After my father's fall and hip surgery, he was sent here. Pros: The facility itself is nice, clean and well maintained. The front office staff are friendly. There is a nice courtyard. They have an impressive gym for those doing physical therapy. We REALLY loved Virginia, Gilbert, Kovita, and Troy. All were so patient and caring. My dad was doing physical therapy for about 2 weeks and I did see a lot of improvement in his upper mobility. Cons: Cant' really pin-point what I disliked about this place but I did feel like the administrative staff wasn't as pleasant. Make sure you are checking on your loved one and that people are doing what they are suppose to be doing. On two occasions when my dad needed help to use the restroom, a person came and said they would return since they needed secondary help. They never came back. My mother informed me of the issue and I texted an administrator. My mother stated that after that people seemed more aware and friendlier. Deann is somewhat friendly but not a good listener. My dad was instructed to cross his legs when being in the wheelchair and he's telling her to please wait for him to cross them and she just continued pushing the wheelchair.... she was too in a hurry. It was genuinely frustrating to watch, as if she was simply ignoring him. For those that get upset about the discharge process, I can't blame you although discharges have nothing to do with the facility itself. That is entirely insurance. Sadly, I feel like there should still be a better and more empathetic way to guide people on what their options are. Thankfully, we had a Plan B for my dad and he was able to return to the facility he was at prior. I just feel like the bedside manner for some employees there is lackluster. read more

I am beyond disappointed and honestly disturbed by the level of care at this facility. What I witnessed here goes far beyond the occasional oversight, it reflects a systemic neglect of basic human dignity and well-being. Our loved ones are not treated with the respect or attention they deserve. Simple needs such as assistance with hygiene, timely meals, or even just being checked on, are often ignored or delayed to an unacceptable degree. It's heartbreaking to see vulnerable individuals left waiting, confused, or uncomfortable while staff appear either overwhelmed, indifferent, or completely disengaged. What makes this even worse is the apparent failure of management. There is a clear lack of accountability and organization. Concerns brought forward are brushed aside or met with empty assurances, and no meaningful changes ever seem to follow. It feels like leadership is either unaware of what's happening on the floor or simply unwilling to address it. An elderly care facility should be a place of safety, compassion, and support. Instead, this environment feels cold, chaotic, and, at times, neglectful to the point of being dangerous. Families trust places like this with the care of their loved ones, and that trust is being deeply violated here. I would strongly urge anyone considering this facility to think twice and thoroughly explore other options. No one deserves to spend their final years in a place where their basic needs, and humanity, are treated as an afterthought. read more

My friend was at this facility. She was often left so long she had a diaper rash and bed sores from improper care. Sitting hours in her own waste. She was told they were short handed instead of being told they were sorry they didn't come sooner. According she was told management didn't hire. read more

I just got out of there as a patient and all the past negative reviews are absolutely true. The lack of communication was worse the staff didn't tell me I was discharged my family did at 8 at night. The nurse stations we empty at all times push the button wait 30 minutes. Medication was always late not ordered until you ran out so you would then have to wait. Not a doctor on sight. PT was a joke machines broken 10 people on the same room with 6 staff. Couldn't move in there. The food lol.....you better call Uber Eats. This place is beautiful, clean, smells good and that's where my compliments stop. Never send a loved one here! I have never seen such a lack of care for people that really need it. They damned near killed my husband with neglect. Couldn't wait to get him out of here! All of my complaints went unanswered. They couldn't give me the time of day or for my husband either! He was in 4 different hospitals over the course of three months! This place was the absolute worst by far! I will be reporting them to Medicare!! Not only are the nurses lax but do are the doctors. The PT Team were enthusiastic when they actually gave PT, but that was sporadic. They say 1.5 hours of PT, you're lucky if it was 30 minutes. I give a big thumbs up to the Wound Care nurses. The only ones that truly cared about their jobs and the patients. Don't go here if you need sub-acute care! You won't get it! This place is a joke and they ought to be ashamed!! read more

The food was mediocre. Everything else was top notch. The nursing staff was empathetic and well trained physical therapy was well equipped and well trained rooms and the facility were absolutely gorgeous and clean. The nurses were nice. The doctors were polite and a physical and occupational therapist really helped me get back on my feet.

I took a tour of this place and I absolutely loved everything about it. It's even better than the pictures. I had already seen 5 other rehabs in Phoenix and Scottsdale and this facility is outstanding to any of them. And their ratio of nurses is 1-12. I ended up choosing another close-by facility just because a doctor we know suggested it and felt it was the best place for my husband given all his current medical issues. This place was new in August 2022 and is still working out some kinks. But it's still far superior to many, many other places I have seen. read more

I was in this facility because I completely fractured my right ankle. I can tell you I had excellent care. I was treated with the utmost respect and listened to by everyone I encountered. I participated in Physical Therapy (thank you Gilbert and Jackie for working me hard while still listening to what was going on with my body), and dialysis (shoutout to Jason and Connie for a job well done and trying to keep me warm. Loved the banter too). Thank you to my CNA's (Devante, Tony, Deann, Rae, Rosa, and far too many to remember), and nurses Lena, Lydia, and many more). Each one of them took very good care of me and every day with a smile. Thank you to the Activities department (J, Christina, Alecia) Oh no, I'm drawing a blank on the last one! Anyway, what a great group!) I had trouble with the food but it was a me problem, not them. I struggled to eat protein. They had a variety of foods and an optional menu if you didn't like anything they were serving. You were given a weekly menu of what was being served so you could change or cross off what you didn't want to eat. This is long winded, but I didn't want to miss anything. If I ever have to go to rehab again, I want to come here! read more

Memory, temporary care. Depending on what shift your loved might be staying there could be really bad/sad. The management doesn't seem to be on the job. Or doesn't care. Nurses station rarely is occupied. Most patients ignored, activities, what activities? Parked in front of tv and left all day. Breakfast food tray sat til afternoon... no help with eating meals. Meds not given. A beautiful facility but I would not even consider leaving my dog with them. read more

Of course, the facility is nice in appearance. I would give zero stars if I could. My Mom was here after a shoulder fracture, and she also has dementia. Some of the staff were unprofessional and not really attending to her needs. Situations such as her breakfast left in her room, so she hadn't eaten anything all morning. A soiled and smelly diaper that hadn't been changed all day, which happened a few times. Aggressive words to myself from the care person that "she needed her ass beat", referring to my mom. Disgusting!! I will never take here there or recommend this place to anybody. read more

This is one of the worst care facilities I have ever taken my grandmother to. The care she received was extremely negligent, and some of the workers were very unkind towards her. They left her clothes in the closet sitting in her own waste from an accident three days before when I came to visit after they changed her. Not only did they lose her clothes in the wash when they took them, but they also lost her glasses and her phone over the course of a month and a half. We only discovered that her items were missing when we visited her, which we did frequently. If my mother, aunt, brothers, cousins, and I had not been so involved in my grandmother's life and attentive to the care she was receiving, we would not have known about their unkind attitudes or their lack of care for her belongings. Additionally, my grandmother would push the call button for her nurse, and it would take up to 45 minutes to an hour or longer for them to respond to her needs. Upon visiting the following morning after she was admitted at night, my aunt and I were upset to hear my grandmother tell us that a young lady who came to give her medication didn't taken to consideration that my grandmother can't move quickly and seemed disoriented. This young lady told her, "I'm not going to help you if you don't take this medication." Keep in mind that my grandmother suffers from arthritis, dementia, and Alzheimer's. Moreover, there was one afternoon when I visited her just before she was getting ready to take a shower. I noticed noticeable red bumps on her neck. When I inquired about them, the staff couldn't provide any answers and asked if it was the first time I noticed them. I questioned how they hadn't seen them, considering they were responsible for bathing and showering her. They dismissed my concerns, saying it was "just a bug bite." It turned out that my grandmother had shingles. My family decided to discharge her because we were not satisfied with the care she received. I asked for her social worker multiple times to discuss the treatment she had undergone. The facility lacked communication on all levels. I don't know how many times I asked to speak to her social worker, and they brought me somebody else. I don't know how many times I requested her nurse for her pain meds that they took forever in the day to give her. Lastly, I was only able to speak with the social worker and Jordan Clay, when we were leaving. I shared with him the negative attitudes of various nurses, the loss of her belongings, and the fact that they administered her medication without ensuring she took it safely. In response to my concerns, the director simply said, "I'm sorry that's the care you received." read more

Similar to other reviews I've read, our experience was not good. The quality of care was not good. We had to constantly ask for things to get done. I spoke to the Director of Nursing, nurses, etc., in order to get what we needed done. This is not how it's supposed to work. Additionally, there are some very unfriendly employees working there which includes nurses, RT, and CNA's. Yes, the building is nice but what good does that do if the care is not there. IMO, I feel the standards of care are not being met at this facility. We had to constantly ask for things to be done Along with poor medicine management. We should not have to visit daily to ensure our loved one is properly cared for. Yes, the facility is beautiful but don't Judge a book by its cover. read more
