We love my Dads CNA Herlinda. She provides beyond amazing care for not only my father but all her patients. She always has a smile on her face and is extremely empathetic to all of her patients needs. She goes above and beyond the call of duty. My father is always perfectly clean while under her care. It is extremely apparent when she does not care for my father on her off days. We are extremely grateful and beyond blessed to have Herlinda as my fathers CNA. We would love to recognize her as not only the best nurse but also as just a God sent Angel that came into my fathers life when we needed her the most. Thank you Herlinda you are the best! My dad and our family love you so much and will be forever grateful for you

Update: This place…read moredoesn't seem to understand that case managers give us all the information that they relate to them. So in addition to our complaint below, they have also lied and suggested that we threaten them and that they had some sort of Accu safety device on our person. Fortunately, we have a witness who was there during the entire event. None of those things happened. You are seriously warned. Do not go here. This is the summation from our IP letter and conversation... For the record, the day and evening nurses were amazing. In no way am I disparaging the front line workers who are doing their best in mediocre, if at times in certain spaces, deplorable conditions. This care center proved to be an utterly unsuitable and deeply concerning environment for patient care, directly jeopardizing the well-being of my partner. Critical medications, essential for maintaining his mental stability, were abruptly and inexplicably discontinued upon admission, leading to severe mental distress and disorientation. This negligence was a profound lack of oversight and disregard for patient safety protocols, setting the stage for a dangerous situation that could have been entirely avoided. Compounding these medical failures was a shocking display of administrative and case management incompetence. The facility's case manager provided false assurances regarding a thorough patient assessment, which simply did not occur, and later dismissed legitimate concerns from the caregiver. Despite explicit prior inquiries about facility security (during a tour in which the 10+ year admin assured them the facility was secure), the center admitted an at-risk patient to an unsecured environment and then actively refused to prevent him from wandering off the premises into a potentially hazardous situation. This deliberate misrepresentation and subsequent abandonment of responsibility are inexcusable. The culmination of these failures resulted in a terrifying emergency where our patient, unmedicated and disoriented, was able to walk into the street parking log, requiring immediate intervention from emergency services. Fortunately a friend was able to bypass the wandering and get him to remain in the parking lot. Due to the imminent threat to his safety we had him transported to the hospital, where the lack of medications was addressed around the shock of the case (hospital staff had choice words to say about the care facility and none of them were good). There is a severe disconnect between the facility's administration, case management, and nursing staff (WHO ARE AWESOME and doing their best despite the management), ultimately demonstrating a profound disregard for patient advocacy and safety. This care center not only failed to meet basic standards of care but actively endangered a patient under its supervision. The room was busted up, the floorboards warped, people were sleeping or disabled on floor mats, there was no Television in the assigned room and someone somewhere was yelling for assistance for over an hour. The room given had full trash cans, partially used items in the drawers, and the food given was contrary to the dietary requirements requested prior to admission. DO NOT GO HERE.

When we ask them where are the items no one knows anything. We buy more, clothes, shoes, grooming products and cell phones. All comes up missing and no one ever finds the items. This went on for almost two years. The health care is worse. They continue to let our love one suffer until an infection would come. They send our love one to the hospital. The hospital get our love one back strong and healthy only to be sent back to this place to get another infection, vicious cycle. This facility has even let my loved one fall in the back of a transportation bus, because they did not properly strap him down before they started transporting him. This maybe a training facility, because they continue to have new staff that doesn't seem to know what they are doing. Several reports has been made to San Antonio Regional Hospital and the State and this place is still horrible. My love one died this week from the miss treatment from this facility, and guess what my family went to retrieve my love ones belongings and they had nothing. No glasses, cell phone, clothes, shoe's nothing. Please take your family member to another place not here. I bet you are saying to yourself why didn't we change facilities. It's because this place take all the insurance money up front, so there is nothing left for you to take your love one somewhere else. As we were there to puck up his belongings, other patients heard us talking about how everything is missing and other residents started to tell us that their things are also coming up missing as well.

Today is 11/12/2025. I'm a Veteran. I rarely rarely…read morecomplain about service from anywhere and I get how workers can be busy and it can take time sometimes. I had a full knee replacement surgery from VA Loma Linda Hospital and I choose here for rehab because I'm closer to my family. Anyways around 11pm I was getting up so I can walk around and the bed wheel was unlocked for some reason and I fell and hit my hip on the floor and I press the emergency button but no one showed up for 15 mins so i literally had to call the front desk with my phone. I hear front desk announcing it but still no one showed up. I called front desk again because emergency button didnt work again but this time they didn't answer my phone call. I can literally hear the phone ring but no one is picking up the phone. this guy said that no body wants to work because they're leaving work or something like that. Really??? What if I fell from the toilet and no one helped me for 15-20 mins. Then what? And this was at 11pm when most patients are sleeping. Also at 2:10am I asked for medicine so I can go to sleep without pain. Nothing @2:30am so i pressed the emergency button and someone showed up and the guy said let me find out. Nothing for another 15-20 mins I press the emergency button again and he said the the nurse who has the key is on break. Really??? I mean wtf? There are over 220 beds here and you're going to take a break with keys on you? Unbelievable. Took almost an hour to get my one painkiller. SMH. I'm in so much pain right now. Hopefully tomorrow morning I can reach back to VA hospital to transfer me to a different rehab facility. Also nurses, LVN, CNA are just chatting outside, joking and laughing. Which is fine but do your job and communicate more. When you're waiting they just leave you clueless

The RN evaluator is also caring, friendly and very capable. If family is involved and communicates they are willing to work with you for the care of the patient. The housekeeping staff is friendly and kind and always want to help. My mom needed doctor appointments and transportation and this was all arranged by the hardworking social worker. She handles the care meetings and coordinates care as if she was a case manager. She handles the hair salon and even additional nursing attention as needed. She walks around and checks in on patients. At discharge she handled all the needed items for home care and worked hard to advocate for my mom. Admission staff was awesome. They were very welcoming and made sure she had coverage with her insurance. They are great to work with. The doctor that oversees the facility was not easy to communicate to in part due to her availability. She is a full time doctor seeing patients in her office during the day. She made an effort to call back after hours but my own schedule also got in the way. It would be best if the doc attended the care meetings upon admission so goals and care can be discussed clearly. It was difficult to pinpoint care plans. Staff would send message to doc that family want to discuss care and it was hard to receive info back quickly or at all. Maybe if the doc was present at limited but fixed hours on site it would help. PT/OT was good and made an honest and persistent effort to assist recovery. It helps if family is involved and asks questions regularly. My mom was independent prior to her medical issue so the goal was set to return her as close to that as possible. You have to make it known to them where she was before very clearly. Otherwise she may be released too early and becomes a fall risk. The food staff is very nice and concerned. They made an effort to give her Greek yogurt often for her digestion. The food is otherwise not very tasty or presentable and comes from the main facility. I'm sure the quality in Claremjnt Manor assisted living is better than in the rehab unit. My mom ate mostly in her room and so could not ask for extra items from the kitchen as those in the dining room. The dining room was filled with behabing persons that need assistance eating and this was hard to see for her to see while eating. She is a social person otherwise. I would definitely recommend this facility. They have helped her recover through 4 events in the past year and a half. No place is perfect but Claremont manor care center does a great job. They want to help and truly care. We are lucky to have them in Claremont.

My brother got accepted in the program and is now more than half way accomplished. Things are getting tough for him with the meetings because he is a shy person but the counselors there are very helpful and supportive. Marvin (case manager) took the time out of his busy schedule to sit with us and counsel not only my brother but me as well. He let us know that it is normal to feel like leaving the program and he let me know that my brother trusted me for letting me know he was having feelings of wanting to leave. My brother is now saying he doesn't want to give up, he doesn't want to quit and he wants to graduate. Marvin went as far as to showing us a graduation diploma to show us the end results. I am so grateful for every person in that program. Bear , Tory , Melanie, Wanda and also the clients. They are all very helpful , His House has a deep family vibe , that is really hard to find in any facility. They are also very thorough in making sure the establishment stays safe by having safety guidelines for visits. I was so impressed to see my brother studying the Big Book. I can go on about the great things His House is doing for the clients. As of now my brother still hasn't graduated His House program, but I know he is in great hands. The staff is very knowledgeable , very educated and very experienced.

I was targeted by Brittney, she would just spread personal information that fell under HIPPA to other clients and staff. I am extremely infuriated at the way they treated me when I was put on a second behavioral contact over a misunderstanding. They then literally watched me pack my stuff and kick me out to the street, and told me to find my own way home. I will be pursuing legal action for this mistreatment.

My family had a very difficult experience with Pomona Vista Rehabilitation Center. My mother was…read moreplaced there following hospitalization and required very specific medical care, including strict dietary restrictions related to a feeding tube and recent surgery. Unfortunately, there were serious communication and care concerns during her stay. Despite repeated efforts to inform staff about her medical restrictions, she was given food that she was not supposed to have. This led to complications, including her feeding tube becoming dislodged and requiring an emergency hospital visit. Even after the hospital provided updated instructions, it felt like important information was still not being clearly documented or followed. In addition to the medical concerns, it was very difficult to get timely responses from staff, and we often felt that our concerns were not taken seriously. This created a great deal of stress and worry for our family during an already challenging time. We ultimately requested a transfer to another facility closer to home so we could better advocate for her care. I strongly encourage families to stay very involved, ask detailed questions, and closely monitor care plans if their loved one is placed here. I am sharing this experience so that other families can be informed and prepared.
