I'd be interested to know how many people die in this place on an annual basis .I'm deeply disturbed by what happened to my neighbor Bill at Prestige in Chehalis. He went to the ER in Aberdeen after hitting his head and was told he'd stay overnight for observation. The next morning, he woke to find himself in ambulance, being transported to Prestige Care in Chehalis. Confused, e arrived around 4:30am,(before daylight) amd was checked in, and told he was no longer able to care for himself and wouldn't be allowed to leave. He called me crying, scared, and confused. He said staff told him they'd call the police and have him jailed if he tried to leave. I told him that sounded like kidnapping and that if he needed a ride home, I'd come get him. A couple weeks later, he called again, begging me to come. He was angry and sobbing, saying, "I want out of this damned place." When I arrived, I signed in at a makeshift desk and was told there was COVID in the facility, so I followed all protocols. I found Bill in his room and told him to grab his things. As we left, a nurse yelled that he needed a walker. Bill replied, "I don't need a walker--I'm going home." She snapped back, "You know you can't go home." Staff began whispering and scurrying around as tension rose. I asked if we could step outside to talk. We went to a fenced-in outdoor area--like a prison yard, enclosed in 6-foot chain-link fence. I went back inside to speak with the nurse in charge. She was rude and dismissive. I told her Bill had asked me for a ride. She said he couldn't leave without a doctor's release. When I asked to speak to the doctor, she said he wasn't available and hadn't answered calls. I asked, "What happens if he just walks out with me?" She said, "I'll call the police." I replied, "For what? He's not a prisoner." She then said no one was allowed to leave because of COVID--even though Bill had tested negative that morning. I asked, "Do you go home at the end of the day?" "Of course I do," she snapped. "Then what's the difference?" I asked. She had no answer. When I turned to walk back outside, she added, "I don't know why you're trying to take him." I replied, "He called me. I'm just giving my friend a ride home." She warned that if he left, she'd report him as AMA (Against Medical Advice). I explained it all to Bill. We planned for him to walk out on his own and meet me at my truck. But when I went to leave, I found the doors locked. I had to ask someone to let me out. Staff watched me from windows and phones. I waited over two hours, but Bill never came. I was informed three weeks later that Bill died in there. Odd, I thought. He was just riding his bicycle 10 miles a day prior to being put in Prestige. I felt horrible and still do. This place operates like a locked facility, but without transparency or legal clarity. What happened to Bill felt coercive, cruel, and wrong. People need to know what they're getting into. The other reviews are spot on! This place is bad. My mom was worse after her stay there. PT was all but non existent; bathing/showering was a joke. She had medical issues that (bedsore, etc) were never looked after. Only time they put a patch over the sore was a couple days prior to her discharge. I imagine this is because someone realized it was worse and they thought "oh sh#t!" better cover this up. All in all, they booted her out the minute insurance was up and sent her home in worse pain, no strength or increase of mobility. Do NOT go here if you have a choice. read more

My mom went into this place walking with walker. Healthy eating regular food. They put my mom in a wheel chair and would not let her walk. I walked her ever day I went over. They put her on pasty food and would not give her regular food or allow her to have food. She was in a nice room on tell her insurance ran out then she was moved to the back in a corner across from a big bay window. Mom was allowed to catch pneumonia and go into a coma. Dr would not treat her right, would not send her to the E.R. tell she went into coma. Mom had to be taken off of life support 7 days later. I lost mom on Thanksgiving day 2015. I had no idea that this place would make my mom worse. 2.5 weeks in the hospital is a spa vacation compared to this place. I had to ask for her meds 3 times. They said there were no orders for timed dispersal. After 5 hours of asking, she finally given a pill....what the pill was, we are not sure. Mom said sugar. Nurse Ratchet is a Disney Princess compared to the CNA caring for my mom. We waited upwards of 30 minutes for assistance to the bathroom. Once I assisted my mom to the bathroom while waiting. I don't think she ever showed up. Leaving her alone was frustrating. In the morning she complained of pain from having to go to the restroom by herself. Oh, and this was the first 24 hours. How can an establishment who's sole purpose is to care for aged and dying get it so wrong? read more

It's been a year and a half since I was a patient of this facility. I still get nightmares of my experience here. I was transferred from swedish medical Center after a long hospitalization. The first thing they do is take away any body autonomy or privacy. I hadn't been this place for more than two minutes When someone with a diaper comes into the Room. She then proceeds to diaper me like a five year old in front of everyone. As the days wore on the many instances is of disrespect and what I perceive as negligence. The social worker who is doing the intake paperwork interrogated to me as to why I didn't like professional sports. . I politely and respectively explain my reasoning and she rolled her eyes. As the Days rolled on, there were many episodes of neglect, such as not helping me out of soil clothing for an hour on two occasions. On one occasion, The laboratory that this nursing home used made a huge mistake in my blood work. Due to this laboratory's mistake I was sent to the Local emergency department for eight hours. This particular nursing home did not and I repeat Did not report my weekly Lab reports to my infectious infectious disease doctor. They also did not inform me that I had a appointment with this doctor. The physical therapy was an absolute joke. I was under the impression that I would get two full hours. However that was not the case. I only got if I was lucky forty five minutes per day. I had a male physical therapist that was a nightmare. He forbade me from listening to any music during therapy. On one occasion after I'd had my first shower in a couple months and was wearing street clothes. He said in a very condescending voice why are you dressed up do you have an appointment . I now have a frozen shoulder and a pinched nerve in my neck due to the Inadequate therapy. The food was add best horrible. During the seven weeks I was at this facility I never received a hot meal. I have since learned that this facility has been cited by the state health department because of their bad food handling practices. I came across a washington state assessment testament that has them cited for twenty four violations of various degrees. I wonder what it's going to take to get this place shut down. Seriously bad!!! I have not seen my mother and wanted to fly in and visit. All was good to go until COVID-19 entered the home. No visitors! So I asked to visit via zoom. I was contacted with a date and time. A day later I phoned. No call! I sat and waited. I called and it seemed as the activity director had forgotten. She gave me the excuse later my mother had been sleeping. No call to tell me that or that she was sorry at the time. She emailed me later saying she was sorry and could zoom in a half hour, hours later. I had missed this email and begged for another time. No response . Called again and was promised a zoom meeting by phone. No response. Today was informed by my brother. My mother has tested positive for COVID-19. This is a week later than I was to have a set up zoom meeting. My heart is broken! This is negligence! I want to see my mother even if for a minute! This is not good for any family!! They knew about the virus! They knew it entered the nursing home. Four weeks before. Why did they not keep my mother safe? She had tested negative before. My beautiful aunt was taken to Prestige Care due to cancer. She passed away yesterday and while we were having her body picked up, there was a nurse whose car was temporarily being blocked. She had no respect as to what the family was going through. After apologizing and explaining that the van would be moved in just a few minutes, she proceeded to go out, search for the van keys and moved the van out of her way so she could leave. One of the other nurses continued to try and make excuses while we were moving my aunts body. "There is No excuse for what she did. As a nurse seeing that one of the patients had passed and seeing her grieving family, she should of had much more compassion. I will NEVER refer anyone here! read more

If I could give this place of business negative stars, I would. This place is absolutely horrible! DO NOT PUT YOUR FAMILY IN THIS NURSING HOME!! Unless you want them to miss out on food, get berated for having an accident in the bed, get told guest can't walk the hall ways by kitchen staff (Elizabeth). Same employee, who walked past me and tried to ram shoulders with me after telling us it's disrespectful and rude to walk around and talk with the patients. I had a few different conversations with patients, one who was lost and wanted to talk to someone "because everyone ignores me" when I found the closest nurse and asked her, she looked at me like I was stupid for asking her where the PT's room was because she was "lost". The nurse walked to opposite direction that the patient was. The next patient was was crying at her door who told me how she cries all the time, and no one ever comes to help or talk to her. My grandma, who has missed meals, berated for peeing the bed, the nurse snapped at us saying " I came in here three times and asked her if she had to use the bathroom she told me no." Then she proceeds to pull her out of the bed by grabbing her pants all the way up her giving her a major wedgie instead of putting the Gait/transfer belt around her and moving her that way. When they got into the bathroom, a different food lady came dropped her food off took the top of the dome. We asked the nurse why she took it, because her food was going to get cold, she snapped and said I don't work in that department I don't know why they do the things they do." We stripped the bed for the nurse and she took the bedding and set new bedding on the bed and walked away. After 45 minutes WE had to go ask them to sanitize her bed and make her bed! read more

Under staffed. Which means when you press nurse button it could actually be hours. Horrible food. Do not listen to patients and their needs. One day the bed was wet at 6am it took until 3 pm to get it changed. Terrible shower situation. Most PT were not good. Head nurse was rude. Director was rude. One day main medication was not given. Just not a good situation. Only worried about money not patients. read more

The staff here cares deeply for the residents. There are daily activities and it is always clean and inviting. They are an eden care facility, which means that there is a resident kitty, and the laughter of kids and elders fills the hallway. If my parents ever need care I would send them here, and feel good about it. read more
