It's been 4 years since losing my mom to Alzheimer's. My mom was at Oak Park for just a little under a year. In hindsight I'm glad that my family and I made the difficult decision to place her when we did and before she was in the later stages of ALZ. i will be eternally grateful to Oak Park Manor and their staff. It takes a special kind of person to do that for a living, day in and day out. The atmosphere at Oak Park is very much like a home setting making the transition easier for both the patient and the family members of the patient. Toward the end stages of the disease my mom was wearing other patients clothes, needed assistance with using the restroom which meant she sometimes soiled herself. She eventually stopped eating and speaking. Not because of anything Oak Park did or didnt do, it was due to the progression of the horrible disease eating away at her brain loosing the ability to function normally. One of the reasons that we selected Oak Park Manor it was listed on the dept of health website as an approved facility who accepts the assisted living waiver program. The program provides financial assistance to those who qualify. If you really want your loved ones to pass away, this is a place to put them!! Caregivers leave the residence in their feces for hours. That's how they end up getting bedsores and UTIs.!! Also, if they don't want to eat, they'll let them starve!! They ignore them when they need help and if they don't want to get in the shower, the rip off their clothes and force them to get in with cold water!! it stinks really bad in there. My loved one lived at Oak Park Manor right before the new ownership took over. They are now called Claremont Hacienda. Apparently, the same staff members that worked at Oak Park Manor still work at Claremont Hacienda. My family and I were very involved in my loved one's care and advocated for my loved one by contacting the staff with any issues concerning my loved one's health and happiness. I deeply believe the staff at Oak Park Manor neglected my loved one, which ultimately led to her passing. I highly recommend you look somewhere else. read more

My mom passed away early June 2019. If you read my post you will see this place is horrible..... a few months before my mom past she had scabies which she got from this facility. My mom would have other people's blankets. Other people's clothes on and often I would see other people wearing my moms clothes. They are not washing clothes or bedding separately which is required by the health department. They are lazy and always looking for a short cut...One of the nurses told me they had a huge outbreak and several residents had them. It spreads from lack of cleanliness and proper care, my mom was not fed all the time, she was soiled often, I would have to complain to get her dressed with clean clothes or even bathed. Since I didn't have POA they just shrugged what I said off..... once my mom contracted scabies her health declined fast. She was always doped up... she wasn't being woken up to be fed or given water..... my mom weighed less than 70lbs when she passed and was severely dehydrated.... when my mom was first put here she was 175 plus lbs....please do not put your loved ones here....... If you care about them you would find another place ..... these people do not care one bit about your family members...... They act caring to your face but in reality mistreat your family members..... I've attached pictures of my moms skin/ please look at them and think really hard about it before putting your loved one here. read more

I recently had to move my grandmother into an Assisted Living community recently--and let me tell you, I was taken beyond my expectations with Oak Park Manor. So many questions and utter confusion arose from moving a loved one into a retirement community; however Sabrina, the Executive Director, and Jennifer, the Wellness Director, not only answered any questions I had, but comforted me in the process as it can be an emotional toll on not only my grandmother's life, but my family's lives as a whole, so I thank them for this, they treated us as their own family members, just like the staff does my grandmother. They are one of few buildings in the area that accepts the ALWP waiver, which is a HUGE help. The building is beautiful and breaks the stereotype for the typical ALWP waiver participant. I had my mother in this place. This place is so dirty and the staff and the new administer is rude and awful. She had run me and my mother on the wheelchair out of the facility because, I told the supervisor that my mother was extremely dirty. I had my father there before and it was the cleanest place and the nicest staff. Now, pff this place is full of neglect I'm going to inform the state and adult protective services . I'm very concerned about the patients who are still there. read more

This place was a god send to us. The staff go beyond there job descriptions. Everyone is so helpful to my mom. I have been there for a few months now and I couldn't ask for a better place. There marketer/maintenance/tech/griller/caregiver who is know as Jack by all the residents but real name is Donell is really a jack of all trades. Who cares for each resident like they are his family. And this is how staff member treat everyone. There wellness nurse is always there whenever I need her and the nicest person you can ever meet. I am so thankful for everything that they do for my mother here. The place is more like your home than a memory care unit. I wish we had known about this place a long time ago. My mother has been to some really really bad places. read more

Oak Park is a warm facility, a homey environment. It was nice to see the work putting in to upgrade the facility. I visited my aunt often. And my favorite spot to sit with her was at the backyard. Whenever I'm there I felt welcomed and I was comforted on how the staff took care of my aunt. They're very compassionate and I saw how much they loved my aunt as much as I did. Especially Jennifer and Marisol. My family and I are grateful to Oak Park for taking on the role in providing care to my aunt while it was difficult for us. Thank you Oak Park, God bless. read more

We are so grateful for the staff at Oak Park Manor in Claremont. They have communicated with us the whole time mom has been a resident. They have hearts of gold. We are so blessed to have found this memory care facility for our sweet momma. I would recommend this place for anyone who needs a placement in memory care. read more

My grandmother has been at Oak Park for several years. From the start they have always treated her and I very well. They notify me of changes, big or small. They helped me with translation (my grandmother has returned to her 1st language), and guided me as to when the best times to visit her are. That last thing is the most important, while they welcome visits at any time, they know their patients schedule and personal likes/dislikes and were able to guide me to a time that proved better for my grandma to be receptive and engaged during my visit time. In other words, thanks to their caring and compassionate care I am able to visit WITH my grandmother, instead of just visiting her. I appreciate all they do. read more

Don't choose this place, save your self the headache. The staff said they would make sure my dad walks daily. The truth is he was either in the wheelchair or in bed. My dad fell out of bed when the newly hired employee was with him, yep he went to the hosp. His shoes were stollen, the staff said if we didn't write his name on the black shoes, the other patient gets to keep them. We called for 2 days, left messages and their response was we had ph problems. Dont take your loved ones here. read more

It is never an easy decision to place a family member in a long-term care facility, however, Oak Park Manor definitely came to our rescue. Oak Park is a locked facility that specializes in memory care. My father has lived there for the past 2 months after a very aggressive and agitated form of dementia unexpectedly plagued my father's pre-existing dementia. Over the course of 2 months, they were able to find a drug treatment plan that worked for him and now he is doing much better. Overall, the residents are always well-groomed, dressed in clean clothes, and well fed. Kristin (one of the head nurses) is truly an angel! We will be bringing my dad back home around end of the month, and the Administrator (Sabrina) said if things change please let them know. I also appreciate that they are remodeling and expanding their facility to welcome more residents in need. Long-term care facilities DON'T have to be sad and depressing places. You can tell that the residents here look out for each other and there is a mutual feeling of compassion, care, and respect. Thank you again Oak Park Manor! read more

You will not find better care for your loved one in Southern California or Portland OR. The quality comes from the loving staff who treats each resident as if they were their grandparent. My father was a resident in 2013 and the first half of 2014. We moved to Portland OR in June of 2014. I really question my decision to move my father with me since his care was so much better at Oak Park Manor. In all the places that my father has been, I have never found a staff of caregivers that were so upbeat, encouraging, patient, and caring. I cannot name which staff members I am speaking of because literally the entire staff has this attitude. The staff seem to help each other out as well. You will find more luxurious facilities (which cost an arm and a leg more) but their staff is watching the clock waiting to go home. Oak Park is clean and is decorated to look like home.
