Hire better staff who have respect for their job and the elderly they care for. 90% of staff looked disheveled and tired. Imagine the incorrect dosage given to patients due to their lack of sleep and alertness. Better cleaning of rooms. Better ventilation. Honestly, they need to be audited or inspected because their environment is similar to a horror movie for the elderly. read more

I was discharged a few days ago after spending two & a half weeks on the fourth floor. I feel most of the staff there is hardworking especially Caroline who tries to help meet the needs of the patients on the floor. Also the physical therapists are professional & kind while providing encouraging direction to aid patients towards their goals. read more

If I could give this hospital a zero I would. completely terrible care for parents and cleanliness. They did not have my grandmother on any type of monitoring after she already had her heart stop in their care, and when we went to pick up her things (not including her phone because it was misplaced and no one knows where it is) they hydrogen peroxide and alcohol we brought was partially open and all of her belongings were wet. when asked about the phone our social worker told us she did not know and had no answer for us. I haven't been here 24 hours and I want out! My bed mattress is solid metal with no cushioning. I just had a laminectomy and ALIF SPINAL FUSION. I have had to go pee sincr last night when i was brought in. I didn't get catheter till 610am-6:200am 05/11/24. As you will see with your own eyes. I had at least 1000cc of urine. The nurses were waiting for Dr. Approval to use a foley catheter. I WANT OUT NOW!!!!! read more

My father-in-law stayed in Warren Barr South Loop from June 1st to June 30th after his hip surgery on May 28th. His experience at this facility was very bad. I would never suggest that anyone get care at this facility trying to rehabilitate. We had family with him everyday from 8am to 6pm, our requests were ignored or took a long time to get responded to most of the time. This is the conclusion from everyone who was there with my father-in-law. Some of the caregivers lack professionalism. It appears that the management did not correct any of the issues. First day, he was in a room with 3 residents. They changed him to a single room after we addressed the noise and smell issues to the manager after the first night. Few days later, they added another resident. The room is too small for two beds. There was no additional seat, and it was difficult to move wheelchair in and out without moving all the furniture around. The other resident had his TV on 24 hours a day. We addressed the issue to both the administration and nurse numerous times. They advised that they were sorry, but they advised they can't help with this issue, since this is the patients right to have his TV on anytime at any volume. They asked my father-in-law to wear ear plugs at night if that distracting his sleep. There was no solution they would offer to fix, so we decided for him to leave before he was fully recovered from surgery. Due to disruptions in his sleep every day and night, the entire process of rehabbing has put him in a state of stress at all times. He didn't have good rest after the first day he had a roommate. This is not the resident's fault, but this is the facility unreasonable policy. Some nurses have very bad attitudes especially the nurse who was on duty on the 4th floor on June 22nd morning shift. We would not be able to name the staff employee because not all of them are wearing name tags, and some of them are part timers from agency. In the early morning we informed the nurse that the bathroom was a complete mess with papers and plastic bags when we stepped in. First, she said I should wait for the CNA or housekeeper. I told her that a resident needed to use the bathroom. She cleaned the bathroom floor and told me "just papers and plastic bags", implying I could of done it myself. I could read all the annoyance and impatient attitude from her facial expression. Later in the afternoon, when I tried to inform her of a plumbing problem with the bathroom sink. She was very rude and told me that I had interrupted her (chitchatting) conversation with her co-workers. She didn't flush the toilet after she emptied the pee container from the other resident. Of course, I just did it myself every time when I saw it. PT is only team I could give a praise to during my father-in-law stay. Our primary PT is Drew and sometimes other PT on some days. Drew has done an amazing job with my father-in-law progress. We are truly happy with everyone from their PT team. If we would get to choose again, we would not choose to come to this facility. We do not want our loved one to suffer and be stressed during their recovery when they should be healing. read more

The nurses sleep at the front desk at night, on camera, and this is why the residents can't get a nurse at night. The residents sleep at less than 65 degrees and you better not complain bc they can't handle critical discussions so the climate is respect this as status, or leave. They feed your family cold food breakfast lunch and dinner. It's a hot meal surely but by the time it reaches the residents it's cold. They have to microwave this. . The rehab is run by therapist who, if they don't like you, will send your insurance company false information that you don't need therapy, which will stop your insurance, then, the billing department will come to advise you that your insurance has ended ..and they want you to believe just by chance that occurs! You ask for the doctor and the nurse will not call or if they do, paint a bad picture. There is no grooming. They rather be on Facebook. This is definitely a beware. The administrator is ok w all of this because if not none of this would exist. read more

My grandma FELL going to the bathroom and was sent to UIC Hospital on a Saturday Feb 13, 2021!!! Families was NOT notify at all!!! We found out on Feb 15,2021 Monday, from the social worker that was helping us !!!! Where the hell are the nurses that was suppose to be assisting her ??? Why weren't family notify when she fell ??? They are trying to cover it up and act like nothing ever happened! My grandma had a BIG bruised from the fall !! 3. My grandma was dropped off at her apartment with NONE of her personal belongings!!!! She came back with NO JACKET IN THIS 10 degree weather, NONE of her earrings, NO hearing aids!! Where are her stuffs???? I had called UIC Hospital and the nurse that was assisting my grandma said she didn't come with nothing! Everyone please don't sent your love ones to this nursing facility. If you care about the well-being of your loved ones, do not place them at Warren Barr South Loop. My mom was here for a few months after dealing with complications (i.e. a stroke) from cancer. The team ignored my requests to let her Northwestern doctors take the lead, instead focusing on the care of an internist who would run tests and change medications without so much as a phone call to me - as her power of attorney. They were routinely over-medicating my mom and then were confused why she was sleeping all day. I tried to troubleshoot the issue with her nurses, social worker, administrators and even the Executive Director. All of them went through a game of them telling us (family) one thing and doing another (with patient). It's a series of "feel-good" conversations, hiding behind COVID-19 instead of taking responsibility for the fact that they are understaffed, under-qualified and struggle to communicate across departments - something one of their employees admitted to me at a doctor's visit. While it certainly has been a challenge with COVID-19, we patiently waited for instructions to see one another. They finally called to let me know they were allowing visitors and when I called to schedule a visit, somehow they weren't allowing visitors anymore. Poorly run is an understatement. I absolutely understand that COVID-19 has put all of us through the ringer. I also expect that someone - anyone - would respond when they're responsible for the health and well-being of someone's loved ones. Every doctor has told us - it will take a partnership between health care workers and family. A partnership with Warren Barr South Loop is the most inept type of relationship. If your goal is to spend a good portion of your week chasing down nurses to verify medication lists and doses are correct in addition to making sure they have correct doctor's appointments and more - all from the outside since they won't let you in - then this is the place for your loved one. Please learn from our terrible experiences, your family deserves better. read more

I had a friend who stayed here for a few weeks after he was in the hospital. His initial intention was to go to the gold coast location because it is closer to his residence in the Mag Mile/Gold Coast area. Because there weren't any rooms available at that particular time, (I guess because they are usually pretty full- who knows) they promised him that they could accommodate him in one week later and offer him a room at the South Loop location until that time. When I visited him there for the first time I was really confused because it looks like the hotel. I thought maybe I was in the wrong building. His therapy went really well and the care was second to none. This is what I felt and this is what his family felt. He really did not want to leave when his room at the Gold Coast location was available in fact he stayed a week longer. I think that says a lot about the place and the staff. He only agreed to switch locations because it it was closer to his family and friends who mostly don't drive. Bottom line is that he was treated with the utmost care by the staff and I feel like they went above and beyond every day to make his life better and to ultimately get him back home. The one drawback that I remember is that, at the time, there was a brand-new therapy gym under construction that he didn't get to use. His therapy quality was not affected though because he was improving the heck of a lot every time I saw him. I saw him 2 times a week if i recall coreectly Im going to to go back to check on the therapy gym...just out of curiosity :) read more

If I could give this place zero stars I would. My mom was sent here requested by the hospital not our choice. She was doing great. Within 2 days of her being in this place this place she started to change. She became really sick. When it got, she a point they weren't even helping her I decided to help her and take her out and they wouldn't let me. They gave me this whole cycle of stupid reasons. Finally, I told them either they call and ambulance or I'll call the police and ambulance myself. Finally, I got her to a hospital she was so sick. They neglected her. She begged me not to send her back. She told me so many things on what they did to her I couldn't believe it. This place killed her. Do not send your loved ones here. Good physical therapy daily M-F. Meh food, per patient: "Hospital had better food." Free parking lot for visitors on west side of Wabash across from front entrance. Very observant of patient privacy protection protocols. Our loved one was taken to the hospital via ambulance from this rehab facility and she never returned here, nor to her home. Warren bar is definitely a good nursing home. The staff is attentive and appear to care about the residents at that location. I love the infused water and chocolate chip cookies that the staff sets out for guess on occasion. Nice touch! As a Jehovah's Witness, I have two other Witness women who are cared for at the Warren Barr that I visit near the South loop. The two women are generally in good spirits. My mother is currently at this nursing home for rehabilitation. The place is dirty and the service is horrible. The staff is rude and unprofessional. When my mom had to go to the restroom last night, she pressed the call button for an hour before someone showed up. The afternoon nurse is extremely rude and when asked her name, she refused to give it to me and my family and proceeded to covered up her name tag. We are working feverishly to relocate my mother as soon as possible. My mother is elderly woman, in a tremendous amount of pain, and should not be treated this way. read more

I am soooo...sooo...sooo....grieved. Every advertisement actuating positive features and outcome, are complete opposite. The only positive, was the downstairs lounge. On February 20th, I received the worse news of my life. This facility may need to be closed down. This is so unfortunate, concerning that finally a nursing home central to every loving family who want to stay connected with their loved one is made so possible, is cut short, loved one immediately discarded, and no one return a phone call answering what? how? and when? You are not even invited to pick up any items that were left by your loved one, or sentiments you yourself had for your loved one in their room. I am so heart broke, truly. I am a mandated reporter and this is one of the worst facilities I have ever seen. I have a brother who is in this facility. Trying to get in contact with someone is nearly impossible. When I did get a return call, all they do is say they will look into the situation and call you back, but weeks go by without a return call. The first social worker made numerous promises but no follow up. When she finally answered her cell phone she was happy to say that she no longer worked there, which is the only reason she answered. So, numerous messages were left trying to determine who was/is the new social worker. After a week, the new social worker finally met with my brother and I was on the phone. I do understand that she was just assigned to him and didn't know all the particulars. I gave her my cell number since she said she would follow up, and provide me with hers, which I never received. I have placed several calls but no answer and no return call. No one can return a phone call but the billing department has no problem stating that your check and pension has to be turned over to them. It is so unfortunate that our health facilities don't care about the quality of service that they give and is only concerned about the monetary aspects of the business. All we are seeking is for the discharge process to begin so he can get the services and equipment he needs when transitioning back home. For sure, they will never receive any money for this mediocre service, lack of professionalism, and nasty facility. If you are thinking of placing someone in this facility, then think again. I will report them to IL Department of Public Aid for all the good that will do. read more

Patients are not being cleaned when they need to. Caregivers are not respectful or compassionate towards patients. The facility is greatly understaffed and occupied by Caregivers who do not Care or properly take care of patients at this facility. Management has a very poor hiring process, not hiring people with the correct credentials or enforcing rules. They're allowing the mistreatment of these people. read more

We had my dad transferred here from WB Gold Coast, if you haven't read that one, read it first. Dad was moved into a private room that looked like it had been used for storage. It only had a broken bed and a tiny beat up old side table and a TV that didn't work. The walls were dirty and had gashes. No staff came into the room after the ambulance attendant's left him. I went to the nurse's desk where I found two CNA staff, one I believe was actually asleep. She said she was tired, the other had a frown on her face and was watching the clock. I asked what was wrong here, she said she was off in a few and was just watching the clock and couldn't wait. We began our search for a nurse, and finally someone came to say hi, and left the room. Ok, now dad didnt have blankets toiletries, no one asked me any questions about dad. So I began to make calls to their admin, which everyone was off, it was a Sunday. I reach Tim, (nursing supervisor and a blessing), which assured me that he would take care of everything the blessing) by getting dad into a better room. He did a great job, however, he is only one. Most CNA's were extremely lazy & just need more training. A couple went over above to help. I felt I could not leave my dad, because he would be left in the room alone with no care. He was a fall risk, but when he would turn his light on, usually no one came. So he would get up alone to go, wet his pants on the way and risk falling. This is the worse, late one night dad called to say he hadn't had his medication. My dad has congestive heart failure and takes a blood thinner and heart meds. I call the nurses station to check on it, however, the nurse told me she was "only one nurse and I would get to him". I kept my dad on the phone for a while and she never came. 9:30 the next morning I asked him did he ever get his meds, he said no she never came. I was outraged! They ran the security tape back, to find that the nurse never went to his room that night. He had missed his medication for 3 days and didn't get it until the next afternoon. Now this is very serious. I wonder how many other times did he not get his meds. My dad's body is weak, but his mind is very strong, so he is able to tell me. This place is lacking so much. It is NOT sanitary. Ice is kept in a portable picnic cooler sitting on a counter at the nurses station, with a scoop where anyone, and everyone can scoop ice. For water, go to the break room where there's a tiny bathroom sink, where you wash your hands, to fill my dad's drinking water picture. I went out to buy bottle water. One day they had no pillow cases on dad's pillows. After complaining, they said they ran out, and was washing them. So they wrapped his pillows in sheets. Too many bad attitudes among the CNA's. There's a couple I felt comfortable leaving my dad with, Pam (so great with dad), Latoya and Shana. I must give physical and occupational therapy thumbs up. That is a good team, especially Drew. I felt that my dad benefited from that department only. You complain and you get all these get apologies and "we will fix it". I ended up tranfering my dad back to the hospital, he developed fluid on his lungs. I wonder if missing his meds had anything to do with it. Hmmm... read more

Facility is really nice looking and clean. However the second floor LPNs (one specifically) is not very friendly or professional towards EMS personnel. Simple question looking for a particular person and the LPN started yelling at crew member and continued yelling down the hallway in an extremely unprofessional manner. The crew decided not to engage and walked away. The final ticket to this LPN was the fact she screamed down the hall asking " do you understand English? Do you understand the English I'm talking to you?" Personally I wouldn't want my loved ones nor myself being cared for by this type of person. They were supposed to be a facility that did therapy and had doctors on staff. And real nurses. Actually they do have those. But everyone looks as if they are in training or over worked. Nice staff and some are really caring. But The social workers are doctors and nurses are questionable. My mom has a very serious wound and they sent her home. After I begged the nurse and doctor to look at her situation again. Even get her back to a hospital but no they sent my beautiful mom home. The doctor and therapist at warren bar told my moms insurance she was ready to go and they see know reason to keep her as a patient. My mom was in pain constantly and they never even gave her test to see why. After 3 weeks home. she was rushed to rush hospital with a whole entire hole in her back from not being moved regularly. Absolutely disgusting! The women that work there are ussual to busy playing with the phones or eating to actually help the patients. I had a friend who was recovering here and he would tell me how the nurses would scream at him and throw him around if he had an accident. The people who work here have as much compassion as members of the SS Totenkompverbande at AuschwitZ. Don't ever let someone you love stay here. This place is horrible, don't believe the pictures you see online this place is a joke they neglect and abuse their patients my mom has been there for a couple months, and it's been a nightmare and when i say that i mean just that. I have complained to the higher up staff on several occasions, and nothing was done the things that i would like to say i can't but please look into this place before you decide to put your loved ones in here. If you want your family to be Mentally and Physically abused this is the place. It's a horrible place and that is the truth. Not all the CNA's are bad there is a couple that i truly like that really take good care of her, and a couple of nurses but for the most part the staff is not caring at all I've been there in with my mom she would push her call light and no one would respond and when i go to the nurses station the nurse would say i don't answer call lights the CNA does that what kind of nurse would say that. My mom called asking could we come clean her up the weekends are horrible no staff it smells. I don't want to attach photo's because they are disgusting. I wish I could give it no stars. It's like a shell it looks good on the outside but filthy inside the patient rooms. My cousin is there unfortunately. At first she was at a facility that was not nearly as modern that did not have a lot of financial support but the difference is you hardly see the medical staff here checking on the patients. At the less advantaged facility, the nurses were very passionate. You saw them visiting the rooms and the trash was never overfilled in the patient rooms. read more

All week I've been struggling to have home health services set up for my terminally ill parent.I've left messages with the social worker who dropped the ball and she's never returned my calls nor that of the facility's own liaison who's been trying to help me. This is unprofessional and unacceptable! My dad was there for a few days and if it weren't for me escalating to the associate Director a couple of times he would not have had any meals he could eat or his medication given as directed. The only reason he was there to begin with is because it was the closest facility that was covered under his insurance. However. If I had known how bad our experience was going to be we would've been willing to pay the 40% of cost sharing to send him somewhere else. No patient or resident that lives here should be treated the way my dad was treated. In my opinion the director, social worker and most of the nursing and kitchen staff are working in the wrong field. They have no business working in ANY skilled nursing facility. read more

Update: I also learned today that they also used to never change her diaper. Apparently she would only get changed when someone would visit and physically go get a nurse. Who knows how long they used to let her sit in that?! I know it's not a glamours job but come on. You signed up for this occupation. For the most part if get a good nurse, which is extremely rare because there's literally like one or two here, they're great. Otherwise, the rest are lazy and have attitudes when you ask them to do anything more than the bare minimum. They'll say they just did something when they clearly didn't. They didn't wash my grandmother for a week once! It was ridiculous and when I asked if they could wash her or give her a sponge bath or wipe her down or anything they said they just did it. I left a slight mark with my eyeliner on the inside of her arm when I went to she her last to test this theory and it was still there when I went back a week later. Keep in mind eyeliner wipes and smears fairly easily so tell me why the mark was only slightly smudged. It would've come off easily with any type of contact. When I asked them to change the sheets because my grandma had wet herself or even getting her a bath/wipe down they gave the eye roll and puff like it was a lot to ask and just mutter that they just did it. Clearly not because if you did her sheets wouldn't be wet. Even if you did just do it and it happened again it's your literal job to take care of your patients. If you have other options I recommend elsewhere. read more

I arrived to see my relative, earlier than usual, and was told that the patient went to PT. I waited inside the room, sitting, and listening to the next door patient's TV. All of a sudden, from the corner of my right eye, I see something zoom by outside. I was sitting close to the window, where you could see the little patio they have for the patients. I decided to place my phone camera in a position where it could record what was going on, and it did.... it was a bunch of rats looking for food, out in the patio. I then began to wonder if Warren Barr had rats inside their institution, and whether they roam around in the kitchen when everyone is sleeping or there is little activity in the kitchen. It gave me the shivers to think that these ill patients can't do anything about everything going on in the institution, either for fear of retaliation by the employees, or simply because they are too weak or old to notice. read more
