Hello, Family care is so important when we leave our loved ones with those who are basically strangers but still administrating medical support. Well, m what I can share about Rialto Post‑Acute Retirement Care Facility id that I have personally observed their nursing staff, Nurse Evelyn, who is personable, responsible, knowledgeable and had my loved one laughing and happy. This matters! Nurse Shanon and Emily are also great! On a couple of occasions when I visited a family member and a very good friend, I noticed the front office staff are welcoming, the administration seems like they are on top of everything and their level of care, responsiveness, and overall integrity have consistently impressed me. The nursing team is both highly skilled and deeply compassionate. They do more than just administer medications and monitor vitals -- they take time to engage with each resident, understand individual needs, and notice changes. On two occasions I have seen nurses staying extra minutes to comfort residents, proactively address concerns (like pain, hygiene, emotional support), and follow up with families to ensure everyone is informed. Very few facilities care like Rialto Post Acute. The leadership at this facility demonstrates professionalism and responsiveness. When I've raised questions or concerns, the administration responded promptly, openly, and with real care. Policies are transparent; staff seem well‑supported and trained, which reflects in how smoothly operations run and how comfortable residents feel. The facility itself is clean, safe, and well‑maintained. Residents are treated with dignity and respect. Activities for engagement are organized, and the staff's attention to emotional as well as physical well‑being shines through. The communication with families is open and honest, helping us feel confident about our loved ones' welfare. Kudos to this hard working professionals! Put my mom here for rehab and physical therapy. This is the worst place. She was there for 11 days and was able to sit up on her own upon arriving. Today (the 11th day) we have taken her out of here. She can't sit up on her own, she's just as weak as when she arrived almost two weeks ago. On day 5/6 her belongings came up missing literally off her body. On Christmas someone took her personal blanket and staff "can't find it." My mother has issues with appetite and eating, the food is terrible as hell. So she hasn't ate, they don't make sure she even drinks water just sits a heavy eater pitcher next to her bed and that's it. My mother is very weak, so lifting a water pitcher on her own isn't going to work. The worst part of it all is that they do not communicate with you when things like this happen. They don't answer the phone at all and whoever the head nurse "Nikki " is, needs to quit her job. My sister walked up to her saying "are you Nikki the head nurse" and she replied "it depends on who's asking" RED FLAG!!!! They do the bare minimum for these people In this place so please DONT BRING YOUR LOVED ONES THERE AT ALL. Literally we have taken my mom out that place and she is back in the hospital. My mother was transferred for post acute care from Kaiser Fontana. The physical rehab care my mother received was top notch! I wish the physical rehab department was accessible to the general public, they are caring, professional and very entertaining. They made physical therapy fun. On the medical care side of my mom's stay, the LVN's were attentive and professional. The CNA's were also caring and attentive. The RN staff especially Deborah, was very professional and always answered any questions I had regarding my mom's care. The environmental services team also were on the ball day in and day out. Shout out to Mr. Chester and Mr. Percy. Special recognition to the activities staff, laundry services, kitchen and nutritional staff, you're all so wonderful!! Also, front office staff is welcoming and professional. Social Services was efficient and helpful. Medical records staff friendly, professional and always helpful. This facility and the various departments work together to create a warm, welcoming and above all caring environment for the patients and their family members. read more

My cousin was placed here after a fall the end of April. He checked in the beginning of May and left the end of the month. Some of the staff was very competent. The counselors, the physical therapist, his doctor and the nutritionist all kept me very informed. However, he has dementia and was assigned to a room in which the other patient was a long-term resident and had a mental issue. He started cussing at me and my cousin in spanish and started pulling the curtain between the two beds open. This caused my cousin extreme distress and I asked for my cousin to be removed immediately which the staff did. They were very helpful in making this happen. My cousin did have a broken back and did not understand that he was not to get up out of bed, so unfortunately, he took two additional falls and it appeared that they eventually restrained him with medications. When I would go by to check on him, he was asleep. At one point a representative for me went to see him and found him lying across the bed, uncovered, blankets on the floor and his diaper completely soiled. She rung the bell for a nurse, but no one came. Eventually she had to go to the nurses station, which was just outside his room to request assistance. She found a nurse on her cell phone who eventually assisted her. She told me to get him out of there ASAP. When I transferred him to another long-term care facility he was wearing clothing that wasn't his, his electric shaver was not with him. ( I had to go retrieve it). And a Mustang silver ring that he was wearing was no longer with him. I did reach out to management via an email regarding both the positives and negatives I encountered. My dad had to go in and didn't last a night there we called an ambulance first thing in the morning and was taken back to the hospital. We found him on the floor with only a thin pillow and paper thin sheet between the cold hard floor and his body. He fell in the middle of the night and his bed alarm was going off no one checked on him until someone on the other side of the room yelled from the hall that he was on the floor. My grandma is there ,I have to say that The nursing staff is very nice to her and helpfull. However...The Physical Therapist there are soo lazy.Expecially this young man name Carl.. Carl is supposed to walk grandma 3 to 4 x A week. But he only does it like once.Grandma has to call for the cna's to call him.Then he makes excuses like hes busy or didnt have time. Meanwhile grandma is not gaining her strength back to walk because of the inconsistency. We keep telling the Supervisor and she just says she will talk to him.He does good for like A week and then back to his lazy self..Why work there if You dont want to help these people that are depending on you for help.Go find A sitting job or something. read more

If I could give no star To this facility, that would be my rating. PLEASE DO NOT , I repeat DO NOT put your loved ones in this facility! Staff is so careless when family members are not around. First off, When u go inside the rooms, they stink like pee and poop. Secondly, patients are left unattended and not to mention they take time to change their diaper. Thirdly , all patients go in and out the facilities smoking and going to Walmart , local restaurants or 7 eleven. They really don't take control of this, can you believe that !!!! Also, don't be surprised if your loved ones personal belongings disappear. Wow, please look for other facilities , do not trust these people. read more

This center claims to be COVID free and tells you that they test all their patients on day one. This is a lie! My father went in with a negative test from the hospital but developed the covid symptoms on day 4, cough, chills and body pain and was never looked twice at. We pulled him out and took him to the hospital where he tested positive and not to mention we found out they were not giving him proper medication. DO NOT PUT YOUR FAMILY MEMBERS HERE! There are so many things that were done that may cost him his health and or life and not inline with a safe, secure and clean facility. read more

My mother was there. Just sent home because of Covid. Was wondering was she tested before sent home to a 70 yr old daughter. To my understanding, NO. Then we find out from hospice, the nursing home was giving my mom Lorazepam 3 times a day and nursing staff NEVER told us. My mother was so looped, never took any drugs. Thank God she passed and in Heaven and never has to go back. read more

My dad was sent here after a stay at San Antonio Hospital in Upland. When he first arrived we thought... What did we do... after learning the staff and moving him once we had him put in B wing. The nursing staff there was very attentive to my dads needs and made sure he was always taken care of and comfortable, I wish I could name some names of the staff we grew to appreciate but that wouldn't be right. The physical therapy department was great. They made us feel like family when we were there watching what they did to get my dad on his feet, the insurance stopped the process and sent him home before they were able to finish the process, but a big thumbs up to the PT dept. Overall it was a pleasant place to have my dad, but we all want our loved ones out and about living life but if a place to rehab is needed, we liked this place, I would personally recommend it to anyone. read more

By far the worst care took father in law there so sorry i did was told he would get hair cut not done did not get shave 5 days but did get bruised arms sent home no oxygen would not give drivers air line which was ares he is on Hospice all they could do is call me all week wanting to sign papers which i did when in went in i would not ever send him back to that money trap ever they are the worst ever o by the way then cslled laughing they found them read more

Avoid this facility at all cost, even if your HMO says it is the only facility available. My dad was there for 3 days for rehab plus IV antibiotics before I pulled him out and took care of him at home instead. His first dose of antibiotic was 18 hours late. Then the next nurse gave the 2nd dose 6 hours too early. He never got a wristband, and I was there when another nurse approached him with his roommate's IV dose (before I stopped her). I had reported his missing wristband several times, and the response was always "I'll have someone get one for him". Of course he never got one, and in a 4 bed room, none of the beds were labeled with a name or even a letter. He was fed ground up unidentifiable slop for meals even though he could eat regular food. Again, this was an ignored complaint. Really, if you care about your loved one, find another rehab center. Oh, and he never got his rehab, either. Just a visit from a person with a clipboard, who just asked a bunch of questions, then left. read more

This is the worst facility to have your love ones at. I call everyday many, many times a day just to hear my sisters voice. It takes a couple of days to speak with her and only when Henry is on staff. I've asked for help getting paper work in order for my sister but no one has called me back. At one point a young man and his staff called. They verified my sisters information and proceeded to tell me that she was doing well eating and taking small walks. The problem with this is my she can't walk....she has one leg. When I told him that this information couldn't not be true, he stubble around and they say, oh, I was reading from the wrong chart. But wait, your assistant verified my sisters information. I was told they have approx. 150 patients and maybe 10 nurses and staff and one cordless phone. Not even close enough to watch after people like my sister. Once I find a way to have my sister remove.... I will. Please, if you want to see and have your love ones, do NOT let them go here. And, do not fall for the false reviews that the staff post on here. read more

This place went from mediocre to worse. Even basic things such as grooming and meals are a low priority to an overworked staff. I seen them through multiple social workers and have never once accomplished anything we set out to do. It's unfortunate I would write a poor review and I actually don't believe the owners care about yelp. The shortcomings in this place could be addressed easily by even a little more presence by management and by hiring more staff. The other stuff are integrity issues. You have to care about your residents sufficiently to be willing to do more than the bare minimum. The caregivers are nice enough, definitely overworked. This place is very caring and nice the staff is always helpful and friendly! My grandfather was here and thwy were always so tentative! I recomend this place for your loved ones they have lots of students that come in and help so there are always lots of friendly faces on aite ready to answer questions and help. Staff are nice enough, seems overworked, my aunt has been here for several years, yes she has had a decline from when she first got here, but shes elderly and in her 80's. To be expected. That's why shes here. Shes always clean, feed and well cared for. Our family always gets updates from the nurses when something changes. Blonde nurse with curly hair was exceptional (didnt get her name), Spent alot of time with us and answered questions very knowledgeable, think she is newer hadnt seen her before. I read all the reviews and I must say not a single one catches the true experience one gets when staying at Rialto Post Acute. Not a single one captured how horrific it is there. I don't think even one of their nurses has a High School education. I would go there if you like waiting for over an hour to get your call light answered. Or if you like lying in Shit for Hours while you're screaming for help. Once I was Lying in my Shit for an hour and when the nurse finally showed up and after I told her that I've been lying in my Crap for over an hour and that I'd like to be cleaned up, she told me that she had to take her break and smoke a cigarette first and then she would be back. She took Two hours before she returned. I ended up laying in my Feces for 3 hours and 25 minutes on that occasion. I complained to the Charge Nurse the following morning and all she said was that some of the nurses were not trained very well., I said Bull Shit none of the nurses are trained worth a Shit. One of the biggest problems is that these uneducated young girls go to Nursing School for 5 to 9 months then end up making only Minimum Wage for all their schooling then end up emptying Urinals and cleaning up peoples Shit. It's not the glorious life that they had envisioned, like they had seen on TV. When they find out that people working at McDonald's make more money than they do they get really pissed off and frustrated then get a chip on their shoulder, no make that up their Butt and they don't wanna beak it. So they end up taking their anger out on the patients..I wouldn't send my worst enemy there. Also if you like your property make sure that you get a lock on your drawers or closet before you leave your room for any reason even to take a shower or it will be stolen, that's not maybe that's WILL be stolen. I had a very expensive phone stolen. (Apple I Phone 6s - $604.00), I also had a very expensive watch stolen (Casio G Shock - $289.00) On another occasion I charged py Power Wheel Chair to full and wrapped the Charger Cord around the battery and placed it against the wall in my room then went to a Doctors appointment. When I returned and went into my room the Battery Charger was gone and when I asked a Maintenance Worker that just happened to be in my room and whose name I shall not reveal as I have a lawsuit pending. He said to me, "Oh when you first charged to the facility and when I brought all your property in there was no Battery Charger there. I said, "Hey Dumb Ass I just charged my Chair to full to go to my Doctors appointment and I God Damn know it was there Muther F***er. What did you do with it you Son of a Bitch?" He said, Ah one of the Nurses must have taken it." I Know that he stole it. When I reported the theft to Social Services they said that it wasn"t on my inventory. I said OMG that"s so Damn petty, it"s part of my chair it can"t be for anything else. They were strict and stuck to it was separate. I had so much more stolen approximately $3000.00 worth and I will post but I am exhausted as I had therapy and a Doctors appointment today. Lastly, do not admit yourself or a loved one to Rialto Post Acute or Corona Post-acute as they are Sister Facilities owned by the same Nazi type people. And you always know that Shit flows down Hill and there is so much Shit at the top so there is always enough to go around. When I feel better I will make an extensive post detailing all the little idiosyncrasies about R.P.A. Take my advice and if you really must stay in a facility then get yourself a Kennell and a Blanket and stay at The Dog Pound, honestly you"ll get better treatment. read more

Run away. They leave patients in dirty diapers for about 6 hours. The bathroom they had in the room was filled and smelled so bad I was getting sick. They forget to give meds. My friend called 911 and they told 911 she was fine. She was having chest pains. They moved her phone out of her reach so should could not call them again. Her room mates verified that it happened. They left my best friend in bed even though the doctor orders stated to get her up every 4 hours and take her to the bathroom. They did not. Their excuse was short handed. If short handed why did they except new patient? I was not suppose to lift anything heavy but I wound up hurting my self because they couldn't lift her to change her without my help. I had to buy and bring wipes to clean her because they didn't have any for the first 4 days. The room was also hot and humid. Had to open window just to get air, fresher air because they room always smelled. When we pushed call light it took hours for them to answer. She wasn't there long because she got pneumonia and passed away. I documented the care she got and it was worse than having no care at all. I also have a witness to what was going on. I was the one doing almost everything for her but unfortunately I could not be there 24/7 unlike the hospital. They knew she couldn't eat by herself and couldn't drink without help. The only time she was feed or got water is when I was there. If I could I would give this place a negative 40. By the last day she was so dehydrated and starving her body couldn't fight anymore. And the other room mates we're also treated like crap. One pushed her call light button on the last day I was there and the staff came in, turned off call light and left. They did not ask what she needed. This place needs to be shut down asap. I called the Ombudsman and now because my friend passed I had to file a complaint with the California Department of Health. Please if you have a loved one there, get them out. If a hospital is trying to send a loved one there, demand they go somewhere else. read more
