My mother was discharged from RWJBarnabas Health and needed short-term rehabilitation, so our family toured Livingston Post-Acute Care. On the surface, the facility presented well with a clean appearance, beautiful rehab gym, pool, carpeted hallways, chandeliers, and a private room option. There was, however, a slight odor in the air during the tour that we tried not to focus on. We ultimately chose the facility because we were told my mother would receive three hours of therapy a day. Unfortunately, after getting her settled in, things quickly became concerning. To be fair, some members of the daytime staff were wonderful. FIVE STARS for nurse Shannon, occupational therapist Mary, physical therapist johnathan, the social worker, and several daytime aides (Jeanette D, AnnJohn, and Maxine) were kind, attentive, and professional. I truly appreciate them. The nighttime environment, however, was a completely different experience. Patients could frequently be heard screaming for help. Outside of the staff members I mentioned, many employees did not appear engaged or fully competent. Leadership was also not transparent, and when concerns were raised directly, communication became evasive instead of solution oriented. Our family was present from approximately 8:00 a.m. to 8:00 p.m. daily to make sure my mother's needs were being met, which turned out to be necessary. At one point, my mother had an outside medical appointment and required paperwork from the facility. We were given documents belonging to an entirely different patient, including that patient's photograph. The explanation felt careless and deeply concerning, especially given the seriousness of patient privacy and identification. There were also several instances where my mother was nearly given the wrong medication, and long periods where she waited extensively for assistance. The odor in the facility became significantly worse at times, particularly later in the day. Additionally, the therapy schedule did not match what was promised. Instead of receiving three hours of therapy daily, my mother was receiving roughly 40 minutes combined between services. This experience taught our family that appearances can be misleading. While some staff members genuinely cared and worked hard, there were too many operational, communication, and patient care concerns for us to feel comfortable recommending this facility without caution. I'm grateful we made it through the experience and thankful for the few staff members who truly supported my mother during her stay. read more

The lack of attentiveness and communication I have witnessed raises serious questions about oversight and patient safety. During a recent visit, a nurse informed me that she waits for patients to notify her if something is wrong. When I asked whether she routinely assesses her patients during rounds, she did not respond. This exchange was troubling and inconsistent with the level of proactive care that should be expected in a healthcare setting. Additionally, my mother's legs have been noticeably swollen for several days, yet no staff member took appropriate action or notified me of her condition. This represents a clear lapse in monitoring and follow-up care. Families entrust their loved ones to this facility with the expectation of compassionate, professional, and proactive care. Unfortunately, my experience thus far has fallen far short of those expectations. I will be contacting the facility director and corporate office to ensure these concerns are addressed and corrective action is taken. I have a compassionate heart for the staff here. I think they are doing their best. The Nurse practitioners and physicians were very kind. The bed and linens were clean, the bathroom was immaculate. BUT, the place looks dated, the furniture worn and dirty... the carpets old and the place smells bad ... I can't figure out out exactly why, maybe just bad air flow, it's just food, musty, body odor... and the staff just cannot care for all the people yelling help, crying out, needing care. There is no one to assist with meals or coordination of things: like when you get the patient out to the bathroom, help them wash up and make up the bed fresh for them. They just don't seem to get what it means to deliver good coordinated care. Do better, please! read more

Terrible place...receptionists and head nurses are unbelievably rude, CNAs try their very best but are understaffed and wildly overworked, furniture and rugs are dirty, equipment is either broken or doesn't function properly, It's dirty and smells. When my spouse, who is immobile without assistance, arrived, he was left alone sitting on a couch for two hours with no urinal or access to a call button. Bells go unanswered forever and when I went to the reception desk on the floor to asked for help, I was met with nasty attitudes. They left my spouse in bed for so long, he soiled himself. With PT only once a day, no one has had time to help him ambulate, which is absolutely essential to his recovery. I am having him transferred to another facility. The administrators keep trying to get me to change my mind and offer to try to make things better, but unless and until the owner decides to hire more staff, improve working conditions so that the staff doesn't feel resentful and overworked, and make major renovations to the place, there is no way to improve on my spouses experience. As one rude head nurse told me: "this is the level of care you get in a sub-acute facility (not true based on previous experience), if you want better care, hire a special." LET IT BE KNOWN, I NO WAY BLAME THE CNA's...they really do try their best and work themselves ragged with too many patients and no help. BTW...They give out gift cards for 5 star reviews--don't believe any of them. read more

Based on my experience, this facility was outstanding for its Rehab Department. I highly recommend this facility for Short Term Stays. You will receive professional PT seven days a week. You must follow their instructions fully. You will see improvement immediately. read more

DISGUSTING, DIRTY, EVERY SINGLE PERSON IS RUDE AND you can clearly tell they don't like there jobs. We had my Dad there and had to call Livingston police dept because in is in a totally incapacitated state he was left crying for three hours not one person answering his call bell. So we called Livingston PD for a wellness neglect check. I spoke to the main nurse a Nay Nay or something similar she was rude, had no compassion and kept egging me on. I said something she perceived to be threatening. Finally stormed down to my Dads room! Finally to find my brother, the officer and my Dad shaking. First thing she says to my father is "you daughter treten me. She filed a complaint against me with Livingston PD. I laughed!!! Next day my Dad was rushed back to Morristown Memorial with a horrible UTI, pneumonia and ECOLI!The! Nice police officer said in a warning honestly if I were you I would get your Dad out of here as soon as possible. SHAME ON THESE INSTITUTIONS FOR STAFFING WITH HEARTLESS GARBAGE!!!
