I expriences with both Elevate Care facilities Winshire Park & Des Plaines locations.This my experience thus far as my mother Ms.Pat stayed in both estabishment for rehabilitation. First The Winshire Park location is tidy could use a few upgrades that the Des Plaines location has as to why its not equal upgrades a big question mark. Ms. Pat stayed at the Des Plaines location temporarily because The Winshire Park location had a serious maintaince issues that needed immediate action, and was taken care of right away. My mother is back at The Winshire Park location currently.Now THE STAFF! (Winshire Park) so far it has been a love bomb experience day one from meeting Yolanda & Kim in charge of admitting and registering Ms.Pat in the facility.I also met Lisa ( The Director of Physical Therapy) Ms. Pat arrvial to explain the process of her recovery. My call with nurse Crystal was very pleasant the evening of my Ms.Pat arrived the night before. I received more than a few update calls in her transitions from the hospital to Elevator Care by the Elevator Care staff.These follow up /update call also occurred during her transition from Elevator Care locations. To be honest it has NOT been the best experience with a few incidents ( I considered circumstances with the incidents),However when I expressed my concerns and grievances to Management /Staff (Kim who was right on it expeditiously). I met with Jeremy (The Director of Elevator Care ) and Christina ( Director of Nurses) I was seen immediately ,heard, and actions was taken right away! Christina updated all the imformation in Ms. Pat files to ensure the nursing staff was equiped to properly assist Ms. Pat so no farther incidents could be happen. Christina also immediately apply my requests.The NURSING staff, head nurse Louis , again nurses Crystal ,then Raphael ( who is/ was my mother /Ms. Pat nurse at both locations) nurses Fendi, Chanel,Dee Dee,Valentine,and today meeting Jill are personable, informative ,and answered all my questions with follow up updated if it was needed.The CNAs Mary & Heather (especially after the incidents took the initiative to make sure Ms. Pat is always good ). CNAs Tatinana, Tess , Gene, two other CNAs I didn't get there names are great they handle my mother care!!!(Ms. Pat is spicy full of life quick witted just so you know) ,BUT CNA Hanes RUN as far as you can!!!!! Why she still there gods knows its always one bad apple I suppose just NASTY UNPROFESSIONAL dispostion. The Physical Therapist Danielle (chef kiss) Chanice, Hannah, Deanine are great ,patient ,yet firm as well as light hearted working with my mother more importantly they do their job well . My experience with LaMonica (she helps with the transition for home care) was brief so far nothing to say because my mother hasn't reach this point yet.Also Tierra ( finances) is very informative. I'm still in the process of my experience Elevate Care 75th exchange, but so far with bumps here and there my experience hasn't been awful.I'm keep you posted once my mother Ms. Pat is discharged. read more

To Whom It May Concern, I am writing to provide comprehensive feedback regarding the care my aunt, an 81-year-old resident with fragile mental health and significant underlying health conditions, has received at your facility. She has been a resident for nearly three years, and her established familiarity with the environment has been a primary consideration in our continued choice of your facility. Historically, I have encountered numerous and severe challenges concerning her care and the facility's administrative processes. For instance, an attempt to address these issues on May 13, 2026, by scheduling a meeting with the Director of Nursing and Facility Manager, proved unsuccessful as neither party attended. Instead, I met with alternate staff, necessitating a reiteration of long-standing concerns that I had anticipated discussing with key decision-makers. A persistent and alarming issue has been the facility's failure to ensure my aunt attends crucial medical appointments. Despite prior discussions and the facility's awareness of her inability to attend independently, appointments were frequently missed due to reported staffing limitations for escort services. Compounding this, there was a consistent lack of communication regarding missed appointments, and the responsibility for rescheduling was not assumed by the facility. I have complained to the Board of Health numerous of times. My aunts financial affairs are not in order with the facility as well. The business office has been getting payments since day one she was transferred to the facility. This is concerning and very disturbing read more

So, I found out that the director at the Elevate Windsor Park Nursing Home off 75th Street in Chicago, told my 80year aunt that's he's avoiding my calls because of a review I did months ago. I have called the director of nursing several times since Thanksgiving and afterwards and until this day he has not returned my call. When he thought about what he said , he told my elderly aunt not to tell me what he had said to her. This raises all concerns and questions now, I did call the manager Ms.Mimi on Thursday of this week when I found this out. I haven't heard from her as of this very day and it's Friday! This is a very serious matter at hands , I don't want anything to happen to my aunt. I will be following up with the Board if Health and I am making this post so that anyone who have a love one at this facility should check on them. I am very concerned, why would a person say something like this? Is this a form of a threat or retaliation because I went on the website and made a review. I do recall the director of nursing asking me to take my first review down but I didn't. I am glad I didn't and now you are being very mean verbally to my elderly 80 year old aunt because you feel some kind of way. People go check on your love ones who resides in any nursing home. This can be considered as elderly abuse or just being cruel to the elderly! I hope the manager look into this matter and I hope the Board of Health as well. This should have never happened or been said or done! The sad part is I have spoken to the director of nursing and I thought we had an understanding but I guess not. I can say Ms.Williams the nurse that's there she take excellent care of my aunt along with other staff members as my aunt has told me and I witness for myself.. But again, I will not allow no one to treat or say anything to my elderly aunt that's not nice and get away with it. They have these reviews online for a reason! The Villa at Windsor Park in Chicago, IL took very good care of my disabled son. As a matter of fact, it is the best he has been cared for in several years. The place was clean and the staff was very professional. read more

First I want to say that if you truly love your loved ones and can find a way to take care of them. Please do so this place is horrible!!! The lack of care, compassion, and understanding to our elderly and handicap is deplorable. My aunt was placed in this facility beginning of August and originally I thought the place was nice because one of the administrators reached out to me and he gave me a video visit. From the phone the place looked nice. But upon placing my feet in the building the first thing I saw was nasty carpet, and patients being talked to disrespectfully. The carpet was filthy and dirty but the place didn't stink it was just in need of a renovation. Jennifer Singletary is the amazing guest services intake person she was sweet as pie. She was kind, compassionate, and colorful. LaMonica Heard the social worker is 100% the best social worker. Alesia Carlock is an informative and patient Business Office Dir. Upon my love ones discharge LaMonica Heard did the damn thang. She got what she needed and put all the pieces in place for her discharge to move smoothly. Alesia Carlock gave me all the Medicare and Medicaid information needed to get all she needed upon discharge. Gabe Schultz was not present and didn't respond to my text about my concerns for my Aunt. The facility overall is in great need of new furniture, new CNA's who care for those in their care and the administration needs to care more for the money and more for the people. Investment in people creates a better investment in the environment. The place overall gets thumbs down. But the people I mentioned are the true Rockstars. read more

During a stressful and sad time, visiting our mother before she died peacefully in her sleep, we can't say enough good things about the staff at the Villa. They were compassionate and friendly. Consumer do your research. There are much better places to receive care. Owner threatens to pull any reviews that don't speak highly of the facility. Ghetto is ghetto; unprofessional staff, atmosphere, and company. Quality measures are a joke; and do not reflect the facility as a whole; it's computerized fluff and lies. Our Dad was never going to recover to the point where we could take him home since he required 24-hour care. The nursing and resident staff at Villa of Windsor Park treated him with dignity and respect. They also were very generous to my family when we called, visited, or had questions about his ongoing health and daily condition. I think things like this happen every day in facilities around the country but no one hears about them because they aren't glamorous or exciting. I truly believe that at the Villa of Windsor Park, the employees' motto should be "It's not about you...it's not about me. It's about the people we take care of every day." This is to the DIRECTOR/MANAGER OF the Villa at Windsor Park. I had a relative who was discharged Memorial Day weekend with nothing but medication. No walker, wheelchair cane, no type of equipment for use at her apartment. One medication had to be bought out of pocket, a walker, blood pressure taker and incontinence supplies as well. Only some of the paperwork and no word on if her Public Aid was even still pending, so she was sent home with nothing but medicine. This was so wrong and unprofessional when it seems she should have been discharged after the holiday weekend. A whole week of nothing in place for her. Not even knowledge of WELLCARE NOT GOING TO PAY FOR ANY SUPPLIES/EQUIPMENT. No apology or anything to her. She deserves an apology, a Thank U as a former patient and even some type of compensation to her, at least an apology.
