I'm gonna get trained as an ombudsman for skilled nursing facilities and I'll be making my return to do reports to the health department. This is dirty and shady. There are some great people that work there and there are some nurses that think they are gods. Not good. They lie and they steal. There are good workers. Ashlynn was great.Dino....great....... They need to train the new people coming in. They need to train the non-caring nurses that probably took the nursing course online during Covid because they have no idea how to divvy out pain medicine to a person who is in need. I waited well over my time. And then they said well you're supposed to tell me you're in pain. I said I've been ringing my bill now for over an hour and a half and I told the doctor and he said oh I'll talk to them. Bullshit he's just as bad. Some big fluffy guy comes in and says he's the doctor. Tells me you will get your medicine on time and the next time it was not on time. I checked myself out. They paid for me to go home because they knew I knew too much. Don't put a family member there if you care about them. There is infection breeding in that place. Look at the pictures I took and that's just a few. Scary and shady that's how I would rate this place. Scary and shady. Now if you are lucky enough to meet Hans Taylor, you are lucky because he is the backbone of that place. He is the angel. He is the really best thing that Place has and if they don't give him a raise, that's sad. He was a sweetheart. The entire time to me helped me out more than anybody there. He even gave me a ride home. Bless his heart. There are angels and he is one of them. The little lady in the bed next to me knows what shadiness goes on there and she told me yeah you be careful of this one and this one and this one she said you have to just play the game Karen. I am so happy with their care and I'd like to share why...I am respected, they honor my dignity and are very professional. It's very hard to be a patient that is in need, vulnerable and is 90% housebound. it makes being in a position like I am in easier when greeted by a caring nurse who is ready to help make a difference. i feel so thankful because i am healing. I know it's my job to be positive and take good care of myself in between my nurse visits and doctor visits. I have an exceptional team of doctors and I've shared with every one how exceptional the Asisted Home Health model of caring for the sick and vulnerable is. I can barely walk because I've been in need of a hip replacement for many years, but due to having an open wound I'm unable to get the hip replacement until my wound heals. I've got 5-6 cm presently June 11, 2026 to go, the wound was 14 cm deep. Since being cared for by Asisted Home Health and receiving aseptic care both at home and at the CMH Wound Center I have healed nearly 3 cm, and I'm so happy! It's slow healing because of radical radiation from stage four cancer, I'm also a survivor. My nurses are so loving to me and their supervisor makes sure I get all the supplies I need for optimal healing, and he's very well respected by his nurses. The leadership is exceptional at Assisted and encourages and supports their staff and nurses. My nurses have shared that they've been employed with this company a long time and are very proud to work for Asisted Home Health. I am nearly 74 in one month, I brag I'm nearly 3/4 of a century and I believe I'll heal by my 75th birthday completely. After being diagnosed with stage four cancer I was given three weeks to live had I not started radical chemo and radical radiation, which I completed barely. But after my second cancer related surgery a large boxer dog broke thru my backyard fence and caused me to get hurt as I was saving my little dogs life from the boxers jaws. An abscess developed 14 cm deep inside me because I had to pull and pull the boxer off my little dog going four feet into the air and down onto cement pavers as I broke my dog free. Sadly later the abscess broke thru my body and developed Into this open wound, it's a very sad story. The good news is I saved my dogs life and Kobe my mini schnauzer lived seven more years but I was really harmed. It's been a long journey healing but I have never given up due to my excellent doctors and nurses who truly care about me. Asisted is one of the best Home Health Agencies I've ever had and I hope they'll assist me to finish line healing this wound. I'm healing, but very slowly. I'm also getting exceptional care from Dr Alicia Haglund at CMH Wound Center, I really like her. And I believe someday around my 75th birthday I will get that hip replacement and be able to walk again and ride my bike and feel the wind on my face again. I loved riding my bikes In our beach city, I have a cruiser and an e-bike. If you have been refereed to this agency don't hesitate you'll be in exceptional care. Stay proactive in your healing and recovery. Help your team of doctors and nurses be successful helping you. They all work hard all day and need our kindness and appreciation of their care. They also need you to do your part and take the best care of yourself in your healing process and recovery. I love Asisted Home Health not just because I'm getting exceptional care but because they respect me and treat me with dignity. Before my doctor retired, Dr John Dexter wanted me to end my healing journey with them. I know they'll be a part of the journey to my full recovery and I'll be one of their success stories, and we'll all dance with happiness. I called Assisted and within hours we met with a nurse and a social worker (Ariana and Jessica) had a bed delivered along with oxygen, a nebulizer, and meds. We were set up with a wonderful health aide - Elvia, three times a week. Throughout mom's journey, the Assisted team were there with loving care and excellent communication. When my mom decided in her last days that she wanted Last Rites, I called Ariana, who called the spiritual advisor and we had a priest at our door within the hour. Community Memorial Hospital (CMH) released my grandmother here…read moreThursday afternoon and it was so creepy that we rearranged to go to another skilled nursing within an hour--I declined to sign any paperwork for admission into VPA. My main issues were: 1-STAFF: There was no staff to assist a new admit. After her transport drop off, I found my 90s grandma with a fractured pelvis sitting alone on an unmade bed, staring at a wall. There seemed to be various nurses, etc., but no one looked up/made eye contact with me, even when I'm sure they could hear me addressing my concerns to the creature they've hired to grunt at the front desk. 2-SECURITY: The woman at the front desk grunted for me to sign in at the electronic station and didn't look up. Also, the front doors are broken in that they are very difficult to open and the accessible button to auto-open doesn't work from the inside, and according to staff they've broken for awhile. Functioning doors is just basic for place that houses many people in wheelchairs. 3-BEST PRACTICES/LEGAL PRACTICES: The same woman at the front desk handed me a stack of paperwork on a clipboard and her only instruction was "Sign." A grunt-command is not a legal, informed consent and the paperwork required for skilled nursing is lengthy (medical and legal). I asked who I could speak with about any questions and who would be going over this with us, and she gestured to herself and said, "What question?" I asked to speak to admissions and/or the charge nurse and she gestured again to herself and said, "Sign." The alarm bells were ringing. Meanwhile, I checked in with my grandma and a CNA had set a dish in front of her that she's allergic to. Since there is no admissions process to speak of, they wouldn't know her allergies or literally anything about her. I have no idea if there are things such as sheets, a call button...no idea. Since they put a bib on her for no reason--she can feed herself--I'm assuming they are in possession of other sad textiles somewhere in the dankness. The CNAs were polite, but they are not decision-makers, and when I asked if this was unusual and who I should be talking to about this odd admissions process they were confused. I guess this is just how they treat people. Finally, the front desk creature drifted into the room, grunting for me to sign the admission paperwork. I declined and began the task of finding a different place where my grandma would be safe. CMH pushed back (it was an unpleasant exchange) until I told CMH that I was headed back to the ER with her. Within 5 minutes the case manager's manager called me back and they had found us a bed elsewhere. Interestingly, they warned me that it might be difficult to get the necessary paperwork back from VPA. PRIVACY- They're not concerned with patient privacy so it turned out that her paperwork was just sitting at the check-in desk and I took it. We waited for medical transport, and we left.

My Mother with dementia was released to this facility with a broken pelvis. I was told they were…read morenot allowed to have bed alarms. I was very concerned she would try to walk without assistance due to the dementia. The care during her stay was adequate but the communication was horrible. My mother did try to get up and walk by herself and fell. I was not notified until an hour after she arrived at vcmc and was told she bumped her head and was taken to chm which she was not. The way I found out was the doctor from the ed called me. I didn't hear from vpa until 2 pm the next day asking if I knew where my mother was. I explained her situation and conditions and I have not heard from them since. I never received any communication regarding her progress and after numerous times trying to meet with the appropriate staff never did. My mother passed away from her injuries. I just want family members to know about their communication and how horrible it was.
