My mother was sent to Heritage Park to continue her recovery after spending two weeks in the ICU. Even before her arrival, we were contacted by Carmen Rivera, the Admissions Director, who greeted us with a warm welcome and immediately made us feel supported during a very difficult time. Carmen also took the time to alert us to a potential issue with my mom's medical insurance that could have impacted her continued stay and care at Heritage Park after the first week. Understanding how sensitive and life changing this situation was for our family, especially concerning my mom's overall well being, Carmen approached everything with genuine care and professionalism. She helped us clearly understand the potential impact and guided us through possible solutions. Her knowledge, experience, and willingness to advocate for what was best for my mom truly became the calm in the storm we needed. Thanks to her guidance and expertise, we were able to resolve the issue within 24 hours. Carmen has been an absolute pleasure to work with, and we truly appreciate the care, attention, and support she provided in making our transition into Heritage Park seamless. My mom has been at Heritage Park Skilled Nursing for a little over a week, and the professionalism, kindness, and compassion shown by the entire administrative and healthcare staff have made her experience extremely comforting. The level of care, attention to detail, and commitment to providing the best service have been very reassuring. We feel extremely confident that my mom is in the best hands here. She has played a key role in helping our family obtain resources for legal authorization such as Power of Attorney, as well as guidance for current and future healthcare support and housing or placement services tailored to my mom's needs. My mom has also shared how much she enjoys her physical and occupational therapy sessions and how friendly and supportive the staff has been. I want to thank every single RN, LVN, CNA, and his social worker, Sandra, and physical and occupational therapist who helped Mike through these very tough times with his recovery. He had a really hard time with his recovery. His whole medical team there understood his frustration and what he was going through. They were so caring and helpful and never got upset with him on his bad days. This was the 3rd facility that he had been in since he fell and broke his hip and fractured his shoulder. Heritage Park Nursing Center - Rehabilitation Center Near Me - Upland, California

Be warned this place was a nightmare, if you have loved ones I would not recommend this place at all. I feel sorry for anyone that is forced to stay there for an extended amount of time. This place made San Antonio and Pomona Valley Hospitals seem like the St.Regis or Ritz Carlton. The food was atrocious, I am diabetic, they kept giving me food that even I knew I shouldn't have because my sugar level was not controlled, for example for lunch and dinner I was given a cold sandwich on white bread with a bag of chips and regular fruit juice. If you know anything about nutrition this was terrible to give to a diabetic with high uncontrolled blood sugar levels. The nurses and their support staff were actually nice but they are so understaffed and overwhelmed that I can really blame them. The physical and occupational therapists are good. However the administrators are biggest lying pieces of crap trying everything to grab every dollar to charge you and your insurance. When I was at Pomona Valley I was put on 6 weeks of IV antibiotics, I did 3 weeks at Pomona Valley and was relocated to Heritage Park to do the remaining 3 weeks and get physical therapy. My third week there, mind my experience had been horrible the entire time, I'm thinking I'm done and will be leaving soon. They tell me that is incorrect, I have 3 more weeks to go and I argued with them about that. The administrator insisted I was wrong and has 3 more weeks to go. So I asked to be put on home health where I could go home and a nurse would come to your residence and administer the I.V. antibiotics(I've had this done before.) The administrator told me I could because insurance would not pay for that(That is completely wrong because from a money perspective it would be cheaper for insurance that way) so it made no sense that insurance would deny that. Forget the fact that the original hospital doctors said 6 weeks, the administrator was making it 9 weeks just fraudulently raise the bills. I only escaped out of there because I had to be taken back to the hospital. I had gained 50 pounds of extra water weight and was so swollen with water I was actually gurgling while breathing. A relative of mine who was a nurse came to visit me and saw what was happening and made them take me back to the hospital. SO BEWARE THIS IS A HORRIBLE PLACE to have your loved ones stay. I spent 2 weeks at Heritage Park, and it was an amazing experience. The staff, the care provided, the facilities, the activities, and the kitchen were all 5 stars. From the moment I arrived, the attention given to me and the care I saw given to patients was excellent. Daniel, Brenda, Blanca, and Arlene in the nursing staff were amazing and key to my recovery, Daniela & Cynthia in activities were a daily pleasure, Austin, Amy, Allison and Chris were great in Therapy. Sandy and her partner in social services were amazing. Thank you, Marisol, for all of your help. When away from home recovering from serious injuries, Heritage Park Nursing was the best. I recommend this facility for any situation as their care was exceptional. Our journey began when our dad was placed in a horrible nursing home directly from the VA Loma Linda Hospital with no warning. One Flew Over the Cookoo's Nest should explain it! After some research I got hold of Carmen in admissions at Heritage Park in Upland, she was absolutely fantastic and worked hard to get him transferred out of Riverside and closer to us. We have been very please with everyone at Heritage and the around the clock care is fantastic. Each time we have been there we are greeted with smiles from everyone, the place smells clean (and is) communication with the RN's, LVN's and CNA's is a true blessing. We felt lucky to have our loved one so close and well taken care of. To top it off our greatest non medical help is with the social workers office. Sandra is not only knowledgeable as to what we needed but took the time to help us transfer our dad out and to an assisted living facility. In addition Mike, the administrator, listened any time we had a need and was on top of it! I could go on about the physical and occupational therapy, the kitchen staff, the custodial and the front desk because they hold there own too. They are extremely caring very timely with their follow-up attentive to Mom and her needs as well as to any requests I have asked. The entire staff are cheerful and supportive of residents and family needs. True comfort to know Mom is well taken care of when family cannot be there. I just finished my sister's "Care Review" with Sandra Franks. I rarely feel anyone in this world is doing their job well. I can't tell you how comforted by the fact that the staff at Heritage Park (including Sandra) are doing EXTREMELY well. My sister has been at HP for a number of years now, and she (given the circumstances of her condition) LOVES it there. We have looked at moving her closer to me, but she is so happy here with the care she receives; we just can't. The facility is clean, well-managed and the staff caring. Mike Akira, the administrator, does a great job with the programs and care schedules. Sandra Franks, the head of social services, is available and ALWAYS helpful. And the nurses! I've been staying at Heritage Park recuperating from an infection. Went out to dinner with friends tonight. When I came back around 9:00 pm, the front door was dark, there was no one at reception, and the door was unlocked - no guard of any kind. Any Tom, Dick , or Crazy could walk in and raise havoc anywhere in the building. This is totally unacceptable. I'm not yet sure how to report this to regulators but I will find a way. I will also transfer to a different facility if at all possible. I feel that this place is not safe. (It's also not very clean.) read more

I have spent the last three weeks at Heritage Park. It has been a a nice place for recovery and preparation for surgery. I would like to thank the staff for ther care. I would like to thank Herlinda, Baltazar, Maria, and Moultan for there superior care as CNA'S. The PTand OT Therapy staff led by Leslie for getting me ready for my next stage in my medical adventure. The Activity staff for opportunities to interact with other Residents. The Nursing staff who were all attentive to my medical needs. All the Nurses did a great job. Can't name all of them. Terrible rehab care facility, even though the nurses there are nice , the facility is run where they just keep patients sedated so they are unaware of their surroundings and do not get any rehab therapy that they went their for. Then they want to discharge them for not making any progress when they keep the patient sedated on Morphine & Norco drugs all the time. read more

This place is a hot mess. The only saving grace was the admissions director Jeremiah who was responsive on a holiday and hit my mom admitted. The staff to patient ratio is awful there were times that my mom had rung the call bell to use the restroom and she soiled herself waiting for 30-60 minutes. At one point a CNA told her she had to wait because she had to pass dinner trays first!!! My mom was put in adult diapers so they wouldn't have to help her to the restroom She was left in soiled diapers do many times she developed diaper rash. Overall the staff are caring but just overworked to really provide the kind of are that I'm sure they are capable and willing to provide if they could. I must say that Grace ( nurse who dispenses Med's) and CNA Edmon are incredible and have treated my mom with kindness & respect Please be aware that it is policy at this facility to give each and every patient a stool softener needed or not. My mom was experiencing extreme gas and stomach cramps and I couldn't figure out why. When I asked one of the nurses what they thought it could be their answer was "must be the stool softener" My mom is a type 2 diabetic but this facility will not prepare the proper meals for her. Here everyone gets the same slop no matter what. And when I say slop I mean slop! The only decent thing they make is grilled cheese or PBJ Cleanliness is another issue. At most healthcare facilities I've been in cleanliness is evident here it's not. I think they may vacuum the hallways once a week. The patient rooms are ok but there's a huge cobweb on my moms wall. They leave paper on the floor for days. I've seen cheap hotel rooms that are cleaner. Nothing has been done to this facility since it opened its doors in the '80's. If you can avoid putting your loved one here please do it. Try Inland Christian in Ontario instead. read more

My friend was there for multiple Months... after 4- 5 MONTHS they discovered the Wound she came in with, which they had Only repeatedly & daily- changed Bandages on, was actually an Infected BONE. It took the suggestion of a specialist she saw to Xray it & see this is why- it was NOT Healing. Every staff member from the front entrance, nurses, nurse aids, physical therapists, PT aides, medical records, business office, food and cleaning crews were kind, helpful, warm and respectful. Asia, one of my dad's first nurse aid was truly warm and kind. Helped me feel at ease knowing she was there helping my dad. Caroline in admin was extreme helpful when having to transition my dad into their nursing facility. Communication was quick and easy. Sandra and Alex in the Social Services office have been nothing but kind and helpful. My dad's sudden change in his health was also life changing for myself and family. I live in Northern California and had to make abrupt and fast adjustments in our lives to care for my dad. Working with Sandy and Alex for all needed paperwork and navigating the health system made it so much easier for me that I was able to continue caring for my dad despite living so far away. I can't say enough for the people that work at Heritage park in Upland California. Herlinda was the most fantastic worker and caregiving women work in there along with Elizabeth.. Plus Ana and all the other workers I forget their names. As far as he goes for a physical therapist, I love them all they were hard on me but they were very good especially Michelle. Michelle is a tiny little blond but strong as hell. Michelle was classified strong as Hulk Hogan The professional wrestler she was tough you got a love her she meant well and did well. I'm not sure I can write the right words to describe the great work these professional nurses, physical therapy team, and staff have done for me. One day I am living a normal life and the next day I can't walk and in great pain. After being release from the hospital I was transfers to this location. I didn't know what to expect but when I arrive my nurse Jasmine Garcia checked me in and made me feel at home. Her welcome and care for me for the next 4 weeks was amazing and I will never forget her. She's my shining star. But I also owe a great thank you for the all my nurses and staff who helped me out so much. Nancy , Hugo , Lisa , Karina , Natalie, Elizabeth , Yaquelin , B.J., Narine , Joel, Kevin , Paul , Debra , Motan, Herlinda, Anabel, Maria, Jennifer, Jessica, Jessica (Many nurses name Jessica, Thank you) and Pasty who always made sure we all had mask to protect us from the spread of COVID-19 spread. I would NEVER recommend this nursing center to anyone. Despite somethings being good, there was too much that was wrong. So I will begin with the bad and gladly share the good. My sister was here for about a month and in that time the facility really only cared about being paid after her insurance ran out. The social worker initially seemed nice; however, failed to really return my calls and questions regarding my sister's care. So I tried to get the LVN's on the unit involved and was passed from one person to another without nothing being addressed, resolved or a care plan to truly aid my sister. I went 4-5 a week day, nights and weekends to visit her and I would hear her and other patients telling me how they were ignored for hours while they tired to get someone to help them with their daily living needs. I grew tired of the social workers or administrators not retuning my calls or messages. So went faithfully at different times to change her clothes, clean her room, change her bed liens and more. The final days of my sister's life at this center appalled me, I could not get her LVN to address her wound care and I begged both social worker or SOMEONE to please help my sister. Sadly, her condition worsened and she is in hospice now. The good is that there were about 4 staff members who really cared, and worked hard Edmon, B. Peterson, Victoria the LVN and Michael. They were WONDERFUL with my sister and THANK YOU to them for what they did for her. And I'm sorry if at times I asked too much; however, it was for my sister and I just wanted her to be happy in the last days of her life. read more

If you care about your loved one do not send them here. My grandma went in with no sores came out looking like she got attacked by a shark. A huge chunk is out of her leg due to these people not moving her properly. Also my grandma had 2 robes and both went missing. They obviously are stealing OR just constantly misplacing there items.. And what is up with the 24 hour request before visiting? Is it because you guys don't take care of them and u have to get them ready for there visit so no one suspects anything? My family is suing this place due to them not taking care of my grandma who will be passing away tonight. Thanks a lot . read more

My FIL is a current patient at this facility, he's a new patient. He's Diabetic and recovering from Quadruple Bypass Surgery. His blood sugar was 300 today, three hundred, because he has not been given his insulin for the past two days. This is unacceptable! His nurse this morning told him his chart says he's diabetic, but doesn't say he needs insulin. I've just emailed your facility. If anything happens to my FIL, Earl Thorn, because he did not receive his insulin, your facility will be held accountable. read more

I do NOT agree w/the previous reviews. My diabetic father was sent there for after surgery wound care and in less than a week he worsened. . . Also, most nursing staff are very lazy and unwilling to help. If it isn't their job the patient has to wait until the appropriate nurse is available. Other than Herlinda his nurse and the physical therapist I felt hesitant to ask questions because I felt my father's treatment was at risk if I miss spoke. read more

My Mom Fe had a really wonderful experience during her recovery at Heritage park Nursing Center. The staff are very helpful from the nurses, CNAs, physical therapist,OT, dietary and social services.They really work collaboratively for the care of my mother. I would like to thank Sandra (SW coordinator), Asia, Karina (nursing care), Hugo & Baltazar (bedcare), Luis (wound care), Jonah (admin), Allen (Front), Carolina (customer service) PT dept., Dr. Werts, along with the rest of the staff that cared for my Mom. She stayed Dec 1, 2021 through Jan. 19, 2022. Considering the Covid restrictions we always felt that the staff appreciated our daily visits. Safety protocols were followed. Mom says the food portions were good. When she wanted something different she was always accommodated. I wish all staff would wear their badges so their names and titles can be seen. This is helpful to the patient and family members. Whenever we had questions or concerns we felt that the staff was interested in addressing or helping us to solve the issue. My mom came home on hospice and this process was supported and facilitated by Sandra who was always available to help with or direct our questions. Thank you Heritage! read more

My grandmother had to come to this facility for a couple of weeks after a fall she had. Let me start off by saying that there are a few people at this facility that are wonderful and really care. However just because the day nurse is great, doesn't make it ok that the night nurse is awful, who let her sit in a dirty diaper for 6 hours until my family was able to come in the morning. My grandmother would call for help for 6 hours, but no one would ever even bother to come in the room because my grandma spoke Spanish and I guess no one in the entire facility speaks Spanish so they just thought it be best to ignore her and not even go in to see if there was anything they could help with. They literally said their excuse for not going in to see what she wanted because she speaks Spanish. My grandma speaks Spanish but she also speaks broken English. How do you ignore a call that is going off for 6 hours? I do have to say that these nurses are pretty great at ignoring calls. Every time I went to visit many call lights are beeping and the staff is just chatting away with each other like the calls don't even bother them. One time I went to visit on a Saturday and a lot of the patients are out in the hallways. As I was leaving one of the patients was in a hallway in a wheelchair. Her nurse was at the desk sitting and chatting with someone else at the desk. The patient asked her nurse if she could please take her back to her room. The nurse responded by saying, "they are having bingo downstairs, why don't you go there." And the patient responded by saying, "I am very tired I just want to go to bed, can you please take me to my room?" The nurse responded with "no, I'm too lazy." It went back and forth that the lady wanted to be taken to bed and the nurse would only respond with "no, I don't want to." There are other things I saw just during the time I was visiting, I can literally go on and on with the wrong that is done here, but you get the point. This place is awful. Stay away! read more

My 98 year old cousin has been at Heritage Park for almost 3 years waiting to transfer to the VA home in Barstow. When he first got there, he was on the second floor where his care was very poor. He was transferred to the first floor where he met Herlinda who took very good care of him. When he was transferred back to the second floor, they would find him rolling his wheel chair to the elevator to get back to Herlinda. Finally we got him transferred back to the first floor. I tell everyone in Administration that Herlinda is their gold standard and she should be teaching all the others how to treat patients. She knows her patients well! She knows when they are starting to get sick, when a health problem is developing, when they are being lazy and don't want to walk or stand, what they are capable of doing and has them clean, dressed well and ready on time to take to appointments . She is firm but loving. I also have a great working relationship with his nurse, Elizabeth. Always willing to hear my concerns and react on them when needed with a smile! Whenever I have concerns about my cousin's care, I can always count on Aimee, Alex and Jeremiah in Administration to pitch in and help. read more

My father was admitted a couple of weeks ago due to a broken shoulder and nose after a terrible fall at his home. My father is 93 years old and the situation has caused a decline in both his mobility and mental awareness. From the start the two who work in the Social Services department have informed me that I need to be ready to bring my father home with me. Today I learned his HMO Regal is trying to put him back in his apartment with a broken arm and no way to feed, dress or help himself. The horrible knowledge of this was bad enough when one of the two in the Social Service department at Heritage Park told me tonight "if something happens to your dad once he is returned home it's your problem" this was then followed by "I asked your father if he would go home with you and he said yes he would". When I asked how dare you assume you understand the situation? His response was "maybe it's time your siblings finally step up and help". Ending with "if you don't take him home we will just pay for transportation and put him back at home" I just stood there in disbelief as if it's not 2023 and I was not in a professional care facility. I have no idea what is going to happen next but I will not stop being an advocate for my father. He needs to be in a place that can care for him and close enough where I can advocate for his care. This place has caring CNAs and nursing staff. The Social Service staff is the group to fear. The insensitive tone and lack of assistance to provide alternatives not to mention the constant pressure towards any available family to make their work easier and simpler. It was not until I pressed that an option for assisted living was recommend. I pray for any family that steps into the small office of these two. read more
