We used the online only application. It went well until the required upload of driver's license and head shot. The Web page does not allow you to upload a jpeg. It makes you submit a picture from a camera in real time. That was not easy on a computer with a web cam. They rejected the photos causing a delay. We were able to submit the new pictures with a smart phone in reply to the text from them. They requested a wider shot on the head shot causing another delay. TIP: use a smart phone to complete the process. It will save time. The process was OK and should have been quicker than 4 days, but it worked They should fix the app to upload jpeg files and show examples of acceptable head shots. Otherwise good service and good price at $109 + tax and less than one week. TIP: print your own card. Organics dispensary refused to honor the digital card on our phone. read more

I signed up for the process to get a card, quickly learned just a little while later that my ability to own firearms would be effected If I got the card. Once they called me to simply start the process I told them I didn't want to go through with it and asked for a refund. The representative over the phone told me that It would not affect my ability to own a firearm (which is a blatant lie) I said I still didn't feel comfortable with it, and they said okay I would be receiving a refund. A few days go by with no refund, I try to call them back but their number goes straight to voicemail and is an automated system. There was about 8-12 chairs if I can recall and a small wall with a reception/office area to handle the Interaction. no smoke or major cleaning agent smells so comfortable breathing. To my surprise the employee at the counter was acting in my opinion a little rude to an old lady having complications setting up her portal. Kept telling the other patrons "don't be like ..... and change your portal info, otherwise you will be sitting here all day like ..... or we will just charge you $25" I was not going to get involved but wanted it to be noted. Doctors are amazing. Attentive and excellent at communication. I felt safe and trusted the doctors opinions and views and felt my information was safe. Cashing out payment process was a little chaotic to say the least. I was paying cash so I guess I was only paying 259 but if you pay with card it's a few extra dollars more. I was given a receipt and stood there for a few minutes. They had given me the receipt but I hadn't paid yet. Damn feels good to have some karma for the future.. I sent an email on the 9th business day just to see the status, (email reply received) was not submitted yet. They texted me saying i should expect my card within 5-7 days. Awesome. But for some reason I don't get any paperwork, nor my application ID. 5 days go by, I don't get any other communication. I text the number back and they tell me that things are delayed and it might take 7-10 days. Alright, no problem i totally get that. Its now over 10 days. I call(automated menu), get told to email, so I email them, no response 24hrs later. So i called DHS today, they tell me they JUST received my application and it may take an additional 7-10 days from today. Why does it take over 10 days to submit an application? And why did they lie to me in the beginning? People are going to dr reeferalz and getting their card the NEXT DAY. Literally so ridiculous and i will NEVER recommend going here. Why does processing take 4 days? I'm a paying customer. $99 per person should be enough to have a team that can process next day at minimum. Also, have fun when there's an error (which occurred for me) because there's NO PHONE NUMBER to call. All you can do is email their support team and never get answered. 4 stars because I got approved and got my card within the same week. I applied online. Got a text message saying I was approved and that I would get an email from AZDHS within 7-10 days. After getting an offer on a job, contingent on a clean drug test I discovered my MM card had expired, voiding my exception for testing positive. Panicked, I called around trying to get an appointment to see a doctor. Every one was a few days out, then it could take a couple of weeks to get my card... Found this place online, where I just had to answer few questions and they would have a doctor call me within 24hrs. I filled it out right before noon and received the doctor's call right around 3pm. The next morning around 11:00 I called in, bc I didn't see anything on my AZ portal. The guy that answered the phone said it could take up to 5 days. Bummer... But I looked today, lo my application was already approved and I had a printable version of my MM card! So it took less than 48 hours from the time I filled out the application to the time I received my MM card. Outstanding! I guess times can fluctuate, based on how busy they are and how busy the state is with processing the applications... but I had an amazing experience and would recommend this business 10/10. read more

I check my status of my application and its denied because the physician put the wrong birthday..... So the AZDHS contacted Marijuana evaluations for correction and they never got back to them so they voided my card and my application. I tried to contact Marijuana Evaluations so many times, no answer. The recording is just a run around, check the website and email. But i've been emailing non stop, no answer. 10 DAYS AND NO CARD!!! There is no communication, they do not answer the phone, I have asked since 04.30.2020 for a receipt, for my records. RESPONSE: wait 7-8 days for a receipt. 10 days later just got the receipt, after blowing up the phone with text messages. Still no response from the EM. There was no date on the receipt to verify when they filled. DO NOT WASTE YOUR TIME, TOO MANY OTHER PLACES TO GO IN TOWN. I think the next complaint will be to the FTC, for not providing receipts for the CC charges upon request. A complaint to the BBB for just having crappy customer service. I experienced the most horrible customer service ever. I was turned down at the door for not having my nutrition assistance acceptance letter, so I went all the way home to get it. Once I returned I gave it to the girl at the front desk. She told me I should be receiving my card within a week or two. It's now been 2 1/2 weeks still no card. So I called and she told me I never brought in my acceptance letter. I do not recommend this location to anybody due to the multiple mistakes that they continue to make.They need to take their jobs more seriously and realize this is Medicine that people need. This is not the first time they've done this either I've heard of multiple people I know that already have their card and got it here and they say the same thing. I will not be renewing my card with them I would rather pay a little more and make sure things come on time. read more
