I couldn't be happier with the care my loved one received here. He is 85 and has dementia, and having a sitter provided when I couldn't be there gave me tremendous peace of mind. I visited five skilled nursing facilities in the area before choosing this one, and I'm very glad I did. Victor gave us a tour of the facility and was extremely knowledgeable, answering all of our questions and making us feel confident in our decision. Prima was an absolute godsend. She was always available by text whenever I needed help reaching my loved one, had questions for the nursing staff, or needed updates when I couldn't be there. She also helped coordinate his discharge, which required extra planning since he lives in a memory care facility and paperwork was needed from multiple doctors. She made sure everything was completed on time and without added stress. Victor and Prima are truly outstanding employees and a tremendous asset to this facility. My father entered this facility with hope for recovery. Instead, he was met with neglect, humiliation, and treatment that no human being--let alone someone vulnerable--should ever endure. From the start, it was clear we noticed he wasn't being treated with basic respect. One of the most heartbreaking moments was seeing my father left sitting in his own feces--even after being given a shower. There was no urgency or care in cleaning him. And when they finally did, he was scrubbed so aggressively that it felt more like punishment than hygiene. See the photos of the excessive scrubbing. Do not take your loved ones here you will be so sorry you did. This facility failed him in every way. This is an update to my last review on Las Colinas Post Acute. I was looking for long term care for my mother and Trevor Lords, the administrator at Las Colinas, assured me that things had changed dramatically. My mother's been here now about a month and I have to say things have changed for the better. My mother gets good care and the food has improved. It is much better. The rooms and hallways are always clean and you always see housekeeping doing a very good job. Trevor Lords and the assistant administrator Adrian Racpan are always available and quick to assist you any way they can. I am very pleased with the care my mother is getting. read more

I don't recommend anyone go to this horrid place. The nurses leave you in your waste overnight. The nurses sleep on graveyard. They don't answer the call button for over 45mins. The nurses will retaliate against you if you report them to the administrator Trevor. They left me alone for 6hrs in my waste. They manhandled me causing me to slide off the bed. Trevor eventually stopped responding to my concerns and offered no assistance. Trevor, Diane & Victor don't care how their staff treat the patients. The nurses are mean and hateful. They won't give you your pain meds if you complain. They wouldn't treat me for Covid even though I was diagnosed 4dys prior. I had to call 911 to get treatment. They didn't return some of my personal property. They feed you cold food. They really treat you bad if your family doesn't come around to check on you. The only thing good about this place is the physical therapy department and a nurse named Megan. I reported them to several agencies. I was only there for 2wks. Out of 14dys only 2dys was uneventful. I wouldn't recommend this place to anyone. read more

My mom was there for 2.5 days it took me that long to get her out of the horrific place. THose beautiful pictures look nothing like the rooms. My mother in law was sent there by the hospital for recovery after she broke her Femur after a fall, I was worried at first because we see several bad reviews about this place, but to my surprise, she was in a room shared with 2 other patients, after a week there, my MIL was quite happy and satisfied with most of the things there, staff were nice and friendly, the place is very clean, every time we went for a visit, there's always crew cleaning the floors, room was relatively clean and does not smell bad, Air conditioning of the room could use a bit of help, so we had to bring in our stand fan, the food was actually decent and staff were accommodating to my MIL's requests, Nurses take their time to explain things clearly and what to expect during and after the stay there, they took care of in-home care as well after the stay My MIL's only complain was that calling for attendant/Nurse/staff can sometime take a while ( short staffed I think, but not out of the ordinary, but once they show up, they really take care of her needs) it's funny that the only time we smelled horrible smell was on the last day as we were walking out of the facility and one of the room must have something spilled, UGHHH!!! My uncle was admitted here a couple weeks ago and we had to pull him out immediately. The staff here were so rude to him. They left him covered in his own waste and would taunt him telling him to call for his wife so she can come save him. Due to this neglect he got e.coli and died a week later. Absolutely horrible if this place doesn't change their way of working I hope it shuts down quickly My grandpa was at this rehabilitation center for less than a week because the care and service was horrific. He had a nurse Teresa who was in charge of taking care of him and the way she treated him in just a matter of days was horrible. The week he was there she took my grandpa for his shower and was incredibly rude the whole time. First of all she washed him with scorching hot water, and she was scrubbing him so hard that he ended up with red marks everywhere. My grandpa had asked her to be gentle and she scrubbed even harder and told him to deal with it. When my grandpa said to please be gentle and careful with both of his dialysis catheters (he has two one for the stomach and one on the neck) she pulled the stomach one and told him that she was in charge and to see if anyone saved him. Then my grandpa said he was going to complain and she said go ahead and shove that complaint up your ass (in Spanish which comes off extremely vulgar). When she returned him to the room he was in a wheelchair naked and covered in shit. Like literally feces and the worst part is we have no idea if it was his own feces or if she put him on a dirty chair because she was upset. She would also do other things like uncover him in the night when he would tell her he was cold or tell him that his wife wasn't coming to help him when he would ask. Writing this brings me to tears because my grandpa is fortunate to have us and we got him out immediately but I cannot imagine how the care is for other elderly patients. This nurse has to be fired and I hope this place changes for the better. For now I recommend avoiding this place at ALL COSTS. read more

Please don't send your family members to this hospital my very first night I was admitted at 8:30 at night. I sat for 2 1/2 hours in a wet diaper because I kept being told they were doing shift change. My next scheduled medication would be at 11:22 so I let them know the minute I got here. After sitting there all night, waiting for them to issue my medication, and giving many reasons why it was done late and be assured it would be here at four in the morning. I know you can guess it never arrived till 1130 the next day. I found out also I wasn't the only one who was sitting with the wet diaper. One person told me that you don't ever see anybody to change your diaper until 2 o'clock in the morning. She said she goes to bed every night with a wet diaper and has to just wait on them. If I could have walked or actually moved I would've done that but I was stuck. When I called the staff and was bawling and told them I wanted to leave they told me they would not help me out of the bed. They would not help me to the front door and they would not help me to whatever vehicle was brought and to top it off I would not leave with my medication. I could go on and on about that place. They claim to be a rehab facility but they are fooling people.I really wanna say is if you care about your family members or anybody going to that facility please take what I say to heart. I know someone from that facility reads these things and claims often when people complain that it's not true they tell you that the person complaining was never a patient there but I was. I even spoke to the head Night nurse, as I was bawling my eyes out crying, trying to get her to explain to me how they could care so little about people. And she stood there with her arms folded grinning at me the whole time. So it's not just one room or one person the way they treat their patients runs Badly through the whole facility. I was sent there because I couldn't walk. When I finally figured out a way to leave my spirit was so broken. I felt so defeated and so un-cared about. Please believe what I say. My husband was recently at Las Colinas after a 4-day hospital stay. He has Parkinsonism and should not be in bed without exercise for that long. He loses all strength in his body and legs and becomes "deadweight" when getting assistance. The rehab at Las Colinas helped him greatly. He had rehab every day and by the time he left after 20 days, he was back to the level of independence he had before becoming ill. That was the goal and it was met. I have to say the my attendance every day may have influenced the good service and I recommend frequent attendance by family/friends for all patients, if possible. Everyone was patient with him and with me. My concerns were addressed. As with most rehab centers in the area, they were understaffed, but everyone seemed to be working hard to meet the clients' needs. read more

Why do the patients not have phones in their rooms, is it a prison or a care facility. Haven't they given up enough I f their independence and freedom without isolating them too? What are you afraid of, they will have the ability to call for help! I am mortified at the lack of respect for my loved one and isolation after being placed in this facility. This place is HORRIBLE. I would give it a zerooooo!! My mother in law was admitted here and the first 2 days she fell off her wheel chair (while supposedly being supervised) and had to be rush to the hospital. A week later she had seizure (which she never, ever had) and again, had to be rushed to the hospital. The doctor there told us it was caused by high blood pressure , which she does have, but maybe wasn't given her medication or something just went wrong. She also had an infection on her behind for not being cleaned properly. This place is just dirty, Staffs are rude, just horrible. When being. Released from the hospital we asked to please not take her back and found a better placement for her in Chino Hills at Trellis Chino and it was the BEST choice. This place was clean, she had her own private room, nice staff.. I can go on and on. read more

Don't send your dear family here! They never answer the phone and Grandma just passed because of their negligence. It's not the staff it's the doctors here couldn't care less about communication of signs of someone declining. The doctors here do not communicate to any family members ever. If you see your family member having signs or symptoms of something pull them out and get them treated elsewhere. Every patient here has the right to call 911 if their emergency isn't recognized by staff here. Grandma developed severe bacterial infection that was over looked by staff and the doctor just so they didn't have to treat her and wanted her out. read more

My grandmother was here at Las Colinas due to her having a stroke and needed to get physical therapy and occupational therapy. All of the staff that worked with my grandma treated her and our family with nothing but kindness. The care is great! Nurses were gentle with her care and her needs. Nurses/staff would answer our concerns that we've had. I would definitely recommend if you need a snf for rehabilitation. read more

Absolutely disgusting here main problem is the higher ups. In example (Trevor) is weird and makes promises he can't keep. The equipment here is either broken or about to be broken ( completely unsafe) which forces cnas or lvns to wait for a lift or bed that's usable. Patients that need an air mattress are denied due to "broken" beds or "just not enough" in which I find mind boggling since they can renovate the rooms but can't purchase new equipment for staff and residents. Secondly the location of 118-123 is horrible poor residents don't even have air conditioning in that section so believe me it gets hot as hell back there. Complaint after complaint and nothing gets fixed point blank they just want your family members insurance money in their pockets so they'll hold them there as long as they can to bleed the insurance dry unless you're "combative" in that case they'll boot you out. Most if not all CNA's here are hard working and actually do care for the residents but there are some that just don't care. If you or a family member needs help think twice before coming here weigh your options out. Plain and simple this place sucks! Including some of the people that live near the building. So be careful with your car or they'll either call the cops to ticket you because you Parked in their "spot" even if you parked correctly and if there's no parking oh well park a couple blocks down you'll be fine according to (iris and Trevor and the other numb-nuts there). read more

Some staff members go above and beyond . They are truly experts and compassionate . Reggie was fantastic. The rehab and p t guys caring. However there are others that are just there. There was a young nurse who failed miserably tending to my father and witnessed her be condescending and sarcastic to my father. This is my mother at Las Colima's on her fist full day. All the staff have been exceptionally professional and very warm. Victor was working at the reception desk and he made sure that my mom was taken good care of with her room. The support staff has also been a amazing with my mom especially Paul in the physical therapy department. That meant a lot to me. In the picture is my mom with her roommates (BFF's). This review more-so revolves around my experience and observations of the hostile and unprofessional behavior exhibited from the "receptionist" Brianna (as she so sarcastically provided me with when I inquired). I came here with my aunt to pick up my grandmother who was being discharged this evening (as I was the transportation). I, along with my aunt and another woman, was escorted inside by a very nice gentleman so we can check-in and wait to speak with someone for assistance (obviously being mindful of social distancing). Mind you, we had been waiting outside for at least 10 minutes and had yet to be properly addressed. An RN came out during this time and we asked if anyone was available and she mentioned there's supposed to be a receptionist but she's never around (interesting)... Out of nowhere, this woman stands at the end of the hallway yelling, "You need to go OUTSIDE, RIGHT NOW!" She emphatically repeated this several times in a hostile manner, not exercising the decency to approach at a respectable distance to advise of this. Trust and believe I will not be addressed in this manner and said so. She exclaimed she was speaking to the other woman as she had told her several times prior she needed to wait outside. Be that as it may, nothing warrants the right you speak to someone in that fashion. Considering you work in a health care environment with sick people, I found it astounding that she had the gumption to address anyone in this manner, with no remorse and an air of entitlement. Even after she expressed she wasn't speaking to me, she continued to be ill-mannered and unprofessional; talking on her cellphone and not servicing those who had been waiting. As it turns out, she's actually one of the receptionists (information provided to me by one of the very friendly RNs). Dear Las Colinas PA in Ontario, California, as someone who is the "front line face" of your facility, she sets the standard on what is to be expected and experienced from the consumer standpoint. Her extreme unprofessional behavior and uncaring attitude does not exhibit the values that your organization so precisely specifies within your mission statement. Please take heed to educate this woman on what it means to have and display proper customer service and ensure that this is something she is capable of communicating when engaging with others. I have already contacted management and I do expect a response to discuss this further. Thank you. read more

My mom was discharged to here from Loma Linda just as a precaution due to past strokes and recovery from Covid. Of course after reading the reviews, we were a bit apprehensive about this place. I know that people look at the aesthetic and write reviews regarding that, but I will base my review on the care she received. Yes, it could be upgraded. But I have to say my mom was extremely happy there, and her therapy was outstanding. They had her up and walking after her Covid quarantine. I do have to mention Maria and Tracy, her CNA's that helped her. They were outstanding!! And I must mention that my dad and I visited her everyday that she was there, once in the early afternoon and in the evening. Did they put on their best behavior because we were there? Maybe, maybe not. But from what we saw and how she was taken care of, Las Colinas gets a 5 star rating! read more

Please don't have your loved ones sent here. Ask for a different place if they are coming out of hospital and need rehab. Very filthy, patients are in the halls, they do not get their patients dressed. My grandmother who was very able to walk was not dressed, left with a window open in a hospital gown and was catching a cold. She was also sent to the "COVID" ward with no symptoms-how convenient. They hold their patients for extended periods and highly medicate to squeeze the most out of medicare. These opinions are based upon my observations and experiences with them. Their rehab department is very professional however, and in my opinion are the only one's that actually do anything. read more

TL;DR "These people kill your elderly loved ones" I recommend steering clear of Las Colinas and San Antonio Regional Community Hospital!!!! My mom was at Las Colinas because she broke her arm and needed around the clock care while she rehabilitates. She was admitted from San Antonio Regional hospital and was on a 14 day quarantine at Las Colinas, mysteriously Las Colinas staff placed another woman in her room during her quarantine and she became ill, she started coughing badly, a day or so later she went to her dialysis appointment and they demanded a covid test, so they sent her back to las colinas for the test; it came up negative, so she went back to davita for dialysis, her catheter was plugged so they sent her to Kaiser to have another catheter placed, at Kaiser Ontario they tested my Mother for covid and she was positive, as soon as I found out, I called for Dr. Ali Mohammed Etemadian, DO, (las colinas staff had him paged, but I didn't hear from him until a few days later). Kaiser held her for a day without doing anything and sent her back to Las Colinas. The next day Las Colinas sent her to San Antonio hospital to change out her catheter, they changed out my mom's catheter, I called the hospital (SARCH) to see what was happening. The nurse told me my mother was discharged 14hrs ago and was waiting for transport which will not arrive for another 3hrs (Las colinas had my mom waiting almost 20hrs after discharge to get her a ride back to the facility). I told the nurse at SARCH (San Antonio Regional Community Hospital) that my mother tested positive for Covid, can they please administer treatment for covid, the nurse (from SARCH) told me that my mom only came for a catheter change so they will not help her with covid, in fact they no longer treat covid, they just treat the symptoms and they gave her "tylenol" for her fever. The nurse from SARCH said that if my mother wanted treatment for covid, she has to go back to las colinas and be re-admitted to the emergency room. At that point I felt helpless and frozen. Dr. Ali Mohammed Etemadian, finally called me a day or so after that and I asked if he can prescribe Hydroxychloroquine and Azythromycin for my mom's covid or give her Monoclonal Antibody Therapy, he told me "no" on the Hydroxychloroquine, but he will order Monoclonal Antibody Therapy from the pharmacy for my mother. Another couple of days go by and they finally get the Monoclonal Antibody Therapy for her, by that time, she could barely breathe. Las Colinas sent her to SARCH due to her trouble breathing (nearly a week after testing positive for covid) and over at SARCH they refused to give her hydroxychloroquine or Monoclonal Antibody Therapy, they said she's not a candidate for it. A day later my mom died, Dr. Ali Mohammed Etemadian sat on his hands and took almost a week to order the Monoclonal Antibody Therapy. My mother died on August 23rd at 4pm and we can't get her cremated because Dr. Ali Mohammed Etemadian still hasn't signed the death certificate, it's 9/18/2021! almost a full month after my mother's death. If there's a lawyer that specializes in this type of case, please reach out to us. Las Colinas I dare you to sue me, you won't like being deposed and what will come out in "discovery" If you like your relatives living, steer clear of Las Colinas and San Antonio regional community hospital, they literally did next to nothing and allowed my mother to pass away.
