The notice shall be sent via first-class mail, via email from an email address on file with the health studio, or DELIVERED IN PERSON TO (Name of health studio operator) at (Address and email address of health studio operator)." I have sustained a massive injury. I have constant pain and a spinal fluid leak and can no longer do any kind of physical activity. I have been trying to cancel since the end of March. The gym manager demanded I come in person, or mail a letter along with my medical documents as proof in order to receive a refund or to process my cancellation.
