Update: This place doesn't seem to understand that case managers give us all the information that they relate to them. So in addition to our complaint below, they have also lied and suggested that we threaten them and that they had some sort of Accu safety device on our person. Fortunately, we have a witness who was there during the entire event. None of those things happened. You are seriously warned. Do not go here. This is the summation from our IP letter and conversation... For the record, the day and evening nurses were amazing. In no way am I disparaging the front line workers who are doing their best in mediocre, if at times in certain spaces, deplorable conditions. This care center proved to be an utterly unsuitable and deeply concerning environment for patient care, directly jeopardizing the well-being of my partner. Critical medications, essential for maintaining his mental stability, were abruptly and inexplicably discontinued upon admission, leading to severe mental distress and disorientation. This negligence was a profound lack of oversight and disregard for patient safety protocols, setting the stage for a dangerous situation that could have been entirely avoided. Compounding these medical failures was a shocking display of administrative and case management incompetence. The facility's case manager provided false assurances regarding a thorough patient assessment, which simply did not occur, and later dismissed legitimate concerns from the caregiver. Despite explicit prior inquiries about facility security (during a tour in which the 10+ year admin assured them the facility was secure), the center admitted an at-risk patient to an unsecured environment and then actively refused to prevent him from wandering off the premises into a potentially hazardous situation. This deliberate misrepresentation and subsequent abandonment of responsibility are inexcusable. The culmination of these failures resulted in a terrifying emergency where our patient, unmedicated and disoriented, was able to walk into the street parking log, requiring immediate intervention from emergency services. Fortunately a friend was able to bypass the wandering and get him to remain in the parking lot. Due to the imminent threat to his safety we had him transported to the hospital, where the lack of medications was addressed around the shock of the case (hospital staff had choice words to say about the care facility and none of them were good). There is a severe disconnect between the facility's administration, case management, and nursing staff (WHO ARE AWESOME and doing their best despite the management), ultimately demonstrating a profound disregard for patient advocacy and safety. This care center not only failed to meet basic standards of care but actively endangered a patient under its supervision. The room was busted up, the floorboards warped, people were sleeping or disabled on floor mats, there was no Television in the assigned room and someone somewhere was yelling for assistance for over an hour. The room given had full trash cans, partially used items in the drawers, and the food given was contrary to the dietary requirements requested prior to admission. DO NOT GO HERE. read more

Do not take your love one to this facility. They steal their clothes that you buy. Every thing my family has bought to this facility for our loved one has been stolen. When we ask them where are the items no one knows anything. We buy more, clothes, shoes, grooming products and cell phones. All comes up missing and no one ever finds the items. This went on for almost two years. The health care is worse. They continue to let our love one suffer until an infection would come. They send our love one to the hospital. The hospital get our love one back strong and healthy only to be sent back to this place to get another infection, vicious cycle. This facility has even let my loved one fall in the back of a transportation bus, because they did not properly strap him down before they started transporting him. This maybe a training facility, because they continue to have new staff that doesn't seem to know what they are doing. Several reports has been made to San Antonio Regional Hospital and the State and this place is still horrible. My love one died this week from the miss treatment from this facility, and guess what my family went to retrieve my love ones belongings and they had nothing. No glasses, cell phone, clothes, shoe's nothing. Please take your family member to another place not here. I bet you are saying to yourself why didn't we change facilities. It's because this place take all the insurance money up front, so there is nothing left for you to take your love one somewhere else. As we were there to puck up his belongings, other patients heard us talking about how everything is missing and other residents started to tell us that their things are also coming up missing as well. read more

This place was awful. My mom was there for 4 months, and in that small time I seen so much neglect. I seen them push elderly patients, ignore them and just be out right mean to them. It made me so sad and depressed to be there. My mom is no longer here and is with our lord, no amount of hate for this place will bring her back. But I will say please please don't put your loved ones there. They get 1 package of wipes for so many patients and when they run out they use towels! It hurts them so much, especially if they have wounds. Also during a transportation with the facility my mom fell out of her wheelchair. They never connected her correctly and her wheelchair moved and she fell out while the vehicle was on the freeway. Being that my mom was blind she couldn't see if they connected her right she just trusted them. Later to find out her transporters were not transporters at all they were the "maintenance man" and the "front desk worker" I don't even think they are qualified to transport. Very sad, I wish they changed there ways. These people need love. Especially while in the end phase of their lives. More joy, more compassion and more love. read more

I would give this place zero stars if I could. Specifically for their afternoon to early morning care shift. My family member went all night in a soiled diaper- when she asked to be changed that evening the nurse ignored her and she had to wait for the morning nurse to ask if she can change her. My family member was there for rehabilitation after a major surgery and she never receive pain medication- or any type of care or physical therapy/ mobility assistance. Not to mention the facility is dirty. Morning shift nurses were nice. And I'm grateful their day shift is on top of things. But their afternoon/ evening/ swing shift is just horrendous. San Antonio hospital placed my brother there upon discharge for rehab. I had previously read the reviews and was concerned. As time passed I realized that I was impressed with the positive personality of staff and how nice and attentive they were to my brother. It's not the most beautiful place or nothing like that, but it's the way they attended to my brother that mattered. The staff has a lot of positive energy and provide music and activities for staff. They were great and I appreciate them for this. The care team , social workers, care coordinators were well informed on options for discharge, etc I was impressed. My husband was sent there without my knowing, directly from San Antonio Regional Hospital across the street. He was placed in a normally 2 person hospital room with 4 other men. (5 total) One of the beds was turned sideways to cram more people in. 3 blaring TV's going at all times and one of the patients with tourettes yelling all night. There are not enough people to attend the patients properly. There was one nice young man and another man that seemed to have some compassion. (I which I could remember their names). Otherwise, everyone else seemed apathetic and overworked. When he entered the hospital he was able to walk. After Villa Mesa's"care" his legs became so weak, that he is, still today, in a wheelchair (4 months later). Dr Phang Cong only briefly saw him maybe twice. I never saw or spoke to him. He wouldn't return my calls. His legs were terribly swollen and he was developing bed sores. I stopped by one late afternoon to see him in a wheelchair, soaked with urine and asking at the desk if they would change his diapers. With open sores, you can imaging how painful that was. Once they saw me, they rushed to clean him up. I saw that his legs were not getting treated. They became puffy, red and swollen with sores. His fingernails were disgusting and his toenails were almost 2 inches past the end of his toes- black and filthy. I got him out of there before he lost a leg. This is a house of horrors-not a care center. read more

My Mother was at Villa Mesa for long-term care for nearly four years. I live less than ten minutes away and was extremely grateful I could be with my Mom every week day 12:30-7 p.m. when lunch was served and 4-7 p.m. on Sunday. My Mom was always clean; her room, of three beds, was always clean and bright; meals were good--I often had lunch there; staff was always courteous and helpful; administrators were accessible and always on-site in the office; outside grounds were kept up and wheel-chair accessible. After lunch, I would push my Mom around the building grounds in her wheel chair and we would spend at least two hours on the front porch feeding peanuts in the shell to a Blue Jay that had befriended us. The Jay would take the nuts from our fingers, off our knees, or the bench armrest and fly away to hide each peanut. We would return to my Mom's room once she got sleepy or when the Blue Jay no longer returned for a peanut. I never had to complain to the staff/administration about my Mother's care. Some names I remember are: Admn. Staff: Denise Peterson, Director. CNAs: Maria Guiterrez, Laverne, Miriam, Bryan, Sam, Juan, Priscilla, Marlene, Ce Ce. Residents: Martha and Ms. Shirley (in my Mom's room), Donna Turgeon (deceased), Mark Montoya (still a resident) whose Mom, Marge, visited every Wednesday, and Patti Stevens. Needless to say, I wish I still had some of those days where I could visit my Mom. read more

This place was far better pre-covid, My grandmother was here for a few years and I know the in's and out's of this place. If you visit daily, they will take care of your loved one like they should, those that don't get visits get treated like "mushrooms" (kept in the dark and ignored). For my grandmother I would request fruit and cottage cheese with every meal (because that's all she wanted to eat) They would do it if the attending nurse would request it, otherwise they wouldn't fulfill the request. One time the food director laughed in my face when I told her I did not appreciate that she's not honoring the meal plan for my grandmother. (*At that time there was a semblance of order, now the director is gone and there's new staff; it's far worse now) There's a new director and the staffing choices seem a bit weird. My mom is now here because she broke her elbow and needs to recover under care, they keep her in a room with the curtains shut and the light off. The won't give her a universal remote for the TV (had to turn on the TV manually, but they even turned the main power off to the TV, which is unnecessary). If they do this to my mom who can voice her own concerns, what makes you think they won't do worse to your loved one if they can't speak up for themselves. I don't know of any better place, but this place is worse than it was a few years ago, I think mainly because of the old director leaving. On 6/16/21 My mom was hungry and they did not feed her dinner because she was out at an appointment, when she asked for food, they said she had to wait until morning, (she's insulin dependent and can die if her sugar gets too low) Her sugar was 61, I had to call in and tell them to give her snacks or she will die. On 6/17/21 Her blood sugar was 76 and they refused to give her food to get it up higher, I have to pull her out of that facility, it's too dangerous for anyone that needs constant care and insulin dependent. read more

If you have the option to select the SNF your family member goes to, Villa Mesa should not be your first choice. The nursing staff I met during my Aunt's time at Villa Mesa were not empathetic, nurturing or welcoming. My aunts exact words were "I'm in hell." I understand transitioning from independent care to having to be dependent of others can be hard but this is where the staff would have made a difference. I truly felt there was no regard or respect of our loved one. My Aunt was transported to SARH ED with sweatpants that belonged to a different patient of Villa Mesa. The sweatpants CLEARLY had a different patients name on them yet my aunt had them on. read more

Do not pick this place. My dad was sent here after his stay at the hospital to recover. We were informed of no visit because of covid. We would call at different hours and no one would ever pick up the phone. He stayed here two weeks. Only was able to talk to him twice. He had a social transport that would take him to dialysis three times a week. The driver dropped him off in a wheel chair and left. They called me saying the facility didn't send the right equipment for him. He sat for 6 hours waiting for the transport back to villa Mesa. Second time the take him to dialysis the driver drops him off and leaves. My dad is not responsive. A few hours later the have him on life support. Never recovers and passes away. They lost his clothes twice. Lost his blanket and jacket. We took clothes to the facility for him. read more

I was very hesitant putting a loved one in this place. I did random visits through out the day and my loved one would be up playing bingo or in the big room they have for residents, the place never smelled like other places I visited, and the nurses are great especially cna Maria. I did have one issue that came up but I stopped a nurse walking by and she got it corrected immediately (didn't know she was the boss). I don't have anything bad to say about the place. read more

This review is well thought out and on behalf of patient and family. Villa Mesa has been a breath of fresh air. My husband had a 4 week stay at this home. I say home because the staff made my husband feel comfortable and well taken care of. He has been at 2 other facilities in the past for care but this was the longest and by far the best. The camaraderie was amazing! He gained weight rather then to loose weight. The communication from the staff to the the family was amazing even though we are unable to visit under the COVID conditions. We really tried to give this facility a benefit of the doubt. We really did..... How this place remains open blows my mind. My grandmother was here for two weeks after a hospital stay and she wasn't monitored correctly. They properly discharged her to come home and within 18 hours of being back home we had to call 911 because my grandma couldn't breathe. Turns out she had THREE LITERS OF FLUID in her chest. THREE! This ended up being another month in another rehab facility because of their lack of care. She was sent home from this place in this shape because no one was monitoring her. On top of that, the place was dirty, run down, had leaks in the ceiling when it rained. The nurses and CNA's let the patients listen to their tvs as loud as they want while people are trying to sleep and rest and get better. My mom and I were here every single day two to three times a day to stay on top of the staff because if you don't, your loved ones will not be taken care of. The staff loves to give lip service as if they're doing such a great job when you confront them about things they are not doing properly, but when it comes to actually taking care of patients, they lack care. Not sure if it's because they are understaffed or what, but it's really sad that patients have to suffer because of their lack of trying to do better. I truly feel sorry for the patients who don't have their friends and family being their health advocates when it comes to this place because again if you do not stay on top of them, they will not do their job. Also the food is horrific. I don't know how they can serve the food that they do to patients. Not even my own dog would eat the food. If you have a loved one who has the possibility to go into this facility, please do everything you can to make sure they don't end up here. Our health care system is bad enough, but with a good fight and being a good health advocate for your loved one, things can change. Villa Mesa, please do better. read more

I don't understand I call days in advance to see a patient. The lady activities directory makes the appointment, I show up and it takes 30 plus minutes to bring her out to visit. The receptionist acts like she doesn't know who my grandma is and always questions is she a patient here. The. They take forever to get her up and bring her back. Although there are numerous nursing homes in Upland and Ontario, Villa Mesa is by far the best skilled nursing facility in the area. The nursing staff are extremely knowledgeable, friendly, and accommodating, and go above and beyond to care for their patients. Management is also just as amazing. They have helped numerous residents and family members in applying for medical, and willingly educate family members to take advantage of resources available for their loved ones. The facility is also exceptional. It is always clean and smells amazing, unlike other nursing homes. I have encountered family members who rave about how amazing Villa Mesa is compared to other facilities. I even met someone who had their wife there for several weeks. He had had her at a different nursing home, but was not happy with the care she received. After a couple weeks of therapy, his wife was able to tolerate walking. The nurses have also helped reduced her pain significantly. He says he will never take her elsewhere. And states after his surgery, he will request to return to Villa Mesa himself as a patient. Taking care of an aging loved one is hard work, and taking care of a complete stranger is even harder. Not all family members are understanding. Most are difficult and hard to please. But the nurses in this facility are different from most. They truly care about their patients and it shows. read more

I have to say that I had never had a personal experience with an SNF prior to my father's admission this month. Not only were the staff friendly and attentive to my dad's needs, I was able to candidly observe their loving interactions with many of the other patients. From housekeeping to the nursing and business office staff, ALL went beyond my wildest expectations by being consistently caring, friendly and thorough. They would call me when I was worried to give me status updates and kept me and my dad's doctor up to date. I've been praising them and giving shout outs to the case manager at San Antonio Regional Hospital. I was so sad when he had to transfer back to the acute hospital due to a health set back, but if he ever needs to go back to a skilled nursing facility, he'll come here. Thank you !!! read more

My first nursing job was at Villa Mesa and this was awhile ago. These nurses are the most caring I know in the area. My 94 year old aunt was having a painful transition and they took her with hospice (Cambridge). I was there for the few days and she was clean, dry and comfortable. Her room was peaceful and the nurses would come by to check on her and me often. One thing that's different here is the staff. They are staff that is invested and have spent awhile with Villa Mesa, this should be a continued goal of the management's part--there are residence that look better than many years ago from the last time I saw them when my employment ended and that is a huge testimony and the way it should be. The residence at Villa Mesa are thriving and well cared for. And even in a transitional period, my aunt was treated so well and so was I. Thank You all. Also, it really is the cleanest place too. Take it from another nurse who has instructed all around the Inland Empire, this place is a good, safe place and it's clean and their nurses are smart. I have never been so upset. This is a disgusting facility. The filth that I witnessed was disgusting! How and why would anyone have their loved one sent there for Rehabilitation. I am at such a loss that San Antonio community hospital would refer such a terrible place... I feel so sad that I let them send my mother their. I trusted them to have the best interest for my mom. They apparently don't or they would have never recommended this place. They treated my mother so poorly. They put her in a dirty room that was freezing cold with dirty floors, walls, baseboards and window valance that was full of dust! So not sanitary. Supposedly this room was where new arrivals went because of Covid.( makes not sense) They put her in a bed that was completely crooked leaning to one side with a dirty sheets, they also put her in the bed with her tennis shoes on. What kind of treatment is that? The Director of this facility should be ashamed of herself. If she is the Director she represents this kind of care!!! This place should be shut down... read more

I would not recommend this facility if you loved one is seriously disabled or they require dialysis They are understaffed and if you are not willing or unable to advocate for your loved one their care may suffer. Necessary medications and/or treatments may not be administered in a timely manner if at all... further jepordizing your loved ones health. The facility is clean and I do believe the staff cares about the patients it is just that there are not enough of them. read more

This a really friendly and caring facility. They even threw a big Mardi Gras themed bash for Mother's Day. The entire staff was serving the patients and their guests. The food was tasty, and I hope it was indicative of what they serve daily. It may not be for some but it is perfect for someone just needing a few weeks to get back to normal after a long hospital stay. read more

My aunt is recovering there, can't say enough about the staff, from admissions to nursing care.

I was told my dad would have a private room, when we got to the facility he was put into a room with 3 other people. His side of the room was not cleaned, everything was left from previous patient! When he was transferred by the transportation, they asked who would get his paper work/medical file with my dad's condition. Every one was trying to pass it to someone else. Very disappointed in staff that no one wanted to assist my dad when we was transported. The charge nurse didn't want to accept the file. I will follow up with administrator first thing in the morning. It was already after 5:30 upon my dads arrival. There is no management after 5pm apparently. Will follow up with more details after 24 hours and speaking to management. Came in the morning next day at 10am. He was in worse shape then when I left him. They hadn't given him any of his 8 medications he is suppose to have every 8 hours. Hasn't been evaluated. Not happy at all with his place. read more

Worst place ever. STAY AWAY. My mother was there for about a year. She had fallen out of bed two times all I heard was excuse after excuse regarding putting on the rails on the side of the bed. I would not recommend taking any family members hear the food is horrible the physical therapy is absolute joke. Anytime there was an issue there was always some type of an excuse. If you have any type of requests again they never follow through with it. My mother has huge bed sores due to lack of care. She was found totally dehydrated and now is in the hospital with kidney failure. I usually don't get bad reviews but I would not want to see any other person having to go through what my mother had to go through at this location stay away at all costs. read more
