I'm giving 4 stars because there is always room for improvement. My mom stayed here for over a month after her back surgery. Most of the nurses I have met are very nice and considerate, considering my mom can't see. Thank you. The PT/rehab team who worked with mom is amazing!! Really impressed with Downey Post Acute. I had my father there and he was very well taken care of. The room was clean. The staff was very helpful and the case manager was excellent. I want to start by saying how great full iam with this place and how much the staff has done for my grandma in particular the social worker Ms. Evelin Hernandez and the director of nursing Ms. Maria Wilson they booth have made this process really easy for my family and have answered all the questions that my family has had it's hard accepting that your family member can no longer be back to how they were before i admire you booth im forever in dept. I am a patient and I have been without pain medication for 24 hours awaiting Doctors signature. Pharmacy is waiting, nursing staff is waiting. I can not attend physical therapy because I can not tolerate the pain and the pain is extremely worse after therapy. I just want to go home and be strong enough to climb the 20 step staircase. How can I receive my medicine so I can attend therapy so I can go home. read more

I filed a formal grievance against this facility. My husband went in fro rehabilitation after a car crash that wasn't even his fault. He has two broken legs and a broken hand. Their site promotes they will never forget that you're their valued guest: NOT TRUE. That each day you spend with them it will be filled with the caliber of service that will make your stay comfortable, safe and therapeutic: LIES. From day one when ambulance people arrived to drop of my husband thank God I was there I saw the tiny little corner where there was this filthy bed where they where planning on having my husband lay down. I stopped them and I show them how dirty it was and that there was even hairs on pillow and all over the bed, and a trash can fill with spit and discarded food a tiny side table with drawers had dirty cups and used toothbrush.. trash under the bed on floor the whole place smelled like poop and urine (not only in the bedroom but the whole place). I asked for clean linens and had to changed them myself. I took plastic bag and got all the trash out.. I finally had my husband lay on the bed and cover him with blankets we had purchased before (not the facilities) while I was putting bag filled with trash outside the room I got back and saw a old man pulling hard on my husbands blankets saying "THIS ARE MINE!" my husband could had gotten hurt he has outside wires coming out his legs. The guy try that two times I told the main Nurse Michelle and she thought it was funny. The receiver RN Tam help me move my husband to another room. I stepped out and was leaving after 3 hrs trying to calm my husband and telling him that everything would be okay I saw Michelle at the reception desk she was on a personal call crying you could see how red her face was and unstop tears coming down her face. I didn't know what to say.. so I said Im leaving Michelle..I got close and hugged her I assume she need it. I left.. who would guess she would be his miscreant nurse who would abuse and mistreat him. She would lie and say she had witnesses to his bad behavior blaming it all on the patient saying things like "His acting erratic, I can kick him out of this place watch me..." to protect her self. I had to call Downey Police for a well being check up because they never answer after 3pm . I would leave messages for Josephine the case manager and to anybody and they do not return calls. The DON (Director of Nurses) Mr Santos is also in it and will defend their nurses lies his motto this is what he told me "There are 3 stories to everything, Yours, Michelle's and the truth." My husbands got infected wound and blood works detected something wrong while he was there . He was there to get well and they weren't even ready for him CERO MEDICINE (he needed an IV with antibiotic so his wounds wouldn't get infected, they did). Thank God I keep a log with names and times. .... There is much more I reported I wont say here but I am reserving the rest of the detailed facts, for when they start the investigation due to the opening a the formal grievance. I feel responsable to take this much further. I don't believe they where targeting only my husband, I believe this is how they treat anybody (I saw elderly mistreatment). I hope this place gets investigated and maybe RN Michelle physiologically reevaluated. FYI Those pictures they display on their website do not depict the real place. The rooms where they keep the patients do not have nice curtains or pictures on wall or flowers with nice lighting. Its is al the oposite. Old smelly and grim. If you can take your family member home for In-home Care. Do so. read more

Worst place ever wouldn't recommend if your family member had a stroke and needed rehabilitation. Nurses are rude and ignore you, worst thing is they want you out as soon as possible. Meaning if you are getting better but still need help and fall a lot they don't care they stress you out and that's bad for someone who just got moved for having a stroke and they cause another one read more

This nursing home has the best case manager Jocelyn, I've ever met, gets things done on time.... my mom went few other places, but case manager so slow in solving patients needs....she has the heart of love for her patients, the nurses, CNA are really nice...so many good things I want to say about this place, you should find out yourself.... read more

Okay I read the reviews and thought it might be an okay place for my dads rehabilitation before coming home but this place is hell on earth. My dad is in his 50s... instead of making themselves available to take him use the restroom they slapped a diaper on him and don't come to him when he calls for about 3-4 hours later. Last night his room mate died because no one would respond to his multiple calls! His room mate choked on his own vomit because no one ever showed up. I'm filing a complaint with this place this is absolutely unacceptable. read more

This is the worst place to put someone you love. My family and i learned the hard way. The lack of empathy, the absence of basic human decency, and disappointment this facility and faculty have shown is despicable. There are many elderly people that need a little more help that ones on family can give, and when you as a love one place them in the care of others, OTHERS who have chosen this job, why? If not to care for someone's grandpa or grandma, dad or mom!!!! Calling this center to get information to get an update is close to impossible; you're either on hold for 10-20 min or it just ringing until the line hangs up. Yet the times I've been in the facility, you see not one but two or three scrub wearing employees either on their personal phone, or having conversations amongst themselves. I do know that these employees go through little to no training but what decent human being would call a patients loved one, upon the time of their death and notify them, that their loved one has expired. Expired as if that was not a person, as if it was a gallon of milk in the fridge. Jasmine, Vanessa, Megan, Jason you've all contributed to elderly neglect and you can't reevaluate why you're in the occupation you've chosen. Well then i hope your grandparents or parents are never put in assisted living centers and if they are, i hope with sincerity they never come in contact with staff members like this facility hires. read more

I dont know where to start this place is awful.... only a few decent staff members. i went in every day and could really observe. i wanted to see my loved one, but every day i visited was hit with money this money that ins this ins that, .evening hours was like a joke could never find any workers anywhere. there are so many negative things about this place. their is not enough room to write it all. do yourself a favor and dont bring your loved one here.....o yes the only reason for the one star, was that i like stars.. sorry the food is just plain horrible. read more

My name is Magdalina kinchen,I'm here in this facility in Downey,I thought when I got here I would be okay,I've broken my leg 3times within the year,so I needed rehab for my leg.the First couple of hours was okay and one or two nurses were on their jobAfter that day I was talked o verbally real bad,humiliated,I needed assistance using the bathroom and getting cleaned up,the nurse that was on duty was very mean ,lazy,and very rude to me,she told me when I ask her to please clean me again because I could still feel the BM all inside and around my back and bottom,she replied to me that she did and what did I want her to do ," stick my finger in your butt" ,is what she replied to me more than once,I became very angry and ask for some gloves which she refused to give me,I then reached for some wipes I had in my purse and wiped my bottom in which so much BM was all over the place,I started to cry out of anger,and continued to wipe and clean myself as best I could,she stood there and watched me cry and clean myself insisting that she cleaned me and wasn't about to stick her finger in my butt!!!! I called my daughter and my sister who works for Sherriffs,I told her what was said and how I was treated,my daughter Nacole became outraged and called up here and was complaining to a woman who said she was the supervisor,her name is Claudia,she was rude to my daughter and I heard her tell the receptionist and other staff not to answer the phone ,that she was gonna keep calling.Just yesterday I received a call from my sister Dorthy Contreras and she told me all my medicine was ready at the Rite Aide in Downey and she would go and pick it up and bring it to me,I replied okay,DIDNT see any reason why I couldn't have my meds so when I go home I have them,but it's no big deal my sister could hold on to them until I go home from here,so my sister dropped my clothes and medicine and a vapor pen off to the facility,Claudia and a young lady named Ashley went thru my things,I've had several things brought here and the staff never went thru my things,I'm sure they did it this time wanting to see inside my bag cause I'm always sending my credit card to my son nicholas Weston who comes and brings me my mail and food,so I really believe they went thru my things to see what they could get or take,it happens a lot to patients, nurses stealing their things and money and being taken advantage of,anyway I kept calling for them to bring me the things my sister brought up here,my sister didn't leave the facility until they were gonna bring me my clothes and meds,but they didn't bring me my things,it was an 1hour and 25 min before they brought me my things,after I kept yelling for my things and my daughter and sister kept calling,Claudia came and told me that she has to go thru my stuff and she would bring my bag in and go thru it in front of me,well she came in with my bag and it was open and untied,she and Ashley had already went thru my stuff,I started to go thru my things and I pulled everything out and my medicine was inside my pair of socks,my sister said they had to have done that cause she didn't,and why would she,it's my medicine with my name on it and the vapor was out just in the bag,nothing to hide,I'm not in jail,I'm getting therapy,not a prisoner.she grabbed my socks and took my meds and the vapor,which made me look at her like she was treating me like a prisoner,she then said I'll have to keep them until you go home,I said "no my sister will come get my things,she then looked kind of weird at me and the girl Ashley,she then told a nurse named Chris to count my meds,he did,he said "MagdaLina there is 26 pills inside,I said ok give them to my sister,he said okay,he left out and I started to think about the quantity,I usually get 60 every month and now I'm confused so I called the pharmacy rite aide along with my sister and daughter and she explained to me that she just filled that prescription less than 2hours ago and there should be 60 meds inside,I immediately called that nurse Chris back inside my room and ask him to count my pills,there was only 26,which is impossible,they filled that prescription of 60 meds,now these are pain meds,Oxycodone 10 mg,. Now I'm furious they stole my pills and they took 1hr and 25 min to bring me my stuff,you tell me and the way she snatched my socks away from me really made me feel some type away,I can't believe they did this to me,I'm 52 yrs old with chronic pain that sometimes never lets up,I don't know why these staff members would steal from innocent people I've done nothing to nobody,the nurse Chris left to go home,but he came back to check on me and to sign a paper stating he counted 26 pills when he counted them,he also seemed to really be concerned about how I was treated and how my sister and daughter were treated,I told my mom what happened,my mother is a nurse of over 40 yrs,she then became office manager of Woodruff care home and the chateau in Long Beach and bellflower,im very angry. read more

I was a patient. This is my experience. I was taken here to recover and take therapy. I got sick here. RN are mean. Maintenance guy and other help are nice. Place smells like urine and human waste. Everything is old and broken (blinds, paint wall chipping, etc) I got kicked out because my wife caught the Nurse in charge in a lie and mistreatment. Nurse felt threaten and manage to kick me out in the middle of the night with no discharge papers or reason why. Thank God for my wife and attorney they acted quick in the middle of the night and I was taken to a private hospital where they caught a infection on time of one of my wounds and treat me for other things that came upon do to mal care at this facility. I was there for almost a week and had to wipe myself after valve movement if the nurses wont come if you call them and if they do they are not trained they literately don't know what they are doing or what are the medicine for that they are giving you. I am now in In-Home Care and my wife help me file a Formal Grievance with the State Health Board. IF YOU LOVE YOUR HUSBAND, PARENT OR GRANDPARENT DO NOT LEAVE THEM HERE. read more

The facility has new ownership and I've seen a lot of good changes taking place. They've added experienced nurses, and I've seen patients get better and stronger from the Therapy department. Take a look for yourself and you'll see that your family member will be taken care of at Downey. read more

It's taken me a year and a half to write a review on this place. All the other bad reviews are spot on with their complaints so please take them into consideration. My mom was placed here a couple years ago, not by choice, but it's what was decided at the time by another family member. And let me just say it's a shit hole of a place to be and I wish I would have gotten her out of there. Till this day it's my biggest regret. She went in there to recover and a year later she died in there. I slowly watched her die for a year and she was so badly mis treated and taken care of in there. Rooms were over packed, not enough staff, lack of care, and DIRTY. Not only was she getting sick every other month and ending up in Icu with septic shock due it a uti or pneumonia, but her teeth were falling out and she ended up on a feeding tube and catheter. A vegetable let's just say. My mom passed at the age of 55, her last couple days on hospice were shared in a room with 2 other people. Yes, not even her own room to peacefully die in. I remember asking the staff when she was being transferred back from the hospital if my mother could be placed in her own room to pass in, she only had a day or two to go and they told me no. So not even 24 hours my mom lasted there on hospice. I highly recommend not bringing your loved ones here. Stay away from this place, that's all I can say. read more

Needs a better phone system, it is nearly impossible to reach a live person when transferred to a department through prompt system. It is very difficult to communicate regarding patients. read more
