I guess I got weary of military grade frames and lenses. Also, I just wanted to have more options on eye care and potentially better technology for my examination. Well, there was one impressive machine that was a true upgrade from the normal dilations that required sunglasses for hours at a time. Even with the extra $35 fee, it was worth trying and I got to see the pics as Dr. Potochick explained what he saw on the screen. No dilation. No funky shield. And thankfully, all was good with my eyes - retina in good shape! 5 Points Eye Associates has been around for a long time, since 1976. Dr. Potochick has been there since 2008 - a well-seasoned veteran in his occupation! Very thoughtful, courteous, and he'll answer all your questions. I was very impressed with the treatment and attention I received there. After the exam, it was time to choose frames. Tabitha took care of me and we worked through about 20 lenses before narrowing them down to 5. Then after working through those 5, it was clear the Ray Ban was the winner! Rev now sports Ray Ban's for his daily eyewear! These glasses are comfortable, look great, and finally, I have progressive lenses! (The military doesn't offer progressives). I'm getting used to my new progressive lenses right now. Been good so far - at least writing this review with them on was pretty easy overall! I chose to write my review on 5 Points when the job was completed before rating and writing my review. I will say that I can recommend them highly. Picking them up today and their taking care of me with solid professionalism and seeing to the completion of being sure the glasses were going to work for me was very impressive. I encourage anyone who's looking for a solid eye care to visit 5 Points Eye Associates! Also, I love the building it's in. Originally, it was built as the Home Telephone Company in 1914 and 5 Points took custody in 1976. A lot of history in this building! It feels like a classic when you walk inside! Recommend: HIGH

The young lady that I dealt with on my lens prescription was very informative and gave excellent customer service. The cashier was awesome, very friendly and was chatting with us, very informative also. She checked us out and we left . We went to lunch and I went to look at my invoice and I notices it did not show my vision insurance copays. I called back to the office and spoke with a nice man, named Darrin. He got my info, looked up my account and said they did not have my insurance info in their system, he got what he needed and said he would get with the optical department. I got a call this morning, he told me everything had been entered and yes, I was due a refund and he could credit my card right then if I could give him some info. He ran the card while I was on the phone with him, told me how much everything was and this $$ was my refund back to my card. He apologized for the office's error. I just want to tell the whole office staff and to Darren , Thank You for your Prompt service of correcting this error. The lady at the front desk asked me for my name and phone number to "put me on the schedule." I told her that I was just looking and did not want an appointment. She pressed me further and told me that they wanted to know who is in the building, which seemed odd. So, I gave her my name and phone number. Then, she asked for my birthday. I'm more or less flabbergasted at this point, since the business is located in Orange Park, not Beverly Hills. I told her to just put down 1/1/2020. Another staff member came out to request my birthday because they want to know who is in the building. Good grief, do they have a lot of thefts in that area? I have never had anyone ask for my personal details in order to look at eyeglasses frames.

My son happened to be reporting for college football the next day- we were not aware he was leaving so early until 2 days prior. Cassie welcomed us and assured us my son would be seen. We discussed the situation and she was excited for him and said they will take care of him. 10-15 minutes later: enter Jerry or Jeremy - through the front door (whatever he is deciding to call himself today). He came into the store at approximately 5:40. He told my son the dr. was unable to see him as they were 'legally bound' to make sure the optometrist can leave by 6:00 pm, that he came in too late and someone came in earlier and they decided to see him instead. My son called me and told me a man came in from break and the situation at hand. When I called to speak to Jerry/Jeremy he kept laughing at me, saying he wasn't on break. Found it hysterical. Said my son was too late and they couldn't do anything because of the binding agreement with the optometrist. I said I hope that the conversation was being recorded and he laughed and said he did too. Being that my husband and I were at Wal-Mart (very close to the store - grabbing dorm room items), my husband heard him laughing at me and went over to the store. Jerry/Jeremy was unable to provide proof of their 'legal' obligation and smelled profusely of certain substances. I go in for the manager's direct line and all of a sudden Jerry/Jeremy says the optometrist CAN see him, but at this point I'm beyond livid all while trying to wrap my head around get my freshman off to college with 2 days notice. I asked for Brian Captio to return my call, I still haven't heard from him. I call today to speak to him- but of course he's in a 'meeting' and 'unavailable.'

I went to Trillium 3 times as my glass were so uncomfortable and painful I couldn't even wear them. Every time an adjustment was made a different part of the glasses caused pain. In addition these are progressive glasses and when the adjustments were made they didn't fit on my nose correctly which caused issues - I can only see farsighted. When the last adjustment was made I was rather concerned and asked 'what if I can't wear these'-I was told to check Amazon for some kind of eyeglass gear and if needed to call the manufacturer. Although effort was made to fix my glasses I don't see why I should be searching for something to fix my glasses nor call anyone. I purchased the glasses from Trillium, although you may not be legally liable, I would expect much more than this. Well I got arm guards for the arms of the glasses and it didn't work.
