My got a new roommate and they did not get along. A single room became available and our family decided to move our aunt. Clearwater was told on a Friday at noon we would move our aunt on Tuesday. When we arrived Tuesday morning to move our aunt, there were no rooms keys for her room. Staff said they would work on getting keys made. Despite checking several times throughout the day for keys, no keys were available. This meant my aunt had to find leave her room unlocked when leaving her room and go find staff to lock her room. I contacted Clearwater the next day, Wednesday, and still no keys had been made. One excuse is that the person who makes/has access to keys was on vacation. (No one person should be in control of all room keys in a senior assisted living and memory care facility.) I went out of town for a couple of days. When I went by Saturday morning, 4 days after my aunt moved to her new room, no keys had been made. When I spoke with staff, I reiterated it was unacceptable. I also told them I would be writing a review and contacting their corporate office. Within an hour, keys were made and the problem was rectified. Clearwater was given four days notice my aunt was moving to a room that had been unoccupied for several days or more. Even after moving, it took four days and my stating I was going to contact their corporate offices to get keys made for her room. Unacceptable.

My father was emotional when he left because of the sweetness they showed him. The head nurse, Cynthia, always communicated with me, answered my texts, called me directly if my father needed to hear a familiar voice, and a pleasure to communicate with her. This decision to place your loved one in a skilled nursing facility is not an easy one, but when you put them in a place that feels supported and you know they are cared for, it helps ease the anxiety of not having them home with you. The nurses and the caregivers took great care of him. He was always clean and was closely monitored medically. He had some events that required him to be hospitalized and they always welcomed him back after brief stays in the hospital. My family would like to mention a few staff members who went above and beyond what was expected of them. They are Cynthia and Edwin. There are many others that provided excellent care. You were there for him when we were not able to. You kept us informed when he wasn't feeling well. When we came to visit him, the place was always clean. I used to see Dr. Castellanos when I was a kid with my mom, roughly between the ages of…read more12-15(2004-2007). Now as an adult I've realized how horrendous of a doctor he was. Our family eventually stopped seeing him after 3 years of being under his care. We went to him because it was hard to find doctors who spoke Spanish in our area at the time. I'll start with the worst. My mom used to be really concerned for her health. Over time we realized her concerns were signs of what she's now developed, diverticulitis and other digestive issues. She often came in with pain and explained her concerns. Dr. Castellanos would tell my mom that she was fine and to take ibuprofen. Any time she came in it was the same thing even though her symptoms progressed - continue to take ibuprofen. Eventually she told my mom she was too worried and was a bother. We finally stopped going to see him when she used the restroom and found blood in her stool. She developed ulcers in her throat and stomach that were very painful when eating. After we stopped going to see him we found out all of her symptoms were the beginning of diverticulitis. In 2009 she ended up going to the ER because she was heavily bleeding when she used the bathroom. A diverticula had burst open and it was discovered she has 3 total that had developed. She was taken to emergency surgery. She's suffered through diverticulitis ever since with multiple ER visits. Dr. Castellanos was negligent as a doctor when presented with symptoms. He could have referred her to a gastroenterologist but never did. We had HMO at the time and eventually switched to PPO but it was after dealing with him. For my own case, I developed gastro issues around 12 and realized I had sensitivity to certain foods that lead to a lot of gas and odd noises in my intestines. I felt shy about it as a kid but was worried because I didn't want to sit in class having those noises come from my intestines because well... it was awkward. My side of the classroom could hear my gas issues and I didn't understand what was causing it. When I told him he blew me off and told me to eat more fiber. I didn't bother more about it or ask because I was too ashamed to ask more. Years later I realized I had food sensitivities and that I need to be careful to avoid dairy. All of this to say - I would absolutely NEVER recommend anyone to see Dr Castallanos. My mom could have sued him for negligence but decided not to. Seeing him still in practice now and talking to family about health made me 20 years later have to go back to write this.

Never received a follow up call or contact on my blood results to guide me through what the results meant. I called the office and asked what the next steps were, they told me if I didn't hear anything then there's nothing to be done. But that doesn't explain the concerns that I'm having with my health and wellness. They scheduled me an appointment with Bianca, who is supposed to be specializing in women's health, but a day before the appointment they called me to reschedule and push it back a week. Her appointment times are terrible. She's only there on certain days of the week, there's no early morning or after work appointments. I specifically told them from the original booking that I would have to use my lunch break for the appointment and would have less than an hour to be there. The Office assured me that I will be seen when I arrive. When they called to reschedule my appointment, I reiterated all of that. I arrived for my 1130 appointment right at 11:30. At 12:05 I still hadn't been called back and checked with the front desk how much longer it would be. From what I could overhear it sounded like there were 3 to 4 more appointments in front of me. Which meant I would've been there well over an hour before I would've been seen. They asked if I wanted to reschedule. I told him to refund my copayment. Thankfully, they did. My initial experience did not outweigh the rest. Don't overbook if you can't see your patience in a timely manner. The least you could do is let them know what they expected weight is when they arrive for their appointment. I've now lost my entire lunch break, was late to getting back to work and have to find a new doctor all in one. If they don't care about your time, they don't care about you.

I didn't have much of a chance to research this place before the doctor sent my mother in law here. That was probably my biggest mistake. My husband informed me they were moving my mother in law here so I drove over there and boy was I in shock! This place is so disgusting, the staff was extremely rude. I walked in and saw how gross her room was I immediately said "we're moving her!" I took pictures of how disgusting this place was and right away they called the head of social services asking me what they can do to make us stay, I said nothing and showed them the filth. They said they would move her to another room that's much better and talk with the housekeeping staff but that it would be too detrimental to move her again this late in the day. They moved her to another room and it was just as bad. We couldn't get her moved because the other places said it was too late to do a transfer and she would have to stay here. We were so bothered we stood with her as long as possible and it just got worse and worse. Patients roaming the halls screaming and all the staff just huddled around chit chatting and are so rude when you ask them a question. The next day when we advised them we were leaving they became very standoffish and wouldn't help it took them Almost 12 hours to release her after us going up and pestering them every hour. Our experience with Arbor Glen Nursing Home was nothing short of horrific. During my father's time there, he fell an alarming 11 times due to inadequate supervision and lack of preventative measures. He became severely dehydrated, which is an absolute failure on their part to monitor basic health needs. The lack of attention and proper care was appalling. While the Director of Nursing (DON) was the only bright spot--compassionate, helpful, and understanding of my concerns--unfortunately, the rest of the staff seemed completely untrained, indifferent, and clueless about providing the level of care residents deserve. Despite voicing my concerns repeatedly, the quality of care did not improve. This facility's inability to ensure even basic safety and well-being for its residents is unacceptable. I would never recommend Arbor Glen to anyone and sincerely hope that no other family has to endure the heartbreak and frustration we experienced. Families entrust nursing homes with the care of their loved ones, and Arbor Glen failed to meet even the most fundamental expectations. read more

Arbor Glen has deteriorated to an awful place to live. Since February, 2016, the care of residents on the whole has become criminally bad. What was once a facility that care about its residents has become nothing more than a haven for drug addicts, the mentally ill, and criminal indigents. Their Medicare rating is a farce as they put on an extraordinary show for the DHS survey team including hiding "difficult" residents by transferring them temporarily to sister facilities. If you want your loved ones to live in filth, noise, and fear, Arbor Glen is the place. Otherwise, do not listen to their lies and promises and take your loved one elsewhere. read more

Fantastic Care!! My mother has been here for the last couple of weeks and we are so happy with her care. The staff are all wonderful especially Rachael and Wacy. We have been to other centers but this place really seems to care about her care. They are quick to respond to her and she really appreciated all of the communication. A nurse recommended this place and at first I wasn't sure if this place would be right for my mom but the nurses and staff are caring and friendly. My mom received excellent care and small issues were always taken care of. We appreciate all the staff at Arbor! read more

Beacon became my second home. I was there every day and often stayed until 1 a.m., especially when my mom was not feeling well. I was very attentive and meticulous about her care, always closely monitoring the quality of support she received. Beacon made sure they delivered consistent and compassionate care throughout her stay. My mom was transferred to Beacon in 2021 and spent the final chapter of her life at Beacon Healthcare Center. My mom passed away on January 4, 2025. Her assigned nurses at Beacon never failed to check on how she was doing. We had hoped she would recover, but God had other plans. Although, saying goodbye was one of the hardest experiences our family has ever faced, I am deeply grateful for the care and compassion she received. Like any healthcare facility, there were a few hiccups along the way. We had some concerns during her stay, but I appreciate that the staff listened to us and worked with our family to address them. Being able to communicate openly and come to a resolution meant a great deal during such a difficult time. The nurses, caregivers, and staff treated my mom with kindness, dignity, and respect. They made every effort to keep her comfortable and showed genuine compassion not only to her but also to our family. Their patience, comforting words, and support meant more than they probably realize. No place can take away the pain of losing someone you love, but knowing my mom was surrounded by caring people brought us peace during her final days. I truly appreciate everyone who played a role in her care and helped make a heartbreaking experience a little easier to bear. Special thank you to Nurse Tran, Ms. Cynthia, Ms. Gloria, Nurse Angelica, Nurse Angelo, Nurse Mary Ann, and Nurse Mary Rose for their dedication and compassion, especially during my mother's feeding times. I wish I could leave more stars because we had high hopes when my dad was transferred here for…read morerehabilitation after surgery. Unfortunately, our experience has been deeply disappointing. Like many facilities, there appears to be a serious lack of urgency, compassion, and communication. My dad went approximately 15 hours without being provided water. On another occasion, he was left sitting in soiled clothing for over an hour and was expected to eat his meal before anyone assisted him. While visiting, I also witnessed another patient fall. The patient remained on the floor for approximately 20 minutes while several staff members stood around without providing immediate assistance. It was upsetting to witness. Communication among the staff has also been extremely poor. It often seems that no one knows what is happening or who is responsible for patient care, making it difficult for families to get answers or feel confident that their loved ones are receiving proper attention. Based on our experience, I cannot recommend this facility to anyone looking for rehabilitation care for a loved one.

The place looks nice on the outside but holy hell, this place leaves their patients completely unattended and neglected. DO NOT BRING YOUR LOVED ONES HERE. There are patients screaming for nurses, patients who need help getting to the restroom being ignored, and it's completely unacceptable. DO NOT BRING YOUR LOVED ONES HERE. We have a family member who needed assistance from recovering from a major surgery, and thought this establishment would be a good place to transition them back home from being in the hospital. WE WERE SO WRONG. DO NOT BRING YOUR LOVED ONES HERE. Recovering from surgery or any medical condition is a profoundly vulnerable time for many patients. It is not uncommon for them to feel down but to deprive them of assistance at an establishment that is meant to look after patients is both unacceptable and frustrating. Such experiences can significantly impact their emotional well-being during an already challenging period. This place is a scam, DO NOT BRING YOUR LOVED ONES HERE. Heed the warnings of all of the 1 star ratings - we failed to and we regret it. NOTICE how all the five star ratings are over 2 years old or from profiles who have less than 10 reviews? Cleanliness was okay. The CNA Martha, Tina, Leti and Rachel and nurse Anthony and Cristina were superb! Very kind and compassionate . Always very helpful and nice. Therapists Aaron, Dino and Hugo were also very kind and patient while giving their therapy sessions. In the kitchen, everyone was kind and the food was great and punctual!

was taken to Glendora Canyon Transitional Care after several…read moreweeks at the hospital due to a septic infection. After that time in the hospital I was unable to stand up and walk. I was weak, had lost some weight and needed to get my muscle mass back. Even though I did not received enough PT due to my weakness, I was treated very well.
