I urge you to carefully consider this before contracting with any hospice provider. Unfortunately, our experience with Faith & Hope made an already painful situation even harder. In April 2025, my mom suffered a massive stroke. She was placed under Continuous Care (CC), which meant she required critical, around-the-clock nursing support. We were told her case would be re-evaluated after three days. However, the care was stopped in less than 24 hours and we were given only 4 hours' notice that services would be discontinued. At that point, it was clear our needs no longer aligned with Faith & Hope's philosophy of care. We scrambled to hire private help on extremely short notice. That night, my mom was in significant pain & experienced agitation. She needed her medication adjusted and an increase of the dosage frequency to help manage her symptoms. The next day, we made final attempts to Faith & Hope to reassess mom's eligibility for CC, even though the on-call night nurse, when we asked, did acknowledge that we were free to seek another hospice provider. We waited all day for a response from Faith & Hope, but heard nothing. Faced with another evening without adequate care, we transferred my mom to a different hospice. They gave us the right care without hesitation. Sadly, our mom passed away only 4 days later. The only positive experience we had with Faith & Hope was with Nurse Theresa. She cared for my mom with compassion, dedication and a true commitment to patient advocacy. Theresa's thoughtfulness and support helped guide my brothers and me through an incredibly challenging time, and for that we are deeply grateful.

    TERRIBLE & TRAUMATIZING EXPERIENCE , Would not provide a suitable wheelchair for my mom, but they…read morereferred me to a person they recommended. I paid $1200 out of pocket, only to find out that the wheelchair was far too large for my mom's small frame. Their referral would not exchange the wheelchair for a smaller size, nor would they refund any of my money for a wheelchair my mom was unable to use. I was stuck with the entire bill of $1200 and they provided no financial assistance whatsoever I ended up donating the wheelchair to the nursing facility. The worst experience of all occurred the day my mom passed away. They delivered my mother's body to the wrong crematorium, which was located approximately 30 minutes from the correct funeral home. lNever received an apology, nor did I receive a call or a single response from them, despite my countless attempts to contact them by phone, email and text. Horrific experience!!! I wish I could give them ZERO stars

Unique Hospice stands out…read morethe best from my first call to Vanessa made me feel very comfortable and I knew right away. I made the right decision from the first visit with Ness was absolutely wonderful when I saw her how she treated my loved one as family and the compassion she had for her Jen was awesome also. Joyce the social worker always checked in and made sure we were doing OK but also checked in with our loved one that was dying of cancer these women are a great team. They made my loved one feel very comfortable and felt like family. We were treated the same. I would highly recommend this company to anyone that's going through the tough time of losing a loved one

    Recently had a family member placed under hospice care with this company. It was a new experience,…read moreso didn't really know what to expect. For the most part things went well, when we needed items to be replenished they did so with a bit of pushback at times but we received them promptly nonetheless. The care from the home health aide and LVN were for the most part great, no complaints there! Thorough and helpful by nature. One concern was the RN that made it a habit on more then one occasion to have my family member clean out their own ostomy bag even though my family member was bed bound and would not be able to properly clean their hands afterwards. I will be reporting said nurse to the appropriate agency. Only other disappointment was after my family members passing, no real attempt was made by their counselor or staff to help with the grieving process which seemed like such a harsh disconnection.

It was a challenging time for us, but the warmth and dedication of the staff made all the difference. We found them on Medicare.gov with 5 stars rating and they definitely deserve this rating. They also have multilingual staff on board which was very helpful for my father-in-law. Starting from the administrative staff (Veronica, Leticia), and Social Worker (Ms. Bi) were very responsive to our calls, very knowledgeable and informative, resourceful accommodating, and always ready to help. The nurses (Sarah, Daisy, Vicky, Socorro, Angela, Fei, Jessica, Agnes, Elia, Mel, Daniel, Rose, Divine, Zenny, Hannah, and several others); nursing assistants (Maria, Alex, Moses, Gloria, Lupe, Juan, Linda, Yolanda, Vilente, Maple and several others); physical therapists (Rowena, Sue, Rosa, and several others); nutritionist (Norma and staff) were all highly skilled but also incredibly kind and loving, patient, and accommodating. They worked as a team and tirelessly helped my dad 24/7. Every aspect of his care was handled with compassion and professionalism. From medicine feeding, incontinence changes, careful bathing, to well-prepared and personalized meals assessed by the oral therapist and nutritionist, and laundry cleaning, they took good care of him. They also kept us informed every step of the way, providing timely updates on his condition. Beyond medical care, they also created a warm, welcoming environment. The facility was kept cleaned and well maintained, and they thoughtfully decorated the place for every holidays making it felt like home.     My grandma had stayed there 2 times and here is the experience:…read more The first time: Grandma said that they wheel you into the shower, strip you of your clothes then spray you with a hose like a dog. Then they leave you soaking wet to get clothes. Imagine how cold that is. The last time: Grandma's condition got worse and was transferred here from the ER. She couldn't move, so they had to turn her every few hours. After a few days, they called to say "you gotta take her back to where she came from. she isn't like this the last time she was here and she is too much work. wherever she was before, take her back now." I said she had been home but now she needs professional help since she is still on IV and we don't know how to do that. Few days after that initial call, we got a call on a Sunday night and they said she stopped breathing. I said what? The lady on the phone said she stopped breathing and now we sent her to the ER. When I saw the ER doctor, he said "her condition wasn't that bad to a point that she would die. something happened at the nursing home". I was there when she woke up from the coma, and she was in such terror like she woke up from a terrible nightmare like the end of the world. I don't have to be there and I know exactly what happened.

So far, I have no complaints when it comes to their care! Therefore, 5 Stars for the nurses there. But.... As far as professionalism, NEGATIVE all stars!!! My mother and uncle used to be her caregivers till they were no longer able to since she had her surgery, they are there for all of her doctor appointments which is several times a month, they arrive early. Might I add, they live about 45 minutes away, and with traffic, that is about an hour or sometimes more. Keep in mind, Vista Cove and the Hospital she visits is only 15 minutes apart. On two separate occasions the people that prepare her to be transported to the hospital WERE LATE!! I am not talking 5 or 15 minutes, I am talking 45 minutes and 1.5 hours!!!! both times the doctor refused to see my Grandmother, so both times we were forced to reschedule!!! The 2nd time they were ridiculously late, my uncle had called them, they said they were having car trouble, whatever... okay so, did you ever think to use another company to transport her? that would be the RESPONSIBLE thing to do! I am a pretty understanding person, I know that car troubles happen, but what are the chances of two separate occurrences weeks apart?!!!! that to me is no excuse!!! someone was slacking off there. So as soon as I hear of this happening again, I contact the MANAGER named "ROSE" She informs me that she was super "embarrassed" of that incident and she apologizes, but she was NOT sympathetic, I could sense in her sentiment that it was NOT genuine and she was rushing me off the phone, to the point where she almost hung up, and I had to tell her to wait! I told her that this was the second time this happened and its unacceptable, once is okay, but this is affecting everyone, its not just about the transportation vehicle, this is about responsibility. It was HER responsibility to not only make sure my Grandma is well cared for, but also to see to it that she she makes it to her appt on time! and you know what Rose told me?.... "Well its not just your grandma, we have other patients we need to drop off too!" Me: "I understand that, so are you saying you are continually dropping them off late too?" Rose: "No, they are not late" Me: "Okay so why is my grandma late all the time?" Rose: "Like I said, things happen, if this is a problem then you need to find another company to transport her" (in a rather rude tone) Me: "So you're saying I need to hire another company to transport my grandma?!" (returning the rude sentiment) Rose: "No , we will find another company to transport her" (at this point, her rudeness subsided) Me: Okay well who is going to let me know as soon as this happens? I want that person to give me a call directly. So, it turns out, the person in charge of transportation is named "Andrew," he is supposed to give me a call. I will update this on her next visit to the doctor and see if any changes are implemented! Bottom line, the manager Rose was not willing to help... only as my tone changed, then she stepped it up. Very unpleasant to deal with.

Current employees are…read morebeing forced to work and not allowed to self-quarantine due to claims of low staffing issues. While licensed employees stayed home to protect their loved ones. Other staff members, predominately Spanish speaking are being forced to report to work. Facility has not been sanitized to re-operation standards. Workers have not been provided Covid-19 pay for their non-stop dedication since March. If you know of a loved one at this facility, please inform yourself. Call and demand answers. The workers here are a tremendous and dedicated team. Help them out if you can.
