Cat or Kat was terrible - I wish I got her full name. She told me because I didn't have power of attorney, I couldn't make decisions on my mother's code status. She also said my mom was incapable of making decisions herself and couldn't sign the paperwork herself. I'm next of kin. That means it's my decision. Cat offered to "sign my name for me" which I refused, because I disagreed with the code status. My mom was discharged from the hospital & placed in Jurupa Hills for long-term care with hospice services planned, it was in the hospital discharge paperwork. So long story short, my mother is unable to make decisions/sign the paperwork, I went through the trauma of deciding my mother needed comfort care & hospice services, only for Cat to give her a full code status meaning she would get CPR, a feeding tube, or a ventilator if needed. That is not comfort care. That is not hospice. This meant my mother was in Jurupa Hills for 35 days just rotting and suffering. She wouldn't eat, shower, drink water, or take her pills by mouth anymore. She NEEDED hospice care. She NEEDED pain management. She NEEDED comfort care during these 35 days she stayed at Jurupa Hills. My mom ended up back at the hospital, I put her on hospice (which I'm legally allowed to do as next of kin, when my mother is unable to make decisions/sign for herself) and what do you know. Within 24 hours of being on hospice, mom passed away. It was time. Mom was ready. And she should have gotten comfort care sooner. I have nothing but hate in my heart for the poor training and lack of knowledge that Cat/Kat has. She has no business working there with sick, helpless people. Other staff at Jurupa Hills were kind to my mother during her time there. I respect and appreciate everyone that took care of her, paid attention to her, and treated her with dignity and respect. There is a young gentleman with long brown hair that works there, I wish I got his name. He was wonderful - THANK YOU. There was another Spanish woman working there that was wonderful - she was kind and attentive, THANK YOU. The facility itself was relatively clean and did not have a smell to it. I have frustrations with no communication from the doctors that work there. I have frustrations that no one read my mother's hospital discharge paperwork to see the part that said hospice services were expected. I would not recommend Jurupa Hills to anyone seeking hospice or comfort care for their loved one. Everywhere probably sucks and I'm sure there's negative reviews regardless of where you send your loved one. My only advice is they will probably get better care in a hospital. read more

The kitchen does not wash the dishes correctly here. The upper staff has the audacity to blame the CNA's that bring me the dishes rather than eliminating the problem from the kitchen. There wouldn't have been a problem if the dishes were cleaned properly from the get go. I have an immune disorder and I can easily get sick especially eating off of other people's utensils, cups, plates that haven't been washed correctly. I spoken to people here but the problem doesn't get fixed. They write up menus and pass them out on Sunday and the menus always has something missing. For example meatloaf and mashed potatoes with gravy, NO GRAVY. Roast beef dip with au jus, NO BREAD AND NO AU JUS. There has been multiple occasions this has happened since I've been here. read more

A dear sister of mine was admitted to Jurupa Hills Facility. I really appreciate how the staff take good care of her. I got the chance to visit her and the staff were very friendly and accommodating. I delivered food to one of the patients. The front door has NO button for wheelchair users. The door is considerably heavy! I had to help multiple people in wheelchairs who struggled to open it. PLEASE get an automatic door opener so they can press the button and not hurt themselves trying to open the door!!! read more

If I could do Zero stars I would, this place is so unprofessional and disgusting. whoever is management clearly is doing a poor job on how they should MANAGE their staff. I read some reviews, and I'm not surprised, My uncle was here, and I am so happy to know he is gone and not under the care of his case manager Jasmine. What is your establishment paying her for? She never returns calls I left numerous of messages, and she is rude as hell and should not be employed there. As I was waiting to rip her a new one, I was standing by the door and can hear her talking bad about other patients. It's extremely hurtful to know that she's in that field, and she talks about patients in that manner. These are people's family members, and it broke my heart. I was highly disgusted to know she would stoop to that level and speak on patients that way. She is very lucky I was on a time crunch. Other people have clearly mentioned her in previous reviews, and I doubt accountability is happening. She needs to be FIRED!!!! So, whoever is reading this please take steps because she is a walking lawsuit, the second I find out who I can speak to regarding this I will be taking it to the highest chain of command. This place is horrible and does not care about their patients! We had our patient in our waiting room for over 3 hours waiting on transportation to pick her up. We tried activating her ride multiple times throughout the 3 hours and they stated they would send someone and didn't. They don't answer the phone either and you cant even leave a voicemail. Who knows what kind of care they provide here but it's nothing I've seen before, so neglectful. We even had one of the drivers tell us he can't pick her up because he's not going to get paid when we had already tried activating the ride multiple times! This needs to be fixed. read more

This skilled nursing facility was great. The CNA's and LVN's that attended me all did a great job. There are two issues that I don't agree with and are concerns to me. I had an infection on my PICC line. My the area of the PICC line was compromised and started to generate hives and redness. I asked the RN is this okay, she stated that the antibiotics that was being administered through my PICC line would take care of it. I did not... Later this day on my way back from my doctor appointment I got really sick. Nausea. I was already fighting off three life threatening infections. I wanted to be dropped off at the hospital but was told that I had to return to their facility first than go back to the hospital when I was right next to RCH. I was not going to put my life in jeopardy. I went anyway. read more

A dirty horrible place we refused to have mom stay. Outside this facility looks beautiful but inside it stinks of urine. They were going to place my mom in a filthy room that had blood on the ceiling, dirty clothes in the far corner, soiled diapers in the closet, and bugs . No thank you -the paramedics who brought mom here said they wouldn't leave a loved one for a minute in this place. We left. Disgusting. My mom was staying here and had to work with her case manager Jasmine. Not only would she be late, she would be late to meetings multiple times. She has a nasty attitude and her excuse is sorry I have a lot of work, lost track of time!!! But we would see her walk in late to work. EXCUSES AND PATHETIC, some of the workers are decent, I requested a different case manager and apparently she stated she is the only one???? What kind of facility only has 1 case manager? I know she didn't want to tell the truth because she didn't want to get in trouble or reported. That's ok cause I'll use this platform to complain about her poor management, communication and professionalism skills. Y'all need to reassess your worker and train them how to respond. Rude, nasty and I will be looking into where else I can report this. Overall aside from her the facility is clean, nurses are nice with my mom but that ladies energy is just by far past poor. read more

Thank you to the people of Jurupa! It has been a couple months now that I have left and I am so grateful for the care I received. The Social Worker Christa made sure I got everything I needed here. I got money again and a place to live because of her. I put a picture of my backyard of my new home! I now live with one of my best friends in a beautiful home she found for me with caregivers that also take care of me. I was so scared and she made me feel safe. This is the worst facility I've ever seen. I was with my friend when he finally got his mother was there for 10 days until his mom was begging her family to get her out. She was told after leaving the hospital they were going to put her in this horrible place for 14 days. First, they never bathe her for the 10 days she was there, she's on kidney dialysis and when she let them know she needed to use the restroom they said just poop in your diaper that it was mandatory for every patient. I could go on for 2 more pages on how horribly this poor lady was treated most people treat dogs better than they do at this place. People that have no compassion should definitely not be working with patient that need some compassion. They really don't care about any of the patients so sad. I will do my best to try to start an investigation into this place and pray these ladies get fired!! read more

It was our lucky day when we came in contact with Sandy . I do not recommend this facility for your loved ones. My mother has been there since the end of May and her things have gone missing three times. Her prescription glasses, her cellphone, her personal pajamas, and the little things in life that make a person happy like perfume, magazines, snacks etc. Social worker and State notified. read more

I went to visit my grandfather and there was no one taking care of him. He is strictly tube feed and it was 3:30pm and he had not been fed. He was was covered in fecal matter and the place was dirty. My brother was sent to Jurupa Hills Post Acute to rehab after a 10 day hospital stay, where they kept him entirely in bed. At Jurupa Hills, they also put him in bed, with diaper. He did not need this at all & could get up to go to bathroom but they would not allow it. His room was the size of a small bedroom, with 4 beds, inches from each other, the curtains pulled & everyone in diapers! Some of the patients there would be calling for a nurse & I went in hall several times looking for one to no avail. The girls working there are very nice & try hard, but seem understaffed. They did not get him up for rehab very often, didn't do much, & in a week the Doctor had Not seen him at all! I brought my brother home so he could have home therapy & actually improve..which he has! read more

Not a good place. Can never reach anyone by phone. Expect your loved ones possessions, clothing, etc., to be lost or stolen. They discharged my sister to an assisted living place and did not take any of her clothing. The new facility told me they brought her with only the clothes she had on. She had a closet full of clothes at Jurupa Hills. She has no change of clothes. She is at the new place with a clean tee shirt they gave her to wear and no pants. I tried calling Jurupa Hills on every single phone extension-both nurses stations, the social worker's office, the business office's line and the receptionist-either no answer or voice mail full. Multiple times of the day, no answers, I did have I the phone number of 1 person, Sandy Kayla, but her response was she has nothing to do with belongings and she didn't even offer to help me, even after I told her no one was answering at any extension. I've had to file a complaint with the Riverside County Health Department. Pathetic lack of organization and paying attention to what they do.Last time I visited my sister, who is paralyzed on one side and only has the use of one hand, her hair looked like it hadn't been brushed in a week. Completely matted up in a ball in the back, it had to all be cut off. At the new place they told me my sister's body wasn't even clean. If I could give 0 stars to Jurupa Hills I would. I think the people who gave rave reviews are either relatives of the employees or were paid off. There is absolutely no way this is a 5 star place. read more

Do not place your loved one here in this facilty. My mother was a patient here for 11 months and in that time she lost 35 pounds! She was always dehydrated and starving. My mother suffered from Dementia and needed help with feeding herself but the employees would ignore her and say they cant make her eat. They would not even take the time to hold her drink so that she could hydrate. The food is horrible and the head nurse was incompetent. We would always find my mom with urnine all the way up her back which means she sat in a wet diaper for hours. The last day there, I walked in and found my mother was going in and out of consciousness and their nurses and CNA's were just all chatting around my mom. This woman was slowly dying and they were just going about their business. My mother was taken to hospital and the doctor said she had 10 days to live. I brought her home and she is still alive today one year later because I took the time to feed her and hydrate her. Shame on you Jurupa Hills! I should have sued you but I did not want the negativity in my life. read more

I'm a former employee at JHPA. And I would just like to take a minute to say that LIZ CARDOZA, the director of nursing, is a horrible employer to work for. She is extremely rude and treats her staff badly. She talks poorly about her employees behind their back and she is highly disrespectful and I've witness her call a few of her CNA's "b*tches" on several different occasions. I quit my job to attend nursing school and during the end of my employment, Liz treated me like trash. She was insensitive and rude to me. She would ignore my calls, texts and emails. She was super unprofessional. The facility and staff are okay, but the owner Chad is super nice and treated his employees well. read more
