The people at Ellis are caring and have helped him to strengthen his muscles to maintain some independence. This most recent time, when the staff at Ellis heard that he was coming back, they were excited and have made all attempts to put him in the same room and accommodate him as best they can. She did not…read moreallow for my sister to make an informed decision about her discharge placement violating her patient rights so, she ended up at Ellis. Upon entering she noticed what appeared to be possibly black mold on ceiling. When I asked staff via phone about it they said they could not see it (see pics attached)- They gave her another room. Hair stuck to wall in shower, a white medium size canister labeled "mouse Trap" in shower, wallpaper falling off and stained underneath, water stains on ceiling, Bathroom commode with rust and whatever else was on it. When my other sister pointed these out to administrator, he said he was not aware. Safety checks and environmental checks are a part of any healthcare facility. It is his job to know. Then the maintenance man came out to glue the wall paper back on the wall. My sister was transferred there post-op for physical therapy. It is a major health concern and risk of infection to be admitted there. I am appalled that Dept. of Public Health Licenses them. There is no way they would have passed an inspection. Filthy, unfit, risk of infection facility. I am sad that some people do not have anyone to advocate for them. NOTE:If your loved one is leaving a hospital, ask the case manager and/or discharge planner for a list of places that are covered by their insurance so you can at least research if they are a good place for your loved one. Also Some Medicare plans are now covering community Health Workers to assist you and advocate for you. Please see pics below and Beware of this facility-It does not deserve 1 star

It was something no family should ever have to witness. He was in a room, far away from the nurses station, alone and in the dark. Nothing on but an adult (soiled--and not newly soiled either) diaper and a johnnie hanging off, with only a sheet for warmth-the AC was set at 61. He was laying sideways in the bed, oxygen cannula off. Half the lights didn't work, the tv that was there one day, was gone the next. He hadn't had his medication in over 24 hours because we were not notified we had to sign off on it being given to him, so he was very agitated. When the hospice nurse (who was wonderful!)came in, she was furious. She immediately went to work to figure out what was going on. We got his meds changed but had to wait over an hour for the nurse to even CALL the doctor to have them signed off on and that was only because I kept on her to call. I asked if he could be moved closer and was told "Oh yeah. That's a good idea!" HELLO???? Is this how all their "hospice" patients are treated? Just left in a distant, dark, freezing room? When I did speak to the staff, it was obvious they were understaffed and over worked. When I asked for extra blankets and to help reposition him, they were kind. Though a few of them did yell and treat my aunt with utter anger and disrespect. However, I am absolutely horrified that any elderly person, never mind a US Veteran has to spend the last few days of their lived being left to die with such little dignity. Imagine those poor people who don't have family to advocate for them? I didn't want to believe the reviews, but wow. They are on point. Something needs to be done. Our elderly and veterans deserve so much better. By the grace of God, my uncle passed about 24 hours later. While I miss him terribly, it was a blessing he is out of that dumpster fire of a nursing home.

This facility will literally will put your loved one at risk. My father was rushed to the ER after I found him slumped over in a chair. His room was next to the nursing station. I asked the nurse simple questions like has a doctor made his rounds today and seen my father and did he eat his breakfast. The nurse would not answer these questions directly, forcing me to ask them repeatedly until she admitted she did not know the answers and would have to get back to me. I then asked for assistance for my father. Two people came to check on my father including the doctor. It was not until the 3RD person came in and they took his vitals. It was a dire situation and he desperately needed O2. Thankfully they relented and called the ambulance. During this difficult time, the nurse who I asked the simple questions reported me to the administration. Jeremy, some sort of nursing supervisor, entered my room (while my father was in distress) and accused me of threatening behavior, which he could not articulate. He then threatened to call the POLICE to have my arrested. LOL Guys, these guys are unprofessional clowns. If you have a loved one who you care about, do not send them here. If you have someone you are trying to get rid of or possibly punish, this is your place.

    My mother was in this facility, and I visited my mom every weekend. She had Alzheimer's. One day,…read morePam, the social worker, called me at work at 4pm. Apparently, my mom had been raging all morning and afternoon. Rather than call me when the raging started, she had her sent to Saint Elizabeth's psych ward and started using the term FTD to describe her condition. She said she had never seen such violent behavior. (My mom was 5'2" tall and about 105 pounds, mind you.) Since I worked for the Alzheimer's Association at the time (which is probably the real reason my mom was sent out, as I was fully aware that this facility did not have the required activities programming to market itself as having an Alzheimer's unit but did so anyway), I called contacts at McLean and had her transferred there. Her doctor there said my mom had no business being in a psych ward. She had Alzheimer's type dementia, and her behaviors were relatively common. I had her transferred to Newton Wellesley Alzheimer's Center, and the staff there cared for her lovingly and with tremendous empathy and kindness until she passed away yesterday. I'm writing this review now because I don't want any family to have to suffer through what we went through. If you are absent and don't really care about your loved one with dementia, then choose Golden Living Center in Dedham. However, if you love your mom or dad and plan on visiting regularly, then place your parent somewhere else. You will sleep better, trust me.

    When I first entered Briarwood I thought that this is probably as good as it gets ,but I still hate…read morenursing homes .First the pros ,Briarwood has great personal ,the staff keep up religiously with your meds ,and vitals.The daily cleaning starts early ,like 6am,there are sprinkler systems ,good programs ,pleasant dining ,family room and library.The staff is always pleasant smiling greeting you ,helping you with wheelchairs.They have a terrific rehab gym and staff. The cons,the food while acceptable is mostly boring.When trays were delivered to me in my room ,the food was always cold ,all 3 meals.In the diningroom ,ice cold poached eggs were the norm and never were soft boiled.There were always 2 choices for lunch & dinner but heavily weighted with chicken,always decent desert ,but as I said the food was boring and cold .Having come from MGH where the food was always hot and decently cooked.On weekends some of the staff were surly and it could take on occasion an hour to summon a care person. The foors are slippery especially when they are washed daily early in the morning.I cannot understand why they would wash the bathroom floors in the mornings when most people just getting up would use them with walkers or wheelchairs ,a real hazard .On top of having a round commode on an elongated toilet ,meaning you had to stand up to clean yourself and with slippy floors found my feet slipping and almost fell down.Having the flusher and paper on the left meant you had to stand instead of sitting in a wheelchair,while having them on the right side you could reach from your wheelchair ,a very dangerous situation which I pointed out to the staff. In the next room was a patient who would rant and scream daily in the middle of the night,many people,some stroke victims trying to rehab, lost valuable sleep affecting their rehab because they and I where too tired from lack of sleep and couldn't function .This went on for the 40 days I was there ,the whole floor suffered and the facilty tolerated it,that was totally unacceptable .Most rooms were mostly double but 4 people shared a bathroom.My roommate got discharged and I got a very sick person who yelped and moaned all night ,frequently tried to get out of bed setting off bells I could not sleep.I got myself transferred but I was so weakened that I fell.The TV'S where very difficult to change channels ,no remotes you would have to press and press and it would take many tries to change channels,no remotes hereThe bottom line,this nursing home by tolerating disruptive behavior caused me and many others great harm.When this mans family came to visit all 6 would pile into the room effectively keeping me from entering my room.There should be limits ,and that is why I only gave them 3 stars.I am quite bitter and by the way my disruptive room mate wound up with a private room ,while I was scrunched into a small corner of another room .

Her room was so cramped that in order to make it possible for her to sit in a chair we had to move all of the furniture. As a result she was always in bed if we weren't there to set everything up. The communication was non-existent. We had a meeting with her care team and no one in the meeting had actually worked with her. The food was not good with no choice and she either had to eat in her bed or in the hallway. Although the nurses and aides were pleasant they seemed very overworked. Stay away from this rehab!

    With some personal details edited out, below is the text of a complaint I am filing tomorrow (May…read more17th, 2022) with the State of Massachusetts Department of Public Health: My mother has been a patient at West Roxbury Health & Rehabilitation since this past December 23rd. The main focuses of my mother's admittance into West Roxbury Health & Rehabilitation were intended to be physical therapy (building towards resuming her ability to walk) and continued mental health care (my mother is manic/bipolar and has been meeting with outside therapists for several years). The former has transitioned from lackluster previously to, for at least two months, nonexistent. The latter has been nonexistent or, to be blunt, neglected. Over time, and has been expressed to me by various nurses and medical professionals employed by West Roxbury Health & Rehabilitation (and its parent company, Bear Mountain), my mother has developed bed sores on her buttocks. The physical therapy component of her care has been limited to some range-of-motion exercises focused on her arms. On April 9th, in conversation with my mother, I learned she had not received any physical therapy since at least February. On the evening of April 14th, I detailed, in writing, the physical therapy I feel my mother needs to receive - sitting on the edge of her bed, followed by sitting in a chair, then a wheelchair, and then exercises focused towards walking. My handwritten request, as stated above, was ignored. In addition, via text exchanges and a phone conversation with my mother's outside mental health therapist, I learned that neither the Director of Social Services at West Roxbury Health & Rehabilitation, Regina Weston, nor any of her corresponding peers at West Roxbury, had never reached out to her, including a request for permission to access my mother's mental health records, in any form or fashion (I provided Regina with her contact information upon my mother's admittance, explaining the importance of contacting her, for my mom's mental health and wellness). Since April 15th, none of the voicemails - four in total - left for Regina have received a response; the same can be said for a handwritten note I left in Regina's inbox on the evening of May 4th (the note was received and removed from her inbox the following day. In sum, a staggering lack of professional competence on Regina's behalf). My mother, quite often, has reported medical issues and not received treatment. These issues include a cold (she was not provided any medicine by the facility; I brought her a bag of Halls' Cough Drops) and abdominal aches which went untreated. She has approximately eight or nine changes of clothes I brought her from home. There have been repeated incidents of the morning nurse on duty of refusing to switch my mother from a johnnie to a change of clothes. No explanations have ever been provided by the nursing staff for these refusals. As well, there have been ongoing infestations of cockroaches and spiders within the facility. I overheard talk in mid-March from one of the nursing-level staff members of an exterminator being contracted to eliminate the infestations. Apparently the attempts were lackluster, or have not been followed up upon. The patients' bathroom my mother utilizes has not had a properly functioning paper towel dispenser nor a properly functioning soap dispenser since at least her date of admission last December. Less than half the time (I visit my mom at least six days per week) is there a replacement supply of soap; more often than not, there is no soap at all. The bathroom in question often features lingering odors of urine and/or feces. The staff bathroom, located within a common hallway, is properly maintained, as I have utilized it on occasion when the restroom adjoining my mother's room is occupied, with a properly functioning and more modern paper towel dispenser, as well as a properly functioning soap dispenser - both fully stocked or supplied. (I hope your Department performs unannounced inspections, because this place needs one. Badly.) Following a comprehensive medical examination at ideally either Faulkner Hospital or Massachusetts General Hospital, my mother needs to be transferred to a facility where her physical and mental health therapy needs will be fulfilled. West Roxbury Health & Rehabilitation has made it clear they are not capable of serving as the facility in question, and they do not deserve an opportunity to prove me wrong. I am filing this complaint with the hope that you can assist in reaching out to the facility's management in a manner which will be responded to. (I have no reason to bear any faith any attempts by me, based upon their established track record, would generate such a reply from said facility management.) They seem content to allow their patients to reside at their facility without adequate care until they're ready to die, even when they have - as my mother does - a home to return to.

On Friday they gave him the wrong breakfast. They gave him the wrong medication at lunchtime (it was another patients), fortunately he recognized it was not his medication and refused to take it. The workers were not responsive to alarms. He had to call me at home when he had trouble breathing because no one would come. On Saturday we took him out of the facility without telling anyone (there wasn't anyone around to tell) and checked him into a hospital.

It needs to be closed or all management fired and replaced along with most of their staff. My mom was in there for a I week and I took her out. She cannot walk and has stage 4 cancer. She was left in her pee for hours, she was left unattended for hours, she was just n a lot of pain from her fall and her cancer they ran out of her prescription and she did not get it for 2 days, the nurses are extremely rude, unprofessional and do not care about the patients. My mom had chemo the staff assured me that they would bring her days before her appointment. The day of my moms chemo appointment my mom called me and told me that they were not bringing her to her chemotherapy appointment. They have rats and the showers do not no work. This is just a few things that has happened to me mom in 1 week of her being there. There is more. Please don't send your loved one to this place. My mom and others have experienced neglect and abuse there. I made serval complaints nothing was done. I call the Mass board and the police and filed a police report. Let's see what happens
