    Formal Complaint and Request for Investigation Regarding Assessment Conduct and Accuracy…read more I am submitting this formal complaint regarding the conduct of Armando Salazar, who conducted my assessment. I believe the manner in which I was treated and the way information was documented during my assessment warrant a serious and thorough investigation. Throughout the assessment, Mr. Salazar's conduct made me feel uncomfortable, embarrassed, and disrespected. After I answered several questions, he laughed and mocked me. I found this behavior highly inappropriate and unprofessional, particularly in the context of an assessment where I should have been treated with dignity, impartiality, and respect. I am also extremely concerned about the accuracy and integrity of the information recorded during my assessment. While the assessment was taking place, I observed Mr. Salazar entering responses into his computer that did not accurately reflect the answers I had provided. To the best of my knowledge, certain answers were recorded incorrectly and did not represent what I actually stated. This concern is extremely serious because inaccurate or misleading information in an official assessment record could potentially affect decisions made about me and may have significant consequences. I expect any assessment conducted on my behalf to accurately document my responses and to be completed honestly, objectively, and professionally. I am therefore requesting a formal investigation into this matter. Specifically, I request that: My complete assessment and all information documented by Mr. Salazar be reviewed for accuracy. Any available notes, records, recordings, or other documentation associated with my assessment be preserved and reviewed. The answers recorded in my assessment be compared with the answers I actually provided. Any inaccurate information identified in my assessment record be corrected promptly. My complaint be formally documented and reviewed by an appropriate supervisor or independent party. I be provided with a written response explaining the outcome of the investigation and any corrective action taken. I want to make clear that my concern is not simply about Mr. Salazar's demeanor. I am raising a serious concern regarding both the way I was treated and the accuracy of information entered into what I understand to be an official assessment record. I respectfully request that this complaint be treated seriously and that no adverse decisions concerning me be based on information that is inaccurate, incomplete, or does not reflect my actual responses. I am asking for transparency, accountability, and an accurate record of what occurred during my assessment. I look forward to receiving confirmation that this complaint has been received and information regarding the steps that will be taken to investigate and resolve these concerns. Robert montuy

Since dealing with…read moreClinica La Familia in Phoenix and having a great experience with staff and doctors ( mostly PA), I moved to Tucson 3 years ago. The closest Clinica La Familia office is in Casa Grande, 1 1/2 hour away. But the staff is super friendly. Front desk, helpers?, nurses, PA. My assigned PA is amazing and I am so grateful for the care I get. YES, waiting time can be way off. I had a 11:20 AM appointment, showed up 20 minutes early, was brought in 1 1/2 hour later and had to wait for another 20 minutes for the PA to show up. Later, for my blood work, the young lady was super new and she kept checking my forearm. I am one of these guys my veins are huge and pulsing in my forearms. But she did super well and I didn't feel anything. Less than 24 hours later, I got the results from the blood and urine tests. The office in Casa Grande is 1 1/2 hour away from my place. Yes, It is going to take some time there in waiting. But overall, It is so worth it for the care I actually get.

I was given false information regarding my insurance coverage which prompted me to pay out-of-pocket for the office visit. After waiting almost 2 hours for "insurance verification" which should have only taken 5 minutes max, I paid the $90 dollar consultation plus the treatment which they didn't resolve (that's another topic). This office later went and double billed my insurance by submitting a claim almost 2 weeks after my visit. This is medical fraud. Their demographic clientele is Hispanic people, I can only imagine how they take advantage of these people in need of medical services. If you visit this place, inform yourself on what they can and can't do.
