My father-in-law received rehab at oak trace a few months ago. This is a very large facility/campus and his building seemed rather new and clean. There were a lot of open spaces for congregating, playing board games, dining, etc. However, the quality of his care could have been better. I think they are short staffed since he missed a few of his regularly scheduled physical therapy appointments. We looked into possibly buying an apartment in their independent living area but they are very expensive and don't offer the same services as others in the area. Overall, ok if you keep on top of staff. read more

The care here is excellent. There are so many people that are caregivers that are taking care of my wife that seem to actually want my wife to get better and come home. My wife has gone through so very much and where she was wasn't helping her at all. I moved her after she was at another facility for 42 days and was getting worse. She fell 9/2 and fractured her right ankle and fractured her fibula and tibula and was non weight bearing. Then she had to have her gallbladder removed. This gave me the perfect time to move her. With the help of Jaime I got a room here. She has improved more in a week here than the 48 days at the other place. I recommend Oak Trace especially if your loved one is also on dialysis. We chose to send my mother to Oak Trace for physical therapy after a hospital stay at Central DuPage Hospital. The facility looked great and it was close. However, upon arrival, we immediately started to experience problems. First of all, mom's nurse, while very friendly, encouraged her to simply relieve herself in a diaper rather than a bed pan, saying that it would be easier for mom (mom had history of UTI's). Things only got worse from there: 1. Our care meeting was a joke. With both of my parents, we had experienced care meetings at various facilities before and were very familiar with the process. However, at Oak Trace, when we requested a care meeting, they sounded confused. The caseworker gave me a date but would only give us a 4-hour window right in the middle of our workday. After some convincing, I was able to get them to agree to seeing us first (they were over an hour late). And when they did see us, they basically told us nothing. What was more discouraging was how the caseworker and one therapist that showed up (spent about 10 minutes with us) constantly tried to lower expectations of recovery. 2. Shortly after mom arrived at the facility, she began retaining urine (the opposite problem from what she had when she arrived.) They inserted a folly catheter, however someone had thought mom still had a stent in place, which would have been removed during a surgery she had weeks earlier. The charge nurse struggled to understand the difference between the dates of procedures on the discharge notes from the hospital and the header dates on the document, even after I tried to explain it to her multiple times, raising serious competency questions. 3. Mom had developed depression when she arrived at the facility, and at the care meeting we requested that a psychologist talk to her, which they agreed to. Our concern was that her depression was getting in the way of her recovery, which we told the staff. Just under two weeks at the facility, mom had seemed to begin to come out of the funk that she was in, and began participating in therapy more, and eating more again. However, the facility then sent in a psychologist who so severely shook mom up that she went into an even deeper depression than before (refused to get out of bed, answer or return phone calls, or even talk to us when we came to visit.) She described the session as horrible, and to us, it appeared to set her back quite a bit, which was the opposite of what we were trying to do. The therapist never came back. 4. While mom was in depression, she often refused therapy. Unfortunately, the facility's staff (not the therapists themselves to my knowledge) but particularly the nursing staff, would tell her that it was okay, and tell us all not to worry because "everything is paid for." We sent mom to the facility for rehab, not to lay in a bed. 5. After mom developed the serious depression after the psych visit, she started to seem ill to us again. We questioned the facility and they claimed to have it under control. Because mom was now getting ill (another UTI), she wasn't getting therapy and began to decondition fast. The staff acted like this was just fine, however she was looking more and more ill to us. 6. The day before we had mom sent back to the hospital, a nurse called me and told me mom had fallen out of the bed when she (the nurse) was trying to transfer her to a wheelchair. So apparently, this nurse or CNA tried to move my mom on her own (even though she required two people's help at that point), and they had put her on a very slippery air mattress. The person who did this also told me mom was just fine. The next day, I came into her room and she was experiencing pain in her legs and looked very ill. The staff claimed that everything was just fine, but my brother and I insisted on having her transferred to the hospital again. I will say that one CNA who came in acted surprised that they did not run an X-ray or send her out to be checked after she fell. When I requested that mom be sent out, the nurse instead called the physician who comes to the facility and told me to talk to him. The physician then told me things regarding mom's health that contradicted information that other staff members told me. He also strongly tried to discourage me from sending mom out for further observation at a hospital, telling me it would be a waste of money because they are not going to do anything for her and the ambulance would cost a lot. Additionally, another caseworker came into the room after I insisted on sending mom out anyway and started telling us it would cost us $500 a day to hold her bed, in what appeared to be another attempt to keep mom in the facility and out of the hospital. We insisted and she was sent back to Central DuPage Hospital, where they found that she did indeed have another rather serious infection that required IV antibiotics. Avoid this place for physical therapy! read more

My experience living in the independent living area of Oak Trace has been excellent, and the food is great.

Two falls on two units. No one has taken the time to get to know my dad. The nurses are quite horrific. If you ask questions you will not get answers. Took two weeks to finally have a nurse find his things they removed from his room which included necessary things like his glasses. There are literally no safety for fall risk and trust me everyone their is in serious risk. Sad!! Bad and they do not care about elderly people at Oaktrace read more

When my Mother moved from the Medicare floor to the self pay floor the attention dropped. When relocated six weeks ago, she had no toilet seat, working clock or a phone in her room. The toilet seat is there yet the phone and working clock have not been addressed. Even when she was sent back to the Medicare floor upon contracting COVID-19, the phone upstairs was never installed. Several staff members requested work orders (WO) my husband and myself each requested multiple WO as well as another sister. This company must place all their resources on the Medicare floor and forget about the other paying residents. Plenty of lip service with little action. read more

My mom was here approximately 3 weeks due to a fractured hip. A few of the staff (Michelle, Chuck, Maggie, Neal) are wonderful. Staff (especially nurses) switch frequently so there isn't consistency of care. One nurse gave my mom the wrong medication that thankfully we recognized before she took it. One gave her BP meds without taking her BP first- as required by the doctor and then argued with her that an aide took her BP already. (Untrue) due to the quality of care and attention I remained at the facility 8-10 hours a day. Every morning, my mom would ring her buzzer to have somebody help her up out of bed and get dressed and then would need to wait for somebody to come. If she needed assistance in the bathroom, sometimes it took 30 minutes before somebody showed up, and then she was left unattended in the bathroom and told "I can't stay with you" although my mom was a significant fall risk. No one came to change the sheets on my mother's bed for three weeks or offer her fresh towels in the bathroom. I took care of both. I also took on the responsibility of helping her wash up and get into bed at night because they were short staffed. The dining staff was very kind and food was prepared well. However, it was often Luke warm. If you ate in your room, you ate last and food was always cold then. Coming to this facility for rehab, we had to fight to receive the care we were promised. We were told she would receive one hour of OT and one hour of PT daily. That turned out to be 30 to 45 minutes of therapy up to five days a week. Physical therapy was very helpful. The OT often juggled three or more patients which meant my mom did exercises on her own. We were never included in a full team meeting. We had one meeting, with one staff member, at the end of our first week. After that, nobody invited us to a team meeting or shared team meeting conversations. They had her discharged going into her second week and already gave us a date at that point, despite progress being noticeable with her therapy. Upon her discharge, we followed up with her primary care physician three days later. Based on a urinalysis taken in the rehab center, we found that my mom had a bladder infection that was never discussed nor treated. The facility actually shared the records with her doctor so they would have known had they looked at the information . I would not recommend this facility to anyone. read more

My mother was approved for rehab at Oak Trace. I visited the facility. It was everything we were looking for. We are excited about the transition. Oak Trace found out that she needed a series of injections in her knee which would provide stability and allow her to benefit from rehab. We offered to cover the cost of the shots ($1600). They said medicare would not allow us to pay. The day she was scheduled to move the reneged on the placement and we are now scrambling to find another placement. We found 2 other facilities that will cover the cost but not geographically convenient. They say they are a non-profit but operate more like a for profit corporation. Beware! read more

My mother is a resident in the assisted living section of Oak Trace in Downers Grove. Like many long-term care facilities in the Covid world, Oak Trace is way understaffed. That's a problem, but Oak Trace has not taken the steps needed to ensure the health and safety of their residents. My mother and other residents have frequently been unable to receive the assistance they need. They'll push their call button for assistance and nobody comes to help. Some residents have sat in wheelchairs all night because nobody comes to help them into bed. It's only a matter of time until a resident is seriously injured or dies as a result of these issues. Oak Trace was acquired a few years ago by Lifespace Communities. Their CEO, Jesse Jantzen, needs to step up and ensure the safety of their residents. read more

Horrible place. My MIL is here. $12,000/month and they don't supply anything- we have to send briefs, all toiletries, all of it. The care is only by an assistant, who runs between at least 12-15 patients. Food is stuck to the floor. Her male neighbor is constantly in her room. A doctor rarely if ever visits. It is horrendous how much is spent on elder care, and how little "care" there is. read more

This place has been Amazing. They have taken such special care of my mother. I was worried as you hear such bad stories of nursing rehab homes. My mom is a celiac and one of the nurses stopped on her own to buy her gluten free bread. They have done everything to care for and give my mom the best experience while she was there. My mom and I were both sad when she was discharged. This is truly an amazing caring facility. read more

My parents were happy when they moved here years ago, but it has definitely gone down hill the past couple of years. The food is not very well cooked, there is tons of construction with little parking, and items have gone missing out of the apartment. On the plus side, there are lots of activities. If you do move here be prepared for 4%+ yearly cost increases. read more

My father has stayed at Oak Trace twice for short visits. The first time they discharged him, they kept his pressure sore boots. The second time they discharged him, they kept his winter coat, shoes and clothing. Be careful about going there with anything you don't want to lose. On the plus side, his balance improved while he was there, so the physical therapists probably do an ok job, and the social worker seemed good, too. She was the only person there I heard from, apart from a conscientious night-nurse who reported a couple small mishaps. I would have appreciated some information about what they were doing with him while he was there. Patients are 100% cut off from family for the first two weeks they are at the SNF. After that, they can have videocall visits or "patio chats" where the family member can see the patient through a window and speak via cellphone. read more

This place is under staffed and disorganized. Communication between staff and family is poor if it exists at all. Don't think that because it's fairly new and modern that this is the place for your loved one. I truly believe that my mom willed herself to die after her brief stay in their so called skilled nursing unit. With lack of staff, she fell frequently ending up bruised and broke a bone. I would not recommend this place to my worst enemy. read more

My Mother spent her final years at Oak Trace . The staff was friendly and helpful. The food was fantastic with a great and varied menu. We did not question or regret our decision to have my Mother purchase an apartment and spend her final years there. However upon her unfortunate passing we have very little communication with the sale of her place. During the initial purchase the sales staff was very helpful in choosing the right floor plan and communicating daily. We signed a contract that guarantees 85% of the purchase price upon sale. We reached out to the sales staff as to the sale of the property and we're told finance handles the sale. It has been 7 months since my mothers passing and we have never been called once as to the sale. I personally called the finance director gave him my e mail and told him to at the very least drop a note every month or so. Never have received an e mail or any communication. In hindsight we should have had a timeframe for the sale an 85% equity return written in the contract. Buyer beware! read more

BUT my mother was in for rehab and she was placed on a floor with many people with memory disorders additionally she was given a room with no pictures on the walls,carpeting(yuk) and NO TOWELS were available. She had to bring them from home. Really? Would have preferred plymouth place or King Bruwart. Immaculately clean and very upbeat! Caution boomers when we are taking care of our parents they are no 5 star facility. The staff was 5 star but that doesn't make up for the environment. read more

Our father was sent to Rehab/Skilled Nursing at Oak Trace, where the care was inconsistent at best. One of the nurses said he couldn't walk to the dining area or even feed himself, (right after we had just walked with him and eaten lunch), and another nurse said he was always walking around, joking with everyone. One morning we went in around 9:30, and my dad seemed almost comatose. The nurse said she had tried to wake him up, but since he didn't wake up, she was just leaving him, and hadn't given him any water, or his morning meds yet. By 11:00, we roused him, gave him water, and he was sitting up and joking. One of the therapists said he was probably dehydrated. Around noon, the nurse came in and finally gave him his morning meds, six hours late. Two days later, my father was having an incredibly hard time swallowing, so the doctor ordered x-rays. It was found that he had pneumonia, due to food in his throat that wasn't going down. Knowing this, they still gave my father dinner and breakfast the next morning, neither of which he could eat much of, and was spitting it up constantly. That morning, they moved him to the 4th floor skilled nursing, where the cost would've been $14,000 a month had he stayed. The room wasn't even as nice a a hospital room...it was small and stark white. Not a nice place for people to spend their final years. Anyway, they also fed him. When I explained that he couldn't swallow food, and asked for broth or something easy, I was told they only had "lasagna and soup that is spicy." I said they had an entire kitchen for the people in assisted living, to which the nurse responded they could only serve the food that was sent to that section, regardless of how the resident was. I said that since this floor was called skilled nursing, I expected even better care, but she told me it wasn't skilled nursing like a hospital, and didn't seem to know why it was called that. By the next day, we were taking my Dad to the emergency room, but not before being told there were no towels or washcloths available. We left two hours later, still no towels or washcloths. We never went back. read more

I had to move my mother here after living with myself and my family for 2 years due to her decline with her late on-set Alzheimer's and Dementia diagnosis. Although she's only been there a few months I find the staff are very caring and they are in constant contact with me if there are any questions, concerns or information they want to share. The environment is always clean and inviting. The staff also made great efforts to make sure to include my mother in their activities and remind her when something was going on that they thought she'd enjoy. I'm excited to see the new Catered/Assisted Living building that's currently under construction that's slated to be open early 2019 (when parking of course will then get better for everyone - guests and staff alike). My granddaughter works at Oak Trace and I decided to visit her to see the community. The residential living building is beautiful! All the staff and residents are so welcoming and friendly! They have a beautiful library and dining room. There was also an entertainer there that evening who was really good! My mother has lived at Oak Trace for 5 years - 4 years in the Independent apartments and 1 year in Catered Living. She also has been in rehab twice since moving in. We have been extremely happy with our experiences and quality of life, independence and care. The entire staff is focused on the safety, comfort and happiness of each person who lives at OakTrace - from the restaurant team, maintenance team, the wonderful front desk team, social workers and the nursing teams. I know my mother is safe and happy, and has people who care about her watching over her each day. The entire team made mom's move to Catered Living an easy transition - I can't say enough about the Catered Living team! We are so grateful to have a wonderful option in our community to give our parents a great life and independance in a beautiful and safe environment! Thank you! read more

My mother was a resident there 3 weeks after moving in she got a pinched nerve in her neck and had to spend 5 months in the medical hospital. The nursing staff made many mistakes and the food was awful. There are many nicer places and much cheaper. When she passed we were given 1 day after her funeral to get furniture out. Thanks... Six month later and we still have not received the monies they owe our family. They say they have to wait till someone takes the unit. Why is that our problem. read more

My mom has been at Oak Trace for a year. I am so glad this is the place we picked for her. I have seen caring and compassion from the CNA's on numerous occasions. The nurses do an excellent job keeping me informed of any changes in mom's health. Mom really enjoys the daily activities, especially bingo, arts and crafts, and cooking. The custodial staff does a good job keeping Mom's room clean. The food service staff is very professional. Thank you all for what you do on a daily basis. read more

My mother is a resident at Oak Trace. It is a lovely facility. The grounds are very pretty and well kept, the food is great, and the staff is top notch. There are plenty of activities and outings for those who are interested, including special interest clubs, wood working, tutoring opportunities, etc. When my mom needed additional help with daily living, the transition to catered living was easy, and I am happy knowing she can receive more care right in the same location when she needs it. read more

If you need skilled nursing, pass up this place! CNA took blood pressure with cuff upside down, patient did not get a shower for 9 days, vitals recorded improperly and people on portable oxygen tanks are wheeled down to the dining room and forgotten as their tanks run out. Many CNAs yell at patients and some nurses talk over patients. Upon arrival to this facility, no welcome, no tours or directions given. Patient climbed into bed to find holes and patched holes in sheets. Room temperature was recorded between 80-85 degrees for seven days before maintenance was able to attain a comfortable temperature on the floor. Reported CNA problems and then they not only retaliate with even worse behavior but also get the few good CNAs to join in to retaliate. Had to pull up a cot and spend the night with my loved one. What an attitude improvement that action brought about! read more

Mom was only there for a few days for Medicare Rehab. Would not recommend. I do not think it is "skilled nursing." Nurses only wanted to hand out pills, not help with questions about Mom or give Mom any help. Care was not adequate and we found another place as soon as we saw what was going on. read more

My mother went there to do her rehab after a knee replacement. It was a nightmare. She arrived with a heart monitor and the RN told her that it was not her job to make sure the monitor was working properly. No one checked her surgical incision or changed her bandages properly so my mom got an infection. The staff is lazy and lied about following through on the therapy schedule. The worse procedure was bathroom assistance. A male attendant took my mother to the bathroom in a wheelchair and she had to go to the bathroom while he watched her, claiming that the door must remain open to make sure she would not get up by herself. He also left the hallway door open so anyone passing by could look at her. HOW DISCUSTING AND HUMILIATING! The adminstrator wanted to know where they feel short of quality care. My suggestion is that he and all of his management staff put on a patient gown and be serviced and ignored by their staff - that should give them a reality check. It is a terrible place. My mom was so depressed and my sisters moved her out of there as things kept getting worse. read more
