This is my last and final message for Mirage Postacute in Lancaster. I don't even like to get dirty, but they really did me dirty. They They made me go against medical advice, Deacon I had no other choice. I was totally acceptable to what they were saying they may leave at night with no medication. I haven't had medication since I have super high blood pressure. Jenny Social Worker is a snake in the grass, Barrett is there. I don't suggest anybody go there or take your loved ones. Johann is so rude I went there for a interview and I had to tell him to walk me out because he wasn't professional at all I applied for a cna position I was early for my interview wore scrubs i have nails on and he told me I don't like your nails repeatedly the first time he said anything about my nails I let him know like I can remove them he just wasn't professional and it's sad especially if he hiring people please train johann because he seriously needs to be trained again on his customer service skills Worst interview ever read more

He has been at the Mirage for 5 days and been moved 4 times. His clothes have already been lost. The family has requested a few things such as coffee and ensure with meals and that has not been done. The television in all rooms is sketchy and the patients cannot even watch a program. It appears that everyone is so worried about the poor ratings they are getting that the patient's needs are somewhat neglected. I had trouble with this facility years ago and it seems that will follow them b4cause I don't see much improvement. read more

My father was placed at the Mirage for a bed sore on his behind. It was a high stage 3 to low state 4 when he entered in July. When he came to our home for hospice, it was rated as unstageable. Not only had it grown to ~3" in diameter, with ~1 1/2" of full one exposure, but it had started to rot. The stench took your breath away. The Wound care folks kept telling me it was improving so I never insisted that they turn him over so I could see it. UCLA also treated him for sepsis several times when he was sent out due to catheter not being cleaned or replaced properly. We had gotten a suprapubic catheter to reduce risk of infection and to eliminate the discomfort associated with regular catheters. UCLA provided detailed instructions that were easy to follow and no one followed them. His urine bag wasn't even emptied regularly. The dietician didn't follow the diet he was supposed to be on in order to reduce stones in the bladder. Their idea of clean is to shower every three days. They wouldn't change that schedule despite the fact that he had psoriasis AND his wound needed to be completely free of fecal matter. They didn't change his diaper regularly enough to keep the wound from almost continual exposure to filth. The family visited regularly and tried to insure that his care was kept up (we couldn't keep him at home due to the wound care requirements). They would increase care for less than a week and then go back to the same sloppy patterns. The facility stinks so badly that it made me gag, even with a mask on. Solid white t-shirts became yellow/light brown within a few washes as their laundry is done with nasty clothes that need to be rinsed thoroughly before being washed. His pants had bleach stains on them so we finally started doing the laundry ourselves to make sure his clothes were clean and fresh. We had one of his doctors replaced due to lack of communication and the next was equally as bad despite my requests for phone calls and when I would show up on days he was supposed to be there, his day had "changed" that week. Joseph, the head of the facility, argued with me after my father's death and tried to claim they had improved. They hadn't improved at all. Archie, their head of social work, made racist statements about white privilege on our first family meeting. He was useless otherwise. We are now missing property AGAIN and they won't return. our messages regarding getting property back. Several items "disappeared" during my father's stay, including his fan and watch. All property was properly checked in through their "system." Linda, at the front desk, is awesome. She was always very kind and welcoming. The PT/OT team was very helpful and kind. Cathy was sweet and obviously cared about him tremendously. Tina was awesome, but she was moved away from his station. Je'Don was awesome, but she was also moved away from his station. The night nurse was so bad, all of the three patients in his room were afraid of him. He was physically abusive to them and also yelled at them. The patients actually told me and asked me to please see if I could get him moved as they were afraid for their safety. Thankfully, he was at least moved away from the station. PLEASE don't use this facility unless you have no other option, then stay on them constantly. Check your loved one's entire body regularly as my father ended up with so many sores. Hospice was shocked to see him when he arrived and we cleaned him up (he was terribly filthy) and put him in clean bed and gown. Hospice couldn't do anything for his wound as it was rotting and he was in end-of-life care. read more

following a car accident, I was so well taken care of of all by all the staff at Mirage, the rehab department helped me walk again in just a matter of 2 months. all of the CNA's were incredible. As a severly handicap individual utilizing the facility as a long-term resident I've watched The Mirage Convelecent facility (formally known as Wellsprings) over these past few years go through many changes. Some of these changes are super awesome good some of them not so good but over these past few months under new administration specifically under Joseph Garibay1 the Mirage is a shining bright new star in the industry! The atmosphere has been uplifted with the addition of new lighting and new electronic fixtures in our common rooms which have brought us into more of a common age of quality of care! I want to thank the CNA staff at station 3 for being stellar stars and our LVNs who distribute our medications and are getting better and timely day by day. The PT/OT dept is beyond compare and the jewel in the crown.......I love you all. Thank you! All in all with the good comes the not so good and as a resident you having a matter of patience as a patient is key to having a cohesive and mutually respected relationship between resident and staff. Impossible to reach my loved one by phone . There are no phones in the rooms , youre at the mercy of the nurses station . They don't answer . Their websit has the option to send a loved one a vi card . But when I try to submit the. are it will not submit . I've tried this mul times .I reached out thru email via the website to ask them to fix it . No response . I reached out to the creators of the website Yolo . asking them to fix it or steer me in the direction of whom ever can . No response . Their website does not have any contact info other than the phone number I already have . I do not live in the city Wellsprings is located in. I'm sure my loved one is feeling forgotten about . That is not ok . I cannot comment on the condition of the facility it self . Ive never been there . No can I comment on the quality of care my loved one is getting . My sister had to go to this place after breaking her leg. Because she arrived from the hospital on a Friday afternoon, she spent the entire weekend without getting any kind of PT or OT. AV Hospital hadn't been great about the PT either, so this was over ten days of not getting up and out of bed. Walking down its maze-like hallways is like a trip through Dante's nine circles of hell. The place is grungy and grubby, and there is actual urine on the floor. The smell is very unpleasant. There is really no privacy for the people there as they are stacked three to a room. It is like an animal shelter for old people. While my sister was there, they were jack hammering the patio to make way for new construction. For days there was continual din and dust. The lady during the weekdays at the desk is crabby. The individual health care workers are mostly lovely people who are so overworked they can hardly get to the patients. The vast majority of them are kind and helpful. The turnover rate here is over 50% so you can be sure the staff is not happy with the conditions. They would love to have more time to spend with each patient. If you need to put your relative in a home, choose a different one. This palce needs a lot of work to improve. read more

The hardest place to review is a place you don't want to see your loved one in. We are here for cracked hip rehab and long term care. That being said, this is a good facility with great employees. Everyone has been very living and kind to mother in law who has dementia. She wakes and is very frightened but the staff tries to help her as much as possible. read more

This place is great I especially love the nurse who took care of my great grandmother before she passed away. My great grandmother lived a long life of 108yrs young. The nurse Tawny Rivera-Leon was great with her and I appreciated her long commitment with her. I knew if I had any questions I can count on Tawny to answer those tough questions. My brother was at Wellsprings for almost a year. I couldn't be happier with the care he received. The facility maintained excellent communication with the family and was very helpful when my brother was transferred. My brother left in great condition. He was at Wellsprings as long as he was because it was hard to find placement during the pandemic. He never got covid as they had excellent monitoring and tested for covid frequently. I apologize in advance if you have the misfortune of having any relative, friend, acquaintance, or even an enemy stuck in this establishment! My nephew had the misfortune of being in this place exactly TWO (2) days. AVH transferred him because he condition was improving. Two days at Wellspring, he was transferred back to AVH, basically comatose and passed away within hours. My nephew's Cardio Doctor couldn't believe he had only been there 2 days. Then, to make matters worse, the bill came, and came, and came! I contacted Wellsprings when my nephew passed, so they were aware. Then the bills came! Called again, this time I asked to speak with billing. Transferred to voice mail for Daphne, Left a message for Daphne to please call me back. No call! It has now been 11 months and still no call back! I got another bill today, and attempted to call and talk Rafael Reyes, the do nothing admin, or to anyone who could do something about the billing and the threats about sending me to collection. I was put on hold numerous times, several different people pick the phone up and asked what I wanted and needed and who I wanted to talk to and put me on hold again for a total of six times. Was never transferred to Rafael Reyes, do nothing Director of Wellsprings, was not transfer to the do nothing billing department Daphne. So now I am writing a review about how horrible, uncaring, unfeeling, disorganized, this place is! If this is how they treat callers regarding money, can you just imagine how they treat your loved ones? Especially the ones that cannot fend for themselves? Corporate offices will get complaint forwarded. Maybe they could get Rafael or Daphne to return calls!! read more

My loved one was in this facility and the Administrator is horrible. My friend and I both have family here and when I called to speak to the Administrator he wouldn't call me back he had his assistant or the social service director call me back. Colleen the social service director was great and helped me but I wanted to talk to Rafael but nope he won't return my calls. I understand they're busy but I know you have a minute to call a patients family member back who has left numerous messages. I'm giving Wellsprings three stars for a few reasons. Now let me list them for you, one, my father was a bigger man. Now I'm not saying he was obese but he was around 240-250 when he was there. The proportion of the meals where terribly small. One meal he was given only 7 french fries. Litterally. We counted. Another reason I didn't give five stars is because I repeatedly told them my Dad was going to need tons of encouragement. You can't just go in and say, "you want to do physical therapy today?" I told them numerous times he would just say no. That you should be going in and saying, "it's times for physical therapy, so let's do it." So some days he wouldn't even receive therapy which was the WHOLE reason he was there. I would visit him DAILY and some days he would just lay in bed and watch TV. My third issue was lack of attention. My father has Brian cancer and a chunk of his skull was removed and one day they called me to tell me he was acting funny and oddly lethargic. I rushed from my home to Lancaster to see my dad's head was leaking puss. That's not cool. That's obviously an infection, they should have noticed! On the other hand, there are some positive things about this Acute center. One thing was that his nurse seemed to actually listen when I would tell them things about my dad. I felt they always took me seriously and made my Dad feel comfortable for his short stay. Another thing is despite the lack of daily therapy, once he did get some he started to do great. The staff is awfully nice and always gave my dad ice cream and he loved it. Thank you guys. read more

If you really love your family, pls don't put them in this place. They give each CNA 10 - 12 patients each to take care of. It's hard and they all can't attend their needs right away. I've met some cna's that are really nice and that they do care about their patients and they love what they do. They don't have an adequate back up generator system and this is a care facility for patients who are in relying on their care and require a fully functioning facility. Do not relinquish any personal information, nor bring any personal belongings, it will disappear. Better to not patronize this facility - you will only regret it. They are not trustworthy. frustration overload! Walk in and your loved one has received no care since around noon and it's 5:20! I walked in at 5, put the light on so that she could receive care. The nurse was in the hallway but neglected to come in and check to see what the need was. The light was on for 20 minutes, staff walking up and down the hallway, nurse actually in the hallway, noone answers the light or comes in to check. I had to call the receptionist's desk and she had to call someone over the intercom. The person came five minutes later-did what she should have done hours before-said she was busy with other patients and had been sent to dining. Mentioned the situation to the nurse (who was the nurse that had been in hallway by the way), she had no comment. Wow! I give one star for the Staff who did their best to accommodate my mother-in-law, but this place has some evil heartless people working in their admissions. Because my wife went against their wishes and removed my mother-in-law until their facility had gotten the proper paperwork and was prepared to receiver her. In response they turned my mother-in-law down. The fact that you can do this to a 71 year old woman who had major life threatening surgery 5 days ago shows that you have no empathy and I pray for your sake that when you are 71 people like you are no longer working in this industry or your time will come. read more

I have been a patient here for about 3 weeks. Like most SNF's, their staff to patient ratio isn't ideal, but better that other SNF's I've been in. The staff are always pleasant, wanting to help, and accommodating. I had an issue with another patient in the bed next to me who was mentally troubled. The man kept me awake all night talking loudly, cussing, and threatening me. The staff quickly moved me to another room. I'm now able to get some sleep. I thank Wellsprings for their caring actions and would recommend them as a decent place to recuperate. read more

Since my cousin has been there he has had the worst care you can imagine. When he first arrived they weren't giving him enough to eat and he lost so much weight he was sent to Antelope Valley Hospital because they didn't know what was wrong with him. When we went to see him they had served him pizza. It looked like something you wouldn't even feed your dog let alone a person. At this point they don't pick up his laundry. It's been sitting they for awhile. Unexceptionable! The nursing staff seems like they are on constant vacation. They need to be replaced with people who want to work and are not there just for a pay check. I was a student there I enjoyed helping people and I can't even understand how they can be so non caring about the sick and elderly. What a shame. They don't give him his medication in a timely matter. So who is monitoring this facility to ensure good patient care. I would be embarrassed to say I was in any type of management there. Hopefully no one in your family will receive this type of treatment. Remember what goes around comes around. The mistreatment of others will come back to haunt you. You get one star because that's the lowest because I really want to give you a zero. But I guess one star is good because he does have a bed. read more
