Just to keep a bit of privacy I will not mention staff individuals, But I felt as though I was family in here and I spent 10 weeks here for medical and physical therapy! Oh yeah and the food was so ONO. I initially was on the 2nd floor in a shared room and thanks to the floor manager I was transferred to the 5th floor by the end of week! She was the bomb I even spent a lot of my free time on the second floor . The fifth floor staff has so much aloha I felt at ease being in their care! The location was convenient. He was here approximately two weeks. The main reason to going to short-term rehab is if they need more physical therapy and occupational therapy to get stronger to be independent. Sometimes they no longer need to be in the hospital, but may be a fall risk so going to short-term rehab is a good idea. Overall, I think he had a good experience. He did receive PT and OT on most days. I believe it's six days a week. First they started him with a walker, then a cane, and then did trial him on no assist device. I also asked them to make sure he could navigate stairs. He told me the food was pretty good. Staff was attentive. Physical therapist and occupational therapist were personable and provided good therapy. Would definitely recommend this place for short-term rehab.

Most of the staff have worked here for years and they are a wonderful group of caring and attentive people. I observe them working cohesively together as a team. As a Registered Nurse, I am satisfied and pleased with the care and rehab provided at Hale Ola Kino.

The second time his stay was longer and he gained his strength back better than the first stay. I know the quality of his care was da best and helped him get stronger daily. I specifically requested The Care Center of Honolulu over other skilled nursing facility for it's excellent care, compassion, and supportive staff (PT, OT, Nurses/Aids and Dietary, and Administrative personnel). The admissions staff - Mike and Lynn are a Blessing since they accepted my husband into this facility in a timely manner when my time at Queens Hospital discharge was approaching. Mike and Lynn assured me that the transition will be smooth and my husband will be taken care of. And he sure was, settled in his room with the great nursing staff of the 2nd. floor. Mike and Lynn made sure that I was mentally happy as well since transitioning from Queens Hospital to Care Center of Honolulu was quite stressful. Mike also called me on a Sunday to inquire how I was coping with the transition and to schedule the orientation to meet the staff that would be taking care of my husband while at Care Center of Honolulu. Thank you Mike and Lynn and please be assured that if I need skilled nursing services, you are on the top of the list. Thank you again for your support and help on the recovering of my husband Words cannot express how grateful and Blessed I am to have the PT and OT Staff take care of my husband. The Staff has been so caring, supportive, and the outstanding quality of care, and always so professional as well as caring for his needs as well as my needs as a caregiver. The PT and OT staff always kept me abreast of his progress and always assured me he was progressing well. I know that his progress at the care facility as a resident was due to the hard work that the Staff has invested in him. The support of Stacy, Kevin, Tasha and I'm sorry I don't have all of their names , I am forever grateful. You all are my angels, and I thank God for your help. I cannot say or praise the nurses/aides/ enough on the 2nd floor who were so supportive, caring, administered excellent quality of care, and always willing to help me when I needed assistance with my husband while I was visiting. The nurses/aides answered my questions with so much respect and care. They followed up with me on my questions and obtained the answers if needed. Their dedication to my husband and willingness to help him eased my mind everyday. They answered all of my questions, and assured me that my husband was improving and contented while he was staying at the Care Center of Honolulu. The nurses/aides are such a Blessing, and I wanted to thank each and everyone of them for the care they provided to my husband as well as other residents, their professionalism, dedication to all that need care at the facility. The nurses/aides are: Ester, Rousell, Venus, Patrick (always made sure my husband had his TV on UH sports and taken back to him room in a timely manner for bed), Mack, Marlene, Rochelle, Sarah, Doreen, Abie, and Justin. If if left anyone out, thank you so much from the bottom of my heart. I am grateful to all of you for caring for my husband. You are a Blessing to us. Thank you for all you do for the patients at The Care Center of Honolulu. Thank you to Dr. Bruce Tamura as well...always willing to call me and discuss the progress of my husband. He called me to give me updated progress of his health and the health issues that he encountered. Also, suggested a healthy diet that would benefit him and his health challenges. I am thankful for his knowledge and guidance in my husbands health. The Dietary staff was go gracious to me and always assisted me when I asked for milk whenever I visited. The Staff never questioned me or hesitated on my request. The milk was used for his snack which was avocado and milk. This snack made him happy as well as I shared this special snack time with him. I am grateful for their hospitality and making my husband happy with his milk and avocado. The maintenance staff was very helpful and attentive to ensuring that my husbands TV was working when he had to transfer to another room. Their help with the remote control/tv installation made him happy since he could watch his TV shows. Thank you for your help. Thank you also to Zeny on always keeping me posted on my husband's participation on activities. I truly believe that socialization and activities are also medicine and help to stimulate his well being. The receptionist were also so helpful and friendly and always greeted me with a smile. She used to call me at home, to call the front desk, so that they would go and help her to the bathroom. She would press the call button but nobody would come, therefore, she would have accidents in bed. Keep in mind that my mom was 91 and did NOT have to use diapers, but they put her in diapers. I can only imagine how degrading it was for her during her stay. Another cause of concern was only 30 minute visitations were allowed because of Covid. I totally understand this, but your time starts ticking as soon as you sign in. Their elevators are so slow so our visit was only 5 minutes due to the wait for the elevators. We couldn't even visit with her! So sad and depressing. Needless to say, I took her out of this place so that she could live her final weeks at home. I sure hope visits and care are better now for the patients and families.

From the very beginning, the staff demonstrated a level of compassion, professionalism, and attentiveness that brought our family tremendous comfort, especially during such a sensitive chapter in our lives. My grandpa's room was always kept clean, organized, and comfortable. It gave our family peace of mind knowing he was in a safe, well-maintained environment where every detail mattered. The care team consistently checked in on him, ensured he was comfortable, and always communicated with us with patience and understanding. I want to give special recognition to Scott, the transporter, whose kindness consistently went above and beyond. He was always so helpful, patient, and respectful. Whether it was assisting with transportation or simply offering a friendly interaction, his support made a meaningful difference. Another person who deserves immense appreciation is Karen from the Business Office. She was a tremendous help to my grandmother throughout the entire experience. Karen was always kind, caring, and treating my grandma with genuine compassion. She made challenging days easier with her warmth and professionalism. Even though my grandpa has since passed away, we take comfort in knowing that during his time here, he received the best care possible. The staff's hard work, patience, and time did not go unnoticed and will always be remembered with appreciation. We wholeheartedly recommend this care home to any family seeking a supportive, compassionate, and well-run environment for their loved one. My first impression walking in was that this was a facility with proper staffing and it would be a…read moresafe place. Especially since they have alot of elderly patients. Many people struggle with care and affordable facilities, unfortunately the result of that could come at a bigger cost. My dad was at Avalon for a few weeks for rehab, Well, needless to say my dad got scabies while he was there, a uti, an infection in his spine and an abscess in his hip when he was discharged from there the same day he ended up in the ER and diagnosed. We have papers to prove the timeline These infections are all contributing factors to why he cannot walk. Further more the Avalon social worker on site refused to give a patient assessment for my Dad prior to his departure . The reason we were given is "Because he is no longer going to be in their care as of Sunday" even tho we asked for the assessment way before Sunday while he was under their care while they were getting paid by insurance.. but once they heard the payments would end on Sunday they wouldn't do it. Well, the picture is clear it was because Avalon didn't want anyone to know his condition upon release because that was not his condition upon arrival. also. When I visited my dad I noticed he had lost more weight, 10Lbs quick in a week, we immediately took action and started sending him a meal everyday to ensure he was actually eating because although Avalon was providing food daily my dad wasn't eating all his food and his rehab nurse was so happy when I bought him food one day while He was doing rehab..this man literally had tears in his eyes that's how concerned this nurse was for my dad. we sent the food purely to ensure dad would eat his food (a shake and a fish sand which) his favorite meal... once a Day..... out of our own pockets. the result was the dietician called me up while I was at work upset that I sent my Dad food stating that they are feeding him, it's like these people are so caught up in their own egos and emotions that they don't even care about the people that are there who need help. Nobody said they weren't feeding him.. If you are an Avalon employee and you feel the need to withhold Help from someone in need because your emotions you should quit today. The job you signed up for is not for you. And if you are looking for a place for your loved one and this place was recommended take what I'm typing seriously. Not everyone there is selfish and willing to put people at risk because of their own personal issues.. I'm sure Avalon has some great people there, some of the nurses and aides were very professional and I could clearly see they had compassion and empathy for the people they are there to serve... but the few that hide in offices like for Example sign on door that says "Social worker" her name is Harlene. Maybe find a job where your I don't care about what's right I care about how I feel" doesn't affect someone who has done nothing to you and needs help and can't help themselves because their disabled. Lastly, on his 2nd to the last day there I walked in to urine on the floor, and my dad hungry. If you have time to call People up about stupid things make sure their rooms are cleaned and they are fed first. After all this was said and done I then got word from a friend of mine that her mother was there and it was not good for her either. Pls do better. Again this is not a reflection of the entire staff I'm sure Avalon has some Great people there but some need to be removed.

BUT... I hope this review isn't offensive, it's really just a quick thought of what I had based on what we're allowed to get @ this time. Okie dokes! Thanks guys!! MUCH ALOHA & STAY SAFE! UPDATE REVIEW... So far mom likes it here. Not really much to complain about & they have mom up and about everyday like clockwork. Which is really nice to hear. We got her mobile chair delivered & this makes her therapy much easier. It gives her independence and builds strength again. I haven't had much of an issue with her care there just yet, so I can say... it's 3 stars so far. . 4 would be a visit into the facility and 5 would be super excellent feedback from my mom ONCE COVID restrictions are lifted. So there's that. And the fact they allowed us to drop off some signs and flowers and balloons and snacks for her and staff there. Haven't really made it out there in a while. Money has been a bit of an issue for now and so we're trying to get things back in order & home. Once all that's taken cared off, we should making frequent visits. But planning a surprise visit soon. Other than that, staff was very kind, helpful & cooperative when I met them they the glass door. They had nothing but kind words to say about mommy. Oh and they're super cute too because they have BINGO NIGHTS, MOVIE NIGHTS, KARAOKE NIGHTS & SO MICH ACTIVITIES FOR THE PATIENTS... they have a lot of activities to keep them busy. So they're not bored at all. That's the one thing I know for sure! Because they gave away the bookmarks that we dropped off as prizes and the patients were super happy because there was enough for EVERYONE. Everyone WON! Hehehe....

He warned me of the bumps on the road and any sudden stops-for my safety and comfort. I checked in and was not very impressed by the surroundings ( its not the Hilton or Ritz). Just very low budget but functional sort of ghetto feel. The scale they put me on could hold a wheel chair and we got weighed together/ handle of scale was broken and hanging awkwardly. Little things like that need fixing or replacing but is never repaired is questionable. 2 star Every bed had a 14 inch personal tv monitor that was connected to the wall hung on a heavy duty hinged armature- that's great. That lady took a shower after finishing her dump and went back next door. My roommate said many different people are using our bathroom but she is not sure why? Aid Michelle said the lady nest door is too big to fit her shower and comes over to use ours. OK 1, 2, or 3x but everyday? I do not know her and its unsanitary.
