Location has changed to Mineral Drive in CDA. I was just there this afternoon to get my son, who is incarcerated, a replacement SSN. Went to Window 1 where I told the rep that I had been in previously and was told by a rep back in 2015 that in order to get a replacement card for him I need to provide proof of his incarceration. It took him over a year but he was finally able to find out who to ask about this proof of incarceration. Well, the SSA rep, who was obviously new, asked another rep about this proof of incarceration. They proceeded to tell me that my son needs to provide a signed note from a doctor that gives his name and birth date. They wouldn't check any further than to look at a list of Acceptable Proof of Identity. The only identification papers I had with me in addition to proof of incarceration, was a copy of his birth certificate, a letter he received when he turned 18 from the Selective Service System, and the Application for a SS card that he had filled out in 2015. I did tell them that my son is incarcerated, not hospitalized! They checked the last time I was in to SSA to request a replacement card, and they said there was nothing in the notes that I did this...I suppose meaning that I lied! Now I don't know what to do. I can't just call the prison. I'll have to wait for my son to call me. He'll probably have to wait another year to get something from a doctor, who he can only see if he has a medical issue. I read on line that replacement SSN cards for prison inmates are covered by a Memorandum of Understanding, RM 10225.125. I hope this is so in my son's case, but SSA wasn't interested in helping me. read more
