The level of care given to my mother by the staff at North Park Post Acute was sub-standard. The staff did not provide the necessary and required support care. Her medical needs were not monitored - although on her chart from her physician and daily requested by family members. She was not given baths/showers, she did not get assistance to dress in the mornings, she did not get assistance to get to the toilet (often took 45 mins+ to get a response from the call buzzer before a nurse/CNA came to her room). The overall care was significantly less than adequate. I discussed my concerns with the staff, but was told it was someone else's responsibility (day staff blamed night staff and night staff blamed the day staff). I even spoke with the nursing supervisor, while she was polite and professional, there were no changes to the level of care my mother received. After a few days, we transferred my mother to a rehab facility in Modesto - the care there is significantly better. I should note, the lobby and check-in desk were acceptable - the patient rooms are not maintained and are in serious need of cleaning (trash on the floor, dirty bathroom: blood from another patients in the bathroom - for multiple hours, that led to blood in the bathroom overnight), and the walls in the patient rooms were in need of repair (very bad patch jobs in large areas of the sheetrock/drywall). Bottomline: Do not send anyone you love or care about here!

The nurses, CNA's, PT's, and OT's all deserve 10+ stars. The team listened to me, and answered each one of my questions. They worked with me daily so that I would be able to go home, and be safe in my own house. I would recommend North Park to everyone. The care and compassion they showed to each and every patient was amazing. I have a few people I would like to give a shout out to. Nurses Kyle, Biddy, Alejandra, Joshua, Zaid, Jocelyn, and all the nurses that helped me during this challenging time. The CNA's Joshua, Gladis, Deana, Louisa, Jasvir, Kiwimder, Kamuii, Debora, Phony, Bell, and all of their co-workers that helped me. The PT/OT department was outstanding. They worked with me compassionately while challenging me during every visit. I'd like to give a shout out to Anne Marie. I hope that's how she spells her name. She was my OT that worked with me every visit, helping me to build up my strength and my upper body, and my balance for when I returned home. She was a kind, compassionate professional, who again treated me just like I was family. There is PT that worked with me most of my days, and her nickname was G. I hope I'm right about that. She had me challenge my range of motion, which helped me continue to improve with my treatment. Again, she was kind, compassionate and treated me like family. I would give this whole facility 500% if I could. There was all so a specialty nurse named Porsche who helped to care for my incision. She is well trained for dealing with wound care. She worked with me multiple times to make sure that my incision was healing properly, and being taken care of professionally. She explained everything to me that she was doing, and she made me feel at ease and comfortable with her work. I want her, thank her for her care, wisdom, compassion. If you are looking for a place for an Alzheimer's, patient to have a long-term care, this is…read moreprobably a very good solution. If you're looking for short-term skilled nursing, I'm not so sure. I brought my mom in here to recover from major surgery. They screwed up the medications for the first 2 to 3 days until I came in and sorted it out myself. And if you're doing this, just be aware, that if the hospital transfers a patient, it can take them a minimum of 12 hours to get the medication to their facility, but no one will tell you this. So as a result, my mother who is suffering from extreme pain due to major surgery, had to wait almost 12 hours for medication because once I realized what was going on, I had two traipse to her house and procure some myself. And even once I thought we sorted it out, they still kept forgetting to give her some of the meds she was supposed to have for her eyes. It was eminently frustrating. They treat everyone like a dementia patient whether they have dementia or not. This is a material problem for people who do not have dementia. If someone is trying to recover from something, it means they will not get the attention, exercise, etc. that they need to actually recover. They'll be told to stay in bed and not move. Not helpful. They are shortstaffed on skilled nurses for the facility. There's basically one per shift and that is the only person who is allowed to do much of anything other than very basic stuff. Another material problem in my point of view The facility itself is very nice, clean, with a variety of separate spaces that are all well maintained. The food was high-quality. There are enough people around that you can feel like at least someone is always on duty and attending to the needs of the residence. Everyone who works there is delightful. To be fair, they listen to my concerns and tried to rectify and respond in a timely manner. If this is considered one of the best of the best, I'm alarmed for what that means for my own old age. It was OK. Like I said, if you're using it for long-term Alzheimer's support it's probably fine.

The rehab department was excellent and gave 5 star service. My parents spoke very highly of Jorge. The administrator Bryan and DNS Chon were extremely helpful when concerns arose and even reached out on a weekend. Exemplary customer service. From the minute she arrived to the moment she discharged, extremely caring staff. I am so grateful that after my mother's stroke she has recovered at almost 90%. She would not be as recovered without the rehab and care the staff provided from Tracy Nursing and Rehab center.

They go fill an ice cooler that you would use to throw cans of drinks in on ice like you would use it for camping.... Shared with all 99 patients and staff ...so of course when they run out the patients drinks warm water all day or don't get served ice at all!! is that part of their state of the art facilities? I don't think so !! They don't even have an ice machine and I don't know what they're talking about ? as directed I marked all my husband's clothes. It's been nine days. He has not laundry done as promised. He had no clothes this morning and they pulled some clothes from somebody else or I should say probably lost and put on my husband. I was rather upset that he had to be put in some ghetto clothes as he called it when he has a nice labeled wardrobe down there enough for more than seven days and he does not have anything. He sat on his bed today with no clothes and a diaper waiting for something to wear as he was mentioning to me he was cold. I guess they do laundry when they feel like it never again will I let the administrators come out to a hospital all dressed up looking good and they put our elderly in these Medi-Cal Medicaid places just to take money from the government and pad their pockets. This is a utter disgrace. I am so thankful that this patient is only there for very short term. My heart goes out to the people that live in this with no real representation and this facility counts on that and I can guarantee you in the state that it is I would like to meet the people that are given at five stars because it must've been under different ownership. This is not that place as far as the accommodations it's fabricated. and the administrators go out hunting for people to put in there. They need the money and they misrepresent by the ad five stars. It's a joke. The only positive thing I is the girls that are actually working. The rooms hands-on with the patients are the only stars in that building. They are so understaffed and overworked it's deplorable.. girls work with kindness in their hearts as they do the best they can under these horrid working conditions. Bless their hearts!!! When her care was less critical I would say it was bordering on neglect . A cna said she was in charge of 15 patents and almost half had to be spoon fed including my mother. Hence, myself and Family made sure to be there so she could eat. My observations were patients who didn't have family step in for them might of not ate well. I heard one rn supervisor talk extremely rough to like a dementia patient to be still and stop moving. It appeared as work burn out. There were many times mom was soiled and staff would not come to help. I had to change mom myself and do best I can to get her to the restroom, changed and brought back to bed. They had no sense of urgency in helping despite telling me to not take mom alone. I waited but when it started approaching two hours I had to do it . I also Had to help cna put mom To bed as she had no help as mom was too weak and a two person job. They got mom's diet order wrong as moved up to solids and kept on doing a blender diet for days. We had to Go out and buy food so she can eat as she hated blended diets and I don't blame her. I was told this happened cause dieticians use a different system and it don't data share with the facility but nobody could make a phone call. There was like a comittee but I don't think they were proactive (except supervisor of nursing) as made excuses For some of my concerns . I am not going to say this facility is the worse but definitely not the best. I was told this place was the best in town. I am Just going to say it's like any other nursing facility from what I hear. When I thought mom was well enough to go home I demanded she leave immediately as they wanted to keep mom longer. I overheard admin mentioning mom was a "good" patient as had dual insurance. I told them at my house mom is not neglected and will get better care as I had all her bags packed. I don't recommend this facility for your loved one and it's definitely not the best in town but can't say the worse cause I haven't seen many . It missed my high expectations and them trying to say get better care cause they are small is garbage.

I will eat almost anything, but I tried my wife's food and spit it out. Every day you complete a form saying what you do NOT want...and that is exactly what they serve. Not once in over a month did they get it right! The rehab staff is nice and good when they work you. But they only work you a half hour; not an hour...or they talk to you the entire time and you get no exercise. I would give the facility a 2-star and the administrator and HR person a negative 5-star. Dom said he was in charge of several different facilities...so don't go to any of them where he exists.
