Between a 2 and 3 star review. My mom-in-law was here for about a week after a hospitalization. We would never place her here again. They are very understaffed and that's probably demoralizing the care team, and it shows in the way they treat their patients and patient families. Dont try to call the nurse stations, no one ever picks up and no one returns voicemails - their staff even said to us that vm won't be returned. Any time we asked a question of the care team, when we could hunt someone down, they didn't know the answer and gave us a blank look. I was speaking with one nurse and he ignored me and was filling out a form for someone else the entire time. You have to REALLY go in person and demand attention and advocate for your family member's needs. For the week she was there, one of the two elevators was down so we took the fire escape stairs (which staff recommended) since the only working one was SO slow. Obviously the patients can't take the stairs so the worst part was it meant there was no access to the whole 6th floor - which is where the physical therapy/rehab space is!! So ignore their pics of PT equipment and the pretty glass roof and view, it was blocked off for all the patients! It also meant they canceled all the communal activities! Also the food is terrible, so mom barely ate anything the whole time she was there. We visited every day and never saw veggies, and saw plain hotdogs (no condiments) and tots, dry gyros, etc., and cheap fake extremely sugary "fruit juice". Nothing healthy at all. read more

My grandfather was not properly taken care of. It is a run down facility and I would not recommend taking family here if you need anything more than the bare minimum. When problems arouse there was a prolonged delay in treatment, not enough nurses+CNAs to patient ratio and the CNA/students use patient rooms as their storage. If your loved one has dementia, do not take them here. They will not get the assistance they need for daily living. I will say Remy (nurse) and Ray (cna) were absolutely amazing and the only good thing from this facility read more

Dont even think about it, they lack they lack check people steal the guest clothing my husband they stole his clothing when he went to the hospital for two when he was sent back everything was gone, including his eyeglasses, toothbrush, electric toothbrush, and all the clothing that I brought him then they put him in another room the same thing happened. No clothing return it took them a long time to relocate his walking chair all in all it was the worst rehab. My husband has been and I spent a lot of time there the the accounting not truthful after you pay what you owe which came to about $6000. They tacked on another $225 without an explanation anyway I was waiting to give this review. Thank you very much for considering going. read more

It's the kind of place where you don't want to send your parents, but you're GLAD they're here. Of course, we check Yelp for reviews and experiences, but that's what we do :) My Dad was here for 4-5 weeks and on day 1, the intake manager was very personable and was able to switch my Dad from a 3 person room to a 2 person room. ALWAYS ALWAYS ALWAYS Speak UP if it's not what you want. The food is not bad. CNA's were very helpful, especially the 2 wound care nurses. Special Thanks to Jean Eastridge, Social Manager, who helped me arrange transportation for my Dad 2x when he needed to go see a doctor. Overall, yes, this place (and most places) does smell like Urine, not the cleanest, but my review would give this place a solid 4 star. I do like that they check your temperature before you enter the facility. Easy Parking. White Zone always available. That's Huge for Visitors as well. read more

This place has its pros and cons. The pros are that the CNAs and nurses are awesome and attentive. The food is okay for a hospital. The cons are that ppl are often talking or singing in the hallway, which makes it difficult to rest. Also, for some strange reason, they don't have ice packs. Really? I just had surgery and need ice packs! There's no way to get in touch with the kitchen to let them know about food changes you'd like to make. A dietician talks to you when you first get here and then nothing. read more

If your case manager recommends this place, do NOT go here. We were promised a single room but ended up in a 3 bed room with 2 other neighbors despite having a covid-19 outbreak on every floor. Management is missing throughout the day and can't be found but will show up to a patient whose insurance stopped paying for his needs (yes I heard the whole conversation because there is no privacy when there are multiple filled beds in a room). I also found cockroaches running around on the floor and CNAs throwing their items into resident's closets to hide. A star is for the nurses because they have 25 patients under their license and have everyone calling for them every minute. If only the state reviews look thoroughly within the management instead of worrying if a nurse washes their hands for 20 seconds then maybe this place would be better. read more

I have been in rehab at City View for 10 months. It is a clean, safe, and well run facility. The staff, especially the RNs and CNAs are hard working, professional and compassionate. The food is good for hospital food. The coffee is not good, I call it prison coffee, but I am used to Starbucks. I am grateful to be in a well run place like this. read more

Care is minimal, there's only one R.N. for each floor, furnishings are cheap and rundown and three patients are crammed into each small room. The TV at each bed is out of date with few channels, poor reception and no way to listen privately so you hear what they're watching in the other two beds. My husband was there for a month. During his stay his catheter was never changed because none of the staff were trained to do it. He developed a high fever and sepsis and had to be taken to the hospital by ambulance. When I saw how sick he was, I had to beg them to contact a doctor. If I hadn't intervened, my husband might have died. There were some nice, helpful people working there, but on the whole the place is a dump and I would avoid it if you care about your loved one. read more

Excellent quality care from top management down to housekeeping staff. Support Care are overwhelming from the Nurses to CNA's.

If you have a loved one going to this place, be extra vigilant! A friend was sent to this facility to be monitored and recover from a multiple fracture on dominant hand.is oxygen dependent, Fall Risk, diabetic, sleep apnea requires a c pap etc. This facility is NOT review patient's Care Plan or basic information!! The aid/nurse took the patient OFF his oxygen, put him a wheel and went down the hall to get his weight. How long can you hold your breath? When family member called to inquire when patient was not connected to Cpap machine, aid/ nurse replied, we only have plugs for the bed, and oxygen. Patient could not sleep, until multiple inquires about the cpap machine. Patient spent one week and returned to the ER due negligence, oxygen Saturation, glucose were never monitored and No insulin given. I don't think the staff was unpleasant and yes I thought they were bus but one week of negligence is absurd . If you have no choice, be vigilant! read more

Awful! I cried when I saw this place that I had to stay at. Absolute filth abounds.I was disgusted to see my bed did not have clean linens . The bathroom was like a dirty storage sp. poop (lots) on toilet seat , along with much soil. bathtub filled with various items, ie: old commodes and other equipment, pillows, and much more. ledge above sink filled with used plastic cups, old vases old toothbrushes and more. The food is near inedible. Last nights dinner was three mounds of different colored mushy stuff. Old, dried food clinging to bedside table. Meds either No delivered or not delivered in a timely manner. I could go on and on. If you're appalled by filth, unsanitary conditions, inept help, and negligent staff, this is NOT the place to come! I'm surprised the health department hasn't shut this place down. read more

If you love your family do NOT allow them to come here. The nurses are beyond incompetent. They are functioning zombies and really have no care for the patients. I'm not sure why they collectively are lazy and don't find anxious and sore patients to be a concern. My mother was suffering from DVT and she waited for HOURS before sending her to the ER, which she practically begged and thought she might die!!! Please do not bring your family here, unfortunately people without family may be trapped here and mistreated. This place needs to close or needs a huge inspection. Terrible terrible place. Nurses DO BETTER, why did you go to school and become a nurse to not fulfill your moral goals. Insane. And the food, literally like prison food. Skilled nursing care. 14+ days 12/3/24-12/23/24 approximate dates. My friend was here, she was supposed to have a shower and her hair washed 2 times per week. After 14+ days, she had never seen the inside of a shower and never had her hair washed. According to my friend, they only wiped her genitalia and never washed her hair. When I found this out 12/20/24 I asked that they give her a REAL shower and wash her hair. The person who should have been showering her and washing her hair never did their job. And when confronted adamantly and angerly said they did. My friend has no memory issues only a heart issue. Plus her hair was absolutely filthy. Disgraceful, filthy, inhumane treatment regarding bathing. Food was always delivered cold. When I visited, I saw many people standing around doing nothing. My friends roommate said she presses her bed button and no one ever comes. They ignore patients. read more

City View San Francisco 4th floor Head Nurse Amanda is the best nurse I've ever had. She helps when I call. Good with making me comfortable,did a great job bandaging my heel just the way the doctor wanted, making sure I do things myself so I learn. She is pleasant and takes my concerns seriously. I was a resident in City View Acute Care from 22 April 2020 through 15 Dec 2020. I normally reside 100m from City View in it's 1980 Senior Housing replacement. My preferred acute care choice is this facility. I've been in several. This was a Hobson's Choice. Food: Four food service managers during my stay. The last [Oct-Dec] was 4 stars. The other three rated 1 to no stars. I don't like runny eggs. After 6 months, I just ordered from the kitchen. I once got a chicken piece with worm holes. I took an image with my phone AFTER I took a bite ! Care Staff: The CNA's, RNA and RN's were exceptional...except for some lifers. Most all were new hires. PT/OT: The providers were good to exceptional, manager not so much. Doctors orders were ignored or done just before my insurance ran out only to fill out timesheets. Reason for Acute Care: 30/60 days for Gallbladder infection. No operations done during Covid 19. I was walking 2 weeks before coming here and in several months, I couldn't flex my legs and I was bed bound. I lost 32 lbs in the first 6 weeks, 20% of my total weight by the time I left...8 months later. I had the choice of here or home...I chose poorly. Doctors: Doctor [1] and PA [1] needed a fire under their feet to consult them...at first. After they acknowledged my issues, 5 stars. They were 200% overworked. Social/Case management: With one exception, case manager[s] and Social worker[s] were lacking or non existent. Mostly new hires. Senior/Middle Management: Lacking to nonexistent. *MOST ALL NEGATIVE COMMENTS WERE FROM LACK OF EFFECTIVE MANAGERIAL OVERSIGHT...IMHO. Facility: It was built in 1947 with 6 floors and only 2 slow elevators. Patients will shelter in place in an emergency or power outage/catastrophe. Bring your own Rosary to grasp in an emergency. Personal Belongs: My only possessions were my flip phone with charger, my keys on a lanyard and my medical alert tag with my dog tags. I left with ONLY my phone and charger ! I was told I would expire here...several times, in so many words...sadly. It took more effort to voluntary leave here than escape my Vietcong Prison...54 years ago. Yes, I turned 75 in City View. I daily thank God for the health of all the residents and staff...management not so much. Please...pray to God for Divine Guidance to choose for your loved one or yourself. Our Lord will provide. Just ask in your prayers for help in this time of tribulation. Go in Peace. read more

This place is so horrendous. I wish I could give it zero stars. I am filing a formal complaint with the City of San Francisco...feces in the bathroom all four days I was there... never saw anyone in a cleaning capacity at all... PT and OT sporadic at best and seems disinterested. I stopped eating the deplorable food after awhile as I just couldn't stomach it. When a fellow patient had to bring me my breakfast tray that was it. Whoever is in charge should sneak out the back door and change his/her name and leave the country. This place is horrible. I went to check on my grandmother at around 4pm and found her absolutely distraught not being able to breathe. She was anxious, and couldn't even speak a word because she was having a really hard time breathing. I notified her nurse, and the nurse said her vitals were fine even though her blood pressure was 244/124. She said the machine was "not working" and could not find one that seemed to be working. She concluded that my grandmother was fine. I said I didn't believe so, and that she usually gets like this when she has fluid buildup in her lungs. She checked her lungs and said "oh, everything sounds fine." I ended up asking her to call an ambulance which she didn't. She only called transport which takes a lot longer than an ambulance. Once transport got there, they were also upset with her because they said she lied to their dispatcher. Nurse didn't mention my grandmother was experiencing tight chest. When emt took my grandmother's vitals her BP was 247/97. What was most upsetting was that when transport got there nurse was giving me report saying my grandmother was fine but that the family was wanting her to go the hospital but that her vitals were okay. When my grandmother got to the hospital the doctor immediately called me that my grandmother was going to go into the ICU. Said if I wouldn't have advocated for my grandmother to come to the hospital, she would have died. So, absolutely DO NOT come here or bring your family member. read more

This place deserves NO STARS. Nitemare disaster with limited NR and they are so unprofessional as unskilled it's a disaster. Poor older people that don't have family support to come daily. The food is below standards and looks like dog food. The facility is filthy dirty and so under-kept. If you need some done you we'll wait all day and keep asking and asking. Staff is full of shit. Rooms are gross if you can please send somewhere else. This facility is before average needs to be closed down. Will attach photos of now gross it is. Saturday I waited all day to get a catheter removed and an IV put back in. One NR in the floor serving 20 people and did an awful job make my dad guss blood and she had to call someone from a diff floor he also pooched server times before he was able to complete. The catheter needed to come out no doctors on Sunday excuse me it's a rehab where are the doctors. I understand people have no other place And are disparate. But better send ur loved ones to the graveyard before this facility. They also like to keep people longer and not discharged due to getting $$$$. Don't allow them to fool u. They are a team of $$$$ they want the insurance $ and will tell u nonsense. Don't be fooled. read more

My sister is staying at City View and she is getting wonderful attention and care. Whenever she calls for help someone responds right away. This is far better care than she had during her hospital stay of 5 days and better than care at a "sister" facility in San Francisco. I have never seen such a large staff available. Good thing they don't have a negative number to give. The absolute WORSE place in the world that you could possibly send a loved one. Hell, I wouldn't even send my enemy there. The only pleasant thing at this place is Jackie, the receptionist. That is it. My brother was at this hell hole for 12 years and died in their lack of care. Two months after he got there, the therapist called me and said that he has reached a plateau in physical therapy and wouldn't be given it any more. It was a waste of their time! But wait, isn't that your job? So 12 years he stayed at a rehabilitation facility of which 2 months he received rehabilitation and 11 years and 10 months he stayed there just so they could collect his money. The pictures you see posted on their website are all staged and the staff for the most part is super lazy. They do not answer call lights or verbal requests. They can never be found when needed. It is very sad. My brother actually called me one time to have me to call the facility to let them know that his roommate had passed away. When I called and got transferred to his floor's nurses station, the nurse put me on hold and checked. He came back and said, "yes, John is dead". Really? Several occasions I had to call for my brother because he had been sitting in a soiled diaper for more than 4 hours with no response from his nurse or CNA. When I called they told me his CNA or Nurse was on lunch etc. It took me to threaten to come down there myself to get action taken. Over the 12 years almost to the day, there are so many stories I could share regarding this place. Nurses stealing from the patients while they are sleeping or in my brother's case, at dialysis and such. What a crying shame that this place is still open. Even when I went to collect my brother's belongings, items missing or can't be located. Still almost 9 months after his death, items not returned our "found". If anyone is considering sending a loved one there, look me up and contact me first so I can talk you out of it! It used to be called Tunnell Center for Rehabilitation and then changed to City View. You can change you name but you cannot change the lack of care given. read more

I've been here for two weeks now, coming from St. Francis hospital, amputation of two toes and a chunk of foot. I was impressed from the start, both the lobby and the fifth floor where I'm staying were recently renovated and look top notch. The nursing staff is great, as are the physical and occupational therapists, I couldn't be better cared for. I even like the food...sometimes! It is what it is, but they try hard, and it's always nourishing. If you need a skilled nursing facility, you can't go wrong with this one, if I need it again in the future I wouldn't hesitate. read more

The only reason I decided to transfer my dad here for physical therapy was because they had a single room and they were able to arrange transportation to and from his radiation for 10 days. When we got to the single room, the bed handles were dirty, the floor was dirty, a previous patient's walker and nail clipper were still in the bathroom. Then we noticed that the window was being held together by a plastic bag. We arrived @ 6 pm on 5/20/24 so they were not able to address the window problem. The next day we brought the window up. They were not able to fix it becauae the window was bent. It was structural damage. I told them that he had been cold the night before because cold air could get in. The nice worker said he would bring this up to administration. Administration has sent workers down workers several times every day since we complain every day. I blame administration for taking in a patient when they knew they had a broken window that cannot be fixed while they have a patient in the room. I blame administration for not making sure that bedrooms are clean before moving in another patient to make them money. read more

I was discharged today from this skilled nursing facility and I had an awesome experience. Good quality of care with nurses and has a great rehab team. This facility smells good and clean. All the staff are courteous, polite and friendly. Special thanks to Beth Mesina in Admissions for her time and effort. She helped me with my request to transfer from Los Altos post Acute to this facility and She made it happen in couple of days. She's a beautiful woman with a great smile and a big heart . Everytime she's on the 5th floor She would check on me and ask me if I feel better or if I have eaten my my meal. I totally recommend this facility and please make sure to look for her. What a disappointment! The rooms, bathrooms, showers and hallways are not sanitary. COVID-19 is not over! Nurses pick and choose what policies and routines to follow. Nothing is consistent. If you have a shortage of nurses, don't take on anymore patients! People are here to heal. They don't need anymore stress. Medications are not given on time and not attended to in a timely manner. A patient shouldn't ask for their meds, to have their sheets changed or have their family members do half your job for you. Insurance companies pay you! This place needs to be audited by the Health Dept.Thank god family was their daily to oversee the care your patient. I can't even imagine what people that don't have family/friends go through. I have to say that Rosie, Janet and Jayce were only staff members that cared, worked hard and attended to her patients with TLC. read more

Staff is extremely lazy. They do not distribute medications correctly and timely, for example they do not give you your medication that is needed to be taken before meals instead they give you all your medications one time and by one time I mean like 19 pills at once! Bathrooms are dirty, I honestly don't know how they pass health department inspections. You ask for bathroom assistance and have to wait forever until someone finally comes to help you. Patients here need TLC to recover and if you can not offer that then quit and let someone who actually wants to work do the job. read more

This place is a nightmare! Its been a few years...my mom was here a few times. Once while I was visting her she had a stroke as I was wheeling her to the bathroom as the staff wouldnt. When I told the nurse she just had what appeared to be a stroke, she just went completely limp, eyes rolled back, mouth wide open and unable to respond the nurse said "just put her back in bed hun she will be fine" I had to call 911 myself and the medics and rescue captain that responded almost lost it when they were told what happened. On a prior vist my mother told me the nurse wiped my moms underarms with another patients towel before she could stop her, the result, MRSA a day or two later. During her multiple stays I have even had to pick elderly patients up off of the floor as staff would just walk by them as they lay there. Bottom line, if you dont look like the staff or speak their language you wont get any attention here. The nurses pick out the family they like and thats the room they would hang out in. The physical therapissts were very good and caring and there was one head nurse during one stay that was great. There was also a Vietnamese gentleman who assisted patients and did the job of at least three people but of course he was smart enough to leave. Maybe the place has changed in the last six years or so but I doubt it read more

My father was transferred to this place by his Doctor. It was a nightmare for us. I went to visit him and the room was dirty I called for help and no one responded, I decided to go out of the room and look for help, and there were a few caregivers just talking. I asked them if they can clean my father's room and they never show up. Finally I requested gloves and I told them. I will do your job. My father look at me with tears and told me. Take me out from here. And I talked to his Doctor and took him to my house. I never ever will take my father back to this terrible place. read more

My 81 year old mom was at this facility for 2 weeks in September we were very happy with the service and the care she received . The staff were very helpful and made sure my mom was comfortable the nutritionist was thorough with her dietary needs. Her social worker Clay was always there to help and made sure she was taken care of . He is very accessible and responds to calls right away . The physical therapists and OT worked with her to get her back on track . I would recommend this facility to anyone who needs care . read more

If I could rate this place 0 stars I would this facility has lost my fathers laundry several times and I just got called saying they cannot find his clothes again and I wrote his name on his clothes I will be coming by with receipts I need to be reimbursed for the clothes I just bought my father he doesn't have much clothes in general and to consistently have to buy some more for him is ridiculous the cost of living in sf is high enough consistently having to buy clothes for him cause they keep loosing his shit is ridiculous I'm sorry but this is atleast the 3/4th time they have lost his stuff and it's pissing me off read more

I have to take away one star for the constant fluctuation of nurses on the floor. Too many registry nurses, changing to another new person night after night. More regular nurses needed on staff. The regulars are awesome, given the huge load they carry to manage a whole floor. The nurses, CNAs and therapists are all super-supportive, and caring. read more

Our son is recovering on the 5th floor from a serious leg injury. The care has been spectacular. The nursing staff has been excellent as well as all the support people. Everything you could wish for. The facility is very clean and a recent remodel is just finishing up so everything is new. I don't understand some of the negative comments except someone told us it changed hands within the last year so maybe that is the reason. The only minor thing is sometimes the elevators are a bit overwhelmed at peak times. All and all we are very pleased with the care he is receiving at this facility. Highly recommend it. read more
