I have considered myself a healthy and fit individual and many times felt I was immune to any problems of health and wellness. I never realized how much the things I was learning would have an impact on my life overall. Not only do I feel better and have more energy throughout the day; I was able to make those difficult lifestyle changes with lots of support that I feel I can take with me for the rest of my life. If anyone has any interest in improving their overall health (not just a weight loss goal-although I did lose over 15 lbs in 3 month's) I would highly recommend a consult at least with Dr. B and her staff.

It wasn't just a doctor's office--it was an experience. A fancy, modern, concierge-style medical service that made you feel like you were stepping into the future of healthcare, not a typical waiting room with fluorescent lights and paper gowns. From the moment you walked in, everything about Forward felt elevated. The sleek design, the clean minimalist aesthetics, the glowing biometric scanner at check-in--it all said, "We're not your average clinic." And they weren't. There was cucumber water. There was tech. There were calm, unhurried appointments that didn't make you feel like you were being shuffled through a system. The medical staff? Incredibly professional, kind, and modern-minded. It felt like they actually listened, and not just for the sake of ticking boxes. You had time. You had access. You had a real relationship with your doctor, not a rushed 7-minute appointment every six months. Forward made you feel like you mattered--not just as a patient, but as a human being. That's rare in medicine today. So yes, I'm sad. Genuinely sad. Sad to lose a service that felt like it was finally getting healthcare right. Sad that something so innovative and patient-centered couldn't quite make the numbers work in a system built for bureaucracy, not people. Dr Brown is incredibly knowledgeable and goes into great detail to answer any questions you might have. I make sure to go in prepared and pepper her with a variety of questions. After each visit I feel I have taken a course on medicine. I can't emphasize enough on how knowledgeable she is! All staff is very friendly and polite. I feel forward is exactly in line with what I want from a health care. Preventative maintenance. Again I try and make sure to ask questions and take the appropriate steps to nip things in the bud before something more serious comes down the line. My only gripe is the monthly fee, but you can't put a price on your health.

    The day that Dr. Hale retires will be the last time I ever step foot in this practice. I have…read morenever worked with a medical office/facility that ACTIVELY hides from its patients like this one. The inability to get a real person on the phone is diabolical. All the procedures, portals and voicemail hell this office has implemented "for the convenience of the patient" is anything but. The convenience is solely for the administrative office which seems to work incredibly hard to avoid interacting with patients. I have been with Dr. Hale for 34 years, and it is the ONLY reason I still put up with their nonsense. It just leaves me so confused that a profession that's supposed to be helping people does everything in their power to avoid them.

I originally called the number provided to the Glendale office, but did not realize it goes to both locations. So when I booked the appointment they sent me to Scottsdale location at 1pm same day, which is far. Once I realized I called back to switch, the same girl answered (you can tell, the Glendale girl has the most annoying voice.) They confirmed to switch me to 9:30am the next day. At 1;40 that same day, they called to confirm they should cancel my appointment, even though I called to change it. So they say no one actually booked that appointment, lets book it now, but that time slot is full. So we book for 1:30pm. The next morning, they called to switch my appointment time earlier, because the provider (who now no longer works there) did not want to stay all day with a huge gap. After them trying to get me to move up, i said i will not be showing up any earlier. Was super worried no one would be there for my appointment. Then, I get there. This place is DIGUSTING. There was a dead cockroach on the floor, what looked like puke residue all over the coach and floor. They brought me back for vital check and just pulled a stethoscope out of a giant pile, didn't clean it, and put it back into the pile after using it after putting it on my bare arm. The patient room was really dirty. I finally see the Doctor, who, I actually felt was knowlegable, Dr. Jain. Got the meds I needed for a trip and said I would schedule a follow up when I returned. Last week, I called to book this follow up and despite telling the same annoying voiced girl that I wanted to book an in-person visit, she scheduled me for a telehealth. Only for Dr. Jain to call to say this needed to be a in-person visit. This appointment was yesterday, we booked for 3pm today. Today comes, and in the morning the same annoying girl called to ask "if I would rather have this appointment via telehealth." I then get annoyed and tell her that she already made me book a telehealth only to have it moved to in person per the doctor. Then, she is trying to get me to come earlier today. I stated, had I wanted an earlier appointment I would've booked it. Then, I just say they do not have their s**t together just cancel my appointment, to which she says "are you sure, you really need those meds." I already had the meds...this was a follow up for bloodwork and to talk about a few other things. They do not pay attention to the information you provide. Their links never work to fill out the paperwork prior. In the off chance it does, it does not matter, they cant access the links so they make you fill it all out again on paper when you get there. DO NOT waste your time at the Glendale location. I do recommend Dr. Jain, but spare yourself and go to the Scottsdale location and specifically ask for their scheduling team when calling.
