I briefly placed her at another facility in San Rafael, but knew within hours it wasn't the right fit. Finding South Marin was a turning point. My mom was diagnosed with terminal cancer, kidney failure, and dementia. The care, compassion, and respect she's received here have been exceptional. A special thank you to Kelly, the social worker, who is knowledgeable, supportive, and truly cares. Carina, the activities director, is also wonderful and brings so much kindness to the residents. All the nurses, assistants, and staff have been wonderful. I never imagined putting my mom in a SNF, but I feel very lucky she's at South Marin. As a former long-term care volunteer ombudsman, I also want to add some perspective. I've seen reviews mentioning delays in care. The reality is that staffing shortages are a widespread issue across the industry due to policy and funding challenges. That said, the staff here consistently do their best under difficult circumstances. Their PT (what she tends to need most) is terrific - the PT therapists seem to experience little turnover as the same individuals have provided PT for my mom over several stays. Despite being very busy, the social workers are also very helpful. And Dr. Susan Nguyen (the hospitalist there) is wonderful - caring, smart, and on it. Rehab isn't fun, the facilities are all really busy and frustrations are inevitable. But we will keep requesting South Marin when my Mom needs it b/c it's the best around.

When he first moved in, the community was only a couple of years old and, like many new assisted-living facilities, was working through some early growing pains. Staff turnover was higher at that time, and there were occasional glitches as the building found its rhythm. Over the past year, that has changed significantly. Quality staff have stayed, new team members have been strong additions, and many of the issues we saw early on have simply disappeared. The stability and consistency now are remarkable. What has remained steady from the beginning is the heart of the community. My dad was welcomed into a warm, supportive environment where residents truly look out for one another. The caregivers are attentive, compassionate, and incredibly dedicated -- they know the residents well and treat them with genuine respect and tenderness. It feels like a place where people are cared for, not just looked after. We visit often, and our family has had many dinners in the beautiful white-tablecloth dining room. Meals are freshly prepared by the private chef, and the food is consistently very good. The Vincent also hosts happy hours a couple of times a week with live entertainment, which is a highlight for many residents and adds to the sense of community and joy in the building. The housekeeping team keeps everything spotless, and the overall atmosphere is clean, calm, and welcoming. Their points-based care system is fair and transparent, and as my dad's needs have increased, I've found their re-evaluations thoughtful and reasonable. On-site services -- like the podiatrist and monthly dentist visits -- have been invaluable. More recently, my dad's health has declined, and we are transitioning him into the Memory Care unit and beginning hospice. It's a very emotional step for any family, but the memory care team has given us tremendous comfort. The unit is bright, clean, and staffed with people who clearly have experience and a deep understanding of cognitive decline. While moving his things, I've had the chance to observe how gently they care for the most vulnerable residents. It speaks volumes about the philosophy that guides this community. Although the surrounding neighborhood is not my favorite, the quality of life inside the building is exactly why families choose assisted living. It is safe, uplifting, and genuinely resident-centered. I've been a front-line medical professional for 30 years and toured every senior living community in Marin when preparing for my parents' transition. The Vincent stands out. They have supported my dad with dignity, kindness, and professionalism -- and they've supported our family through some difficult turns as well. I stayed at the San Rafael Healthcare and Wellness Center (SRH&WC) for 20 days because I had fallen…read moreand cut a big gash in my right calf. After 4 days at Marin General, I needed to go to a skilled nursing facility and the MG staff strongly recommended that I go to SRH&WC over the other facility that my Medicare covered. It was the right call. All things considered, the stay there was quite pleasant. The facility was cleaned daily and more often as needed, the management staff was caring and professional, and the nursing staff were all wonderful, kind and proficient. I had physical therapy 3 times/week, and there is a small gym. Although I didn't take much advantage of it, the activities (thank you, Whyll and Cassandra) were varied and entertaining, ranging from live music, drum circle, bingo and trivia contests, and nightly movies. They even offered religious hours on Sundays. Something for everyone. I would be remiss if I didn't acknowledge by name some of the exemplary staff members (and my apologies to those who I'm not including -- again, they were all great). First, the management team of Macqueline Soliman-Plantin (Admissions) and Ana Benitez (Social Services) were absolutely wonderful: professional, thoughtful and caring. From their initial welcoming visit through to my departure, they handled all the paperwork and phone calls with my insurance company, ensuring there were no costly surprises. They were also terrific with my family and friends. I should also include Maddy Tyson (Receptionist) in the above list. She, too, was great, so kind, polite and professional. Everyone in the Nursing Staff (there were a lot) was professional, intelligent and attentive, so I will only mention a few names: Priscilla (Wound Nurse), Hector and Marie (RNs), and Wesley (CNA). I also want to thank the two Physical Therapists: Alex and Tricia. Two somewhat different styles, but both helped me build up my strength and balance so that I was able to be released after 20 days. They really helped me get back on track physically. Finally and a surprise, the food was actually quite good ranging from above average to excellent. (Not to toot my own horn, but I am a gourmet/gourmand and an excellent cook: I know delicious food, and I'm quite critical.) There were three truly standout meals: a tunafish salad, cheese enchiladas and roast turkey with gravy, stuffing, etc. All meals came with veggies and/or salad, and a starch (potatoes, pasta or beans). Kevin, the chef, put out different meals the entire time I was there, and I was able to have him adjust my meals to fit my dietary needs. Again, this was the only time I stayed in a skilled nursing facility, and I hope I never stay in one again, but my stay at SRH&WC was about as good as I think it gets, and much better than I expected. The Center provided a smooth transition from the hospital to rehabilitation. The nurses are skilled and are supported by a team of doctors who provide quick response to any care issues. The Center also can provide ancillary services such as Podiatry, Dentistry, and Optometry. San Rafael Healthcare and Wellness Center is a smaller center, but it punches well above it's weight in services like physical therapy and food services. At the same time, it's small enough that a patient can know the whole staff by name.
