Review on food that is provided to patients. This is the nighttime meal. This is more than sad. Nutritional value questionable. Less than half a cup of beans. Particularly unappealing. My wife was in Care one at Concord for a back issue and some rehab. They almost killed her. I removed her by ambulance to NWH which I am positive saved her life. Same issues as this place. No reponse when ringing for the aides. leaving patients insoiled clothing longer than 24 hours. She was there for rehab I had to pay for 14 days up front and she was there for 7 days and they did ONE day of rehab and then chanrged medicare for 6. If anyone recommends any local Care One Facility RUN! I then had to chase them for three months for my refund of 6 days read more

My grandparent were both sent here for rehab and they are both being removed! my grandmother has had her clothing go "missing", she has been forced to spend all night soiled because she called for help to use the bathroom and NO ONE EVER CAME and she was left like that ALL NIGHT!! As well as my grandfather!! He has been left with a blood pressure of 88, his feet are swelling, he has had so much pain and discomfort in his stomach after eating and they DID NOTHING!! he has finally been brought to the hospital to get ACTUAL CARE! and my grandmother will removed and brought home to live in HER house with someone that ACTUALLY cares about her and her well being!! i would never in a million years ever suggest even my worst enemy be placed in this terrible prison!! I had called 4 different times to get in touch with her nurse or whoever runs that nightmare and every single time i was sent to answering machines and never got a call back! i will be gladly driving 830 miles to get to her and to get her out of there!!! i hope this place gets shut down before someone loses their life from their neglect!!! read more

Made a reservation for my mother to stay for a week. Was told to call a month in advance to confirm reservations and book a tour. Called Devin the admissions director for 2 weeks straight M-F at least 2 a day as directed by the front office. Left multiple voice mails,messages ,emails and texted his cellphone. Has not once returned one message. Last call I made I was told he was in his office they transferred me and went right to his voicemail. Very unprofessional!! read more

My grandmother was not dead five minutes at 10 pm at night and they asked us to clean out the room right then. DISGUSTING. This was the end to the "care" received here. Earlier that day, I arrived at 11 am to visit her. I found her slumped, crumpled in a ball in a wheelchair, with her head over her knee, with a soaked drool rag on her lap. I lost my mind. She was actively dying and they just left her like that. I know the activity staff loved her very much. I thank them for their love for her. She fell more than once while literally in the actual hands of nursing staff. Her insurance didn't allow us to transfer her out of Care One. We tried. We met with admin to ask for communications regarding her care. Melissa the Director of Nursing professed she'd be available and "call anytime" and "we hear you, and we'll be better." She never returned a single call. Literally the week after the meeting asking for better communications, they put my grandmother on a puréed diet and didn't even let us know. She did not recover from the last fall. I thank the people who gave her the care she deserved. Every human deserves good care. Sadly, this place needs to see that. My grandmother was a beautiful, giving soul, even with Alzheimer's . She didn't deserve what she experienced. No one would. And to ask us to clean out the room lesfive minutes after her death at 10 pm at night....Shame on you. And now they are asking to contact me? They have my number --it is in my grandmother's main paperwork as is my mother's number and I left it on the administrator's voice mail and the DON's voice mail countless times. This place is awful. Utter incompetence at every level. My mother is being cared for here, and the nurses take forever to administer any care and response times are upwards of 15-30 minutes when they are buzzed. Concerns and recommendations are ignored. Even the janitorial services like replacing toilet paper rolls and paper towels in bathrooms are overlooked. Mind you, these patients are barely mobile, how are they supposed to reload toilet paper and paper towels? read more
