Here the facility and staff are top notch. It is modern, clean and well maintained and, most of all, the nurses and aids were fantastic. My needs were well cared for and the staff anticipated many of my needs. They even treated the "grumpy old men" (and there were lots of them) with care and polite understanding. The cleaning staff were "Johnny on the spot": I was amazed at how quickly and prompt they were at picking up and cleaning any mess. The food is good. I am not a connoisseur but was very happy with it. Each resident had an individual, but common, menu. The individual meals were tailored to each patients needs and desires. I was mildly surprised that when someone asked for additional items they were usually provided. Again, the dining staff were very friendly and accommodating. I had approximately 15 meals there and only one was disappointing. As most people, I do not really want to be in a NH unless necessary, but when I need it, I will choose Augustana again.

Emerald Crest seemed like a decent place at…read morefirst, it didn't have those gross smells, the staff were nice, it always appeared clean, the rooms are spacious & have private half bathrooms. My grandma has dementia and started to decline rapidly over her time span here to the point she couldn't walk anymore or feed herself. Our family would visit often enough to where the day staff got to know us pretty well. My grandma would often have bruises and cuts and would cry and say that the male staff at night were hitting her and handling her rough. We would pop up from time to time and find the male staff "hiding" or sleeping in the visiting room. When confronted why my grandma would still be sitting at the dinner table hours past dinner, not changed or properly helped for bedtime routine they would brush it off because they always assumed we would come do it. We ended up putting a camera in her room which captured her falling due to her being left in her room for hours a day and needing assistance going to the bathroom and they wouldn't check for hours. Also found that they would often remove her from her room at random hours to "go to the bathroom" which was odd because she had a bathroom in her room. We even offered a commode that they could roll right up to her bed so she wouldn't have to travel far. We often had to clean her room (which is fine) because we wanted it to be properly cleaned & for her to be comfortable. We knew someone else's mother who was placed here while my grandma was here. She was very early in her dementia diagnosis and more aware. She also sustained bruises and said it was a male staff at night mishandling her. The staff member was moved to another house which is disheartening & terrifying. After my grandma had a sustained multiple injuries to the point we felt the decline in her health was due to lack of care the nursing home suggested we move her to the "hospice house" because they generally have more staff (but the flip side to that is everyone in that house needed A LOT more care and there was never enough staff). While in the hospice house they did push for my grandma to actually go into hospice because of the level of care she would need they obviously could not handle. My grandma had to wear diapers but she could still tell you when she needed to use the bathroom. We've caught them telling her to go in her diaper vs doing the work to put her on the toilet. She didn't like the feel of using the diaper and when she would use it they wouldn't change her for hrs which caused more issues. We'd often show up and she would almost always need to be changed. I feel like my review is getting a bit long and I've only covered 1/2 of the issues here. My grandma is no longer at Emerald Crest due to one too many preventable falls under their care where they tried to blame us when in fact that the staff neglected to care for my grandma while she was in their care.

    I worked here for two years, two long. I can say that the staff at PHS Bloomington care about their…read moreresidents and their work, with it showing each day. In fact, they serve a little TOO well, with little reward or benefit from the administration. Sadly, PHS Bloomington is fraying at the seams because of the toxicity that is present there, thanks to the Campus Administrator. Micromanagement, workplace abuse, quiet firings, you name it. Do your due diligence when picking this facility for yourself or your loved ones, because that toxicity does trickle down to the resident's care.

The place is aesthetically pleasing to look at, and it doesn't have that "smell" that some of these types of places are known to have exuding from them. Mom was happy with the room and the quality of the care. And she was especially happy with one nurse, who cut up her food. (Which, after having a shoulder replaced, really helped because she only had use of one arm at the time. It's a little disconcerting, though, that no one else seemed to realize that cutting up food with one hand for an elderly person would be a problem for her. And my mom isn't the type to say anything about it.) There was one major problem, at her release, as my mom was given a narcotic prescription to manage her pain. (She did, after all, have her shoulder replaced, which is major surgery and there is pain associated with the recovery of such an invasive procedure.) The problem was that the doctor left off the amount of narcotic pills for the pharmacist to fill. And the pharmacist CAN'T DO ANYTHING WITHOUT A SPECIFIC AMOUNT for narcotics. (They won't give you anything other than to suggest taking Tylenol!) And then we couldn't get a hold of the doctor, as (apparently) they're pretty much MIA for the weekend after 2pm on Thursdays. And, in my mind, this really was an severe oversight that so little help was available for my mom after the fact. Though I wouldn't let this little story dissuade you from coming here, BUT if you're going to be in need of prescription on an out-patient basis, I'd be doubly sure that it's properly filled out; particularly if you'll want to get it filled after 2pm on a Thursday.
