I see a lot of work as they're trying to make improvements. For a visitors, employees, and residence of this facility. I had a question where my friend was located at. They were very quick to look in the computer and guide you which way to walk closer to the room. Smiles on each of the workers. For they truly care of what type of care they give to each resident that lives there. They work with post acute patients that come out of the hospital. Claremont Heights provides physical therapy so the residence can be more independent so they can go home quicker. The beautiful activity room was wonderful to see as this is a heart of each residence, getting better and stronger and feeling loved. They provide the medication each residents needs. The check-in kiosks is so easy to understand and use. Once you walk in the lobby, the staff is very happy that you were there very kind and always has a smile on their face. The only downfall I noticed is the facility. Some rooms have cracked in the ceiling and in the wall. There needs to be a custodian team to keep the rooms swept and cleaned to whether you'll find trash on the ground. To all the nurses that work there the CNA's that work there the kitchen crew that works there. I am so thankful that every resident has a smile on their face, knowing they are safe and well loved. There are vending machines and some courtyards so the residence can buy their favorite snacks. The roses and the colors of the gardening are just so beautiful. It feels like you were at home. When you walk into the lobby, there is music being played. And there is a TV screen and it shows the most amazing colors of every season like a picture window.

When I received the news that my father had passed, the first thing I was told after that was I need to be there instantly! Excuse me! I live in hacienda heights, and I had to go to West Covina to go pick up my mother. I could not be there instantly I told the male nurse over the phone. You need to train your staff to speak in a more sensitive way. Especially after a bad news had given to the family. "I am so sorry your dad has passed, however; we do have policy the body has to be removed in x amount of the hours. Please drive safely to get here asap when you can." This was the way, your staff should have spoken to me, 2 years ago. I did not complaint before because I was in total shock! Do Not take advantage of people's grief and depression again! The RN evaluator is also caring, friendly and very capable. If family is involved and communicates they are willing to work with you for the care of the patient. The housekeeping staff is friendly and kind and always want to help. My mom needed doctor appointments and transportation and this was all arranged by the hardworking social worker. She handles the care meetings and coordinates care as if she was a case manager. She handles the hair salon and even additional nursing attention as needed. She walks around and checks in on patients. At discharge she handled all the needed items for home care and worked hard to advocate for my mom. Admission staff was awesome. They were very welcoming and made sure she had coverage with her insurance. They are great to work with. The doctor that oversees the facility was not easy to communicate to in part due to her availability. She is a full time doctor seeing patients in her office during the day. She made an effort to call back after hours but my own schedule also got in the way. It would be best if the doc attended the care meetings upon admission so goals and care can be discussed clearly. It was difficult to pinpoint care plans. Staff would send message to doc that family want to discuss care and it was hard to receive info back quickly or at all. Maybe if the doc was present at limited but fixed hours on site it would help. PT/OT was good and made an honest and persistent effort to assist recovery. It helps if family is involved and asks questions regularly. My mom was independent prior to her medical issue so the goal was set to return her as close to that as possible. You have to make it known to them where she was before very clearly. Otherwise she may be released too early and becomes a fall risk. The food staff is very nice and concerned. They made an effort to give her Greek yogurt often for her digestion. The food is otherwise not very tasty or presentable and comes from the main facility. I'm sure the quality in Claremjnt Manor assisted living is better than in the rehab unit. My mom ate mostly in her room and so could not ask for extra items from the kitchen as those in the dining room. The dining room was filled with behabing persons that need assistance eating and this was hard to see for her to see while eating. She is a social person otherwise. I would definitely recommend this facility. They have helped her recover through 4 events in the past year and a half. No place is perfect but Claremont manor care center does a great job. They want to help and truly care. We are lucky to have them in Claremont.

If you do have family there, and have issues, please call the LA County Ombudsman's office at (800) 334-9473. You can also contact the Medical Board of California www.mbc.ca.gov to file a complaint. My family had a very difficult experience with Pomona Vista Rehabilitation Center. My mother was…read moreplaced there following hospitalization and required very specific medical care, including strict dietary restrictions related to a feeding tube and recent surgery. Unfortunately, there were serious communication and care concerns during her stay. Despite repeated efforts to inform staff about her medical restrictions, she was given food that she was not supposed to have. This led to complications, including her feeding tube becoming dislodged and requiring an emergency hospital visit. Even after the hospital provided updated instructions, it felt like important information was still not being clearly documented or followed. In addition to the medical concerns, it was very difficult to get timely responses from staff, and we often felt that our concerns were not taken seriously. This created a great deal of stress and worry for our family during an already challenging time. We ultimately requested a transfer to another facility closer to home so we could better advocate for her care. I strongly encourage families to stay very involved, ask detailed questions, and closely monitor care plans if their loved one is placed here. I am sharing this experience so that other families can be informed and prepared.

I visited her on her first day / stay here. The outside temperature was 100 degrees. When I get to her room 34B. Her bed was near the patio door. The curtain was wide open, sunshine shining completely throughout the room. There was a small 12 inch fan mounted to the wall and on full blast circulating warm air. I pulled out my room temperature thermometer. It displayed 85 degrees. I put my hands up towards the air conditioning vents. Warm air was flowing. I immediately complained to the charge nurse. She contacted maintenance. I never saw or heard from maintenance on that particular day. The next day I arrive. Same Conditions . They contacted maintenance again. This time I see maintenance personnel. He looks the room over and asks "What's the problem?" The charge nurse tells him "The family member has his own room thermometer that's showing this room is 85 degrees". He leaves and later returns. He placed a large commercial room fan to blow throughout the hallways to cool this section of the facility. I later see my mom's catheter drain bag laying on the floor, opposed to hanging from the bed rail, as it should. Throughout the 4 days that I visited my mom. I could barely communicate with her. She had been given Morphine, Ativan, and Haloperidol. She's been so sedated that's why I could not communicate with her. Prior to her 5-day stay. She was NOT taking either of the above-mentioned drugs for pain , discomfort, or agitation. Then, upon her arrival home after being discharged. I removed her blanket . To my dismay. I see her left knee is extremely SWOLLEN and hyperextended from her lower leg below the knee. Her knee looked like a mountain peak. Holy Cow! What happened here? Her knee appears to be broken, in my opinion. I then contacted Community Extended Care of Montclair. When I ask what happened to my mom's knee. They say "It may have happened during her transport". Things are not adding up here.. The transportation staff say they were informed that my mom was given Morphine prior to being transported. However, she never needed pain medication prior to going to this facility. And now, she's being dependent on these pain meds for pain caused by, while at this facility. Sounds like a cover up to me. I filed a Formal Complaint with the California Department of Public Health. This place needs to be held accountable and shut down. The doctor called me this morning ( October 14th, 2026 to confirm it is dislocated and fractured. Additionally, the medication that was sent with my mom to Community Extended Care of Montclair. The medicine was still inside their containers. The instructions on the bottles stated "Take .....during respite care". However, you sent (5) Amlodipine tablets. Currently, there are (4) tablets remaining in the bottle. In the Lasix bottle container you sent 10 tablets. However, there are (9) tablets remaining in the Lasix container. She was instructed to take "2 tablets by mouth daily ( during respite care ). Meaning, during her (5) day stay she only had (1) Lasix tablet and ONLY (1) Amlodipine tablet. She was deprived of her medication, while in the care of Community Extended Care of Montclair.

The grounds are well kept and everyone from the landscapers, the cleaners, to the medical personnel walking to and from are always so pleasant. They, as well as other residents, always seem to take a moment and give a friendly hello. From what I hear, the services provided by the Gardens fit the desires of our friends who reside here. Another wonderful plus is that the Gardens is a pet friendly community! It is certainly one of the friendliest, loveliest, all-inclusive retirement communities I have visited and they truly deserve their top ratings.

Initially we felt the service and care was lacking the attention my MIL needed. I'm not sure if they were short staffed based on the number of patients they were caring for but their overall care seemed to be sub-par at best. The first few days my mother in law was there she was on the "dependent" list meaning she needed help feeding herself and getting her fluids. While I was there the person delivering her food tray just dropped off the tray and no one came to feed her while I was there so with this said, based on my experience we felt she didn't really eat or drink much for the first few days. She said she was eventually received help to eat and drink. Also one day her sheets were wet and needed to be changed and it was quite a while before someone came into the room so she could ask them to change the sheets for her after buzzing for help. The facility is also very loud so it was difficult to sleep. On a positive note she did say she liked her PT and the OT. They were very patient and very helpful in trying to get her rehabilitated.

They are asking questions that, even in my most desperate moments of researching my symptoms, I haven't thought about. I'm just blown away. They are so kind, knowledgeable, validating, and supportive. So grateful. Lisa at the front desk has gone out of her way to assist squaring away referral issues that were caused from my dr office. They weren't even her mistakes and over the months long issue she she stayed late, spoke on the phone, and grabbed me out in line to ensure she could be the one to work on my issue since she knew the most, to ensure I got the necessary appointments. Just absolutely blown away by this place. Idk if this makes any sense, I'm in the parking lot after only my fourth appointment, but if you are struggling with chronic pain stemming from hypermobility, this is your place.

I am very disappointed with my experience at the Casa Colina Physician's Clinic. I arrived on time…read morefor my scheduled appointment, yet I was left waiting for approximately 40 minutes before being seen. What makes this especially frustrating is the double standard. Patients are expected to arrive on time, and if a patient were 40 minutes late, the appointment would likely be canceled or need to be rescheduled. However, when the physician's clinic is running 40 minutes behind, the patient is simply expected to wait. I understand that unexpected delays can happen in a physician's office, but 40 minutes is a significant amount of time, and patients' schedules should be respected as well. At a minimum, patients should be informed when there is a substantial delay and given an estimated wait time. Casa Colina Physician's Clinic should hold itself to the same standard of punctuality and respect for time that it expects from its patients.

Claremont Manor is a special place which has the benefit of providing almost every kind of senior care & housing you could imagine. It is staffed by administrators who are friendly, caring and approachable. They and the auxiliary staff make our stay here happy and enjoyable. So much so that they made us feel more like friends than residents. The Manor offers great food, beautiful surroundings, interesting events, fun parties and outings to restaurants, shopping, museums& other places of interest near & far. In short, Claremont Manor is a wonderful place to live.
