Staff is dismissive and unwelcoming. They are not patient or family friendly. My loved was admitted and RN on duty at approximately 7-8pm was rude. Entering the facility for the first time myself and spouse of admitted patient were past by at least 3 staff members who did not acknowledge or greet with basic hello and entering the room the RN didn't say a word and walked out of the room of which I had to go to desk just to get information on what next steps of admittance was. Not place I would recommend but unfortunately there are not many options. read more

It's funny when I see reviews that say the nurses and staff ignore patients call lights...what they don't understand is after a staff member will adhere to the call light the patients sometimes have behavioral problems and turn the light back on the second the staff leaves the room. read more

This place is a nightmare from day one! The staff is beyond incompitent! The nurses are horribly rude and lazy. Sat there the 1st night for 3 hours before anyone would even come and access my mom and give her a blanket. No water was available. The RN Supervisor who has no business being an RN SOMONE SHOULD CHECK HER CREDENTIALS, was useless and had a major attitude, because she was too busy to check my mom in and that we would have to wait. Meanwhile 3 hours passed and she finally came in to say someone would be by to take care of my mom. Well the next 2 weeks didnt improve. The place is a joke. They gave my mom medication that they shouldnt have. When I asked to talk to the nurse about meds, Nurse Jazmine got extrememly upset because I was questioning her. Long story short, I took my mom out after 2 weeks because they moved her into another room with NO T.V. left to look at the ceiling 24 hours a day. that towards the back of the building and minimal care was being given. The food looked like vomit. Literally. It was the same food everyday. The physical therapy was 15 minutes a day if youre lucky. After speaking with management, administrators, and someone who claimed to be the director of nursing but find out she wasnt I had enough of the B.S. Lies and lack of everything. The Dr. Hage perscribed my mother a prescription that IS NOT to be taken with her other meds! This place looks nice from the outside and once you see whats really going on that they are not staffed with the right kind of people with actual credentials and the place is dirty! The back looks like a boneyard of old broken down beds and wheelchairs, landscaping is all dry. As all other reviews, if you care about your family member PLEASE DO NOT PUT THEM IN THIS PLACE!!!!!!!!!!!!!!!! ITS HORRIBLE. read more

If you love your family please never take them here. I still think about how horrible my grandma got here. Especially under the care of a family member who didn't care to be there nor really help her. My moms sisters side of the family claimed she was brain dead(they just gave up) but we've had proof of her texting and writing. Hopefully they read this one day bc they are just as guilty as asistencia. My side of the family were very vocal in demanding help and being there everyday to make sure she was getting proper care. The staff member at the time Melissa(white woman) would purposely wipe incorrectly even after being confronted. She was also confronted for speaking about our family for demanding better care. She mocked us. My grandma developed a horrible rash and pain. My grandmother was becoming more and more stable. That was the point and there were days and weeks her PT wouldn't come. She had rolled out of bed various times (they NEVER answered patient buttons) so I can't image what help my grandma needed that they neglected. They grew tired of us complaining so they pinned her bed against the wall by the window. Farther away from being heard or helped. She developed pneumonia being close to the window aside from the lack of care. The only nurse who helped as much as possible was a Philippino woman but even she could only do so much. The day covid happened and the exact time everyone was told to leave was the last time I saw my grandma. I told the nurses she wasn't well and wanted to know where these bruises came from...but I was dismissed and so were my concerns. I hugged and kissed my grandma and promised I'd be back. Us not being able to be vocal for her and the horrible care of this place put my grandma 6 ft. I hate this place and what they did to her. They had spills left for days and machines not working properly. Horrible staff & leads. Male patients roaming the ward and being predators in the female rooms. My grandma being one of them. I can only image my grandma's last days there. Being harmed and silenced. Please for the love of God never bring your loved ones here. I have more photos and videos. The staff was ok. But trying to get my friend home was like pulling teeth . Anna the social worker / director was awful and no help at all. She doesn't call you or help you get things set in place to make it possible to get my friend home which is 4 hours north of Redlands. We had to deposit $500.00 + $15.00 for using a credit card to get the oxygen in order to get home. She got no idea of how to properly do her job. We are still on the road getting home and they call us to ask when we bringing the oxygen bottles back . Very insensitive and rude, we got a certified letter the next day saying they'll charge the card now the bottles are not back yet . Unreal! Of coarse we got there bottles back luckily we could set up a string of different family members who live near there so it was possible to get the bottles back on a week. No help leaving that place. No calls of what needed to be in place for my friends to help after 5 months in the hospital. It would have been a better experience had the social service director and her two minions stayed in her office and not exposed us to her inaction and no compassion. I also see the light to signal when the patient needs something is set to off as soon as a worker comes in and if the needed a different worker to help with something the worker comes in turns light off and says they'll be back but then no one comes for a few hours. Which is not at all helpful. I gave 2 because they need to do better. read more

This place is one tent away from a full blown circus. I could write a book on how awful it is. Starting with the head of respiratory who never seemed to order enough supplies. I had to order inner cannulas for my husband because his supplier wasn't supplying and he didn't know where else to get them... Director of Nursing is a POS! Said my hubby over reacted when he said he couldn't breathe. Unless you've been trached Michael you have no business judging these patients! Not to mention i had to fight my way to have him transferred to ER on multiple occasions. The first one RT supervisor said he was fine, yet coded the minute he got to Redlands community... airway was 95% closed. Many more in between ... The last one, Rosa the RN failed to call 911 after an artery ruptured. She arranged transportation. She called it normal tracheal bleeding !!! cleaned him up as well as the blood splattered everywhere. This place needs to shut down. Days later the DON assured they had stopped the bleeding ... clearly they had no idea. Not to mention I didn't know if I was visiting my loved one in prison or a rehab center. Nurses have no bedside etiquette nor do they reposition or change your loved one as they should. Call lights are ignored and go unheard as they wear ear buds... I could go on... oh ya CNAs have no respect for patients... they verbally abuse them... who knows what else happens ... after things happen don't bother asking for details, it's all internal and they won't disclose unless you sign a waiver dismissing them of any wrong doing. They act like they own your loved one and they no longer have rights... They deserve 0 starts but had to give one to post. read more

Dad was a resident here for a little over 3 mos. Absolutely grateful to his care team as a whole, he had great caring nurses but I have to highlight his RT's. Ron, Director of Respiratory Care and his staff were amazing, the goal was to remove the Tracheostomy ASAP and thanks to the quality care of all his RT's he was able to breathe on his own again. Also, big THANK YOU to Joel in physical therapy. He came in from the first day and said he would have him up and walking and he delivered. Quality PT work. Of course we had some hiccups along the way with the facility but overall great service and I would recommend for Rehabilitation care for your loved one. Also, very clean facility. read more

This is one of the worst facilities I've been inside. The over powering g odor of urine and feces is unbelievable! My last visit was for over 90 mins and when I walked in alarms were going off. When I left those alarms were still screaming but now there were more than 3 ringing. No one went to check on the patient. One was in my brothers room and no one ever came. My brother looks like a skeleton. They say they are doing therapy with him but he said no one comes to see him at all. I questioned the nurses but they all claimed they didn't have any info on my brother. She took my number and said doctors would call me but no one called. They said he can only eat by a tube directly into his stomach but my sister was feeding him soft foods by spoon. Then they lied and said they fed hi. Food too but my brother said the haven't fed him even 1 time. This is a place they throw people into to let them die. Get your family out. They will kill them with their neglect!!! read more

Sometimes people do not take reviews seriously, I was that person until my grandmother was admitted over the weekend. I do not even know where to start. There is so much to say about how this facility operates. Lets start off with the unsanitary conditions of the patients room/bathroom, unfriendly staff, lack of professionalism, patients help call button being ignored for hours, medical orders not being followed, and patient diet plan implemented is a joke. I am happy to say that we removed my grandmother from this facility three days later. If you plan to leave your loved one here I recommend being present for visiting hours which is 8am-8pm 7 days a week to make sure they are being properly cared for. If that is the case then you should consider home health care just like I did. read more

the first day my dad was admitted he sat in pee for a hr because no one would help me because the nurses just sit around and wait for the cna's !! No one notified the cna that he had came so she didn't know he was admitted until I pressed the red nurses button which I was told to do because no one would get up and help me . The Cna was at lunch so we waited a hr!! the only good cna there was Eva very nice and helpful !!! My dad seem to decline in the 6th days he was there and everytime I came after work he had peed on himself and was left in the wet underwear and socks !! We snatched him right out of there after he started acting confused and incoherent again and thought he had a Ute he stroke !! Their was a little turd poop left on the floor and Justine the social workers saw it and walked away/over it so it wouldn't mess up his Yeezys . Did not notify staff and was on the floor the whole two hours I was there . Unclean not sanitary and dirty !!! They have patients in the hallways just sitting there with no one attending to them looking like they are left to die !!! read more

This facility is a joke I was there with a friend visiting her family member and I overheard a staff member I think it was a supervisor tell one of the guys that he's lazy and doesn't care about his job because he isn't getting all his work done while he's there but I have been there a few times and I have seen him working so hard but he will be int he middle of doing something and the cnas and other staff will ask him to do something else so then he has to stop what he's doing to go take care of that. I don't think that it is right for supervisor's to say things like that to their employees. read more

Surprised this place is even open. They were in violation of a few things and almost killed my mom when she was sent there for recovery. They didn't hook up her feeding tube for 5 days. This place deserves 0 star and should be shut down. Only one doctor for the whole facility with 100+ patients and the doctor probably never saw any patients in person. Very short-staffed: one nurse, 2 LVNs and very few CNAs. My dad's back pressure wound was left open in the stained diaper with poop the whole 1st night in the facility. Urine collection bag is covered in no way for observation, cloudy urine was finally tested 3 days after reporting. The call light could be on for hours without any response. I can go on and on for many many problems there: If you just have a broken bone, maybe you can survive there and walk out. But if you had a stroke or respiratory problems, this place could be your death bed. Please DON'T send your love ones there! read more

Overall a nice place. They took care of my dad for about two weeks. The front office staff is very helpful and are willing to answer any questions you might have. Bernard, Mirza and Rosie were always very helpful and nice to my dad. Any time we needed them they'd be there or at least give us an update on when they can be there if they were needed by another patient. The physical therapist Heath was very attentive and actually had my dad laughing when he'd go in. Some of the other PTs who went in to see my dad didn't want to push my dad to do the necessary exercises but Heath was different and was very optimistic. read more

I am very concerned about my aunt . I called Asistencia Villa for 2 weeks if someone answers I'm put on hold then 20 minutes later I get hung up on . I've asked for the directors name no reply and that person is never there I'm told she's on a mission . WHAT? What does that mean ? A mission ? And not once has the director called me back. No reply. So ask to make an appt. Or a time so I can talk to my aunt's doctor , still no answer or receptionist puts me on hold then I get hung up on again . Who do I need to call to find out how my aunts condition is a update on her health. Who do I call? Im very concerned . Something there is very wrong. I got to make arrangements to get my precious aunt out of there . I hope she's not being abused. I have every right to say abused, because there is no communication . And I'm not a rude person . My brother is there and I go there two to three time a a day to check on him He had a severe stroke and he is disabled anyways and sometimes when I go in there and screaming because he's in pain he sitting there soiled and soaking wet and nobody is around then I have to go and get somebody and they act like oh he was just like that they have too many patients and not enough staff and I feel so bad that my brother is there right now when he gets released can't wait for him to get out of there read more

This facility is terrible. My grandmother lived and died there and it eased the worst experience. I had to file complaints with the county and local ombudsman to receive any serious assistance. She suffered from a skin disease that was obtained during her stay there and was never resolved. The doctor determined her cause of death without seeing her. The facility contacted a funeral home without consulting their paperwork, so I had to track down my grandmother's body. Since her death I have been placed on hold for more than 30 minutes to get answers only to be told the person i need to speak with is gone for the day. I have been hung up on multiple times while holding. Please, if you care for your loved ones, find a different facility. read more

Do not let your loved one go here. ESPECIALLY for respiratory needs. ONE Respiratory Therapist for 22 people. My mom was brought in Monday night and they refused to hook up her feeding tube until the following morning. THEN they did NOT SUCTION or do the breathing treatments as needed. She was sooooo dehydrated she was BEGGING for ice chips and ended up trying to drink saline. We were told she was going to have a one on one caregiver overnight and THAT DIDNT HAPPEN. She ended up back in Kaiser ER 48 hrs later because her feeding tube was yanked out. I could give soooo many more details. But it's a nightmare... do not let the "5 star" public health dept score fool you. read more

Let me just say I am not sure how this place is still open. My brother declined dramatically in there care. Staff was nice enough, except for the director Dianne. I came to get my brothers stuff after he passed away and her lack of compassion made me lose it, she looked at my mom and said, "what a great job you did raising that one" as if my brother didn't pass away the day before. My memories of my brother being there was him constantly calling and crying because of how mean she was to him, he struggled with an ED and she made him eat in front of people in the dining room, which he was NOT comfortable with. I could go on and on about this place, but moral of the story I have heard MANY people go through the same sort of neglect with their family members. I even ran into one of their CNA's at the store admitting she has witnessed countless moments of patients being neglected. read more

DO NOT SEND YOUR LOVED ONES HERE: There is a stench in the air as soon as you enter the outdated foyer. My father had 2 strokes and slowly started regressing at this facility. Within 30 minutes of being there he urinated on himself. The nursing staff said they couldn't clean it up because they had to wait for physical therapy to clear him to get up or for them to move him. When asked when physical therapy would be by, I was told the following day. So instead of letting my dad sitting in his own urine for 24 hours, I found a CNA Eva, who helped me thankfully. I got him set up and cleaned, then went home. Upon calling him later with no answer, I called the facility. No one answers the phone there, ever. I called for about 30 minutes before I just got out of bed and went up there in my pajamas. The nurse Carrie who greeted me upon my arrival let me know his nurse Lakeisha (I think), was on her lunch & Carrie could not determine whether or not my dad was given his meds (no system to ensure patients get meds), although he was scheduled to get them at 5pm. When Lakeisha returned from her lunch at 9pm, she brought my dad his meds and checked his blood pressure for the first time since he had been there since 12pm. This probably only happened because I asked about it. The cause of his strokes is uncontrolled blood pressure, so he needs his meds. Within the first 8 hours I knew my dad needed to be removed from this facility. I tried to get a hold of Jodi the case manager various times and she either wasn't there or in meetings. I finally had to go down there to talk with her. Also, the NP/doctor is very inaccessible. I would come during the scheduled time/days he was supposed to be there and the nurse would tell me he was gone for the day. On the day I removed my dad from the facility, I had to chase the doctor down in parking lot after I saw him in foyer, 2 hours after the nurse falsely had told me he left. He then said we should send my dad back out to Kaiser due to the systems and regression I spoke to him about. I lowkey suspect they were happy to get rid of me and my caring family constantly asking them to do their jobs. Furthermore, it is dirty, it stinks. I was there on Friday and there was feces on the floor. The staff just walked over it continuously. I saw the social worker Justin look at it and avoid it as he was walking. It was disgusting and there for 4+ hours. They also leave other patients in the hallway to die it looks like. Oh yeah, and they stole my dad's blanket I bought him too. He was freezing in there for days because they took and it and did God knows what with it. One nurse said that the another nurse probably took it and gave it to a patient they like, so yeah, good people over there. read more

This place is AWFUL. They left my mother in law unattended in a wheelchair for over 30 min in the middle of the night, then denied it and said it was her roommate that was in the wheelchair. The police got involved, this place made up stories to cover their butt. My mother-in-law and her roommate are both witnesses to what happened. Thankfully my MIL was able to call her husband (in the middle of the night) from the roommate's phone to tell him she had been left unattended and was told if she wanted to get back in bed she would have to do it herself. Mind you, she just had a stroke not long before and had been in the hospital for six weeks with no strength to walk yet. They treated my father-in-law horribly, they would not let him go in the facility to retrieve her things bc in the midst of all this an ambulance was called bc she FELL OUT OF BED!! They made him wait outside and brought her things to him. This place is truly unbelievable. I wouldn't recommend it to my worst enemy. read more

After working here, and in response to Lou and Daniella: I too, am an RT that works for the company and after voting to go to 12 hr shifts, it in turn gave us an extra 8 hrs. So actually that "cut in hours" is actually the same amount of hours you would work in a normal work week. I have also worked with the Admins of this building for many years and NOT ONCE have they mistreated or mishandled a situation for ANY of their patients. Unfortunately with covid, the protocols and CDC guidelines change everyday. I would recommend this facility to anyone who's needing a place for their loved one. read more

I was a patient at Asistencia Villa for 6 weeks, following a two week stay in a hospital. I was recovering from surgeries for a tracheostomy and a PEG feeding tube. I just loved the dedicated employees at this facility! No one wants to be in a nursing home, but they made me feel as much at home as possible. They always welcomed my family members and my dear wife. I needed a lot of help because I was being tube fed and my tracheostomy needed a lot of attention. The nurses and CNA's were very on top of it and kind. When I asked for help, the nurses greeted me with a smile and knew what to do in an emergency, of which I had two. The cleaning staff were hard working and friendly. The social activities team came by every day and offered ways to try to find recreation. The doctors I dealt with were kind, listened and gave me consideration of my concerns. The physical and occupational therapists were very caring and gave me the tools and fitness I needed to be able to live at home once more. The Respiratory Therapist Staff was outstanding and made being there so important. They changed the components of my tracheostomy intermittently each day and always checked in on me to see if I was doing well or needed breathing treatment. I just really appreciated the training Ron and his RT's gave me and my family. I highly recommend this Skilled Nursing rehabilitation center and I would go back if I were in need of similar care for my tracheostomy related respiratory needs in the future. read more

I am also a heart patient and that is what bothered me what if I had a heart attack i would have died before someone answered call light. I have been to several different rehab facilities and this is by far the best one I've seen. It is very clean and the equipment is new and up date. The night shift at this place is just plain neglectful. My mom's roommate urinated on herself in bed because the staff was not responsive to her request for assistance. My mother, who had bypass surgery and can not use her arms because it can reopen the wound, had to use her arms to get herself to the bathroom because the CNA was too busy to help her.. The staff, when I quizzed them on my mother's limitations, stated they had no idea she could not use her arms. A supervisor excused that lack of information to my mom being "new". The following night I stayed there to ensure she was being properly taken care of. This, even after I was assured that "this will never happen again". When my mom needed to use the bathroom I walked down the hall and observed 3 people sitting at the nurses station. I went back to my moms room and pushed the "call" button. The CNA on duty was filling ice glasses in the hall and did not see the light above my moms door. Being night, there was no audible alarm. After 5 mins, I walked to the nursing station, same three people sitting there, and asked them how they knew there was a call for assistance. A woman pointed to a screen not more than 3 feet from her face and said it shows up there. When I told them the CNA was busy, they told me they wait 7 mins and then they will go check. They also said they have no way of knowing if the CNA was able to get to the call or not. Only by the indication on the screen staying for an extended time would they know. I spoke to several CNA's that told me that if a patient spills on themselves and requires a clothing and bedding change that they may be indisposed for 15-20 mins. During this time, other calls in there section may very well go unanswered, as was the case when the resident urinated in her bed. I met with the head administrator and head of nursing. She seemed mildly miffed that she received a text at 1:45 in the morning regarding the incident. Even though this was a very serious, quality of life issue for residents, she made no attempt to contact me. During the very short meeting, I made them aware of the fact that I took video of a staff member repeatedly defending their, "sit-on-their-ass-for-seven-minutes" night shift protocol. Instead of reassuring me that this was not their policy and they try to get to every resident in a timely manner, they were more concerned about the legality of me taking a video without the staff members permission. Yep. More concerned about CYA than the actual well being of a resident. Not, "we will make sure that staff member is properly briefed on our expectations for resident care", no, one of the women decided to play armchair attorney with me. In fairness, there are some very good people there. Our experience with day shift was good. It seems there is a standardization and training deficiency between shifts. Unfortunately, the woman in charge decided to turn the meeting into a yelling match, something I refuse to do, so we never got to discuss these other issues. When I told the head administrator that we were moving my mother today because of these issues, her response was, "Thank God!". Simply put, they would rather a problem "go away" than deal with it. read more

My brother has been under their care and unfortunately is still under their care. Since he's been there, he has made two trips to Redlands hospital and the first time he was severely dehydrated and malnourished. The second time, the doctor at Redlands hospital said he was crazy dehydrated and was diagnosed with sepsis! Luckily my nephew was there to contact his mom, my sister-in-law, she rushed over and literally Told me that he looked like he was on his deathbed! I don't understand why he is being kept there! The first time I visited him there, my brother was choking on his secretions, and I was trying to find someone to help, and when I got to the front desk no one wanted to make eye contact with me, there was no urgency at all, and they just stroll down the hallway just to let this other guy know that my brother needed help, but he thought his paperwork was more important then my brother choking on his secretions. He then came in with a bad attitude and did the suction but that was after the fact that I had to use the bedsheet to dig in my brothers mouth to take out all the stuff he was choking on! I also noticed within A three hour period when I first got there, That his sheets were stained and later that day my sister-in-law came in the room I was talking to him and then noticed that he soiled himself and was not changed. I told her I saw that when I got there but I couldn't smell anything and I thought the stain was just normal or maybe an old gown. But when she left him up to check that's when we are smelt it and she immediately went to the front desk and complained. She was met by the main director and the two nurses that came into his room And failed to do their job. Instead of showing their hospitality, they were pointing the blame at so many directions, I thought they were popping invisible bubbles! I'm definitely doing my research, I will report this place, and I promise I will take action with an attorney! And this goes for everybody else there, I'm very vigilant And I noticed that 90% of the employees there or flat out lazy, they rather be chatting it up, rolling their eyes, and earning that weekend party money. If you're in this line of business or occupation, I suggest you think long and hard about Care, Compassion, Hospitality! For chrissakes, put yourself in your patients shoes! These patients are your last line of hope! They depend on you to take care of them like you would your own! This ain't no game, These are peoples lives! Like, why does my sister-in-law have to hook up the feeding machine after an other nurse came in and didn't know how to hook it up and just left it there? If she wasn't there, my brother would've went unfed! And how do you get severely dehydrated two times and get sent to another hospital for the cure you could have given him in the first place! This place is terrible!! If I could give BLACK negative stars I would, I didn't even want to give one, but had to in order to post. They basically had our mother in a drug induced coma so they wouldn't have to take care of her, as the nurse's were working 12 days straight, due to understaffing, but pretty sure they just didn't want to be bothered. The 3rd day our mother was there, they had her so drugged up on Adivan, that her BP was 80/35 and her HR was 104. She was quickly sent to ER, the med staff in ER were shocked!! They had to give her 1500 mg of a drug reversal to "wake" her up. Unfortunately, she was set back over a week on rehabbing her. If you love and care about your loved ones, DO NOT send them here!!! read more

A relative of mine stayed at this facilty to recover from a surgery. Her stay was great .. nurses were very caring and tended to all her needs. We appreciate the care they provided . :) read more
