This is a very old facility and it looks it. My mother was there for 15 days. I feel she got decent care and the staff were all very kind. PT staff were great. But they are understaffed and she often had to wait quite a while for assistance. Bathrooms in patient rooms are terrible, way too small for walkers and wheelchairs. The outside needs some TLC. We chose it as it was the only place in Fallbrook contracted with Palomar Hospital. read more

After my car accident and 4 days in the hospital I was transferred to Fallbrook Skilled Nursing. I'd read other reviews and was skeptical but one review mentioned that the physical therapists were great and that was the reason I was going so chose them. The admissions woman, Janet, even called me beforehand to answer any questions I had. I was SO pleasantly surprised at how good all of the staff were! Every one of them I came in contact with was professional and had great attitudes. They are a bit understaffed but never let that issue control their day. There are wonderful people working there that let me know they were there to HELP me get better. The physical & occupational therapists were great. Special shoutout to Carmen, my CNA, who has a heart of gold and the room brightened every day she was at work. The food was also very good-the cooks do a great job! read more

My husband had been in my care, with some assistance from Hospice. When he suffered a bad stroke, I was unable to care for him any longer at home. The only bed available at the time was at Fallbrook Skilled Nursing. Living in Temecula, I was terrified at having him at Fallbrook at such a small facility. As it turned out, it was a blessing that he was admitted there. The staff was out of this world, caring for my husband as they did. Most of the staff were always ready with a caring hug for me, and always willing to lend an ear when I needed to talk. They went above and beyond in the care...............and always made sure that my husband was comfortable in his final days. Over the years, my husband has been in several Skilled Nursing Facilities, and although he received adequate care at these facilities, not one compared to the exceptional care he received at Fallbrook. Also, the love and compassion that was shown to me. I also observed many residents that appeared very happy to be there ! The facility is exceptionally clean !! My 91 year old mother went here for rehab after a serious fall. She was treated with care and compassion by the medical staff, rehab staff and the administration during her three week stay. I was with her every day for most of the day and saw how hard the staff worked to keep the patients comfortable and taken care of and to encourage them with their own cheerful attitudes. My mother suffered heart failure and passed away (through no fault of the staff) while in the facility and their response to the event couldn't have been faster or more diligent, and their response to me couldn't have been kinder or more comforting. For that I am very grateful. There is no getting around the fact that this is a nursing home, and therefore not a happy place. The patients are old and confused and the facility is fairly old and generically hospital like. However the gardens are a nice place to sit with your loved one in the evening. I hope it never happens, but if I needed to go to a Skilled Nursing Facility I would be happy to be taken care of by the staff of the Fallbrook SNF. read more

My 98 year old mom is receiving excellent care here. Have to think those who criticized this facility had little perspective and knee-jerk reactions. No medicine is given patients without the doctor's order which he will consult with you. (The MD is excellent.) End of life situations bring out the best and worst in people who are seeking help and sometimes miracles, and it seems they are looking for someone to blame. It won't be the people at this facility. Been down this road several times with elderly family members recently, the care here is top-notch. read more

My mother received exceptional care from many throughout her 90+ day stay. The only negative pertaining to this facility would be the night staff. One excessively large bahama mama is particularly nasty and is to be avoided if at all possible. I would recommend this facility for cognitive patients only and would highly advise nanny cams due to this unfortunate fact. read more

Worst place EVER. "If I can give this place zero stars, I would." I would NEVER recommend anyone to come to this facility. My father was here and was completely mistreated. He suffer a fall and no one called me nor my family. We didn't know until the next day we visited him, when asked to take him to a hospital, the nurse said it was not necessary. We did not like her answer, so when ended up taking him to the hospital and he suffered a concussion. I want to also mention something i have been observing. When you have "nurses" giving food to those who cant, make sure that you pay attention and do it with CARE. I watched patient be mistreated and nurse yell at them, while they all sit and talk about other patients in Spanish and are on there phone. This is so ridiculous. read more

In visiting all of the nursing homes in the valley, this one seems to be the best choice. The nursing turnover doesn't seem to be high. I have seen the same faces for several years. Most of them are caring and attentive to the needs of the patient. Staff has a regular routine of getting patients out of bed each morning into the activity room and after lunch back to their beds for a nap before dinner. The food I've observed and tasted myself is good. Activities could be more varied, not just BINGO. Activity Director could be more creative in developing new ways to occupy patient's days. Would be nice if soothing instrumental music could be played in the hallways. Like most nursing homes there seems to be not enough staff (CNAs) to quickly attend to the needs of all the patients. Mostly I've observed they do the best they can. The building smells clean although needs updating badly. They could do a better job cleaning the wheelchairs, beds, trays, etc. Many times it looks like caked on food left for weeks. There are 3 beds to a room in the long term care area. 1 or 2 beds in the rehab section. Yes, they provide water mugs for the patients at bedside but those that cannot reach for it have to rely on a staff member to actually GIVE THEM the water to their mouth. I only hope they are attentive to those needs when family members are not around. I never observed them turning patients unto their sides, they are continually on their backs. This concerns me due to the high possibility of developing pressure sores. If you need to speak with someone in Administration, good luck! No one returns your calls. I have more respect for the CNAs than for anyone in Administration! If you have a loved one in a nursing home, visit them frequently, check for pressure sores or any unusual marks or behavior. You are their advocate, ask questions and follow up. No one cares for your loved one more than you. Volunteer for an hour or two a week. The staff would appreciate it and you will gain more in return than if you spent that time on yourself. read more

Horrible horrible place! My mom was here for rehab and to get better after fallen sick due to her diabetes and losing muscle strength. They stopped giving her physical therapy and told us that she reached her functional gains, they weren't even trying. They instead kept her medicated and she was out of it every time we went to visit. The food is horrible(as most hospitals are) but they had options and never gave it to her. I had to bring in food for her as she was close to losing 80lbs there. While there, she got a horrible cough and when I asked why she still had it after a couple weeks, they shrugged it off saying they didn't know and there was a cough going around. It wasn't until she had to be sent to the ER for difficulty breathing only to find out she had pneumonia. Well as soon as she was better, she was alert and wasn't drugged up, so instead of sending her back here, we've had her transferred to a different facility and she's a different woman, shes receiving therapy and she's back to herself. Happy go lucky and not depressed. I swear they just medicate them and stick people in their beds. read more

I have taken a group of singers from my church to sing on a regular basis and the staff and patients are pleased for our doing so ..we are a volunteer group ...most of the people I know who bring music to them is on a volunteer basis. God this place is a hole. Run down in every way and they have absolutely no pride in anything about this establishment. There are written signs on the exit doors saying "no exit", every door and floor board is scuffed and dirty, and the TV's never work. You walk down the hall to a cacophony of ringing alarms that are never answered by the staff. If you care about your loved ones, find a different place, cause you'll be moving your family member out of this place as soon as you can. read more

My 91 year old grand mother was placed in this center yesterday by another family member. Concerned about the reviews I immediately drove to the center with my mother how is a retired RN of more than 30 years. My Grandmother was laying in bed with the sun shining directly on her. Her lips were dry and she was thirsty. We asked for something she could drink and after 15 minutes I had to go ask again. The trash can in the room and the bathroom was full of soiled articles. This morning she was rushed to the hospital as she was non responsive. Apparently she had been sedated. Why would any facility that provides medical care sedate a 91 year old woman with no mobility and limited speech. Pure incompetence and laziness that's why. For the love of God don't take you loved one here unless you don't care and want them to die. read more

This place is shady. I performed music for them and they avoided paying me for five weeks (still no check) with every excuse. I was supposed to be paid the date of service. When I tried to collect on the bad debt I was told my services would no longer be needed in the future - as of I'd ever go back there to sing for free! HA! They added insult to injury! If they treat occasional vendors this way, how do they treat their feeble, vulnerable senior residents? And where is all the money going???? read more

Health care becomes more difficult with Insurance companies dictating to the professionals what can and cannot be done and recently with Obama Care this just became much, much worse! Despite these challenges placed on facilities like Fallbrook Skilled Nursing Facility they exceeded our expectations in every department! The food is flavorful and always presented in a beautiful manner. Fresh flowers and tablecloths with place mats. Dining room decorated to coincide with the particular season, Holiday or special event! Physical Therapy was amazing and very, very caring. Genuinely got to know who they were dealing with to understand the psychology behind each person and what motivates them. Nurses calling us personally to make us aware of any changes in medication, or physical status. One time I forgot to sign up for the Hair Stylist and the front Receptionist put my Mom on the list and had them bill us for that service on the next months bill! No other nursing home would have anyone who would go to that length since she knew we wanted Mom's hair done that week for a special event. This level of care requires staff going way beyond their jobs but rather becoming personally familiar with their clients and their clients family members! Many times it was difficult because of course I did not want to see that Mom was declining. I wanted to see that she was going to be getting better and walking more not less and in denial some times over what was happening and going to be happening. Again despite all these emotions of having Mom there the Fallbrook Skilled Nursing Home staff was kind, understanding, fed her when we could not be there, kept her skin free of bed sores and changed her often enough to keep her healthy and free from Urinary tract infections. The entire Staff went beyond, I see them going beyond for many, many other patients as well. When Mom was in her final 2 days they had a large private room so they transferred us all there so we had as much privacy as they could give us at the time. The special Barbecue, Antique Car show, Petting Zoo & DJ had at least one of these amazing 90+ yr olds up dancing!!!!!! Fallbrook Skilled Nursing Home works very hard to achieve that 5 star rating they have earned! The Holiday Family meals with photographs were really, really great food too! Decorations, flowers, hours of work gone into all these special functions. I have never had these special functions with any other Nursing Home. Entertainers coming to sing, play piano, all kinds of activities everyone works so hard to bring life to the residents. I could write many more specific examples of how much care the staff takes too. People came up to us with genuine tears in their eyes over the passing of our Mom. It is not a nameless, faceless environment and we were lucky, extremely lucky to have had both Mom & Dad there off and on recouping for various falls and issues. Based on the reviews I was not expecting much. This was the only facility out of 7 choices with an available bed for a man (most rooms at these facilities are shared and they can't mix genders and most patients are female). The day my Dad was transferred here they were having their annual vintage car show and BBQ, complete with petting zoo, sno-cones, face painting, and live music. One thing is for sure, these people know how to have fun! The staff are extremely caring and kind. The nurses are attentive and do regular rounds; Ryan and Anna are particularly helpful and responsive. The physical therapists are professional and are obviously concerned about their patients progress. A few things that surprised me: the cook came into my Dad's room the first day and said, "hey, I know you're a vegetarian, so what do you want to eat, I'll cook you anything". The food smells yummy and my Dad has few complaints. The other night he and his roommate wanted some sherbet, sure enough the kitchen brought them some. Also, every day the CNAs bring everyone a big travel cup of water with ice and they come around regularly to fill up. There's lemonade, coffee, and tea always available, plus graham crackers, cookies (sugar free and regular), applesauce, and yogurt usually available at the nurses station. My only complaints are the security might be a little tighter, although they say they've never had a problem (probably because they are well staffed and on the look-out), the social worker has never called me back (I think she must be very busy so when I need her I go there in person), and there is too much beeping from the alert systems they have in place when someone gets out of their wheelchair, off their bed, or in the wrong location. You do get used to it, but at first it is pretty distracting. Overall, I am very pleased and my Dad is very happy here. They have him back on his feet, using the restroom himself, shaving, etc. He is expecting to be discharged this week after a 2-1/2 week stay. read more

About a year ago, we were able to have my mother admitted to the Fallbrook Skilled Nursing facility as a resident. After caring for her at home for 8 years, we were no longer able to provide her with the level of care she required. After a years-long battle with Parkinson's disease, she succumbed to her illness a few days ago. The staff and management there were just wonderful supporting us in every possible way as her health declined. We will always be grateful for that. During the year that she was there, we were amazed at how much effort is placed into making the residents feel at home. Each month, their schedule includes visits by musicians, clergy and others. The staff is very attentive to the needs of each resident. During the years leading up to her stay at FSNF, my mother spent shorter periods of time at other San Diego county area nursing centers following surgery and other medical treatments at hospitals. We must say that, compared to some of these other facilities, FSNF is kept sparkling clean and does not feel out-of-date. We spent many pleasant afternoons in their beautiful garden area with Mom as she just loved to enjoy the flowers and trees. On holidays, the management and staff invite family members to join their loved ones and other residents for festive luncheons. We really enjoyed participating in those events. Just last weekend, they hosted a classic car show and picnic at the facility. We could see how much the residents enjoyed the event. I want to reiterate how thankful we are for all the personalized care and attention they provided my mother over the last year. It was comforting to see how management and staff members were personally dedicated to the well-being of each resident. An example of this special care by every department there was purchasing an eye glass holder so Mom's glasses were not always at the tip of her nose. Finding extra clothes when I wasn't able to bring clean ones right away. People acknowledging her by name as they walked by and brushed her arm to see how she was. read more
