The memory care director is easily accessible and often found on the floor helping. The memory care has a full time RN with his office located in memory care, the meals are prepared on site and served warm and fresh. Everyone from the director, aides, chefs, entertainment director and housekeeper were involved in my dad's care. Moving a loved one into memory care was not easy, but during my father's year at the Springs, he was well cared for and was able to stay at the Springs and pass peacefully on hospice care. My father had been in multiple facilities in the Eugene area and it was not until we moved into the Spring's Memory Care that we felt he was safe, well cared for and stimulated with activities.

One of the issues was apparently that I didn't say hello and properly introduce myself to an RN. I was also told I wasn't asking enough questions while training with a med tech. As a new employee, I was there to learn, and I was actively listening and paying attention. I felt I was judged over small things instead of being given a reasonable opportunity to improve. In my experience, some staff were overly sensitive and management did not handle the situation fairly or professionally.

She died on Oct 2nd and they had the audacity to try to bill for the majority of Oct. On the staff's watch one of the nurses turned off my moms sensor on her bed and she had a bad fall. SHE PASSED AWAY 10 DAYS LATER! She did not have any previous physical ailments prior to move in and she was only 68 years old. Hospice mentioned that they think she had a broken hip but was never taken to the hospital because we were told she was transitioning and morphine was administered to help with the pain. I stayed there for 7 days watching my mom die and the stuff I saw was insane. The lady in the room next to mom was screaming bloody murder at 5am saying she was cold, I can only assume they were giving her a shower because I tried to take a shower on 3 different occasions and had the water running for over 30 min and it never got warm. Another occasion when my mom was not responsive a nurse came in there and stuck her hand down her pants to see if she had peed and right before that she asked me to leave the room. She was wearing a diaper that showed a blue line if wet. She did not change her gloves as she proceeded to go about her business throughout the room. My mom had to be rotated every 2 hours to avoid bed sores and they were using the community pillows from the living room, my mom died on those pillows and we asked what they wanted us to do with them they said they would keep them. no covers or anything, so if you see America pillows don't put your face on them. I can go on and on. I was in shock and still am in shock with the whole experience. If I could go back in time things would have been handled a lot different but I guess I should have questioned a lot more! Please do not move your loved one here, especially if they are unable to express themselves verbally.

As she improved I was allowed to help mom with toileting. The rehab team did a good job. She progressed and worked hard to get out of there. They were under staffed and had very young training staff. Mom's room never looked as nice as those pictures barely had a chair for me to sit in. And had to hunt for a second metal folding chair for a second guest. It was summer so it was warm enough and sometimes too cold for mom. AC running. Due to Covid we were all wearing masks. They did have a good security system for visitors.

Spring Valley Assisted Living suffers from multiple issues: 1) Slow Response Times - Call buttons left ringing for over 30 minutes (mother had to sleep on the couch one night in her own feces), 2) Unsanitary Conditions - Strong urine odors in hallways or unemptied trash, 3) High Staff Turnover - Constantly seeing different workers, 4) Poor Nutrition and Food Handling - takes up to an hour to receive your food in the dinning room, meals are cold, and dining room fails to accommodate specialized dietary needs (e.g., diabetic diets, spicy, etc...), and 5) Main hallway Smells Like Smoke - employees crowd the exiting smoking. Family members have approached Management on several occasions, unfortunately Management is totally unresponsive. Continued response in all areas is, "We are working on it." This brings me to my # 1 complaint - Service Levels. There are none. Contracts, policy and guidance, and brochures outline care, they do not document acceptable/unacceptable levels of service. Absolutely no metrics!! Patients and their loved ones do not know if they are receiving the care they are paying for because they have no idea what level it is. I would like to see an audit from an independent agency! Anyway Spring Valley Assisted Living is a Very, Very, and Very expensive Shit hole!

The location is pretty close to town, but they really need to focus on the quality of the food- this is really crucial as the cost of the location is rather high with respect to the quality of the food. The activities were not few and not participated by many. There are some great staff here though, and they really want to try and make it work for the residents, but it appeared they were always short-staffed. For the cost, it is not an optimal place.
