The Center at Parmer is fantastic! The nurses don't make me wait for a pain pill, but are supportive when I wanted to decrease my usage and space out my doses. The CNAs are excellent! They provide supplies, keep my room clean, and support me in my efforts to regain my independence! The Center at Parmer has an excellent gym and therapy department! PT/OT encouraged me to learn skills to increase my strength, balance, and flexibility. The kitchen staff made food that was tasty and nutritious. Not gourmet, but hearty and varied. All the staff treated me as a person, not just as one of many! I highly recommend The Center at Parmer! read more

The therapists are absolutely wonderful. So caring/attentive, and comforting. The daytime nurses were all great, although there were many times that I never saw any nurse at their waystation. Nighttime staff always seemed to be inassessible. Many phone calls from the families and call buttons were never answered. Several items "missing" from my husband's room. With the administration, communication issues were common. No one seemed to know what the rules were. Different departments each seemed to have their own rules - or no rules. My husband was brought to the ER room twice in the middle of the night, and I was never told until the next day. A booklet would be really helpful for families. E-mails went unanswered. One question asked of 4 of the 5 administrators received four different responses. Once, I asked 3 administrators and 4 nurses if I could have a mobile chiropractor come in and work on my husband's back which had been wrenched on a fall. I was told there was no problem. So I paid in advance - only to have the person turned back at the front desk, (because of "safety" reasons). read more

I will start with the good: the nursing staff is very good. Both the nurses and the techs work hard and are very caring. The nurse practitioners who oversee the medical care are knowledgeable and provide good care. The physical therapists are very good and caring. Administratively, this place needs a lot of work. A simple handbook telling the patient and family what to expect would be helpful. For example, let the patients and family know that within x hrs of admission, a PT assessment will be made; let the patients and family know when a doctor's appointment has been made on their behalf and what the procedure is for transportation, etc. The food was good, but my husband became bored with it after a while. Just spent 7 days in there for fractured femur and same side hip replacement redo. Had pt 4 times by 4 different people for 45 minutes each in 4 of those 7 days. That's all!! Most of hired help was non caring. Food was terrible. DO NOT ALLOW YOUR FAMILY TO GO TO THIS FACILITY......Why, read below!! my father was in this facility. from the beginning things started off badly. nursing staff is lazy and uncooperative. it's like you bothered them for any questions you had. my dad was left with no dignity. leaving him without pants and a diaper, when he's never needed this before. they continued to leave his pills laying on table expecting him to take, but he couldn't see them because he has no peripheral vision. these pills that were left were for his blood pressure, very important pills. my dad fell out of bed 3 times, fortunately for him he didn't hurt himself. last straw was when he had chest pains and couldn't breathe, they took him to EMERGENCY ROOM at hospital BUT never informed his family until we went next morning to visit him. he was all alone at hospital for hours! My mom was at St. David's Round Rock, IMC for several weeks. The care is good but they are under staffed so I had to be there every day for many hours to ensure my mom drank and ate. When she was released to a nursing facility that I chose (after looking at Google), she wasn't sent with a ventilator which she needs everyday (they discovered this in the hospital due to CO2 retention). So, again, I felt I had to be there daily because they only came in and changed her diaper and there wasn't much therapy. and sure enough, after one week I noticed she was not right and emailed them Sunday night. The following day she was transferred to the ER. The time it was St. David's Georgetown due to Round Rock being full and this time, she was in ICU. Georgetown was GREAT but also short staffed so again, I'm there everyday but this time my drive is farther. After another few weeks she was finally going to move to a rehab place. This time it was the Center at Parmer. Mom was nervous and apprehensive because you never know how you'll be treated in such a vulnerable condition. Well, first off, the made sure a ventilator was installed BEFORE she even got there. The room and facility are beautiful and the food was delicious! One day, in passing, she mentioned that she didn't feel too good and they immediately ordered labs. Most importantly, they were kind, they took GREAT care of her and therapy and staff were wonderful. I was finally able to skip a day or two for self care and I didn't have to worry about her a bit! She was there for a month and is now back home. She absolutely would go back if need be. This place is a class act! Secure, safe, comfortable and again, amazing care. Special shout out to PEARLA, TASHA, JADA, JOSEPHINE, TRINITY, ADELE Thank you, ladies, for all you did for my mom. This facility is amazing. I had to get care do to some complications from a surgery I had. Staff was excellent. In fact I would like to shout out Ingris, Hector, Sophia, Jerry, and Amy. Each of them made my stay great. PT and OT were amazing as well. I felt safe and cared for throughout my stay. It saddens me to have to give such a low rating for anything I review. I would have given this place 1 star but I upped it bc of the following people. Annie NP, LaQuita case manager, Catherine front desk, CG who no longer works there, Lanesha, Tonya, Josephine, Camille, Akayla, various 3rd floor nurses and CNAs. BUT in all my conscience I can not recommend this place other than us having the blessing of meeting those above. My 87yr old mom was admitted here on Christmas Eve for a shattered right knee. Even upon arrival it was a bit lacking. There were inconsistencies to her care from day 1 esp at night. Many times no one was there to change her. She didn't get showered for weeks. I didn't really want to believe it reported it to CG who made sure things changed. She was good about that. But still later I heard about inconsistent care. But we gave them the benefit of the doubt bc it was the holidays and shortage of staff was everywhere. The people I mentioned above did their best to attend to her when they were there. I stopped by everyday to make sure she got the care she needed as well. My mom had to stay almost 3 months to wait for her knee replacement surgery. This facility is nice and new and food was good but we didn't know better because we never had any other facility to compare them to until we had to move to PAM RR Rehab and was met with how real care should be The real complaint we have here is when she finally was set to have surgery and she had to be discharged for a few days. We were then told by Clyde that we didn't need to come back here since she can go rehab at a long term care facility. I was a bit shocked upon hearing that because it didn't feel like we were welcomed to come back. And on the day of her surgery I get a call from them saying she should not come back here and they arranged for her to go to a nursing home. I was floored. We didn't even know if she would make it in surgery and now if she came out she had to go somewhere else. She had her surgery at St. David's RR hospital which I highly recommend. She had to stay there 4 days which was a relief because it gave us more time to figure where to send her for rehab. St David's helped immensely. They were not pleased with the Center for not allowing her back. They made some calls to them and guess what I received a call back from Clyde apologizing and saying for her to come back because it doesn't look good. It was hard to send her back. I had to humble myself to take her back all bc my elderly mom wanted some familiarity. We went back and was admitted to another floor. At first I was hesitant because it's different but it actually was a blessing in disguise. The nurse and aides are kind and attentive at that floor. BUT 5 days after she got there we were told by Ricky, an administrator, that she had to leave. He told every medical staff we owed $. Their revenue director and CEO both confirmed to me that we don't. But since they are having financial issues they are basically letting her go because after 21 days Medicare pays less and her secondary picks up but since they are not a Medicaid facility they aren't getting their full share. I was very disappointed because upon admittance she signed a contract which clearly states she's covered up to 100 days. We were told Friday afternoon to leave and to find her another facility. Everything was closing on a late Friday and poor LaQuita the case manager was busting her butt trying to help us. Good thing I still have contacts with PAM from her home health care before. They did everything they could to get her in but guess what? The Center did not send in her paperwork all weekend and not even the following Monday. PAM had to send reps to come collect her paperwork and got to basically assess the place. They accepted her and was going to admit her next day Tuesday but the Center did not want her one more night to save $ so my poor mom had to be bussed to Round Rock near 10pm. Once at PAM we realized what extraordinary care is supposed to look like. Found out there she never got wound care so her bandages were literally stuck on her skin and she was stiff as anything because she did not get the PT she needed all those months and especially on her post surgical knee. This place rehabbed her well to go home in 2 weeks and treated her like a human in need of care. An elderly person who needs to get back on her feet not kicked to the curb because of her financial situation. I seriously hope they change their ways. It's a shame such a nice new facility can operate with some nice people who work there but run by some cold hearted administrators. Please be well informed before you are admitted here especially if you have to stay awhile. Read the other negative reviews before placing your loved ones here. And be a bit wary. read more

Especially if they are needed LIFE-sustaining oxygen!!** We just had the worst 48 hours of our lives at this so called "Skilled nursing rehabilitation center" ...NEVER never bring your loved ones to this facility it is a hot ass mess. I got my dad out as soon as I could!!! I couldn't make up the events that occurred if I wanted it to it was just unbelievable. Words can't describe this place it just should be shut down I am making a formal complaint to the Texas Department of Aging and Disability Services this place must be held accountable for their actions. Keep in mind I kept my dads consent to treat forms for over 24 hours and no one came back for them imagine if something happen to him they would have been complexly reliable for that!! Just pure negligence!! I wish I could erase the last 48 hours of my life with that experience!! I can't imagine if others don't have an advocate like me in their corner. This is such a HUGE eye opener for me. I literally can't begin to to tell you all the events that occurred but let's just say I almost called 911 at 3am hearing my dad screaming for help on the phone cause no one came to help him for 15 MINUETS because he couldn't breathe and his breathing machine messed up... They didn't even know how to use a bipap machine a LIFE-sustaining tool?! Which Clyde lied and said everyone is certified trained on the machines!! Why admit someone if your facility & your staff isn't trained for this?! Let's just say I didn't leave his side after that until I got him out of that hell hole!! No wants to be held accountable for their actions from the Admissions office to the Case Workers or the nursing staff. It may be beautiful on the inside but the people made it just horrible!! The majority of the nursing staff was likely on some kind of visa or program or traveling nurses. Most of the nursing staff is from South Africa and do not speak fluent english. Most of the staff is complete rude and have no manners. All the staff is literally incompetent they need fully ethics training ASAP!!! Back to the basics people it's nursing 101. I had a meeting with the Executive Director of the center and he showed up and HOUR and 15 minuet late to our meeting enough said... He showed me exactly what this place is about and how things are run. They didn't even try to make things right... But welcome to corporate america ladies and gentlemen. We already been through enough!!! On the next and let's just say we are in a much better environment I cried with so much relief after leaving. Found a place with a high level of care and he will get what he deserves going forward. I hope you guys listen and consider that you shouldn't take your loved one here and spare the heartache/drama and level of stress we just went through. Nursing staff are unhappy and don't care about patients. And that attitude comes from the top down from the ED and especially the Director of Nursing. Place looks nice and they promise the world before you choose them but then continue to lie about services once admitted. Physical therapy only wants to work with the most able. I can not say enough great things about a few of the staff at the Center. I appreciate two CNAs who care deeply for my mother, Xiomara and Napolean! They are fabulous in listening to what my Mom needs! Mom's nurse Nicki is a superstar/rockstar combination! I had a situation for my mom where I felt like I was stuck in the middle of a concern I had spent days (literally!) trying to solve and couldn't get a solution. Nicki stopped what she was doing when I approached, looked at me and listened with compassion, and literally solved my problem. Rehab center? Hardly. My sister was bed ridden and neglected here for 30 days while the insurance was paying them. Once in 30 days gave her a wipe down bath. Yes my sister was heavy and a burden but that's what this place is getting paid handsomely for. If you love the person that needs care, don't take them here. Looking back, my experience at this facility felt like a scene from a horror movie - you know, one of those black and white fifties movies with Bette David who is locked in a mental hospital. 1. In the early evening, I was moved to Parmer in an ambulance from St. David's and ushered in by two women, one on each side of me, holding on to my arms at my elbows. I asked, "What is this place? Why am I here?" They stammered and stuttered. No direct answer. 2. I was taken to a room and given the most disgusting piece of food I have had in a long time: A large rectangle of slightly warm hamburger floating in grease and topped by an inch-thick slab of cheddar cheese, also slightly warm. I stuck my fork into the cheese and decided not to try it. 3. I asked the women if I could take a shower. They told me they were admitting me to a room that had just been made up and that I was to keep it in perfect condition. We arrived at the room; I'm still being escorted with silent women and told not to get any water on the floor from the shower. 4. After the shower, one woman comes in and expresses being annoyed about the wet towel and the slightly damp floor. I ask her where the bell is if I need help. She makes it clear I am not to ring it too many times. 5. During the night I rang the bell so I could go to the bathroom. A woman came in to stand by as I got out of bed. She is obviously disgruntled about my bell ringing and made it clear to me. 6. I decided during the night that I am leaving. I called my daughter around three a.m. and tell her to come get me, that the place is awful. She's not happy about that, but she is a good daughter so she says she'll come by to get me around 8:00 a.m. 7. At 7:30 a.m. or so, I pack my tote bag and walk out of the room. An orderly asks me why I am leaving and I tell him I left some important papers at home and I have to go get them. He is okay with my answer. 8. As I am headed for the elevator, a large woman with a Caribbean accent who is standing at the med cart across the admitting room from me, yells in an angry voice, "Where you going?" I ignore her. She yells again, angrily, "Put on a mask, put on a mask." I ignore her and keep walking. I get in the elevator, push the "1" button, walk out of the elevator and outside where I sit on a curb waiting for my daughter. 9. The Director of Nursing comes out and talks to me while I am sitting on the curb. She wants to know why I am leaving. I tell her that her staff is very rude and impolite. She apologizes and acts surprised but says she'll look into it. She wants me to come back; I refuse. 10. My daughter arrives and I leave. The above took place last year in Sept. I have fluctuated about writing a review. After having read the one and two star reviews, I decided to write about my experience. BTW, I am a retired R.N. read more

not the best care for your loved one while you're out there - many people pay for this kind of care and is very sad to see this places with bad service- definitely feel bad for all the people that stay here and have no siblings to check on them . Not sure if its the lack of facility workers or its the fact that they don't have very good supervision around for nurses- but some nurses here are very rude with bad attitudes When you asked them for help . They have a hard time getting their job done . You press a call button and they don't come to you untill like 30 minutes late -without over exaggerating . What does a patient do if they don't have a sibling to notice this type of situation ? You bring your loved ones here thinking they are going to be ok and well taken care of while they stay here -but nurses here have a big issue getting to them and giving them the attention they require. You can actually see nurses talking /gossiping to other coworkers or on their phones much more then actually working. This happens a-lot during the evening shifts . My siblings and i come visit mom everyday and we flip around all day long and all agree that nurses are not very efficient or professional here especially with those patients that need the extra attention from them . If they don't like to take their career serious-and taking care of people is so hard - I don't see why they are in this profession. They should leave it to nurses who actually would want this type of profession . Very nice place but workers here don't seem happy here read more

They will kiss your ass to get you in. The CNA are always sitting down on their phone and 2 to 3 lights going off. Lazy bunch of people. The facility was great. my mom was there with a very complicated wound and the care she received from the wound care staff was amazing. She also received excellent care from the nurses and direct care staff also. The physical and occupational therapy was outstanding. If we need to have in-patient care again The Center at Parmer is top of the list. read more

Admitted a patient without acquiring their proper equipment before admission. They ordered a CPAP instead of a BiPAP and did not catch their own mistake. They placed the wrong equipment on the patient causing the patient respiratory distress. The family caught the mistake after listening to the patient and looking up the equipment. Notified management about it and how the discharge order is clear of indicating a BiPAP. They stated they were unable to locate a BiPAP and sent the patient to the ER via EMS. The patient went over 36 hours without BiPAP. The patient ended up in the ICU, when if they had done their due diligence the patient would of never been without a BiPaP. Patient also indicated she was sitting in her own waste for many hours without any one changing her or coming to attend to her needs. I'm so disappointed in this outcome. The patient came to get rehabilitation, not get worse. read more

The Center At Parmer - Physical Therapy Near Me - Austin, Texas

My 101 year old mom was sent to this facility on a Tuesday for hospice respite care. Her care team originally thought she had a UTI and would get better quicker in a 24 hour care facility. After finally testing her for UTI on Thursday it was determined that she did not have UTI. The first day I took my mom to this facility everyone was extremely nice and helpful. I told the staff that my mom was a fall risk and asked if they had bed alarms. They do not so I told them to at least have the bed as low as it goes. The Director of Nursing told me that they would not leave my mom alone in her wheelchair in her room, that instead they would bring her to the nurses station where everyone would have eyes on her. The next day was the same. Very caring nursing staff with the same protocol of not leaving my mom sitting alone in her room. Again they took her to the nurses station to keep eyes on her. When I came on Thursday it was a different story. Did not see any of the previous staff. It was 10:45 and my mom was still in bed in her pajamas. I went and asked a nurse to help her up and get dressed and go the bathroom and she told me to go back the room and push the call button and someone would come. Thanks! Then on Friday morning I got a call saying that my mom fell. I was told she had taken off her shoes and slipped out of her locked wheelchair because she had on improper socks. When I got to the facility my mom had been checked over by a hospice nurse and they said other than some bruising she was ok. Thank goodness. My mom was very shaken. She kept saying she had tried to do everything right. She seemed so upset I'm not sure what they had said to her. She stated that she was scared and was adamant that she wanted to leave and go back to her apartment. I called hospice and told them I wanted her discharged. While we were waiting to be discharged one of the CNA's came in and started apologizing over and over again for what had happened. She told us that she had my mom in her wheelchair in the bathroom brushing her teeth and washing her face and got a call to another room so she left her at the sink. When she came back she said that my mom's wheelchair was in the room and she was laying on the floor. She said that she got a pillow to put under my mom's head and went for help to get her up off of the floor. She again was apologizing and said that she should have taken her down to the nurses station instead of leaving her in the bathroom. TOTALLY DIFFERENT STORY! When I confronted the nurse about this she said that my mom had not been in the bathroom and that it was my mom's fault that she fell because she took her shoes off. I told the nurse that because she was a fall risk that she shouldn't have been left alone and should have been taken to the nurses station. The nurse told me that they have a lot of fall risk patients and they can't have them all at the nurses station. I told her that my mom had been in the nurses station before and she told me that she had not. (I was actually sitting with my mom in the nurses station the first day) The nurse also made a snide comment to me about leaving my mom by herself when I had left the day before. I had to point out to her that I had made sure she was in bed when I left and told the nursing staff when I was leaving. She was super RUDE!!!! It's too bad that a few have to ruin it for the majority! This is a "fancy" looking rehab facility but don't let looks fool you! read more

My father spent a week at this facility and his health immediately began declining the minute he made it through the door. He had knee surgery and needed inpatient rehab. I reluctantly chose this place due to the other reviews. I should have gone with my first instinct. His first night there he was refused pain meds because there "wasn't an order" for it. Even though I PHYSICALLY brought the pain meds from the pharmacy myself. He did NOT receive his pain meds until around midnight. The staff did NOT follow protocol & procedure upon his release or the Doctors released orders such as: his daily need for his compression socks during the day, to reduce the swelling and fluid retention on his legs and knee the brace he needed NIGHTLY for his recent knee SURGERY Keeping said knee elevated to reduce the swelling. My sister noticed alot of this the moment she made it to the facility and assessed him. Immediately after, she spoke with the main medical professional in charge. There were many days when I would show up to see him and he had been marinating in his feces and urine for hours. If his medical record was observed you would know he just got over a SEVERE UTI and was at high risk sitting in his fecal matter for hours. He called 911 on several occasions that I discovered by looking through his CALL LOG AND NOT BECAUSE THE STAFF MADE ME AWARE! He stated he was trying to get assistance and kept being ignored. When I inquired about this no one knew what I was referring to. He was ASSAULTED by a staff member whose name I have yet to receive. The staff member that pushed him, did so because he "wasn't moving fast enough". After it was reported to "upper management" I was gaslighted by the director/clinical liaison and questioned as if these accusations were false. I was even questioned about my occupation (my sister's as well) as if that mattered? One of them even stated that they "heard" my father wasn't compliant with certain things and was grumpy. Well I would be too, if your staff was pushing me around/rough with me. The clinical liaison was so nasty to me over the phone when I called to move him to another facility. If you love your loved ones don't bring them here. I feel so horrible for bringing my father here. The only good thing to come out of this was the african american lady at the front desk. She was always so sweet and helpful to both my father and I. His physical therapists were amazing as well. I believe her name was Kathryn. read more

My 90 year old father stayed at The Center for three nights between a hospital stay and a more permanent bed becoming available near his home. The facility is new and very clean. The people were attentive to my Dad and available to me to answer questions and give Dad attention. The PT/OT people did a good job, too. Special note to Reilly who got it done for me, Alma, his nurse and Mimi, the PA who gave Dad her attention. read more

I regret taking my mom to this center some of the staff was good and others were very bad. Nurses Nereida, Alma, Jerry, Hector, and CNA Maria were great but CNA Epsy should not be working in this kind of carreer because she does not like her job and to do this type of job you have to like your job. Furthermore my mom was there for almost a month with a UTI that the doctors and NP in this facility were not able to control. NP Lovely and NP Mini under the supervision of Doctor Cardenas were not able to control a simple UTI. In the month that we has there Dr Cardenas only showed up once and that was after I asked for him to see me in the room. Because I did not like the way that my mom was being cared for I left this place and drove to San Antonio to Methodist Hospital where my mom got a lot better in only 2 days of treatment by Dr. Maria Chapa in Methodist Hospital. While at the hospital Dr Chapa at Methodist Hospital told me that the antibiotic that was being used to cure my mom was not effective with the bacteria that my mom had when she was at the Center at Parmer. Also while at the hospital the staff asked me why did my mom have a pic line, and when I explained that the nursing director Joe from The Center at Parmer said that this was the most safe way to have access to my moms body to be able to administer IV meds I was told that this was not true by the nursing staff at the hospital and that Pic Lines are not the best resort for this type of situation. I was told that mid line is better because it does not go directly to the heart like a pic line does. I WILL NEVER TAKE ANYONE IN MY FAMILY TO THIS PLACE AGAIN THE DOCTORS AND NP THERE ARE A JOKE. read more

BE AFRAID!!! Pls check the reviews on Google as well. My 87 yo FIL broke his leg, and has been here for 2 weeks, and NOT ONCE has anyone been able to share his care plan for PT or OT. His case manager just started, doesn't return calls, and when we do finally connect, doesn't know his care plan. My FIL is just sitting in his room waiting for meals, vitals, and medications. I called the Exec Director Ricky Lopez. His mailbox is full, and he does not answer his phone. The patients are just sitting in their rooms, slowly dying. This place is not well staffed nor well run. Our LT care facility is now on the case, and is reporting the same experience - can't get a hold of anyone; repeated messages left, no return calls, no information shared. It's virtually impossible to figure out what is going on with our loved one. We showed up - with an attorney - to force them to sign the discharge papers. RIDICULOUS. Not good for patients who need medical care. My dad was progressing so well and with in 5 days of being there he passed away. He was immobile and would leave him for 2 hours before they would clean him. He needed medical attention and they are basically only a PT place. He needed his heart and lungs to be monitored. He had a trach tube and needed suction. The nurse said he needed to learn how to cough up his own pfleghm. He kept saying he couldn't breathe and needed to go back to the hospital. That if he stayed there he was going to die. We trusted them to take care of him. We would have never left his side if we thought he would pass away soon. Then they were not helpful or compassionate when he passed. Didn't give us any direction. He passed away at 2am Finally at around 4pm we got the mortuary to get him but the director of nursing was SO rude and so put out. She told me I needed to figure it out or she could just call the police to take him to he city morgue. She was AWFUL!! The funeral director finally got there and too charge and put her in her place. They did not even say any nice words of condolences. Just were Annoyed he was still there. He begged to be taken out of there, oh how i wish we would have worked faster to get him out of that hell hole. Don't send loved ones here if you want them to get out of there alive! read more

My mother was here for two months. This time frame coincides with the amount of time we tried to set up meetings to discuss her evaluations (medical and physical), progress, goals and diagnosis...we were never able to have a meeting. Either the person we needed to meet with all of sudden was not available or they would set up a meeting and not show. That's right, they would set up the meeting and not show up. The medical staff was nonexistent. In the two months my mother was there, she saw the medical staff maybe once, twice at most and then they did no evaluation. I would guess that there was almost 100% turnover in administration while she was there. The nurses barely left the nursing station if you can even find one. Communication with the patient and family was basically zero. I would not recommend this facility to my worst enemy. read more

I had a complete knee replacement on October 4th. I needed a place to stay with great physical therapy and great care. I was there 20 full days. The staff was kind and friendly. The nurses were always smiling and always attentive. The occupational and physical therapists were caring and pushed me to do more each day. The food was outstanding in my opinion. Not one time did I send a plate back with a crumb remaining. I can't say enough good things about my experience there. I felt blessed to be there - and left many new friends with only great memories behind. read more
