If you are looking for a nursing/rehabilitation center for a loved one Trellis Chino seems to be a great place. We had to find a place fast for my mother in law after her surgery and Trellis kindly accommodated her and our family. New facility, literally walking distance to Chino Valley Medical Hospital. Safe neighborhood, easy access location - very close to the 60 freeway. Facility is big and very clean, nurses are attentive and rehabilitation center looks excellent. Daily rehabilitation with patients and very good food service. My mother in law has her own private suite room with a window and the bathroom that is very big. So far she is very happy in this place and I definitely recommend. Date of visit March 2023

Having had experience with 2 other "skilled" nursing facilities I must say this place has been a godsend! We had Casa Colina to compare with which gave great care and had good rehab but it is more of a hospital setting so not much room to move about and not being able to have grandchildren visit was tough since age restriction was 14 and visitation hours were strict. Then Glendora Canyon was another to compare with and I took my dad out and straight to the hospital after sharing pictures of his lack of care with his Dr who was not happy and requested admission to re-evaluate where to send him. Then I took a tour of Trellis and knowing the previous place had said they could do the needs for his care but didn't live up to any promises of that care I was skeptical BUT Trellis has delivered on all they said they would do!!!! He is receiving PT daily as the orders say! The wound care is amazing! Eddie is a true gem! So much so that the City of Hope surgeon following my dad is so impressed she wants to steal him... but Trellis deserves him too and is so lucky to have him from what I have seen other places care like he provides is not happening THANK YOU EDDIE! I have not even once had to request wound care and they just are doing it routinely and are very organized to be sure it is done and done correctly. You have literally taken my dad from wanting to give up because there was no progress to happy and motivated simply by keeping him informed on the progress. Now this is a skilled nursing facility so there are daily moans and groans of patients and sometimes that is the nature of the person and the mental ability to process why they are there. I in no way see any lack of care and my dad is 100% capable mentally and building physical strength back but he is great at evaluating and has said how wonderful everyone is. The facility is so so clean! I see maintenance cleaning the floors to perfection and spraying and wiping all surfaces, great maintenance staff keeping this place up! The layout is so well thought out so there is common areas with nurse station in the middle. Tables around with couches and lounge areas which in the very center is a great outdoor space. My 3 young children come with me to visit and it's a great environment we can enjoy too! Food is food, and by that I mean some days are great others are ok but nothing to complain about because you are always fed and it's made with nutrition in mind. I love how they put out a coffee bar which allows you to move around if able and gather if wanting to it's a nice way to serve! The room is great it's private with its own cabinet, end table, tv and bathroom with shower. Has made it nice to build that transition to home life! Bed has been comfortable and amazingly everything works which seems easy enough to expect but practically nothing worked at previous places. Thank you from the bottom of our hearts for all you have done to get my dad stronger and healthier and finally healing after a long journey! Everyone from the admissions person who promised us we would get good care, the young lady signing us in, the kind students, on top of it and motivating PT team, the efficient and kind nurses, chefs, maintenance keeping things spotless, to the Drs we appreciate you all and cannot praise you enough for your wonderful care you provide.

Trellis is a very clean facility, the staff is friendly and the environment if beautiful. My mom said the food was very good too.

She definitely gives a lot of priority to her people and you will understand what I mean if you ever work here..... she gave the OK to the Night Shift CNA to take pictures of solid residents to send to her as "proof " , and all that shift does is take pictures and complain which that's HIPPA , yet they sleep through the shift and according to some of the residents, they only get changed once during the night...... the work environment is very hostile , they're on you about pagers and walkies , I worked at Bayshire in San Dimas , it's about the same vibe of this place AND IT IS SO MUCH BETTER , WE NEVER HAD TO CARRY PAGERS OR WALKIES & it is non hostile ...... if you ever work here you'll leave this place feeling super burnt out & depressed , not to mention their " attendance policy" it's horrible, she let people leave early because of "their kids " but I witnessed young girls being really sick, throwing up with fevers and she would give them verbal warnings for trying to leave home early , please think twice about working here not to mention they didn't have a DON and as far as I'm aware they still don't.... So you're putting your families lives in the hands of a facility who can't even get a grip..... don't work here & if u do u were warned .

My family had a very difficult experience with Pomona Vista Rehabilitation Center. My mother was…read moreplaced there following hospitalization and required very specific medical care, including strict dietary restrictions related to a feeding tube and recent surgery. Unfortunately, there were serious communication and care concerns during her stay. Despite repeated efforts to inform staff about her medical restrictions, she was given food that she was not supposed to have. This led to complications, including her feeding tube becoming dislodged and requiring an emergency hospital visit. Even after the hospital provided updated instructions, it felt like important information was still not being clearly documented or followed. In addition to the medical concerns, it was very difficult to get timely responses from staff, and we often felt that our concerns were not taken seriously. This created a great deal of stress and worry for our family during an already challenging time. We ultimately requested a transfer to another facility closer to home so we could better advocate for her care. I strongly encourage families to stay very involved, ask detailed questions, and closely monitor care plans if their loved one is placed here. I am sharing this experience so that other families can be informed and prepared.

I see a lot of work as they're trying to make improvements. For a visitors, employees, and residence of this facility. I had a question where my friend was located at. They were very quick to look in the computer and guide you which way to walk closer to the room. Smiles on each of the workers. For they truly care of what type of care they give to each resident that lives there. They work with post acute patients that come out of the hospital. Claremont Heights provides physical therapy so the residence can be more independent so they can go home quicker. The beautiful activity room was wonderful to see as this is a heart of each residence, getting better and stronger and feeling loved. They provide the medication each residents needs. The check-in kiosks is so easy to understand and use. Once you walk in the lobby, the staff is very happy that you were there very kind and always has a smile on their face. The only downfall I noticed is the facility. Some rooms have cracked in the ceiling and in the wall. There needs to be a custodian team to keep the rooms swept and cleaned to whether you'll find trash on the ground. To all the nurses that work there the CNA's that work there the kitchen crew that works there. I am so thankful that every resident has a smile on their face, knowing they are safe and well loved. There are vending machines and some courtyards so the residence can buy their favorite snacks. The roses and the colors of the gardening are just so beautiful. It feels like you were at home. When you walk into the lobby, there is music being played. And there is a TV screen and it shows the most amazing colors of every season like a picture window.

When I received the news that my father had passed, the first thing I was told after that was I need to be there instantly! Excuse me! I live in hacienda heights, and I had to go to West Covina to go pick up my mother. I could not be there instantly I told the male nurse over the phone. You need to train your staff to speak in a more sensitive way. Especially after a bad news had given to the family. "I am so sorry your dad has passed, however; we do have policy the body has to be removed in x amount of the hours. Please drive safely to get here asap when you can." This was the way, your staff should have spoken to me, 2 years ago. I did not complaint before because I was in total shock! Do Not take advantage of people's grief and depression again! Train your staff better! Thank you!!!!

My mom spent about 2 months at CMCC. This facility is filled with caring cnas and lvns at nursing…read morestation 2. They have worked here for a long time and know what they are doing. The RN evaluator is also caring, friendly and very capable. If family is involved and communicates they are willing to work with you for the care of the patient. The housekeeping staff is friendly and kind and always want to help. My mom needed doctor appointments and transportation and this was all arranged by the hardworking social worker. She handles the care meetings and coordinates care as if she was a case manager. She handles the hair salon and even additional nursing attention as needed. She walks around and checks in on patients. At discharge she handled all the needed items for home care and worked hard to advocate for my mom. Admission staff was awesome. They were very welcoming and made sure she had coverage with her insurance. They are great to work with. The doctor that oversees the facility was not easy to communicate to in part due to her availability. She is a full time doctor seeing patients in her office during the day. She made an effort to call back after hours but my own schedule also got in the way. It would be best if the doc attended the care meetings upon admission so goals and care can be discussed clearly. It was difficult to pinpoint care plans. Staff would send message to doc that family want to discuss care and it was hard to receive info back quickly or at all. Maybe if the doc was present at limited but fixed hours on site it would help. PT/OT was good and made an honest and persistent effort to assist recovery. It helps if family is involved and asks questions regularly. My mom was independent prior to her medical issue so the goal was set to return her as close to that as possible. You have to make it known to them where she was before very clearly. Otherwise she may be released too early and becomes a fall risk. The food staff is very nice and concerned. They made an effort to give her Greek yogurt often for her digestion. The food is otherwise not very tasty or presentable and comes from the main facility. I'm sure the quality in Claremjnt Manor assisted living is better than in the rehab unit. My mom ate mostly in her room and so could not ask for extra items from the kitchen as those in the dining room. The dining room was filled with behabing persons that need assistance eating and this was hard to see for her to see while eating. She is a social person otherwise. I would definitely recommend this facility. They have helped her recover through 4 events in the past year and a half. No place is perfect but Claremont manor care center does a great job. They want to help and truly care. We are lucky to have them in Claremont.

This rehab facility is horrible. It is not rehab at all. My mother became sick within this…read morefacility because of their poor care. My mother was sent here to strengthen the bacteria balance in her gut. She was forced to wear a diaper and was not permitted to use the toilet, and she is not incontinent. She would tell them I have to go to the bathroom and they'd say well then go in the diaper and I'll come right back and change you, and they wouldn't be back for hours leaving her to sit in her own waste. The food she was served was so bad sometimes she did not know what she was eating, but she had to eat because she was so hungry. I sent her food from Instacart so that she would have some healthy food to eat in her room. They would limit her water so that she would not urinate which resulted in delirium. She called me in the middle night and her mouth was so dry. I could hear her lips sticking together because they were refusing to give her water. They ignore the call lights. They would leave her in her poopy diaper for hours at a time, causing her to get UTIs. The delirium that she acquired at this place from their inadequate care was so bad that my California family thought that she was in her end times and was going to leave her there. When I called the office and kindly asked them to change her diaper for three hours to change one diaper, they hung up on me. I heard the nurses with unkind angry yelling tones yelling at my mother while I was on the phone and they did not know the phone was open. I texted the management of the facility trying to get my mother help and 90% of the time they ignored me. When company is there or they know the phone is open they are not hostile but as soon as there's no one around, they are. I've already called Ombudsman's to check out this facility. It should not exist; It is that bad. I drove 1800 miles to rescue my mom from this facility two weeks ago. The emotional abuse is one of the hardest hurdles to help her over. Do not take any loved ones to this facility. I will be reporting them to anyone who will listen. My son sent this up accurately when he said "that facility is nothing more than a pound for old people."
