The staff is super friendly. Something did not resonate with me about the prognosis I received and I found another holistic practice for a second opinion. Not only was original Prognosis not fully accurate, the cost would have been 1,000% higher than I ended up paying. I think Hazen can become a great holistic dentist once she's gotten more experience under her belt.

My insurance was a "high" plan with United Concordia. My wife and I had periodic maintenance in May and June, 2024 with no issues. In October 2024 I received an email saying I had a past-due bill of approximately $90. Since I had no outstanding bills I printed the email and went to the office to check it out. I was told there had been a "price increase" that wasn't applied to the last two visits. Since it had been so long they would waive the bill since I was a "valued customer" but price increases would apply to future visits. My wife's next visit was that afternoon. Her bill was significantly higher than what had been normal but I had been told to expect that. Three days later I got an Explanation of Benefits from United Concordia. It outlined the treatment for my wife and stated that we had paid the "out of network" price. I had always been "in-network" so I called United Concordia. They said Dr. Hazen had dropped out their network on 1 May 2024. Dr. Hazen's staff didn't tell me that, just that there had been a "price increase." A few days later I got an email bill for another $80 for my wife's visit. I went in and was told they had expected a particular procedure to be paid for by insurance but wasn't. I paid the additional bill and cancelled my November appointment so I could review my dental insurance options during my upcoming open season. When open season arrived I went in to Dr. Hazen's office and asked for which companies they were "in network." They told me United Healthcare. I changed my insurance to United Healthcare and rescheduled my November appointment for January 9, 2025. When I arrived I gave the receptionist my new insurance card and said I wanted to verify I was "in network" before my appointment began. The insurance coordinator came out and spun a story about how they were in network with some United Healthcare plans - but not mine, despite what they told me earlier. That was it. I'm done with Dr. Hazen. Her business practices and customer relations are abysmal. Insurance plans seem to be some classified subject about which they won't give you a straight answer. Time to find a new dentist.

The Pharmacist's name is Mitch and he is as friendly as they come. Mitch will always try and get you the best deal possible so just ask, he also accepts most insurances. While you wait for your prescription to be filled don't miss out on some of their OUTSTANDING ice cream, the BEST!! Even if you don't have a prescription to fill come for the ice cream, I have had it other places in town and this place has just about the best!!
