My Mom was there for 4weeks in September of 2022. I was with her every day all day. It sometimes…read moretook 20-30 minutes to get staff to respond to call button. As she improved I was allowed to help mom with toileting. The rehab team did a good job. She progressed and worked hard to get out of there. They were under staffed and had very young training staff. Mom's room never looked as nice as those pictures barely had a chair for me to sit in. And had to hunt for a second metal folding chair for a second guest. It was summer so it was warm enough and sometimes too cold for mom. AC running. Due to Covid we were all wearing masks. They did have a good security system for visitors.

I was placed in a single room with a shared toilet. The room was freezing cold there were no blinds or curtains for the windows. It was the first of November 2023. There was a constant beeping of what sounded like a pager. I only had a call button and a controller for the TV which I lost in the dark because I couldn't control the lights. First, full day. Lots of surveys as though I was going to be there for 4 weeks -- I think not!! Very little true PT like I had received in the hospital. So I began doing all the exercises that I knew how to do. Food was worse than a bad cafeteria and cold. Everywhere it was cold. I could go on and on but I think you have the idea. After two more days, my wife and I finally convinced them to agree to release me. What an ordeal and one I hope never to repeat. Advice: check out facilities before you think you may need them.

My step-daughter said she heard good things about the Creswell PT so I went. Glad I did! Josh and Diego are a great team. They developed an evolving program for me. They built up my core. After major spine surgery, my physical therapy--an essential part of my recovery--was interrupted due to billing issues that could have been resolved much sooner. I contacted both Kaya (receptionist) and Lauren several times and relayed instructions directly from Trillium Insurance, but no action was taken. I was repeatedly told I was incorrect and that claims could not be resubmitted. As a result, my care was delayed for two and a half months. Once Lauren finally contacted Trillium, the issue was quickly resolved and the bills were paid--showing this could have been handled much earlier. As of today, all bills are paid, yet no one from the clinic or management contacted me. I am still without physical therapy due to this delay. This lack of communication and follow-through significantly impacted my recovery.

I have been going to this clinic for 3 years with my whole family . I had my annual cleaning and…read morex-rays in July. Patty, the receptionist, asked if anything had changed on my end regarding address and insurance, to which it had not. Didn't pay anything for that appointment (because that appointment was preventative care and covered 100% by my in-network insurance). But I needed to come back a week later to have a cavity filled and was quoted $90 because they were in network with my insurance. Had my cavity filled and Patty told me that the price was actually only $75 per my insurance, which I paid. Then, I find out a week later that they went out of network with my insurance 6 months prior, they never told me. When Patty asked me if anything had changed on my end, that would have been a good time for her to let me know that things had changed on their end . Their website clearly states they are in network with my insurance (Aetna) (Patty didn't even know they had gone out of network 6 months earlier ) and now they are charging me for everything and refuse to work with me on it . They are taking no responsibility in this matter. I have been very kind and professional in trying to resolve this matter and they have completely ghosted me and they keep sending me bills. Super disappointed in their lack of care and unprofessional manner.

Aside from the incredibly devoted staff, the activities and delicious food, there are some of the best nurses and nurses aides I have ever worked with! It is extremely challenging, in many regional facilities, to find and keep quality long-term-care staff, and CGPA manages to retain some of the kindest, most professional staff I have ever worked with. Additionally, the parent company, PACS, is a company with an outstanding reputation, and a demonstrated track record of providing very good care in multiple states. Despite living 35 miles from CGPA I would choose them for me or my family over any facilities in the Eugene or greater area hands down! They are excellent!!
