The caregivers are truly special, especially Isabel and Gabby, whose kindness and dedication never went unnoticed. The Health and Wellness Director is simply amazing. Nav is exceptional at what she does, but more importantly, she leads with her heart. She took the time to truly know my dad and to understand what our family was going through. Once something was brought to her attention, she made sure it was addressed promptly and thoughtfully. Her kindness, patience, and gentle nature meant more to us than words can express. While there is room for improvement with the med techs, and the move-in process could benefit from clearer communication and better explanations upfront, the compassion and care we received outweighed those challenges. I am deeply grateful to Nav and the entire Vincent team for the love, support, and comfort you gave my mom and our family. My friend who is living at St. Vicent contacted Scabies. I understand from others who live there that there is an outbreak of scabies. This is due to unclean conditions. My friend has had scabies over a month now. I would not recommend this place for memory care. read more

A Truly Exceptional Community -- The Vincent, San Rafael My dad has been living at The Vincent since March 2024, and the experience has grown into something truly exceptional. When he first moved in, the community was only a couple of years old and, like many new assisted-living facilities, was working through some early growing pains. Staff turnover was higher at that time, and there were occasional glitches as the building found its rhythm. Over the past year, that has changed significantly. Quality staff have stayed, new team members have been strong additions, and many of the issues we saw early on have simply disappeared. The stability and consistency now are remarkable. What has remained steady from the beginning is the heart of the community. My dad was welcomed into a warm, supportive environment where residents truly look out for one another. The caregivers are attentive, compassionate, and incredibly dedicated -- they know the residents well and treat them with genuine respect and tenderness. It feels like a place where people are cared for, not just looked after. We visit often, and our family has had many dinners in the beautiful white-tablecloth dining room. Meals are freshly prepared by the private chef, and the food is consistently very good. The Vincent also hosts happy hours a couple of times a week with live entertainment, which is a highlight for many residents and adds to the sense of community and joy in the building. The housekeeping team keeps everything spotless, and the overall atmosphere is clean, calm, and welcoming. Their points-based care system is fair and transparent, and as my dad's needs have increased, I've found their re-evaluations thoughtful and reasonable. On-site services -- like the podiatrist and monthly dentist visits -- have been invaluable. More recently, my dad's health has declined, and we are transitioning him into the Memory Care unit and beginning hospice. It's a very emotional step for any family, but the memory care team has given us tremendous comfort. The unit is bright, clean, and staffed with people who clearly have experience and a deep understanding of cognitive decline. While moving his things, I've had the chance to observe how gently they care for the most vulnerable residents. It speaks volumes about the philosophy that guides this community. Although the surrounding neighborhood is not my favorite, the quality of life inside the building is exactly why families choose assisted living. It is safe, uplifting, and genuinely resident-centered. I've been a front-line medical professional for 30 years and toured every senior living community in Marin when preparing for my parents' transition. The Vincent stands out. They have supported my dad with dignity, kindness, and professionalism -- and they've supported our family through some difficult turns as well. I'm deeply grateful for the care my dad has received here, and I hope this review helps other families looking for a place where their loved one will be truly cared for. read more

I'm 74, have lived in Larkspur since the early 1990's, raised my now spread out Family, married to a licensed now High Risk Ob/Gyn RN, and am a semi-retired attorney who has specialized in Injury Related and Medical Negligence litigation. I come from a Family of Doctors and Nurses. In Mid 2024, I became seriously ill and septic. Multi-organ failure, dialysis, endocarditis. and other life threatening illnesses. On discharge from Marin General after 60 days, I next went to Kentfield Rehabilitation Hospital, where neglect, poor follow-up, malpractice by the staff and medical personnel almost killed me a second time. After 2 week rehab stints at Sutter Ralph K Davies MC then Jewish Rehab Home in SF, I moved to the Vincent on recommendation by a nursing home placement consultant. I was not anticipating that recovery from my illness would not be 100%. I am a former national caliber Rugby player and have survived Stage 3 Prostate Cancer for 17 years, have a Fused Lower Back, and both shoulders replaced with Titanium total joint implants. I aggressively pursued PT before arriving at the Vincent and NEVER planned to become a "Resident." When I arrived in late October, 2024,it was "BP": Before Pari (Manoucherhi), the Executive Director. The facility is relatively new, well designed and furnished, 3 floors only, has a nice courtyard and garden, large group living room, library, lower level secured garage, appropriately sized dining room, not so great fitness room, activity room and separate lounge. However, as pointed out in other reviews, the Gallagher Family Owners/Builders mistakenly hired the Oakmont Senior Living a national, publicly traded Management Company to operate it. When I arrived, Management was being transferred to WellQuest Senior Living, a much smaller privately held company based in Salt Lake City and managing mostly California Senior Living Communities. While the housecleaning, food service and quality, medication management/attention, transportation coordination, were adequate, when contrasted with negatives such as being located on the US101 frontier road with noise etc, a shopping mall with CVS/Walgreens, grocery stores not within walking distance for most residents (Northgate Tierra Linda) the monthly pricing was not supported given the many competing Senior Living Communities, More are under construction in Marin and completion will only increase. Very importantly, staffing morale was very low and turnover a real problem. Fast forward exactly one year to this moment. We are now After Pari. "AP." This is a different Vincent. Employee turnover has dropped considerably. The Health Management team led by Nav and Gabby is outstanding. Ebi, the shuttle/Do Everything else Director and Cindy, Activities "boss", facilities team, housekeepers, are without peers. Dining room servers all could wait tables down the street at La Toscana, one of the very best Italian restaurants in Marin. Our chef, Aileen, could run that kitchen as well. Meals are nutritious, fresh veggies and fruit, vary every night, and creative. Weekly unit meetings occur and resident specific needs are addressed. Monthly Management/Resident meetings led by Pari and attended by all department chiefs are held. Grievances are aired and resolved. Bottom Line: My nurse wife and I checked the competition out thoroughly: Tamalpais/Sequoia, Smith Ranch Road,Aegis, Villa Marin, Drakes Terrace, Cogir, etc. I even visited a Oakmont property in beautiful Hawaii where my son and daughter in law live. Not even close to Vincent's standards. One last observation. The WSL owners know this business. Leadership starts at the top and is proven by results. Shout out to them as well. I'm not planning on going anywhere else! read more

Our family moved my brother-in-law into the Vincent 3 months ago where he is very happy and very well taken care of. He needs a great deal of physical assistance and meds which the staff of the Vincent provides at the highest level. Although he was reticent to ask for help at first because he didn't want to bother anyone he now realizes the staff is there to help him and he is thrilled with the support and the caring attitude of each and everyone of the staff members. The med tech department is top of the line in their attention to his needs. The coordination with his doctors is challenging but the staff perseveres until we are all satisfied his medication requirements are met. We find the staff and the management involved, communicative and extremely professional. The facility is always clean and cheery. We visit often and are greeted by name and feel so welcome. Besides serving delicious food the dining room is the center of activity since the residents get to know who they are living with. My brother-in-law always has stories about this new person he met at a meal and how fun it is to learn about each other's lives. It's wonderful to see him developing his own community of friends at this time in his life. The outdoor space is very inviting for the young grandchildren and our feisty dog. We are creating wonderful memories for the grandchildren and ourselves to see Papa B. in a safe, clean, happy and warm environment. Out deepest thanks go out to all the staff at the Vincent for all they do to make this time meaningful and memorable for our family. You're the best! read more

On the positive side, the building itself is well-designed, cheerful and attractive; and the plants and trees in the courtyard are beautiful. The sales people are warm and friendly, as are some of the staff. The food is decent, sometimes excellent. However there are huge issues with turnover. They are on their seventh executive director in three years, and their fourth activity director. A couple of the executive directors were excellent--seeming engaged, stepping in to help out when the dining room was short-staffed, etc--but most seem like they are going through the motions. There is similar high turnover in the caregiving staff and servers in the dining room, as well as the chef. Some of the staff seem genuinely kind and engaged, but many of the staff seem poorly trained and uncaring. I have come to visit and sat at a dining table where the chair was soiled with a big blob of food...I asked a server to wipe the chair clean and they just looked at me and didn't respond (and didn't ever wipe it.) I came back two days later and the same blob of dried on food was still there. The servers have left us waiting for long periods while standing over in the corner of the dining room apparently doing nothing. When maintenance is needed in my dad's apartment, it often takes weeks for someone from maintenance to attend to it...even if it is something rather urgent, like a clogged toilet. There is an issue with the building where it shakes and rattles at times...not sure if that is coming from the HVAC unit or some other large machine, but it is unpleasant. No amount of asking management to address the problem has fixed it. Currently all the announcements sent to family seem to be written by an AI chatbot--with that cloying sentimental tone of ChatGPT. So much for the personal touch. The worst lately is that the activity assistant Brittany was rude to my dad, acting impatient and dismissive. The other activity assistants have been warm and friendly--but Brittany doesn't seem to get that being kind and being good company to the residents is part of her job. And the activities have even gone downhill lately--they used to have nicer parties and events to help attract new residents to the building, but now that they building is full they don't seem to be putting in the same effort. If they could keep some staff in place longer than a few months each and build a trained community, this place could be great, but at the moment it really falls short. read more

My parents have been at The Vincent pretty much since it opened two years ago. It is such a hard decision to move your parents into assisted living, and hard again to choose a place. It's like choosing a preschool for your beloved children...who is going to take care of them as you would? The Vincent obviously is very nice. If feels like a big mansion that you have quarters in. The courtyard is so sweet, and my parents love the theater and activity room, especially the ping pong! It feels like a cruise ship in that there are always fun things going and people out and about. I know because I am there every other day, at different times during the day. The most important thing is that the people at The Vincent are amazing. From the front desk to the med techs to the wait staff to the activities, every person there that I have talked to over the past two years has been kind and attentive. There has been a lot of change in management, but it has felt seamless. Additionally, the residents there are engaging and have such personality! They all know each other and some have become good friends. It's like a sorority or fraternity. I see some groups meeting in the library for bridge, some like to hang out in the activity room, some like the big living room, and others are walkers. Another group is always down for the movie in the theater. I'm always happy when I am there...the people are just so delightful. I think the biggest indicator that The Vincent is awesome is that my parents are happy. They feel seen and heard, and they are engaged. Sometimes you just want to hang out in your room, but if you get restless or want some company, there is always something going on, or even just someone hanging out to talk with. I hope if and when my husband and I have to go into assisted living, we find a place like the Vincent! read more

BEWARE!! Avoid this beautiful looking home... While the staff members are very nice, the management of the facility is understaffed and they are missing processes to take care of their clients. The day before we moved my mother into the facility, I stopped by to meet the executive director for the Vincent, and asked if their staff is ready for my mom. I also asked her to maybe check in a few times more often at night during my mom's first week and she said "no problem". The next day we moved my mom in and the person at the entry desk said hello and that they were expecting my mom. After bringing my mom to her room, we had a quick lunch and returned to the room in anticipation of meeting the staff and management as the executive director had promised. After 2 hours most of my family left, except my sister who was going to spend the first night with my mom. My sister called and said that no one ever came up, and at 8pm no one showed up to give my mom her Medicine. My sister walked into her halls and finally found a nurse who said "she was not aware that her mom was coming". Even after that, not one nurse showed up during the night, which was promised every hour or so... In the middle of the night my mom fell out of bed and my sister hit the panic button, and it took 28 minutes for someone to arrive. The nurse said " I have so many requests showing my sister the extensive help requests". The second panic button resulted in another long wait. The next day our family pulled my mom from the facility. The director apologized, and I asked her to send an apology letter to my mom, and after 30 days we are still waiting for the letter... So sad that such a nice facility can't deliver the service promised at a VERY high price... read more

I am doing a preliminary search for communities for my mom, whose about a year out from making the move. I just had to share how impressed I was by this beautiful community! The receptionist was very professional and made sure I was comfortable as I waited for their Sales Specialist. Christina was also very professional and kind and offered a wealth of information as we toured. The residents looked happy and engaged and overall the feeling I had was very warm and positive. Looks like mom may be moving to The Vincent when it's time! read more

If you want your loved one to be in a community that provides excellent care that is safe and stimulating, I strongly encourage you to continue to look and NOT to place your family member in The Vincent. As a family member of a resident in Memory Care, I was the founder and President of the Memory Care Family Council and was extremely involved in most aspects of the community. My main focus was on the care and safety of the residents. But after 2 years of advocating for improvements that were repeatedly promised and never delivered, moved my mom from The Vincent, as I could not in good conscience continue exposing my mom to substandard and negligent care and an environment where the main source of activities was mindlessly parking residents in front of the TV. For 9 months, there was not an Activity Coordinator and very minimal stimulation for the residents. Issues include poor leadership and lack of follow through at the top management level. The promises are illusory and most likely will never came to fruition. Poor leadership at the top results in poor hiring choices and mismanagement of the entire Memory Care unit. There have been 6 Executive Directors since Wellquest took over; lack of continuity and clear vision for the community is a huge problem. The Vincent hires caregivers with no experience or training in caring for residents with dementia or Alzheimer's. Sadly, the majority of the caregivers are apathetic and demonstrate a severe lack of compassion for the basic challenges that arise with memory care residents. The training required by the state does not compensate for experience and knowledge in how to redirect residents -- or how to calm residents when they are agitated or combative or aggressive or sundowning. This is compounded by the fact that senior management lacks the tenacity and follow through to hold caregivers accountable when they consistently underperform, resulting in a system that perpetuates a culture of substandard care. Since WellQuest has taken over management of the community, there have been over 10 families (one third of the entire unit) who have moved their loved ones to another memory care community. Many of the caregivers hired cannot speak sufficient English, resulting in a severe lack of communication with family members and more importantly with the residents. The NOC shift possesses significant problems due to lack of supervision. NOC shift caregivers have been found sleeping on the job, watching TV and socializing with each other rather than checking on the residents during the night or doing residents' laundry. The NOC shift is also responsible for doing the residents' laundry. They were consistently negligent and incompetent. Most of my mom's clothes were ruined; clothes were faded, shrunk, zippers broken, torn, misplaced and lost. It was very common for clothes not belonging to my mom to be put in her room, with her clothes given to other residents. Labeling the clothes was of no consequence; the caregivers ignored the labels and made no effort to sort accordingly, despite repeated complaints and promises to rectify the situation Medication errors are a common occurrence in both Assisted living and memory care. The Med Techs have insufficient training and understanding of pharmaceuticals and often mark meds as given when they have not in fact been given to the residents. I am aware of many instances where medication was marked as given when residents were not even on site. I personally experienced errors where a medication was marked as given, but when questioned, the Med Tech was forced to acknowledge that the medication was not given. I am also aware of a recent incident where a Med Tech confided in a fellow caregiver about a medication error but advised "Let's keep this between us." Sadly in this community, it's common for caregivers to cover the mistakes of others and lack of accountability is rampant. Activities are lackluster and "one-size fits all," with zero custom tailoring "based on resident's current and past interests and experiences" as promised. There is a severe lack of activities in the Memory Care unit, which once went 9 full months without an Activity Coordinator. The main source of activity for the residents is sitting in front of the TV for hours on a daily basis. From the perspective of the Family Council, the Memory Care unit (referred to as "Journey") is not a priority. In the past 1-1/2 years since WellQuest took over management of The Vincent, there has been a chronic lack of leadership and support. There have been 4 directors in Journey 18 months, with most of them having limited experience in managing a memory care community. As a final note, several of the 5-star reviews were written by the Wellquest management team. The most recent review was written by someone who toured the community that has zero experience living there. read more

I have a relative that moved to assisted living at The Vincent. It is new and clean but the daily care has continued to decline. Most importantly, medication management or lack thereof, has gotten worse. The Vincent is owned by Gallaher Senior Living, a development company in Santa Rosa, Bill Gallaher is well known in the area. Initially, they hired Oakmont to manage the facility. As a family, we weren't thrilled with Oakmont's reputation of being private equity focusing on cost over care. Then Gallaher replaced Oakmont with Wellquest Living, a private company who builds and manages senior facilities. Their presentation to the residents and families sounded great. They were a small, privately held company that didn't manage facilities they didn't own but because they had a relationship with Bill Gallaher, they agreed to be a third party manager. Our hopes were high that the medication management would get better with the new management company. Unfortunately, that was not the case. I've lost count of the number of Executive Directors they have had over the last 3 years, at least 6 or 7. The last 4 have only stayed 2 months each. It is almost impossible to fix a broken institution with revolving management. They routinely miss doses of medication and don't tell patients or family that they didn't administer the doses. The only response they gave, after being asked, is they ran out of the medication. Seriously? Their job is to ensure the medication inventory for EACH patient is adequate. Pills are supposedly bubble wrapped for each resident at the commercial pharmacy, Medtechs are responsible for matching the prepackaged medication to the resident. I have personally communicated the medication errors to Wellquest's senior management but have never received an explanation of why the problem hasn't been resolved. The Department of Social Services has also cited them. Unfortunately, most residents and/or family won't know there is a medication problem until the resident starts showing symptoms or they end up in ER. The only way to ensure a resident is getting the correct medication is to have a family member or caretaker visually verify the medication while it is being given. They won't allow caretakers to administer medication so you are relegated to managing their Medtechs. Please check the history of inspections and complaints for any facility you are considering, go to the California Department of Social Services (DSS) website. You can sign up to be notified of any changes or updates to a facility. Just be aware that not all medication mishaps are reported to DSS so we don't really know the extent of the problem. read more

My wife has been in Journey Memory Care for over a year at TheVincent (formerly Oakmont San Rafael) and has received responsive, loving and skilled care from the caregiving staff, Gladys, Magnolia and Mildred. Please do NOT send anyone you care about or can't put visuals on DAILY to the Vincent Memory Care unit. It is awful! We put my mother in law there over two years ago when it originally opened as Oakmont. We knew there would be some hiccups as it was a new facility and needed to get their legs underneath them. They never did. Oakmont then sold the business to The Vincent which has numerous locations around the country and assured us they were experts in their field and that the care our loved ones would receive would be an upgrade from Oakmont. Wrong! It got worse. We lost 3 chefs, had 3 different Executive Directors and 2 different Memory Care managers (with a significant gap in between). All in a years time! What this caused was total chaos and unaccountability. The care staff wasn't being watched which meant that they in turn weren't really watching the residents. Anyone who has had to deal with someone with dementia knows that they NEED stimulation, they NEED more care than the standard Assisted Living resident and in our case my mother-in-law NEEDED to be fed at every meal. What I saw was that it happened probably 50% of the time, possibly less since I couldn't be there on a daily basis to put on eyes on her. She lost at least 15 pounds during the past year and when she finally passed she weighed 80 pounds. I could go on and on about how this place is run but I think you get it. The worst part came AFTER she passed. Let me preface that we had told the memory care Manager that staff was welcome to any of the furniture that we had in the room. My issue was that as I was in there cleaning, two staff members unlocked the door and giddily entered as if they were coming to check out the goods. After they saw me in there they quickly left but I found it odd as they had NO reason to be in there. The MC Manager then proceeded to come in with the person manning the front desk and started claiming items and showing him what was left in the room. This was happening while both my husband and I were in there cleaning out her final few possessions. I found it rude, unprofessional, and incredibly insensitive seeing as his mother had passed only hours prior and they were treating her room like the Alameda Flea Market. It was disgusting. As the director of a Memory Care unit where people will pass often, I was shocked at her lack of etiquette on how to act when someone finally does. Her staff also needs to be trained in sensitivity (as well as many other things), clearly. This was our last interaction at the Vincent and quite honestly it felt like no one cared that our loved one had passed on, they just couldn't wait to get their greedy little hands on her "things". She deserved better and so did we, but I really hope they enjoyed the furniture. read more

My dad recently spent the last months of his life in memory care at The Vincent, following an illness that caused him to suddenly and irrevocably enter into a delusional state. After many frustrations at a skilled nursing facility, we were relieved to find kind and quality care for him in their lovely environment. Honestly, many members of the staff treated him with a level of attention they would give their own father, even during moments of difficult behavior on his part. The unit director and staff nurse were wonderful to interface with and they also knew when to let us know we should involve hospice. My family and I are forever grateful. read more

The Vincent is a beautiful, new facility which is a facade for what has been a tumultuous experience. My mother, 90 years of age with dimentia, has been in memory care since the place opened and there has been nothing but turnover with staff and hugely inconsistent service. Anyone good they hire leaves, and there is both a training and leadership issue. our experience is in memory care only so cannot speak for assisted living. there are some wonderful families who we have commiserated with who also have loved ones there and perhaps the only good thing. read more

My Mom was in the Memory Care unit at The Vincent for approximately 18 months. I will start with one positive - there are a handful of good caregivers in the Memory Care unit. Unfortunately, those few good caregivers can't compensate for all of the others who are either not trained appropriately or who just don't seem to care for the residents and their well-being. My Mom was left often dehydrated and was not helped with her meals. She would repeatedly ask for help and would get little responsiveness from the caregivers. There were consistent medication issues. We were told that she would be toileted regularly. That didn't happen - she had numerous UTI's because she would not be changed frequently enough or toileted appropriately. Items were constantly taken from her room, including her personal supplies. Her wedding ring "went missing." In 18 months, we went through 4 Executive Directors and 3 Memory Care Directors - and there are little to no activities or stimulation for the residents. The TV being left on is what is "entertainment" for them. We had planned to have our personal caregiver transition out after a few months once my Mom had acclimated but we could not do that, as we knew she would not receive appropriate care from the staff. We moved my Mom out last month, and since she has left and had 24 hour care, she has become more verbal and is overall much better. Her teeth are much better - clearly her oral care was not a priority at all for the staff. I will hope for the residents who remain that things improve, but I would absolutely not recommend the Memory Care unit at The Vincent. read more
