The people at Wolf Creek Care Center in Grass Valley, took absolutely wonderful care of my mother following a stroke. All the staff members there were warm and kind and did their best to help mom recover in as much comfort as possible. Vicki McDaniel, the social services director there, went out of her way to make sure that mom got a TB test, a Covid shot, and that the necessary paperwork was filled out and emailed to get mom into an assisted living facility closer to family. In 2023, my father received a diagnosis of stage IV cancer, necessitating full-time care. Residing in Southern California while I was in Nevada County, I faced the challenge of relocating him closer. Through the invaluable assistance of Samantha Phillips, a dedicated professional aiding families in such transitions, (now a nurse at Wolf Creek Care Center) we secured a place for him at Wolf Creek Care Center in Grass Valley. The care he received there was nothing short of exceptional. At Wolf Creek, the compassionate attention from the CNAs and nurses was complemented by the heartfelt dedication of social worker Vickie and activity director Debbie. These two remarkable individuals not only fulfilled their professional roles but also became cherished friends to my father. Embracing his new community, he took on the role of president of the residents' council, actively participating in monthly meetings to enhance the quality of care. He held the entire staff in high esteem and affection. Wolf Creek Care Center became more than just a facility; it was truly his home. Given his inability to walk and my limitations in providing the necessary care, it was a relief to know he was surrounded by a team that treated him like family. Their unwavering support and kindness have left an indelible mark on our hearts. For families seeking a nurturing environment where their loved ones will be treated with the utmost care and respect, I wholeheartedly recommend Wolf Creek Care Center. Following my father's passing on January 16, 2025, the staff graciously invited me to join their team as a volunteer. Now, I will have the honor of visiting and supporting the residents they so diligently care for, continuing the legacy of love and community that my father cherished. I stayed at Wolf Creek for about 3 1/2 weeks after getting lassoed with my dog's cable line and breaking my femur. I can't say enough wonderful things about my time there. All of the staff were warm, kind and helpful. From the top administrator to the doctor, nurses, cna's, kitchen staff, PT and OT staff, social services and cleaning staff. Seriously everyone was amazing. I developed such a lovely rapport with each and every one of them. As happy as I am to be home, I couldn't have asked for a better experience in a skilled nursing facility. If I ever need one again, I will definitely go back there! read more

5 STAR Review ***** Very Pleased with Wolf Creek! I wholeheartedly recommend Wolf Creek Care Center! The place might not look like much but that is so not important in the big picture. From the very first day I left my husband here I felt comfortable and knew he was safe a well taken care of. He is here because he needs care due to mobility issues and the care is above and beyond expected! The whole entire staff is amazing! Very cheerful, kind, helpful, respctful and professional! Vicky the Social worker and Keith the administrator are both not only kind but listen and are very attentive to any concerns myself or our family have. The facility is very clean and does not smell bad! I need to add also that my husband is a kidney dialysis patient. Jody the Van driver gets him to his appointments on time 3 days a week and is there early waiting for him when he is done. In the beginning the kitchen staff came and talked to my husband daily and custom tailor his meals to his diet and preferences. This SNF facility is aweful. My mother received the worst care possible. The Nursing Assistants are rude, inconsiderate to the needs of the patients, they just don't want to be bothered to help. They stand around in the hall gossiping about other co-workers and their bosses. It is a constant negative environment. You can hear the nurses and Managers talking about other patients and their problems. The facility is disgusting, the rooms are very small. There is no privacy anywhere and nothing is confidential. The bathrooms are only cleaned once a day and you have to share the bathroom with 5 other patients. The building always smells horrible. I would never recommend this facility to anyone. The building should be shut down. It is definatley the worst of the worst place to try to recover from. You would get better treatment living on the street. read more

I placed my mother here for respite for 5 days while I went out of town. It was awful. The aides didn't listen to my mother when she needed to be rolled over slowly. The aide just manhandled her and then hurt her leg. When I came to sign her out to go and take her home I noticed they had her in some man's tee shirt. I had a suitcase full of her clothes in the closet, where they were supposed to be. Then I was waiting for the aid to come and dress her so that I could take her home. They asked me can't you just take her like that and put a blanket over her? I told them my mother would not leave here without her pants on! After I got her home I changed her from a wet brief to a dry one and noticed she had fecal matter inside her and was never properly cleaned. I felt so awful for leaving her in such a horrible place. read more

I used to work there... everyone talks about everyone behind their back... nepotism is encouraged there... people think they can show up to work and leave from work whenever they want cause they have "family" who does schedules... they allow 2 people to work on NOC shift even when the census is low... and still want to get everyone up and dressed... the CNAs talk about the patients and family members very unprofessional. If someone has an issue with someone, they won't go and confront that person... you have a CNA who take 30 mins to change one person and leave their calls lights going off for almost a hour. Also when other people go to lunch a (certain CNA) won't answer their hall partners lights and they allow it to continue!!! You have CNAs who are on call but won't call them to fill a shift when someone calls of.. makes you feel bad when u have a personal injury. There's never any towels when it comes to drying patients off after shower.. they'll take any patient just to fill up a bed. One patient hit a CNA in the face and they went to bat for him.. highly unacceptable read more

My brother was admitted for the hospice care. Unfortunately, besides there is no privacy offered as each room has 2 patients, hospice patients are not in a separate unit, and privacy for the patient and family do not exist. Honestly, when family was not present, care reflected it. Had to request help for basic needs often. In retrospect, Hospice appeared almost invisible since he was in a care home. This was not the hospice service I experienced with home care. I hope you and your loved ones have alternatives to a convalescent home. read more

In September of 2022 we came to the point that we could no longer take care of my wife's sister and was very fortunate to be guided to Wolf Creek Care Center. The entire staff has proved to be so caring, loving, and helpful. we don't know what we would have done without them. By entire staff I mean administration, nursing, cleaning, all ! On all of our visits we have seen nothing but a very clean well cared for environment. Residents are clean, Hair combed, and watched. There are group activities and food times, or individually served if needed. We are very thankful for the care sister is receiving and is happy there. We highly recommend Wolf Creek Care. I had heard some unfavorable comments about wolf creek and am happy to say from my perspective they were unfounded. She has received excellent care at all levels. I have had two parents who landed here with broken hips over the past year during Covid. Wolf Creek is a basic facility, no frills, not an expensive private institution - but I have to say - the care and support they both received was impressive and effective.. Every member of the staff - from the nurses and doctors, to the physical therapists, to the administrators and maintenance staff - all were kind, competent, thoughtful and encouraging. There were challenging medical issues that were handled and resolved perfectly, but the difference here is that the caring staff made all the difference in my parents recovery. I have a family member who was transferred here from the hospital 2 days ago and I have called them at least 10 times. WHY SO MANY TIMES? Because NO ONE ANSWERS!!! I thought I had the wrong number! I checked it again and again - NO ANSWER. Finally after 3 minutes of ringing the 4th try someone came on and had to transfer the call to my sibling's nurse, then the nurse takes the cordless phone to my sibling. It's an awful cordless phone - it cut out on us 2 different times and AGAIN I had to call several times before someone finally answered. On the 6th try of the first day I asked the guy who picked up after 3 minutes of ringing, why I have to call this number, doesn't my sibling have a phone in her room? He said, "no one has phones in their rooms. I have to transfer it to the nurse and the nurse takes the cordless phone to the patient." It was 5:20 minutes before I finally was able to talk to my sibling the 6th time I called. When I complained to my sibling she repeated what I said to the nurse. I asked to speak to the nurse. She said, "Why don't you just call her room?" I said because NO ONE gave me the personal phone number for that room in fact I was told those rooms do not have phones. She laughed and gave me the number. My question is, what was this crap about the patients not having a phone in their room? Apparently that was NOT true. So I was finally able to call my sibling's room. The second night I wanted to talk to the nurse and called 2x and NO ONE EVER ANSWERED - after ringing 3 minutes a message came on that said "the wireless person you are trying to reach is not available at this time. Please try again later". WHAT IN THE WORLD???? This IS REDICULOUS. TOTALLY REDICULOUS. I think I just stepped back 30 years by calling that place. I have definitely experienced better places! I hope my sibling can get out of there in this century. PS: Curtis, thank you for your response however the number you left IS the number that nobody answers. I will remove my review (or upgrade it) when someone starts answering my calls - today I called that number three times and twice there was no answer. Maybe you could put a bug in someone's ear to PICK UP THE PHONE when a call comes in. Thank you. PPS: I edited this and upgraded this on 1/09/17 as my family member felt she had good care here. This place has bad juju! The staff is underpaid resulting in a lot of turnover and a lack of consistency in caregivers. I spent several weeks as a skilled nursing patient and would go out of town before going back.. read more
