Odd since the only info you need for an appointment is the patient's name and birth date. The remaining "personal" information would be the policy holder of the insurance which would be me. Pretty sure my 21 YO doesn't know who the insurer is, my birth date or the last four of my SSN which is what they need to submit the insurance claim. Not sure that the less than user friendly staff understands the concept of HIPAA in this context. I am the person providing all the of the "personal" information. Never been told I couldn't make an appointment for someone in my family because of HIPAA. Happy to take my business elsewhere.

In the past, every doctor-patient interaction has been robotic and it was such a relief to escape from that monotony. My doctors were very kind souls and helped me plenty. I was told my health results and given accurate and understandable advice. Duly also has an app where all my files are saved and I can view my test results at any time which I think is awesome. The app also has a feature that lets me talk to my doctors. Also, whenever i go out to events, it's sort of a funny coincidence that Duly sponsors so many of them. Even worse, was the glaucoma "specialist", Dr Joshi, couldn't remember me from one visit to the next and constantly gave me bad information until I corrected her. After I was promised no wait, and waited 30 minutes, I decided to take my business elsewhere. When I received my chart, it was full of BAD information. There were illnesses I never had listed in history. The unforgivable is that I was diagnosed as having Glaucoma in December 2011. I received many tests, took eye drops nightly and even had a laser surgery. I met my new dr today. I do not have glaucoma.

Once again, Dr. Vitale and staff did an amazing job fitting another twenty five year old pair of Ray-Ban Wayfarers (tortoise frames) with prescription "transition" lenses. In the past year, I've had Dr. Vitale's office transform four pairs of regular sunglasses into prescription sunglasses, including an expensive pair of Harley sunglasses, and, another twenty five year old pair of Ray-Ban wraps (tortoise biker frames) All four pairs work great while riding too! In case you're wondering about the twenty five year old Ray-Bans, back then a good friend was the Chicago rep for Ray-Ban, and GAVE samples of whatever I liked. Go to another eye doctor and they learn that VSP said I used my frame benefit. So this office never cancelled w my insurance company- which I'm sure they were paid for. I am giving two stars instead of one because if you can avoid being seen by Dr. Nuccio the rest of the office isn't bad.
