My 81-year-old father came here for a 5-week stay following an emergency out-of-state colostomy. He arrived in very poor health, and we were incredibly anxious about his complex wound care, ostomy management, and overall spirits. From day one, the team completely turned things around for us: The Leadership & Staff: Ashley and Crystal on the administrative side, and Heather, the head of nursing, are phenomenal. Along with the nursing staff, they met every hurdle with kindness, patience, and swift action. The employees are always smiling, which speaks volumes about the culture there. Expert Medical & Wound Care: Despite a very challenging stoma and wound, the medical team coordinated all his specialist appointments, resolved his ostomy bag issues, and managed his wound care so perfectly that he healed beautifully without needing a wound vac. They even took care of ordering all his ongoing supplies. Therapy & Rehabilitation: Thanks to top-notch physical and occupational therapy, my dad is now walking completely independent of his cane and is in great spirits. Facility & Environment: It's a very welcoming place. They offer great daily activities, a library, and resident outings. We loved the dedicated family areas where we could bring in food and eat together. We had initially read a few negative reviews, but our actual experience was flawless. We saw nothing but wonderful, compassionate care for every resident. I would highly recommend Palm Valley to anyone looking for rehabilitation for a loved one.

RUN FAR AWAY -- Our experience here ended with my father being rushed back to the hospital…read more I cannot stress this enough: I would never allow another family member to be cared for at Palm Valley Post Acute. When my father was discharged from the hospital, we were told he would have a private room. When he arrived, that did not happen. He was initially placed with a roommate who made him extremely uncomfortable and afraid to even sleep there. Eventually, they moved him into a private room, but what happened next was heartbreaking. Staff wheeled him into the room and seemingly just left him there. When we arrived, we found my father sitting alone in his wheelchair in the middle of the room, staring at the wall. No television was set up, no one appeared to be engaging with him, and no one seemed concerned with making sure he was comfortable. He was just sitting there by himself. And that was only the beginning. My father had a Foley catheter and nephrostomy tubes that required careful monitoring. There was visibly significant blood and clotting in his drainage bags and tubing. We have photographs documenting exactly what we saw. Despite our concerns, his lines were not flushed for approximately four days. One nurse, Claudia, finally took the situation seriously and flushed his lines. She discovered that one of them had become blocked. Claudia was amazing, and I am incredibly grateful for her. She recognized that something was wrong, acted when others had not, and in my opinion, she may very well have saved my dad's life. It got so bad that my dad had to be rushed by ambulance to the hospital. Thank God the nurse sent him to Banner Estrella, where they took amazing care of him. He was immediately evaluated and treated for an infection after days of problems with his lines at Palm Valley, including blood, clots, and a blockage that was finally discovered after his lines were flushed. What makes this even more upsetting is that I raised concerns from day one. I emailed my concerns directly to the unit nurse and clearly explained the issues we were already seeing with my dad's care. That email was ignored. I never received a response, no one followed up with me, and none of the concerns I raised were ever meaningfully addressed. Then my father fell in the bathroom while he was at Palm Valley. Our family was NEVER notified. We found out later during a FaceTime call with him. When we confronted the staff about it, they just laughed it off and showed zero seriousness about what had happened -- especially the kind of seriousness you would expect when caring for an elderly, medically fragile patient who had just fallen. And somehow, the communication got even worse. When Palm Valley ultimately sent my father to the emergency room by ambulance, nobody from the facility called us to let us know he was being transferred. Think about that. We found out that our father had been put into an ambulance and was being taken to the hospital because HE FACETIMED US FROM THE BACK OF THE AMBULANCE. Not a nurse. Not a supervisor. Not the facility. My dad had to call us himself from the ambulance. That is absolutely unacceptable. My father had recently been hospitalized with serious medical issues. He required nephrostomy and Foley care, monitoring for bleeding and obstruction, dialysis-related care, and close medical attention. This was not someone who could simply be wheeled into a room and occasionally checked on. What we witnessed was a frightening lack of communication, urgency, monitoring, and basic attention to a medically fragile patient. There were also conditions inside the facility itself that deeply concerned us. There were stains throughout the facility, and at one point we heard another patient repeatedly screaming, "I need water," yet no one appeared to go into the room to help them while we were there. Seeing and hearing things like that made us seriously question the level of attention patients were receiving. I understand that post-acute facilities care for medically complicated patients. I understand that complications can happen even when someone is receiving appropriate care. What I cannot accept is a patient falling without his family being notified, visibly bloody drainage and clots, lines going approximately four days without being flushed, a blockage being discovered only after one nurse finally took action, concerns emailed to the unit nurse being ignored, and my father being transported to an emergency room without anyone bothering to notify his family. Families place an enormous amount of trust in a facility when they hand over the care of someone they love. Palm Valley completely destroyed that trust for our family. If you have an elderly or medically fragile parent or loved one and are considering Palm Valley Post Acute, RUN FAR, FAR AWAY FROM THIS PLACE.

They took almost a week til they gave him a shower. The staff left him in the shower by himself for 30 mins. A cleaning lady found him by himself and had to contact the staff to assist him back to his room. My father in law actually got worse in the facility. The hospital told my sister in law to take him out and take him to the Banner Hospital. I definitely do not recommend Estrella. The staff is completely careless. It's an unprofessional facility.
