I was able to help for a few days after her discharge, but it was clear they would need additional support for her to remain safely at home. The hospital nurses were wonderful, but my mom needed assistance even with basic tasks like repositioning in bed and using the restroom, as her left arm was immobilized. Our hospital care coordinator connected us with Amy, the client care manager for Family Matters, who met us at home right away. She performed a thorough evaluation and took the time to understand my parents' needs and preferences.
