Prominence Medicare required a $40 copay for the initial evaluation ($10/PT) and pre approved 16 visits. I had to suspend PT due to lower back pain and Dr requiring lumbar injections prior to an ablation therapy appointment and requested 6 weeks after ablation before resuming PT. Premiere closed my file until I could begin core strengthening PT in the future.
