My sister, fortunately, was referred to stay at Gracey Wood II a little over two years ago for hospice care. I have experienced a lot of different care centers for family members over my 77 + years but their facility is BY FAR the best and most consistent quality care center I have ever seen. Every time I would visit I had nothing but the best experience with their entire staff, protocols and cleanliness. They have been such a relief to me and my other family members to know that our sister was treated so well, humanely and professionally. She passed away two days ago and we will always be so thankful for the way they took such great care of her! Thank you to Gracey Woos II and their entire staff and administration! Professional management that knows what they are doing. They truly care about residence and staff members.

The new update makes this facility look much nicer. Wish the rooms had been done as well but the staff is caring and nurse practitioners are consistently in the building and available. The staffing ratio is good as well. read more

I'm redoing my original review. Gracy Woods II Living Center on Bittern Hollow has become a sad example of what you get with a Medicaid nursing home. I don't know where it all stems but somebody's making a lot of money and it sure as HELL isn't the real workers here! They're all way underpaid and overworked. All but $60 of every single resident's monthly Social Security check goes to people at the top and the residents and aids are left with scraps. I'm not in a good place to write anything nice other than the fact that there are a few ANGELS that hold this place together and they know who they are. They have heart and work here to actually help people. But don't bring your loved ones to live and die here. Check out the penalties and citations on Texas Health and Human Services. This review is for Gracy Wood off Metric Blvd. Roaches in our room! Had not been mopped in Whoo knows how long. My brother in law is 40 had an 18 wheeler accident about 13 days ago. In icu for a week then they sent him to this awful nursing home. He broke both ankles both wrist 15 ribs and more. The nurses were nice while family was there but at night he said they wouldn't respond to him buzzing the red light and a nurse even told him he's not gonna help him to the toilet and the shower is down the hall. He need to get himself there. Never would give him pain meds thru the night and he even had to crawl to toilet and wipe with pillow case and make his way back. They are awful‼ I told the VP, director of nursing and the administrator of nursing home. They all had excuses to everything I said. I have pictures of old piss in his room, roaches. His bed had no rails he was left in the back. Thank God! We got him out of there. He had the worst experience there. I plan on taking them down! Nurses would move him to where he was crying of pain during the night then say we keep getting in trouble cuz ur family keeps talking. So they neglect him and don't give meds. I went everyday and talked to everyone everyday this week. They look at u like ur stupid. I need to do more. WHAT ABOUT ALL THE OLDER PEOPLE THAT HAVE BEEN THERE FOR MONTHS, YEARS, that have no voice and get this treatment! Let's all report them! GRACY WOOD on Metric blvd in Austin. The new DON and administrator are bullies to the staff there. They have good people leaving because of the way they are treated. Staff are allowed to show up for a 6 AM shift at 7 or 7:30 AM and not being held accountable. If your loved one is being placed in there, make sure they have no wounds. My mother stayed at Gracy Woods 1 for Respite Care and I would not recommend this place. Where shall I begin this place was sold to me by the director telling me to give this place a try that they may not be the newest but they had a hood staff. Well I went to go see my mom on a surprise visit on Sunday to find her in clothes I dropped her off in . I asked her nurse what was that gave me some excuse that they don't give them their clothes Then I went back to find her in soiled clothes and no one offered to help me bathe her. My father has been a resident at Gracy Woods 2 for about a year and I couldn't be happier with the care. Everyone is kind and quick to acknowledge any needs or requests. The nurses are especially kind to me. The staff keeps the rooms pretty clean and they have fun events for the whole family. I also appreciate their care meetings they schedule with me to make sure all is well. The front of the building has been redone and it's gorgeous. I really feel like I found the best place possible. I reached out to Gracy Woods II to arrange a short-term stay for my husband. The Admissions Director, Sandi, was extremely helpful. I had to secure the most recent clinical notes and a list of his medication. She followed up and provided updates on the status of my husband's admission status. The helped to calm my nerves during this process. The nurses were very thorough in the review of my husband's medication, and I was appreciative of the attention to detail. My issue is with the care that my husband received. My husband arrived at Gracy Woods II late Monday afternoon on May 9. He was taken to his room. The furniture is very dated. The first bed in this room did not have protective rails, and there was a short in the controls. The sheets on the bed were extremely bare. The second bed in his room didn't work at all. The hospital table legs were rusted. Sandi helped locate another bed to bring into the room, but the protective railings weren't wide enough to protect anyone from falling out of the bed. When my mother-in-law visited on Wednesday, a day, and a half after admission, she noticed a couple of things. One, when they changed his brief, they didn't clean the penile area at all, and only used a baby wipe on his bottom. Second, there was an open wound on his rear end that was bleeding. He didn't enter the facility with this wound. In less than 48 hours, there was a significant wound on his rear end. This was unacceptable. I realize that there are schedules and other residents who require attention. However, for wound to form within 48 hours speak to a negligent level of care. My husband will not return to Gracy Woods II. I realize that he won't receive the same level of care he receives at home. However, developing a wound within 48 hours is unacceptable. read more

I am writing to express my family's deep appreciation to the staff at Gracy Woods Nursing Center in Austin, Texas. My younger sister passed away on June 19th from terminal cancer. The GWNC staff, to a person, showed themselves to be professional, kind and very compassionate in their care and treatment of my sister. She was comfortable and kept pain-free until the end. This Nursing Home and Rehab almost killed me! They break almost all the State Inspection rules, set up to protect us senior and elderly Americans. I was physically and verbally abused, ( not sexually) but instead of this place letting him go- they just covered it up! They are all like a big family, but patients are just in the way and their paychecks. I kept telling them I was dehydrated, even though I drank water or very diluted tea all day. They never tested me or allowed me to go to the hospital with a order to get checked out. I barely got out alive, a lady helping me, had me on a waiting list elsewhere, and after only being there a few days ( a new Nurse and Rehab, ended up in the hospital after my blood pressure was so low, it took many days, nights in the hospital to recover from very low blood pressure and being dehydrated. My Dad has been a resident at Gracy Woods I since August 2018. Of course I was nervous about how he would be treated and the care he would receive. This place is amazing! The staff really care about each of the residents. They are treated not only with respect, they are treated with love! I try to visit my Dad daily (its not always possible) and I have gone at various times of the day. I see the residents doing activities and the staff engaged in helping them. I've seen the staff hug, and smile, and genuinely treat the residents kindly. They seem to enjoy their jobs, and enjoy the people they care for. His CNAs are wonderful! They are gentle and kind to him, even when he is grumpy or argumentative (Alzheimers) Its not easy taking care of elderly patients, but they do a fabulous job! The nurses keep me updated on his progress and are always available to tell me how this day has been. The administration really worked with me to get through the red tape of his admission. I have been so blessed to know my Dad is so well taken care of. read more

My husband is a quad and requires extra help and equipment. The staff here have been most helpful and friendly. After some initial issues about the care, we have had very good care. With only two reviews before mine, they must take off the reviews. The facility did smell and the little bugs were annoying. Trash in my friends room still hasn't been taken out (three days now and running) and she is in isolation. Bugs are coming out of it now. Super nice physical therapist assistant. My oh my I don't even know where to begin with this facility. First of all let me say they are quick to tell you that they are a five star facility and that could not be so far from the truth. The building is very unkempt as you're walking down the halls the urine and feces smell hit you right in the face. No one seems to know there right from there left. On top of that they have people running the facility that shouldn't be. They have a wound nurse that is supposed to be checking the wounds and doing follow up treatments. She spends most of the time texting on her phone and in the office. The administrator hardly ever leaves her office only to come to the unit to see why no one is answering call lights. The same for the ADON and DON. This place is being ran very poorly and I'm surprised the owners hasn't come to do a surprise visit. They also have a facility called Gracy Wood 2. It's like night and day. I would recommend anyone wanting to put a family member in a GW facility that they choose GW2 and not 1. They spread it on thick with the glitz and glimmer but take it from one who knows for a fact. Run for the hills there not worth the foundation it was built on!!!!!!! read more

This is a very ,very,very, accurate review and,is meant only to help those seeking the best possible care for their family and,loved ones.This review is not meant to make Gracywoods 1 seem like a bad place or to disrespect the company which owns Gracywoods 1 which is Harden healthcare/Trisun,only to be 100% honest.The nurse assistants frequently do not have towels to wash the residents,gloves to prevent cross contamination and,bed pads to properly keep the residents clean and, dry.The equipment to monitor vital signs is always broken and,not enough of them for the amount of residents.Most of the facility is carpet which harbors bacteria and,smells so bad in some rooms that it makes you gag.They will tell you the carpet is sanitized weekly but,it would have to be cleaned daily in order to take care of all of the bathroom accidents and,blood on the carpet from the residents falling and bleeding everywhere.The bathrooms are nasty with clogged sinks ,toliets with broken handles and,over flowing trash on 500 hall more so on the weekends.Be careful while touching doorknobs,they often have bowel movement on them or some nasty looking white stuff.If you walk in to visit during the day around lunch time and,there are residents in the hall,they more than likely will ask you to help them to the bathroom because they have been waiting so long due to not enough nurse assistants to meet the needs of each resident in a timely manner.The door locks do not work properly and,are not 100% secure .I could go on and,on but,I feel this is enough to help the people I have an extra soft spot in my heart for,all people whom need help and, are unable to do for themselves.God bless you and,yours and,I pray that you find a loving and,trusting enivironment to place your loved ones. read more

Where should I start?? 1) Nice pic but misleading. 2) Place was not clean. 3) 4 days before renal diet was put in place for a diabetic. 4) Administration of meds was uncalled for. 5) bedding was horrible and a none working toilet. This place was a nightmare from the get start. No one took responsibility for anything. I could go on and on. I will say Missy the dietitian was helpful... after 4 days and got mom on a renal diet. I had to get in from being out of town to come find mom in a place. Mr Alvarado pt was awesome with mom. There was an aide in the afternoon that was very nice as well. Unfortunately, I did not get her name. Sam and some of the nurses should go back to school and learn some bed side manners. The experience in this place for mom was unexceptable. Mom checked in Friday evening 9/4 for two weeks and was wheeled out of this place Wed afternoon 9/10. Elderly people need more attention than they are getting here. Take care of them as they took care of you. I will never ever send anyone to this place. I will start researching other places for when and if the time comes for mom, hubby or me to be placed in one of these places. Money talks BS walks!!! read more
