The structure of planned activities, restrictions, variety of educational classes, physical therapy and medical care- all designed to provide a higher level of life and recovery for people, who really need it. I believe no one family on its own can do , what is offered in Heritage Park N.C. Our loved one is very satisfied with services, including meals. We feel so grateful that she is in good hands there. The Heritage Park N.C. staff has been great, professional and a pleasure to deal with. Mental illness is very hard to handle on your own, and to know that you are not left helpless -makes the whole difference in the world ! It is not easy to do, what the staff ( all levels ) is doing daily, takes special hearts and knowledge. Thank you. Thank you very much.

Carmen also took the time to alert us to a potential issue with my mom's medical insurance that could have impacted her continued stay and care at Heritage Park after the first week. Understanding how sensitive and life changing this situation was for our family, especially concerning my mom's overall well being, Carmen approached everything with genuine care and professionalism. She helped us clearly understand the potential impact and guided us through possible solutions. Her knowledge, experience, and willingness to advocate for what was best for my mom truly became the calm in the storm we needed. Thanks to her guidance and expertise, we were able to resolve the issue within 24 hours. Carmen has been an absolute pleasure to work with, and we truly appreciate the care, attention, and support she provided in making our transition into Heritage Park seamless. My mom has been at Heritage Park Skilled Nursing for a little over a week, and the professionalism, kindness, and compassion shown by the entire administrative and healthcare staff have made her experience extremely comforting. The level of care, attention to detail, and commitment to providing the best service have been very reassuring. We feel extremely confident that my mom is in the best hands here. The entire Heritage Park staff has been amazing and a pleasure to work with. Working with Anna Alcaraz, Director of Social Services, has been both a pleasure and very insightful. She has played a key role in helping our family obtain resources for legal authorization such as Power of Attorney, as well as guidance for current and future healthcare support and housing or placement services tailored to my mom's needs. My mom has also shared how much she enjoys her physical and occupational therapy sessions and how friendly and supportive the staff has been. The nursing staff has been very attentive to my mom's needs, always taking the time to listen, explain treatments, and address any concerns. Overall, our experience at Heritage Park has been exceptional.

Update: This place…read moredoesn't seem to understand that case managers give us all the information that they relate to them. So in addition to our complaint below, they have also lied and suggested that we threaten them and that they had some sort of Accu safety device on our person. Fortunately, we have a witness who was there during the entire event. None of those things happened. You are seriously warned. Do not go here. This is the summation from our IP letter and conversation... For the record, the day and evening nurses were amazing. In no way am I disparaging the front line workers who are doing their best in mediocre, if at times in certain spaces, deplorable conditions. This care center proved to be an utterly unsuitable and deeply concerning environment for patient care, directly jeopardizing the well-being of my partner. Critical medications, essential for maintaining his mental stability, were abruptly and inexplicably discontinued upon admission, leading to severe mental distress and disorientation. This negligence was a profound lack of oversight and disregard for patient safety protocols, setting the stage for a dangerous situation that could have been entirely avoided. Compounding these medical failures was a shocking display of administrative and case management incompetence. The facility's case manager provided false assurances regarding a thorough patient assessment, which simply did not occur, and later dismissed legitimate concerns from the caregiver. Despite explicit prior inquiries about facility security (during a tour in which the 10+ year admin assured them the facility was secure), the center admitted an at-risk patient to an unsecured environment and then actively refused to prevent him from wandering off the premises into a potentially hazardous situation. This deliberate misrepresentation and subsequent abandonment of responsibility are inexcusable. The culmination of these failures resulted in a terrifying emergency where our patient, unmedicated and disoriented, was able to walk into the street parking log, requiring immediate intervention from emergency services. Fortunately a friend was able to bypass the wandering and get him to remain in the parking lot. Due to the imminent threat to his safety we had him transported to the hospital, where the lack of medications was addressed around the shock of the case (hospital staff had choice words to say about the care facility and none of them were good). There is a severe disconnect between the facility's administration, case management, and nursing staff (WHO ARE AWESOME and doing their best despite the management), ultimately demonstrating a profound disregard for patient advocacy and safety. This care center not only failed to meet basic standards of care but actively endangered a patient under its supervision. The room was busted up, the floorboards warped, people were sleeping or disabled on floor mats, there was no Television in the assigned room and someone somewhere was yelling for assistance for over an hour. The room given had full trash cans, partially used items in the drawers, and the food given was contrary to the dietary requirements requested prior to admission. DO NOT GO HERE.

The facility itself is clean, well maintained, and welcoming which gave us all a peace of mind. What really stands out is how the staff treated us like family. They listened to all our concerns, answered all our questions, and kept us informed. We're hoping with their continued support, my brother will continue to make tremendous progress in his recovery.

This rehab facility is horrible. It is not rehab at all. My mother became sick within this…read morefacility because of their poor care. My mother was sent here to strengthen the bacteria balance in her gut. She was forced to wear a diaper and was not permitted to use the toilet, and she is not incontinent. She would tell them I have to go to the bathroom and they'd say well then go in the diaper and I'll come right back and change you, and they wouldn't be back for hours leaving her to sit in her own waste. The food she was served was so bad sometimes she did not know what she was eating, but she had to eat because she was so hungry. I sent her food from Instacart so that she would have some healthy food to eat in her room. They would limit her water so that she would not urinate which resulted in delirium. She called me in the middle night and her mouth was so dry. I could hear her lips sticking together because they were refusing to give her water. They ignore the call lights. They would leave her in her poopy diaper for hours at a time, causing her to get UTIs. The delirium that she acquired at this place from their inadequate care was so bad that my California family thought that she was in her end times and was going to leave her there. When I called the office and kindly asked them to change her diaper for three hours to change one diaper, they hung up on me. I heard the nurses with unkind angry yelling tones yelling at my mother while I was on the phone and they did not know the phone was open. I texted the management of the facility trying to get my mother help and 90% of the time they ignored me. When company is there or they know the phone is open they are not hostile but as soon as there's no one around, they are. I've already called Ombudsman's to check out this facility. It should not exist; It is that bad. I drove 1800 miles to rescue my mom from this facility two weeks ago. The emotional abuse is one of the hardest hurdles to help her over. Do not take any loved ones to this facility. I will be reporting them to anyone who will listen. My son sent this up accurately when he said "that facility is nothing more than a pound for old people."

My mom suffered a severe stroke and has been under Superior Home Health since November 14, 2026…read more Considering that Sweden left my mom without a nurse to settle her in the home on our very first day of service with them, I shouldn't have been surprised that their assistance would be lacking and uncaring. Jaja (the care coordinator) delayed what should have been a routine urinalysis by twenty days. Her behavior was unprofessional, unpleasant and uncaring 100%. I asked for her supervisor Josie to call me which never happened. Today I finally got to speak with Ena, the Director of Medical Care only to be "redirected " to another home health agency. Again, 100% unprofessional behavior. If the director of medical care penalizes a patient for advocating for her well being and health, imagine how the rest of the staff behave. I asked to speak with Ulysses (the director of this agency) and I was not able to do so. I would never recommend this agency and if I could give them zero stars I would. But that's not an option.
