As a precaution for a man in his 70's he recommended a DRE which I agreed was prudent based on our discussion. He found a large palpable mass on my prostate and recommended I have a biopsy which his office schedule the next week. The mass was malignant in 6 of 8 tissue samples taken. At the visit where we reviewed the biopsies Dr. O'Connor patiently went over the finding and laid out my alternatives in a clear time related way. I chose surgery and he said his colleague, Dr. Christine Tran, was one of the most talented surgeons to come out of the Cleveland Clinic. Dr Tran removed my prostate 3 weeks later with almost no consequential impacts. I have had none of the common issues with continence or normal function. These two remarkable specialist along with my GP, Dr. Nagell, saved my life. The post surgery biopsies had the magic negative margins descriptor while noting the cancer was graded an 8 on the Gleason scale. In plain English it was an aggressive cancer that had not metastasized and it was completely removed with no impacts to the normal functioning of my biology. I heartily recommend them all and a special thanks to Dr. Tran who is one hell of a skilled surgeon.

I waited over two years to post this review as I know I can't change my decision, but perhaps I can help others learn from my research. I had a vasectomy done in late 2018 and months later started developing Post Vasectomy Pain Syndrome, a condition I was not even aware existed until after the procedure was done. At first these guys were great, they ordered ultrasounds, made time to hear my concerns but once the pain continued and no obvious answers were found, I was referred out for pain management and that was that. Two years later am still experiencing ongoing daily pain that will likely be lifelong despite a reversal earlier this year. I was willing to pay the premium price tag for the individualized attention of the practice, but if you are part of the 15% of men who develop any longterm complications from the procedure--you are on your own because they'll act like PVPS doesn't exist. Welcome to Hell. PVPS affects upward of 15% of men according to a 2016 study. (https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4854072/) The consequences are lifelong and very difficult to pinpoint. Their consent form is a laughably thin single page document which cites the possibility of some post-operative pain and complication, but PVPS and lifelong pain is mentioned exactly zero times. These guys are great salesmen. You walk in thinking, "It's simple, quick, effective, and has no impact on anything other than fertility and this is white glove service. False. Nowhere does it mention the well documented possibility of PVPS which has been known for decades. Had I been presented with true outcome risks and a true understanding of the longterm impact I would have walked out the door, and you should too. As Warren Buffett famously said, "Never ask a barber if you need a haircut" and never let a urologist sell you on a vasectomy.

First referred by my Primary Care in Fairfax, I continue being a patient even though travelling from Ashburn to Fairfax isn't easy. However the staff is great and Dr Patel in particular makes himself very available to patients either in-person or via e-mail. I find him very attentive to my concerns and as an overseas traveler, he ensures I have what I need in my trips. He also ensures follow-up as necessary with labs and takes the time to explain the findings. He and his staff are prompt regarding prescriptions and in the past were also very helpful getting insurance approvals for certain medications. A long drive but worth the attention patients receive.
