Subject: Formal Complaint - Predatory Billing Practices and Unauthorized Appointment Scheduling…read more To Whom It May Concern, I am filing this formal complaint against DAVID VANDER VELDT's OFFICE Meadows Eye regarding a $25.00 "no-show" fee resulting from an appointment I never authorized or requested. I believe this company is engaging in a predatory "auto-scheduling" scheme designed to generate fraudulent revenue from unsuspecting patients. Statement of Facts Over one year ago, I visited Meadows Eye for an eye examination. Unknown to me at the time, the office--without my consent or signature--automatically scheduled a follow-up appointment in their system for exactly one year later. I was never consulted about this date, I never agreed to it and had no idea and appointment had been created FOR ONE YEAR LATER. One year later, I received a bill for a "missed appointment" that I never made. Furthermore, the office has sent a notice threatening to report this "debt" to the credit bureaus if I do not pay. Nature of the Dispute This is not a simple clerical error; it is a systemic scam. By unilaterally placing patients on their calendar without permission and then charging a fee when the patient inevitably "misses" the unknown appointment, DAVID VANDER VELDT's OFFICE Meadows Eye is effectively extorting money under the threat of damaging credit scores. Charging for a service (or the absence of one) based on a contract or appointment that the consumer never entered into is a deceptive business practice. Desired Resolution I am asking for the following actions from DAVID VANDER VELDT's OFFICE Meadows Eye: Immediate Voidance of the Debt: The $25.00 fee must be removed from their books entirely. Written Confirmation: A receipt showing a zero ($0.00) balance must be mailed to my address: Direct Contact: I require a phone call from the Billing Department (PO Box 80450, Las Vegas, NV 89180) confirming that this charge has been retracted and will not be reported to any credit agencies. Cease and Desist: I demand that DAVID VANDER VELDT's OFFICE they stop the practice of unauthorized auto-scheduling for my account moving forward. Attached, please find a copy of the fraudulent bill and the credit threat notice. I look forward to your prompt assistance in resolving this matter and holding this business accountable.

Recently I woke up one morning with my eye in severe burning pain. I called the office and they got me in immediately, which I was so grateful for. Dr. Abrams took his time and carefully examined every part of my eye to figure out exactly what was going on. It turned out I somehow had a tiny sliver of glass lodged under my eyelid, which had worked its way out, but he was incredibly thorough in making sure everything was okay and that there was no damage. What I appreciate most about Dr. Abrams is his great bedside manner. He's calm, patient, and truly makes you feel cared for. The entire office runs smoothly as well -- quick, efficient, and professional. It's rare to find a doctor who combines skill, compassion, and efficiency, but Dr. Abrams does exactly that. I'm very grateful for the care I've received over the years and highly recommend him to anyone looking for an excellent eye doctor.

This is an eye center that serves many seniors with vision problems, yet patients are directed to use computer kiosks that require entering a significant amount of information. It feels more like data collection than a simple check-in, including prompts asking for permission to receive marketing emails and texts. I watched several elderly patients struggle with the screens, which led to visible frustration. For a vision practice, this process should be much more accessible and user-friendly. The wait times were also disappointing. If you schedule an appointment, you expect to be seen reasonably close to that time. Being called back 40+ minutes late and then waiting even longer defeats the purpose of having a scheduled appointment. Patients' time should be respected. With a more senior friendly check-in option and better time management, this could be a much better experience.

I originally came in for a simple chief complaint, on-and-off red eyes. But what I received was far more than I expected. During my visit, Dr. Reddy took the time to listen carefully, ask detailed questions, and thoroughly explain possible root causes. He told me that my recurring red eyes might be linked to a systemic autoimmune condition, specifically autoimmune arthritis. What shocked me the most was how accurately he described all the symptoms I've been silently struggling with...lower back pain, hip pain radiating down my leg and foot, skin hypersensitivity, joint pain, and severe morning stiffness. I never even mentioned these to him, yet he described them exactly as if he had known my situation for years. It felt like someone finally connected the dots that every other doctor had overlooked. I honestly joked to myself that it felt like a psychic reading, LOL, but in reality, it was simply an incredibly skilled doctor who truly understands the human body far beyond the eyes. I walked into an eye clinic expecting help for redness, and I walked out with clarity, validation, and a referral path toward the right specialist who can finally address my underlying condition. I had no idea an eye doctor could be the one to help uncover what might be the true cause of my worsening symptoms that have been affecting my daily life. Dr. Reddy is the perfect example of what a physician should be, knowledgeable, thorough, compassionate, and truly invested in his patient's well-being. After a few years go by and "SandraLee" that's what I ended up naming her, it grew so big it was causing my eye to water. My eye thought something with it because SandraLee was massive! I made an appointment and met with Dr. Reddy and his staff. Everyone was very kind. I was so nervous when I went back but the procedure was so easy and quick. I told her that in my previous doctor's records, she would find the signed Medical Necessity Script needed for the oral surgeon and prosthodontist to file the treatment plan with Aetna. Every time I talk to her and mentioned it, she changed the subject or ignored it like it was not her problem. My lower jaw was sticking out one inch to the left and I could not eat. I went down to 92 lbs and eventually went into the hospital for failure to thrive. She asked me what I thought the problem was and I told her it was from the TMJ. It was causing so much pain that I could not open my mouth to brush my teeth or eat and the anxiety it caused was unbearable. I mentioned several times that I had two businesses, but I could work until it was fixed. The misalignment of my jaw was so severe, it knocked out almost all of my teeth which now made it a medical for dental claim. I have been permanently disabled since 2008. It took me over a year to find an oral surgeon and a prosthodontist that could repair the damage. They said it would take one year of oral surgery, and they could only replace my now damaged teeth with implants and that dentures were not an option. My oral surgeon also said that because it had not been treated, I developed Periodontal Disease and was at risk of having a stroke. Mary told me she had 1500 patients and she could not remember everything, so I was totally neglected until I almost died. I was invisible to her and everyone at P-3 Medical Group. My sister was so mad and upset that she flew to Las Vegas and called Mary as an emergency. She reminded Mary that there was a previous Medical Necessity script on file, but I needed a new one from her. I did everything on my own, which included finding the specialists that could treat my injury and getting verbal permission from Aetna to proceed with the claim. She told Mary I had to do everything on my own without her help and all I needed from her now was the Medical Necessity script so they could file the claim. Mary refused to give me a Medical Necessity Script and told me to go to the UNLV Medical Dental School. Both my sister and I hit the roof. She STILL did not understand that TMJ is an insurable medical condition. There is no way that the UNLV dental school could help me. No dentist could help me because it was a TMJ medical problem. Her neglect ruined my health and my life. It forced me to cancel all of my business clients and contracts. I am 5 foot 6 and still only 92 pounds. Complaints have been filed with Aetna who I had been working with closely on this claim. A lawsuit is pending against Mary Nara and P-3 medical group for neglect and punitive damages.
