The RN evaluator is also caring, friendly and very capable. If family is involved and communicates they are willing to work with you for the care of the patient. The housekeeping staff is friendly and kind and always want to help. My mom needed doctor appointments and transportation and this was all arranged by the hardworking social worker. She handles the care meetings and coordinates care as if she was a case manager. She handles the hair salon and even additional nursing attention as needed. She walks around and checks in on patients. At discharge she handled all the needed items for home care and worked hard to advocate for my mom. Admission staff was awesome. They were very welcoming and made sure she had coverage with her insurance. They are great to work with. The doctor that oversees the facility was not easy to communicate to in part due to her availability. She is a full time doctor seeing patients in her office during the day. She made an effort to call back after hours but my own schedule also got in the way. It would be best if the doc attended the care meetings upon admission so goals and care can be discussed clearly. It was difficult to pinpoint care plans. Staff would send message to doc that family want to discuss care and it was hard to receive info back quickly or at all. Maybe if the doc was present at limited but fixed hours on site it would help. PT/OT was good and made an honest and persistent effort to assist recovery. It helps if family is involved and asks questions regularly. My mom was independent prior to her medical issue so the goal was set to return her as close to that as possible. You have to make it known to them where she was before very clearly. Otherwise she may be released too early and becomes a fall risk. The food staff is very nice and concerned. They made an effort to give her Greek yogurt often for her digestion. The food is otherwise not very tasty or presentable and comes from the main facility. I'm sure the quality in Claremjnt Manor assisted living is better than in the rehab unit. My mom ate mostly in her room and so could not ask for extra items from the kitchen as those in the dining room. The dining room was filled with behabing persons that need assistance eating and this was hard to see for her to see while eating. She is a social person otherwise. I would definitely recommend this facility. They have helped her recover through 4 events in the past year and a half. No place is perfect but Claremont manor care center does a great job. They want to help and truly care. We are lucky to have them in Claremont.

I never talked to a live admissions representative. I left NUMEROUS messages on the voice mailboxes of Suzanne, Giselle & Katrina. No return phone call, what-so ever! This is unacceptable. What a job. You never have to return telephone calls. If this facility cannot return a phone call, I am suspect of what type of medical care they are giving to their patients. If you do have family there, and have issues, please call the LA County Ombudsman's office at (800) 334-9473. You can also contact the Medical Board of California www.mbc.ca.gov to file a complaint.     Pilgrim place is a very well established facility I rate this place with five stars they're…read morestaffing there is wonderful I was there for 3 days and they had set up an appointment with me with their social worker physical therapist nurses to do all his medications and what his future goals were going to be staying at Pilgrim place... Physical therapy they have a great room set up with stair lifts and exercise bike and a pole walkway to rehabilitate patients. Meals are also great there I have no complaints there even though my dad is sensitive and eating a few items as he is a cancer patient. My dad was very impressed with the nurses there Dawn to be one of them. Grace who I worked with as I had to file an appeals process with levanta and United healthcare she helped me get this process through as United healthcare was trying to kick out my dad. Unfortunately I filed civil rights against United healthcare and levanta. But that's what it took in order for me to get my dad to stay there longer.. Grace was quite surprised on what I did. They have a very polite staff there. And people that like working with you and care about your loved ones. They also have great activities as far as puzzles a theater room to watch movies popcorn... And a well-provided lounge for guests with water and coffee... I would definitely like to have my dad return here if he could...... He was there back in 2022. And he had a great experience there that was the reason why he wanted to go back to Pilgrim place.. some of the staff members remember my dad there when he was there in 2022 which was a very nice touch since that was 3 years ago to remember my dad as I said these people are definitely there for you definitely a great staff..

    My family had a very difficult experience with Pomona Vista Rehabilitation Center. My mother was…read moreplaced there following hospitalization and required very specific medical care, including strict dietary restrictions related to a feeding tube and recent surgery. Unfortunately, there were serious communication and care concerns during her stay. Despite repeated efforts to inform staff about her medical restrictions, she was given food that she was not supposed to have. This led to complications, including her feeding tube becoming dislodged and requiring an emergency hospital visit. Even after the hospital provided updated instructions, it felt like important information was still not being clearly documented or followed. In addition to the medical concerns, it was very difficult to get timely responses from staff, and we often felt that our concerns were not taken seriously. This created a great deal of stress and worry for our family during an already challenging time. We ultimately requested a transfer to another facility closer to home so we could better advocate for her care. I strongly encourage families to stay very involved, ask detailed questions, and closely monitor care plans if their loved one is placed here. I am sharing this experience so that other families can be informed and prepared.

    My Mom had been a long term patient here from 2017 until July 1st this year…read more This facility is Medi-Cal approved and houses 140 beds. They always have a refreshment cart at every nurses station. For every shift there is a Registered Nurse that administers medications to all the patients on their side of the floor. I know there are at least 2-4 stations. At 06:30 every morning, the nurse was there to check my moms blood sugar levels, and give her meds as needed. Somewhere between 07:30 - 08:00 she was visited by another aid to give her Breakfast. ( She was on a Diabetic Diet) Throughout the day there were nurses to change her, and she was bathed at least once a week. They also painted her nails, cut her hair, and if there were activities she felt like joining, they would lift her with a Hoyer Lift into a special chair and acompany her to activities. There were times I had walked in and witnessed Kareoke going on, and this place is very festive on our holidays. My Mom was always clean, and didnt smell. They took excellent care of her. This facility has a pretty good structure of how they implement care. I think if your loved ones are nice the nurses will appreciate this. You have to remember, this is a huge facility, so of course there may be times when your loved one will have to wait until a nurse can get to them, and sometimes, nurses get sick too. I read some of the other reviews, and I do not agree with horrible conditions patients are in. There are patients that may be incapacitated, or confused, some of them just hang out in the hallways, and its the nurses that are always watching out for them. My Mom has been in different rooms and has experienced other patients that do nothing but cry out, but not because they weren't cared for. That was probably the most annoying moment for my Mom. I have seen the same nurses care for my Mom for years, when she had to have emergency gall bladder surgery, I was glad she was there for them to notice it, and get her to the hospital right away. She had different nurses when she came back. But they were all so good to her. I think if you get involved with your loved one, and take some initiative to get to know the staff, you will find that everyone from the Business Office Manager, the receptionists, the activities director, the Social Services, and the Registered Nurses along with the extra help, these are all genuinely caring people. These people are doing the things that you aren't or won't to care for your loved one. It's not an easy job! If you're concerned about your loved ones laundry, then do it yourself and just drop it off to them. If you are worried about your loved one not getting enough attention, then go visit them more often. If you think that your loved one isn't treated fairly, consider your loved ones attitude, or state of mind before placing blame on anyone at this location., They are all paid to do a professional service and that is exactly what they do. I will close by stating that out of all the local Medi-Cal approved skilled nursing facilities in Pomona, Upland , and Ontario, that this one right here, has the most genuine and caring staff over all the other facilities that my Mom has been in. Community Extended Care of Montclair is the Best! Thankyou for taking such good care of Marylou Castro. Everyone here deserves 5 stars! If there is an open bed, take it!     I would never trust this place with my family again…read more My godfather was recently placed at Community Extended Care at Montclair to complete a 10-day course of IV antibiotics. While most staff were courteous, our experience with one particular nurse--the nursing supervisor, Jamie --was troubling. I was the patient's translator and god-daughter (he does not speak English). Whenever I raised basic concerns of the patient, such as nausea and stomach pain, Jamie repeatedly dismissed the needs ("he never told us that"), gaslit me, or deflected by shifting blame("it's all the previous hospital's fault"). When I calmly asked a clarifying question, Jamie, the nurse, responds with personal defensiveness, saying things like "I do so much for this family," or "I don't like the way you're talking to me," rather than addressing the patient's actual symptoms. (she seems unable to tell legitimate patient needs from attacks or blame) As a result, Jamie engages in guilt-tripping: "so you don't know what medication he was taking and you're calling to complain about what? I can't go off of nothing." (I called to complain about the patient's pain) Jamie is the one who failied to read the patient's files/packet, which clearly states what medications he was receiving. Worse, Jamie escalates by calling the POA to reframe my advocacy as "problems caused by someone without medical knowledge". Of course, POA did not believe in anything Jamie said, but had to pretend to be nice to her just to deescalate so that she would do her job and address the symptom. This kind of triangulation and manipulation is disturbing. This placed the patient -- a vulnerable, elderly person -- in the middle of a power dynamic he never asked for. Advocating for my family should not result in retaliation, mischaracterization, or fear that the patient may receive worse care as a result of advocacy. I hope this review serves as a warning to others, especially families caring for elderly. Communication and accountability are not optional -- they are the baseline. Due to Jamie's behaviors, I doubt this facility's ability to provide this.
