Excellent Care! My husband was admitted to Ellison John on 12/27. When admitted, he had a tracheostomy and a feeding tube and was admitted into the subacute ward. 2 weeks later he was decannulated and eating regular food and was moved to a regular room, and today, just 3 weeks after being admitted and well ahead of schedule, he's going home. We credit his amazing recovery to the staff of Ellison John. They have been kind, and professional, and they've kept him motivated to move forward. We're very grateful! read more

Had my mother in law at the facility. They did not take care of her properly. We took better care of her on our visits than they did. We constantly had to remind them to have my mother in law takes her meds etc. I highly discourage anyone from checking in a patient there. read more

This is the worst care center you can send your family they are not professional at all, hire new grads that dont know what thier doing when it comes to the residents they have roaches , it's unsanitary disorganized and the staff are inconsiderate they dont care about their workers ! I'm presently at Ellison John for rehabilitation on January 9 2025 after a left knee replacement. The CNA's are not trained very well. I had a near catastrophe in the shower. The CNA didn't put the break on the shower chair and I caught myself sliding out of the chair when the chair tilted forward. I wasn't allowed to get out of bed until PT says it's ok. I'm still in bed, two days later !!! I CAME HERE FOR REHAB OMG! Ive been given cold food as well. read more

Poor care/borderline elder abuse, nursing staff no where available, CEO Shane Carlson refuses to return calls/fails to come into the office, deceitful staff, lets patients sit in their feces because no staff is around, no access to rehabilitation services because staff don't show up. This facility is absolutely terrible. I am so disappointed and just in disbelief with how unprofessional and careless this facility is when it comes to their patients. My father is currently staying there and has had multiple falls and has been left on the floor for who know how long until they found him. Every time I visit the facility I hit the call button for the nurse to time them and they never fail to take at least 30 minutes to an hour. My father got a Picc Line inserted and I warned the facility about it and told them they must watch him at all times because HE WILL pull it out. After having the picc line for less than 72 hours he pulled it out and the facility never even bothered to contact me or my family about the situation. After addressing the problem to my father's nurse she proceeded to explain to me in a way that she felt the picc line wasn't necessary because he was taking oral medication. My family and I have been waiting 2 months for these infusion appointments and due to this facilities lack of care and awareness they have canceled every appointment of his for the next 3 months and my father is no longer going to receive the care he severely needs. Please beware of this horrible facility and I would only suggest this facility to those who don't care about the health and wellness of their loved ones. read more

The only transitional care this snake pit offers is to help the patient transition into a coffin!!! Staff doesn't care if patient lives or dies. One racist aide ignored patient's requests for help. Nurses come to answer call light, turn it off and do nothing for the patient. Patient left laying in her own feces, more than once, for 45 minutes due to staff neglect. Staff doctor, Aung, says she will correct the problems but the problems still persist. Dr. Aung does not communicate well with patient's doctor. Does not believe when patient's MD orders staff doctor her to change patient's diet. Overall quality of care is absolutely abysmal! I wonder what the annual patient death rate is here. read more

I go there to visit my sister there is (6) light on waiting for the CNA's to come the help is poor the food is sometimes salty The Dr. Over the facility Dr. Mouse he never comes to the facility and if u call him he never returns phone calls. I was Patient dumped here by AVH. upon arriving for aftercare, approved by Kaiser, which I belong to, for recovering from surgery knee replacement. an unknown man wanted to strip search me right away by trying to undo my hospital gown, . I push him away he got offended! and began to dump my belongings on the bed, he spoke broken English! looking for drugs, while talking to the female in charge. I'm sitting on the edge of the bed, that was not properly fitted with pillows to prop my leg up, on the bed, she said they don't have extra! He was standing on the other side, she was on the other side of the bed while focusing my attention on her, In shocked. He stuck a needle in my forearm, while I was not looking, I call family member that could not get to me, so my family member ordered me a uber to take me home , where there was no one, I hobble along to front door crying in pain cause it was hard to walk, a long corridor to front entrance and waited for an uber, which he did not help me get in the car! the young women in charge said the Hosp. should have told you, we cannot support your recovery, here, but you can leave if you like! sign this paper work, I did. this place needs to be inspected asp patient abuse! I was abused and dumped here and shot with a needle , touched improperly to be strip-search, and when I wasn't looking in my forearm while talking to the women that said I can leave! I left! in pain in my hospital gown and nothing under it, and caught bronchitis, I was elderly abused! I couldn't even put on my shoes! one hand on walker one hand holding my hospital gown close, escaping this facility. I am a senior I have good medical Insurance wow! I call kaiser the next day they came out to my home to attend to me! I'm still in shock and in pain and traumatized! read more

My family almost died from his family continuously giving him alcohol, as well as stealing his wallet, phone, and briefcase. As well as having spousal rights I have POA Durable. I have repeatedly asked Social Services and the front desk to have calls not disturb my family member as well as no visits per his requests. It's been over a month and the facility Ellis John Rehabilitation Center has not done anything about the request. I am was told also that he has no rights to use the window in his room. Horrific! Unfortunately this shithole is the only "Transitional Care" facility that Kaiser uses in the Antelope Valley. A person might come here to 'transition' but there's very little 'care' involved unless you provide it yourself. Stay vigilant and on site, as much as possible, if you have a loved one admitted here. Keep track of patient belongings too; staff discarded my mother's hearing aides. There were a few good therapists in the gym, but the overall experience was subpar even by DOC standards!!!! read more

I wish I could give this place a wonderful review but that would be a lie. This place disgusts me!!! Drs and nurses warned us but we didnt have any other option. We were hoping it would be a good experience and we went in with an open mind This place was an absolute nightmare from the beginning. Going into a "skilled" place is a joke. My dad regressed so much in this place. He would have done better at home. Everything was like pulling teeth and lies from upper management. Honestly not sure how upper management can even feel good about themselves. Most of the staff was sweet but going for rehab and then sitting in bed for practically 3 months is unacceptable. $17,000 a month just for room and board to lay in bed. Dad went in for rehab and never even stepped into the room. We were told he would be able to wear regular clothes as well and was always in a gown. They were cited for giving medication that shouldnt have been given. Why does he have diarrhea??? Oops we were giving him two stool softeners the entire time. That only made his potassium plummet. Sitting in stool and urine for hrs in the beginning. This place needs big time help for the future residents and patients! I truly could never have fathomed how horrendous every single day was at that place. read more

GREED OVER CARE. They DO NOT CARE about family members providing care and support to their loved ones. Ghetto CNA attitudes from LA imports fresh out of CNA school and full of nasty attitudes. There are some very caring Nurse staff, and PT people, but the ugliness overshadows the good ones. Subpar food and endless miscommunications with kitchen staff regarding dietary restrictions. Read and trust the other reviews. DO NOT LEAVE YOUR LOVED ONES HERE UNATTENDED. WATCH THEM LIKE A HAWK. Very nasty attitude coming from one social services member trying to kick my mother out because I cleaned and, properly, treated her wounds on her toes. I just about jumped through the phone at this nasty lady threatening me. Mom accidently ingested medicated cream that they left out on her table because she thought it might have been mayo. They kinked her oxygen hose and left it. They unplugged her oxygen and left it like that for at least one night. I have never felt more overwhelmed, frustrated, helpless, and furious with the piss poor customer service and neglect. This facility also has TONS of very bad inspections due to their incompetence. Fines for being understaffed. There are CNAs doing the jobs of LVNs and visa versa. Wrongful death lawsuit back in February 2023. It should be a negative score. I worked in the ICU of the local hospital, patients came in regularly septic with MRSA. Other professionals i know that have worked there say they are fully staffed, but each staff mamber has unethical work (too many patients) assignments. If you love your family member, don't bring them to Ellison John. If you hate them, this is the perfect place. read more

It was a horror from the start. Being stuck in diapers for hours, going without water for hours, attitude from nurses, it was horrible. Left me in a room by myself for days...kept me constantly drugged up on a combination of morphine, oxycontin and adavan. They bullied me when I tried to leave...it was a horror and made me vow to NEVER end up in a nursing home which is what it really is.​ read more

Unfortunately I had to select a star. They do not deserve any stars. This place should not be allowed to operate. It's not a good place for your loved ones. My mother was there for nearly 8 months, after a stroke, we never left her side, specially after seeing she was not properly cared for, despite the multiple complaints to all leadership there was no follow through or actions taken to address and avoid from continuing to happen. My mom's mental and physical condition were impacted and she relied on their medical expertise to provide help, she was given medication that wasn't documented and no one knew what was given to her, just that they knew she received something, not concerned at all that my mom had been asleep all day and not waking up, this happened 3 times. My moms medication was changed and caused an allergic reaction and not once did they stop to think or assess her condition as they did see excessive itching throughout her body. My mom would be left without her feeding tube reconnected for hours until we would find it and question it. The responses we would get would be oh I thought I connected her after her shower this morning. My mom had a trache, she would constantly try to pull it out, or she would disconnect her oxygen, well unfortunately she disconnected her oxygen and was lacking oxygen for a long period to where they called us to notify us that her oxygen level was very low and she might need to be sent to ER, so kind you if their supposedly checking her why did it take that long to take action of her monitor was going off??!! They're careless. My mom eventually also removed her Rachel and when they also noticed it was too late to put back in. Again my mom is not mentally there. I can go on and on. You can hear monitors going off and there's no sense of urgency. They're rude and unprofessional. The worse to deal with are MJ nurse lead, she's horrible and zero professionalism, I could not believe her lack of care to address the fact my mom was given medication and she could not find what it was, she's the one who told us she knew my mom was given something but didn't know what, she looked at her chart and stated it was not documented, very non challant stated she'll wake up later, I'm sure if it was her mother she would not have been so calm! Liliana social worker, I just don't see how she can be fit to even have a job when she's asked who's the GM for the facility she' stays quiet, you ask questions about who to contact to file complaints and she remains quiet. She scheduled meetings that had already taken place. Cecilia, Tatiana case managers, trying to get a hold of them was insane, when I finally did speak to them I was told they were filing grievances for all the issues we had and yet they filed what they wanted and not the major concerns. There's no follow through and you get nothing but excuses from both. Allison administration I spoke to her because she was asked to call me on behalf of the GM Shane, after sharing everything we had experienced she was very apologetic and stated it was unacceptable and that she would immediately look into it and call me a few days later, needless to say she never called me. I had to reach out and all I got was I apologize I thought Cecilia was reaching out to you, I said really wow I did tell you she doesn't call back or follow through why would you really think she'd call, and you said you were calling me back! My mom could have died here, it is to this extreme, we brought her home because we just could not continue to put her through this unhealthy environment. SAVE your loved ones and find a good place for them. This is not a good place for anyone. read more

I went to drop off some items for a resident on a Saturday at 2 p.m. and was turned away because it was not a weekday. As it is they hold your items for 3 days to quarantine them before giving it to the resident and now without any notice they will turn you away if you come on the weekend. Nowhere on their website does it state the rules but they still turn you away. What is the reasoning for this? I get when they have to abide by health department guidelines but this is not one of those. Is it because they're too busy? I think not. The girl that came to the door was just sitting there. Every time one of you employees at the "care" facility answers your cell phone or sends personal text messages, every time you stand around gossiping or speaking ill of a resident, you're wasting time that could be sent making one of the residents more comfortable. The nurses here are the most negligent and rudest!! I'm a patient here and the way I'm treated is causing me to have severe depression and anxiety!! The nurse a few nights ago got upset with me because I asked her to move my personal property before moving me so it wouldn't fall and get broken so she started yelling at me at the top of her lungs for 5 minutes so I asked her to leave and the other nurse asked me what happened and as I'm explaining to her the rude nurse came back in and proceeded to yell at me! Today my nurse came in to feed me and I asked her to remove my pillow under my legs because it got wet and she removed it and said there were no more pillow cases, which I know was a lie... so she went to put the pillow back under me still wet and I said ew, no please remove it! she got so mad, started huffing and puffing and threw them pillow across the room and refused to feed me and left the room! I have never experienced such poor patient care before! I'm so miserable here! Please, if you care for your loved one then don't send them here! read more

Not surprised that there are COVID cases at Ellison John. I know that it must be challenging to provide care in the midst of a pandemic. The only way covid could be transmitted in a nursing home is by the staff. All residents have been quarantined for months. read more

So, as you can see Dana Cook, left me a message to call her, so we could discuss this. Amazing. It's been over 6 months, and with all the complaints my husband and I registered, no one, has ever bothered to contact us. I responded to someone's post on Facebook, regarding our experience at Ellison John's, and lo and behold, the next day, there's Dana's comments. Isn't that just amazing? So, I figured why not, I'll give her a call. I guess, she left that message just to show how much she "cares". Left 2 messages. She couldn't be bothered to actually contact me. I'll say it again, if you have to put a family member in a facility like this, be their advocate. Watch the staff like your family members life depended on it, cause it probably will. My Dad has mild diabetes, controlled by diet, these people kept giving him sugary foods, and then had to give him shots, to get the blood sugar under control. The man is and was 90 years old, he was constantly being given his meds at the wrong time, so he never seemed to know what was going on. They told me, this 90 yr old man, who was constantly in a daze, should have known better than to eat the sugary food. I thought they were supposed to help him, but im pretty sure, if we hadn't gotten him out if there, when we did, he would no longer be with us. The only good people that we had there, were the physical therapists. They actually seemed to care. read more

Nurse Alyssa, Nurse Rosie, and Nurse Fe and all the other nurses that attended to my dad were all so caring and helpful. Dr. Swaroup, and the case managers Cecilia, Glenda, Silvia were great and they all made sure my dad was well taken care of. PT worked well with my dad and got him stronger. Everyone really went above and beyond in their care for my dad, and I really appreciated it. They took the time to get to know him and what works for him. They were also in constant communication with us the family, which really alleviated any worries that we had. I highly recommend this nursing facility and all the people who cared for my dad. read more

I have worked at Ellison John as an RN Supervisor for more than one and a half years and after I relocated down LA and worked at other facilities, I can now really tell the difference. The staffing is so much better at EJ with nurse patient ratio 1:24-25 as compared to 1:32-35 in other facilities. With the new DON there has been so much positive changes and I was so glad to be a part of this progressive team. The facility is spotless clean, has spacious rooms and high ceilings (which is almost impossible to find in any other nursing homes). The nursing staff communicated well and cared about their residents amidst their hefty work load which is by default if you are in a medical field. The therapy team is amazing and it's so satisfying to witness the residents recovering and returning home! It's the best nursing home in the area and I know for a fact it is only going uphill with its skilled services. Thanks to the new DON and the administrator who supports the growth!! read more

If you care at all about your loved one DO NOT put them in this horrible place. I had a bad feeling upon arrival, very unorganized and extremely rude. Madly who is a receptionist didn't bother to have my dads prescriptions sent to the pharmacy to have them there when he arrived from the hospital. This was only to be a short stay anyway and I made it shorter by taking him out. They only have one RN on staff. The rest are LVN's with little to no knowledge of how to care for anyone. My dad has hypoxia and when I told them I got the blank stare. I quickly figured out they didn't know what that was. I told them I was coming to get him that day. There is a male intake nurse who is extremely rude, while your telling your family members medical history he's acting like your bothering him, wasting his time. He simply said I don't have time for this? Really? They act like your an imposition. So I got my dad the next day. The social worker tried to arrange private transport for my dad to my home. She couldn't so she told 4 CNA's to help us get him in the car. She told us to pull around back and they would bring him back. After an hour of waiting, I went back in, apparently they lost my dads clothes that I had taken in the hour prior. They' were bathing and shaving him, and cut him on his chin while doing this, after I asked not to. It was 30 degrees out and very windy. I only asked them to get him dressed. When they finally found his clothes, they brought him out in the wheelchair, then proceeded to walk away. I asked where they were going they were supposed to help out him in the car, my dad at the moment is very weak, and is 6'7 so it's difficult to maneuver him with only 2 people while like this. They refused to help us, after their boss told them to. We ended up having to call the fire department to come, which was a good thing due to the firemen themselves telling me that it's the best thing for me to get him out of Ellison John. I knew I was doing the right thing, but it was confirmed by others who take people in and out of this awful place. This shouldn't happen to anyone, however my dad is a 75 year old veteran and didn't deserve to be left in the cold shaking, while " medical professionals" are refusing to help. If they weren't going to help they should have left him inside at least while we made other arrangements to transport him. They chemically restrain their patients. When you go there you can hear a pin drop. I know they gave my dad something due to his behavior. All they do is lie. They call you 6 times a day to tell you how wonderful your loved one is. They told me he ate two sandwiches, when he didn't have his dentures in. When I got him home he acted like he had not had anything to drink in months.... I could go on and on. I wouldn't let my dog stay here. This place shouldn't get one star. read more

Staff is very slow to respond to non emergencies like requests for maintenance or bringing medications on time. Devon is a skilled and attentive wound care specialist. Most nurses are kind and polite.... just slow probably due to understaffing and heavy patient load. Facility is nice and offers common space for meals and socializing with guests. If you're loved one stays in this facility be prepared to be their advocate on a regular basis or they will likely be forgotten and feel cast aside. read more

Awful. Incompetent doesn't even begin to describe this place. The facility is nice enough, and everyone is clearly working, but they're way under staffed and the left hand doesn't know what the right is doing. When my father was placed here for short term rehabilitation after a hospitalization I immediately checked Yelp to see what kind of a place it was. I was terrified after reading the reviews . I called Kaiser and begged for a new facility placement. They basically told me it was this or nothing and they they had to "pull strings" to get him in there. We hoped for the best and made sure the facility knew that he had a vigilant team of advocates watching out for him. We called and visited everyday. They still got his meds wrong, his strict diet was not followed (as once we watched, from his window- because of Covid- as they fed him French fries) sending his phosphorus through the roof, the staff BROKE his very expense CPAP machine forcing him to become oxygen dependent after his stay, and provided the biggest joke of PT (Physical therapy) imaginable. The staff is nice enough, but nice is insufficient when caring for someone who is recuperating from a serious illness. I wouldn't wish this place on my worst enemy. It's shocking that they are in business. read more

My mom had hip replacement and the night shift nurses left my mom in a wet bed for 2 hours even when she asked twice for help, she was choking and she used the button for nurses assistance and no one came till her roommate banged on the wall and used her button, she almost fell out of bed with one leg hanging over and again no one came till her roommate again called for help. It was horrible. The food was horrible and she and her roommate wouldn't eat their food. I had to bring in Ensure drinks so she would get weak and shared with her roommate. There were other things that I can't mention. If your looking for this kind of place DON'T come here! read more

This place is so bad especially the night shift mostly all the patient are asleep I had a roommate who was on hospice who treated so bad she waited hrs for her pain meds & to be changed there are some nurses that care the bad nurses I would love for them to live there love ones here so they can get treated the way we got treated here what goes around comes around I would give this place a 1/2 star. This place has been heaven sent! The staff too! Agnes, Anna, Kristine, Janice and the RTs I forget their names but Ernest is the main one I remember have been so wonderful to my mother. She's been here 4 days and I already see so much improvement in her. The staff is so compassionate and attentive. There is a strong core of dedicated staff who help make the best out of a large variety of challenging health issues. It wouldn't be honest to say everything is perfect, and it isn't, but there are wonderful staff there who see their role as more than just a job and it is reflected in the care and attention that they give to the patients. The physical therapists are dedicated and encouraging to the patients and show that they care. The same goes for the speech therapists. As always the best care you can expect for someone you love involves you making sure it happens. Family and friends are an integral part of a patient's recovery process. Complications should be expected, many are not due to the facility but due to insurance and Medicare complications. Shane, the administrator, makes himself available to listen, acknowledge and does what he can to make things better. He is personable and interacts with the patients and staff. read more
